Systemic treatment of cutaneous leishmaniasis: A systematic review and meta-analysis.

BACKGROUND: Cutaneous leishmaniasis (CL) is increasingly reported in previously non-endemic European countries. Although often self-limiting or amenable to topical therapy, systemic treatment is indicated in specific situations. Evidence guiding systemic therapies remains limited and heterogeneous. OBJECTIVES: This systematic review and meta-analysis aimed to synthesize available evidence on systemic treatment regimens for CL. METHODS: MEDLINE/PubMed and Embase were searched for clinically and/or parasitologically confirmed CL receiving systemic monotherapy with meglumine antimoniate (MA), m

Obesity-Associated Inflammatory Reprogramming Impairs a Granulomatous Response to Leishmania infantum.

The demographics of leishmaniasis in Brazil have shifted in recent decades, coinciding with dietary changes and an increase in overweight/obesity. Cutaneous leishmaniasis becomes more treatment refractory in obese individuals, highlighting the impact of obesity on disease outcome. The impact of the overweight/obese state on infection with Leishmania infantum, which causes visceral leishmaniasis (VL), is unclear. We previously showed that an obesity-prone diet restricts L. infantum expansion in the livers of mice, but the immunological basis remained unexplored. Herein, we investigated whether

Diagnosis of localized cutaneous leishmaniasis and treatment outcomes following carbon dioxide cryotherapy performed by health centre staff: A pilot implementation study in Ethiopia.

In Ethiopia, the incidence of cutaneous leishmaniasis (CL) is estimated to range from 20, 000-30, 000 new cases due to Leishmania aethiopica. Children and young adults, in rural communities with limited access to health care are most affected. The current standard of care, pentavalent sodium stibogluconate is only administered in specialist centres, has limited effectiveness and is associated with severe adverse effects. Cryotherapy is recommended in national guidelines at the primary care level to treat uncomplicated CL, but is rarely available due to limited numbers of trained clinicians and

A pilot study evaluating the tolerability and effectiveness of miltefosine and sodium stibogluconate for the treatment of cutaneous leishmaniasis in Kandahar, Afghanistan.

BACKGROUND: Oral miltefosine (MF) achieves per-protocol cure rates of 68-77% against Leishmania tropica in Iran and Pakistan, but it has never been compared with the standard of care in Afghanistan, where only intralesional (IL) or intramuscular (IM) antimonials are used. We, therefore, assessed the 6-month efficacy and tolerability of sodium stibogluconate (SSG) and MF, including MF pharmacokinetics, in L. tropica-predominant Kandahar city. METHODS: This single-centre, open-label, pilot trial randomised patients aged ≥5 years to either SSG or 28-day, partially supervised MF that was allometr

Reactivation of Cutaneous Leishmaniasis in a Rheumatoid Arthritis Patient Undergoing Tocilizumab Treatment.

Cutaneous leishmaniasis is a neglected vector-borne disease caused by Leishmania spp., with viable parasites potentially persisting after clinical cure. Immunosuppressive therapies may favor reactivation of latent infection. The case of a 62-year-old Brazilian woman with a 25-year history of rheumatoid arthritis who developed a painful ulcerative lesion on her right leg 1 month after initiating tocilizumab therapy is reported. Molecular testing confirmed Leishmania (Viannia) braziliensis, consistent with reactivated cutaneous leishmaniasis. Tocilizumab was discontinued, and liposomal amphoteri

Enterococcus mundtii CRL35 as a Dual-Route Treatment for Cutaneous Leishmaniasis: Preclinical Safety and Efficacy Assessment.

American tegumentary leishmaniasis (ATL) remains a therapeutic challenge due to the toxicity and limited effectiveness of conventional agents. This study evaluated Enterococcus mundtii CRL35, a food-grade lactic acid bacterium, as a dual-mechanism therapeutic platform against Leishmania (L.) amazonensis. Initial screening identified E. mundtii CRL35 as the most active strain among four enterococci tested, demonstrating high selectivity against promastigotes (SI = 95) and intracellular amastigotes (SI = 25). In a BALB/c murine model of cutaneous leishmaniasis, three treatment modalities were ev

An assessment of the social, economic and environmental challenges amongst community members with cutaneous leishmaniasis in the Northwestern Syria Conflict Zone.

Leishmaniasis has been endemic in Syria for hundreds of years. Cutaneous Leishmaniasis (CL) has become an urgent public health challenge in the conflict-affected Northwestern Syria (NWS), where there has been significant community displacement, healthcare deterioration, and environmental degradation. We investigated the socio-economic, health awareness, psychosocial, and environmental awareness of community members afflicted with CL in the Aleppo region (NWS). A questionnaire was completed in June 2024 by 320 randomly selected members of the community with active or past CL in the districts of

Polymeric micelle loaded with lapachol for topical treatment of mice infected with Leishmania (Leishmania) major.

