TXNRD1-driven enteric glial remodeling promotes neuronal dysfunction in Hirschsprung disease.

Hirschsprung disease (HSCR) is characterized by distal intestinal aganglionosis and persistent dysfunction of the enteric nervous system after surgical treatment. Here, we integrated single-cell transcriptomics, network-based virtual perturbation analysis, proteomics, human tissue validation, enteric glial cell assays, neuron-glia co-culture systems, and an Ednrb conditional knockout (cKO) mouse model to investigate TXNRD1-dependent enteric glial remodeling in HSCR. We found that thioredoxin reductase 1 (TXNRD1) enrichment was associated with expansion of an SPP1-high/CXCL9-low enteric glial s

A diagnostic cascade from cholecystitis to Meckel's diverticulum rupture: a case report.

BACKGROUND: Meckel's diverticulum (MD) is the most common congenital anomaly of the gastrointestinal tract, yet it rarely becomes symptomatic in adults. Perforation is an uncommon and often overlooked complication that may mimic more frequent abdominal conditions, particularly in the postoperative setting. MD presenting shortly after unrelated laparoscopic surgery is exceptionally rare and can pose a significant diagnostic challenge. CASE PRESENTATION: A 36-year-old woman presented with acute right upper quadrant pain and was diagnosed with calculous cholecystitis. She underwent laparoscopic

From Collodion Membrane to Alopecia: An Insight Into IFAP/BRESHECK Syndrome.

Ichthyosis follicularis, atrichia, and photophobia (IFAP) syndrome is a rare genodermatosis characterized by follicular ichthyosis, alopecia, and photophobia; classical X-linked disease is caused by MBTPS2 variants, whereas autosomal dominant forms are associated with SREBF1 variants. The severe IFAP/BRESHECK phenotype may include brain anomalies, intellectual disability, ectodermal dysplasia, skeletal defects, Hirschsprung disease, ear and eye anomalies, cleft palate, cryptorchidism, and kidney abnormalities. We present a 7-year-old male patient with a collodion membrane at birth, severe icht

Meckel's diverticulum leading to axial torsion and volvulus of the small bowel.

A Meckel's diverticulum is a blind-ending pouch that arises on the antimesenteric border of the terminal ileum. It is the most common gastrointestinal anomaly and the result of an incomplete obliteration of the vitelline duct during the first 8 weeks of gestation. The 'rule of two' is a mnemonic which refers to key characteristics of Meckel diverticula: it is found in 2% of the population, it is 2 inches long, 2 feet from the ileocaecal valve and twice as common in males. Only 2%-4% of those with a Meckel's develop complications in their lifetime. The majority present in early childhood with c

H-type anorectovaginal fistula with anterior ectopic anus in a 15-month-old girl: a case report.

INTRODUCTION AND IMPORTANCE: H-type anorectovaginal fistula is a rare anorectal malformation characterized by an abnormal communication between the rectum and vagina despite a normally formed or near-normal anus. Diagnosis may be delayed due to the absence of obvious external anatomical abnormalities, making clinical recognition essential, particularly in resource-limited settings. CASE PRESENTATION: We report a 15-month-old girl presenting with fecal vaginal discharge and chronic constipation since early infancy. Clinical examination revealed an anterior ectopic stenotic anus with vaginal fe

Population Differences in the Epidemiology, Phenotype, and Genetics of Hirschsprung Disease in the United States.

IMPORTANCE: Understanding population differences in epidemiology, clinical presentation, and genetic architecture remains a major challenge for all rare genetic disorders. Hirschsprung disease (HSCR), despite being the commonest cause of neonatal intestinal obstruction, has been poorly studied with respect to its significant heterogeneity across U.S. populations. OBJECTIVE: To characterize self-identified race and ethnicity differences in HSCR incidence, clinical presentation, and genetic architecture in the United States from diverse data sources. DESIGN SETTING AND PARTICIPANTS: We used re

One-year outcomes for patients undergoing antegrade continence enema procedures without prior rectal enema therapy: a Pediatric Colorectal and Pelvic Learning Consortium study.

