A Prolonged Case of Mucocutaneous Leishmaniasis in a Central American Patient.

Mucocutaneous leishmaniasis (MCL) is a parasitic infection transmitted through sand fly bites. This disease affects people who are living in or traveling to endemic regions around the world. Cases of MCL remain underreported due to its clinical presentation and potential misdiagnosis, with cases in older adults rarely described in the literature. We present a case of an elderly Salvadoran farmer who developed MCL involving the nasal septum following travel abroad and a subsequent ulcerative lesion in the right lower limb. We discuss his diagnosis, treatment, histopathology, and outcome.

Immunopathology of Leishmaniasis: Molecular and Cellular Mechanisms of Tissue Injury Provoked by Chronic Infection.

Human infections caused by parasites represent a serious worldwide health issue, especially in underdeveloped countries. According to the World Health Organization, Leishmania ssp. is responsible for more than 70,000 deaths worldwide, mostly due to untreated visceral leishmaniasis (VL). Other species that cause non-deadly cutaneous infections are also relevant; they frequently leave disfiguring scars in infected patients, as seen in those with mucocutaneous leishmaniasis (MCL) or localised cutaneous leishmaniasis (LCL). Mechanisms of tissue injury provoked by chronic infections with different

Clinical Characteristics and Determinants of Treatment Duration in Cutaneous and Mucocutaneous Leishmaniasis: A Comparative Analysis.

BACKGROUND OBJECTIVES: Cutaneous (CL) and mucocutaneous leishmaniasis (MCL) are vector-borne infections with overlapping etiologies but distinct clinical presentations and therapeutic challenges. Data on factors influencing treatment duration in these forms remain limited, particularly in endemic regions. To compare clinical characteristics of CL and MCL and to identify predictors of treatment duration among patients treated with intramuscular pentavalent antimonials. METHODS: This retrospective cohort study analyzed 306 laboratory-confirmed leishmaniasis cases (274 CL, 32 MCL) diagnosed via

Evidence Gaps and Global Patterns in Leishmaniasis Control: A Scoping Review of Clinical, Diagnostic, and Treatment Strategies.

BACKGROUND: Leishmaniasis is a neglected tropical disease with diverse clinical forms, heterogeneous geographical distribution, and persistent diagnostic and treatment challenges. Although substantial primary research exists, evidence remains fragmented, limiting integrated and context-specific control strategies. This scoping review synthesizes global evidence on disease distribution, diagnostic performance, and treatment outcomes, and identifies critical evidence gaps. METHODS: A scoping review was conducted in accordance with the Arksey and O'Malley framework and PRISMA-ScR guidelines. Fou

Insights into Divergent Leishmaniasis Pathogenesis: A Quantitative Flagellar Proteomic Comparison of L. mexicana, L. amazonensis, and L. infantum.

Leishmaniasis, a neglected tropical disease endemic to 98 countries, affects more than 300 million people worldwide. The disease is transmitted through the bite of infected female phlebotomine sand flies harboring Leishmania parasites. Although infection initiates at the cutaneous inoculation site, Leishmania species exhibit distinct tissue tropisms, resulting in three primary clinical manifestations: cutaneous (CL), mucocutaneous (MCL), and visceral leishmaniasis (VL). The initial stage of infection involves the engulfment of metacyclic promastigotes (MPs) by host phagocytes, a process mediat

Mucosal leishmaniasis in Brazil: spatiotemporal patterns from a 23-year retrospective study.

Mucosal/mucocutaneous leishmaniasis (ML) is a severe form of tegumentary leishmaniasis (TL). This study analyzed spatiotemporal patterns in the occurrence of ML in Brazil between 2001 and 2023. All ML cases reported to the Brazilian national surveillance system were included. Joinpoint regression models were used to assess temporal trends in ML incidence and in the percentage of ML cases among TL cases (%ML). Spatial scan statistics identified spatial and spatiotemporal clusters of ML incidence at the municipal level, while global and local (LISA) Moran's I statistics evaluated spatial autocor

Epidemiology of Imported Cutaneous Leishmaniasis: A Narrative Review.

Cutaneous leishmaniasis (CL) is a neglected tropical disease (NTD) traditionally associated with rural poverty in endemic regions but is increasingly encountered among international travelers, migrants, and displaced populations. Climate change, globalization, and mass human mobility have altered the epidemiology of CL, expanding the geographic range of competent vectors and increasing the burden of imported disease in non-endemic countries. Imported cases are of particular public health importance because they may serve as sentinels for autochthonous transmission in regions where vectors and

Cutaneous leishmaniasis: aetiopathogenesis, epidemiology, clinical features, management and research gaps.

Cutaneous leishmaniasis (CL) is a protozoan parasitic infection caused by Leishmania species and transmitted through the bite of infected sandflies. The disease is endemic in tropical, subtropical and Mediterranean regions, where it presents as localized skin ulcers, often leaving scars that can lead to significant morbidity and social stigma. This review focuses on the aetiopathogenesis, epidemiology, clinical features, management strategies and research gaps in CL. Understanding the complex immune response involved in the pathogenesis of CL, as well as the environmental and socio-economic fa

From dogma to data: charting a path forward for clinico-immunological research in Ethiopian cutaneous leishmaniasis.