Cutaneous leishmaniasis (CL) is a neglected tropical disease. The treatment is restricted to a handful number of drugs that exhibit toxic effects and limited efficacy. The development of new alternative therapies, including the identification of effective drugs for topical treatment of CL, is of utmost interest. The aim of this study was to prepare, characterize and evaluate the efficacy of topical therapy with lapachol-micelle in the experimental treatment of CL. The lapachol-micelle was prepared using polaxamer 407. A preliminary stability study was conducted for 15 days at room temperature.

Optimisation of a Topical Miltefosine Carbopol-Based Gel for Cutaneous Leishmaniasis: Antileishmanial Activity After Storage and Drug Response.

We previously demonstrated the therapeutic potential of a 0.5% miltefosine carbopol-based gel (G-MTF) in experimental cutaneous leishmaniasis (CL). To support formulation optimisation before clinical evaluation, this study assessed whether G-MTF retained antileishmanial activity after storage at different temperatures and durations and evaluated its dose-response following topical application to CL lesions in BALB/c mice infected with Leishmania amazonensis. Storage effects on some physicochemical and morphological properties, sterility, and MTF detection were also assessed. In vitro antileish

Immunopathology of Leishmaniasis: Molecular and Cellular Mechanisms of Tissue Injury Provoked by Chronic Infection.

Human infections caused by parasites represent a serious worldwide health issue, especially in underdeveloped countries. According to the World Health Organization, Leishmania ssp. is responsible for more than 70,000 deaths worldwide, mostly due to untreated visceral leishmaniasis (VL). Other species that cause non-deadly cutaneous infections are also relevant; they frequently leave disfiguring scars in infected patients, as seen in those with mucocutaneous leishmaniasis (MCL) or localised cutaneous leishmaniasis (LCL). Mechanisms of tissue injury provoked by chronic infections with different

Insights into Divergent Leishmaniasis Pathogenesis: A Quantitative Flagellar Proteomic Comparison of L. mexicana, L. amazonensis, and L. infantum.

Leishmaniasis, a neglected tropical disease endemic to 98 countries, affects more than 300 million people worldwide. The disease is transmitted through the bite of infected female phlebotomine sand flies harboring Leishmania parasites. Although infection initiates at the cutaneous inoculation site, Leishmania species exhibit distinct tissue tropisms, resulting in three primary clinical manifestations: cutaneous (CL), mucocutaneous (MCL), and visceral leishmaniasis (VL). The initial stage of infection involves the engulfment of metacyclic promastigotes (MPs) by host phagocytes, a process mediat

Mucosal leishmaniasis in Brazil: spatiotemporal patterns from a 23-year retrospective study.

Mucosal/mucocutaneous leishmaniasis (ML) is a severe form of tegumentary leishmaniasis (TL). This study analyzed spatiotemporal patterns in the occurrence of ML in Brazil between 2001 and 2023. All ML cases reported to the Brazilian national surveillance system were included. Joinpoint regression models were used to assess temporal trends in ML incidence and in the percentage of ML cases among TL cases (%ML). Spatial scan statistics identified spatial and spatiotemporal clusters of ML incidence at the municipal level, while global and local (LISA) Moran's I statistics evaluated spatial autocor

Epidemiology of Imported Cutaneous Leishmaniasis: A Narrative Review.

Cutaneous leishmaniasis (CL) is a neglected tropical disease (NTD) traditionally associated with rural poverty in endemic regions but is increasingly encountered among international travelers, migrants, and displaced populations. Climate change, globalization, and mass human mobility have altered the epidemiology of CL, expanding the geographic range of competent vectors and increasing the burden of imported disease in non-endemic countries. Imported cases are of particular public health importance because they may serve as sentinels for autochthonous transmission in regions where vectors and

Genome Sequencing of Leishmania tropica in Tissues of Moroccan Patients Reveals Microfocal Transmission Underlain by (Pseudo)Clonal and Sexual Reproduction.

Leishmania tropica causes cutaneous leishmaniasis (CL) from North Africa and Ethiopia to India, and is reported to be transmitted from human to human through sand fly bites. While this species is characterized by a high genomic diversity in all the area of endemicity, there is very little information on diversity at a microepidemiological scale. Here, we concentrated on an epidemic Moroccan focus of CL and studied transmission patterns by comparative genomics of parasites in human patients. We used a culture-independent method of genome sequencing, applied directly on dermal scrapings. We iden