PURPOSE: Antegrade continence enemas (ACE) delivered through surgically placed channels help achieve social continence in children with colorectal disorders. Historically, a successful rectal enema program was required before ACE creation, but high-volume centers increasingly perform ACE upfront, particularly for patients intolerant of rectal enemas. This study describes a cohort of patients who underwent upfront ACE for bowel management within a multicenter consortium of high-volume pediatric colorectal centers, with 30-day and one-year outcomes. METHODS: We retrospectively evaluated patient

Concomitant Meckel's Diverticulum With Focal Paneth Cell Metaplasia and Acute Appendicitis Presenting as Chronic Periumbilical Pain in a Child.

Meckel's diverticulum is the most common congenital anomaly of the gastrointestinal tract but remains diagnostically challenging in children because of its varied and nonspecific clinical presentation. Concomitant Meckel's diverticulum and acute appendicitis are uncommon and may further obscure diagnosis. We report a 9-year-old boy who presented with chronic periumbilical abdominal pain of 3 months' duration, with acute worsening over 3 days accompanied by vomiting. Physical examination revealed localized right periumbilical tenderness without signs of peritonitis. Laboratory investigations de

Bibliometric analysis of clinical treatment in Hirschsprung disease: trends and emerging hotspots.

BACKGROUND: Hirschsprung disease (HSCR) is a congenital gastrointestinal malformation defined by the absence of enteric ganglion cells in the distal bowel. Although surgical resection remains the primary treatment, the field has witnessed rapid evolution toward precision minimally invasive strategies, including laparoscopic/robotic-assisted procedures and standardized perioperative protocols. The expanding literature volume necessitates a systematic synthesis of the research landscape. METHODS: We retrieved HSCR clinical treatment publications from Web of Science Core Collection and Scopus (2

Report of Meckel Diverticulum Mimicking Stricturing Crohn's Disease in an Adolescent Boy.

Inflammatory bowel disease, specifically Crohn's disease (CD), can present with hematochezia, iron deficiency anemia, intestinal obstruction, elevated fecal calprotectin often overlapping with other conditions. We report a 16-year-old adolescent boy with presumed stricturing CD whose anti-tumor necrosis factor therapy failed to respond. Persistent iron deficiency anemia and elevated fecal calprotectin prompted reevaluation imaging, raising concerns for the Meckel diverticulum. Surgical resection resulted in resolution of anemia. This case highlights the importance of reassessing a diagnosis in

Adult Onset Hirschsprung's Disease With Overlapping Presentation of Sigmoid Volvulus: Two Case Reports With Literature Review at Tikur Ambessa Specialized Hospital, AAU, Ethiopia.

Adult-onset Hirschsprung's disease (HSD) is uncommon and frequently leads to diagnostic difficulty after infancy. It is a congenital illness that causes intestinal obstruction due to a lack of ganglion cells. Approximately 1 in 5000 live newborns have this illness, and 95% of HSD cases are identified in infants. This case report will fill a knowledge gap at a low-resource setting where published papers on this topic are extremely rare, and will provide insight into diagnosis and surgical management options that are feasible in resource-limited environments with good outcomes. In this case repo

Sequential Track and Intestinal Gathering ("STRING"): A Novel Technique for Multiple Intestinal Biopsies.

BACKGROUND: Intestinal biopsies are essential for the diagnosis and treatment of Hirschsprung's disease and visceral myopathies. However, intraoperative sampling may be inconclusive, leading to diagnostic errors and unnecessary extensive resections. Current laparoscopic approaches remain technically demanding. The aim of this study is to present an unpublished, safe, and effective technique, Sequential Track and Intestinal Gathering (STRING), to perform multiple laparoscopic intestinal biopsies and ensure diagnostic accuracy in pediatric patients. METHODS: Retrospective observational descript

Comparative Outcome of Extraoral versus Intraoral Approaches in the Management of Isolated Zygomatic Arch Fractures: A Systematic Review.

INTRODUCTION: Meckel's diverticulum is a common congenital abnormality of gastrointestinal tract in children. Planar scintigraphy using Technetium-99m pertechnetate is widely used in the diagnosis of Meckel's diverticulum. Single photon emission computed tomography/computed tomography (SPECT/CT) fusion imaging may help to locate the Meckel's diverticulum lesion. We now present a Meckel's diverticulum case which tends to be missed. CASE DESCRIPTION: The patient was diagnosed with Mecke's diverticulum by planar scintigraphy in 2007. After seven years, a recurrence of hematochezia made the patie