BACKGROUND: Rooted in long-standing assumptions and adapted from classifications mainly used for Latin American cutaneous leishmaniasis (CL), the nationally recommended clinical categories in Ethiopia for CL remain limited to localized cutaneous leishmaniasis (LCL), mucocutaneous leishmaniasis (MCL), and diffuse cutaneous leishmaniasis (DCL). However, these categories are associated with immune mechanisms which have not been validated in the Ethiopian context and thus risk misrepresenting the true clinical and immunopathological diversity. In this opinion piece, we will therefore outline key k

[Multilocular mucocutaneous leishmaniasis with perforation of the anterior nasal septum: a rare differential diagnosis].

Leishmaniasis, caused by protozoa of the genus Leishmania, comprises visceral, cutaneous, and mucocutaneous forms. It is mainly endemic in South and Central America as well as the Mediterranean region, while being very rare in Europe. Mucocutaneous leishmaniasis may affect the nasal and oral mucosa, and the current case highlights the importance of interdisciplinary collaboration: both cutaneous and mucocutaneous lesions with nasal septum perforation were present. Close cooperation between the departments of otorhinolaryngology, infectious diseases, and tropical medicine are key to ensuring ra

Cutaneous Leishmaniasis in Patients under Biologic Therapies: A Spanish CLINI-AEDV Multicentre Study.

Anti-TNFα agents have been associated with the reactivation of granulomatous infections, including leishmaniasis. Cases of atypical or recurrent cutaneous (CL) or mucocutaneous leishmaniasis (MCL) have been reported in patients on biologics. We conducted a retrospective analysis of a multicenter database to describe the clinical characteristics and management of CL and MCL in patients receiving biological therapy in an endemic area. Clinical features, diagnostics, therapies, and outcomes were analyzed. Seventy-one patients were included (63 CL and 8 MCL). The most common underlying condition w

Laboratory confirmation of tegumentary leishmaniasis cases in the southern Brazilian Amazon, 2007-2022: Temporal trends, spatial patterns and associated factors.

Laboratory confirmation of tegumentary leishmaniasis (TL) has been prioritized by the Pan American Health Organization (PAHO) in the Americas, given the potential toxicity of antileishmanial treatments and broad differential diagnoses. This population-based study analyzed temporal trends, spatial patterns, and determinants of laboratory confirmation of TL cases reported between 2007 and 2022 in Mato Grosso-a highly endemic state in the southern Brazilian Amazon. Temporal trends in the percentage of laboratory-confirmed cases were assessed for the state and its five mesoregions using joinpoint

Risk of symptomatic leishmaniasis in patients with inflammatory bowel disease receiving biologic therapy in an endemic Mediterranean region: a population-based case-control study.

BACKGROUND AND AIMS: Leishmaniasis is an emerging opportunistic infection in patients with inflammatory bowel disease (IBD) living in Mediterranean endemic areas. However, the magnitude of the risk and treatment-related determinants remain unknown, and no controlled population-based studies have addressed this issue. We aimed to estimate the incidence of symptomatic Leishmania infection in IBD population and to identify associated clinical and therapeutic risk factors, particularly immunosuppressive treatments. METHODS: An observational study was conducted in the Valencian Community (Spain) b

Strain-dependent duodenal structural alterations in hamsters infected with Leishmania (Viannia) braziliensis.

Leishmania (Viannia) braziliensis (LVB), the main etiological agent of cutaneous and mucocutaneous leishmaniasis, may cause atypical manifestations in immunosuppressed hosts, affecting organs beyond the skin, including the intestine. This study investigated whether the duodenum is involved during experimental LVB infection in hamsters. Overall, 48 female golden hamsters were divided into a control and infected group with five LVB strains. They were euthanized after 90 or 120 days of infection. Duodenal samples were subjected to histological and morphometric analyses, including evaluation of th

Mucosal leishmaniasis of the nasal septum: diagnostic pitfalls and management considerations.

BACKGROUND AND OBJECTIVE: Leishmaniasis is a chronic parasitic disease caused by a protozoan of the order Kinetoplastida, transmitted through the bite of infected sandflies. Its mucosal form with sinonasal involvement is exceptional but clinically relevant, potentially causing granulomatous lesions, nasal obstruction, and epistaxis, mimicking inflammatory or malignant disease. In the Old World (Europe, Asia, Africa), it is usually associated with Leishmania infantum, whereas in the New World (America) L. braziliensis predominates, generally following a previous or concomitant cutaneous lesion.

Epidemiological and clinical features of leishmaniasis in a tertiary hospital in the state of Paraná, Brazil.

American tegumentary leishmaniasis (ATL) is an endemic disease in Brazil, but comprehensive reports on its epidemiology and clinical characteristics remain limited. The potential shifts in the disease's epidemiological profile emphasize the need for updated and in-depth studies. In this retrospective longitudinal observational study, we analyzed the epidemiological and clinical features of patients with ATL who received care at the Clinical Hospital Complex of the Federal University of Paraná (CHC-UFPR), a key referral center for the disease in the state of Paraná, southern Brazil, between Jun

Challenges in diagnosing mucocutaneous leishmaniasis caused by Leishmania (Mundinia) martiniquensis mimicking oral histoplasmosis in a patient with advanced HIV disease from Southern Thailand.

Differentiation between leishmaniasis and histoplasmosis remains a significant diagnostic conundrum, especially in immunosuppressed patients, due to overlapping clinical presentations and histopathological features. We report a case of a 53-year-old Thai man with advanced HIV disease from Southern Thailand who presented in 2020 with a chronic palate ulcer and hard palate perforation, initially diagnosed as histoplasmosis and treated with itraconazole for a year. Three years later (2023), the patient developed systemic symptoms, and biopsies confirmed visceral leishmaniasis. Given the increasin
