Inflammatory bowel disease, specifically Crohn's disease (CD), can present with hematochezia, iron deficiency anemia, intestinal obstruction, elevated fecal calprotectin often overlapping with other conditions. We report a 16-year-old adolescent boy with presumed stricturing CD whose anti-tumor necrosis factor therapy failed to respond. Persistent iron deficiency anemia and elevated fecal calprotectin prompted reevaluation imaging, raising concerns for the Meckel diverticulum. Surgical resection resulted in resolution of anemia. This case highlights the importance of reassessing a diagnosis in

Adult-onset Hirschsprung's disease (HSD) is uncommon and frequently leads to diagnostic difficulty after infancy. It is a congenital illness that causes intestinal obstruction due to a lack of ganglion cells. Approximately 1 in 5000 live newborns have this illness, and 95% of HSD cases are identified in infants. This case report will fill a knowledge gap at a low-resource setting where published papers on this topic are extremely rare, and will provide insight into diagnosis and surgical management options that are feasible in resource-limited environments with good outcomes. In this case repo

BACKGROUND: Intestinal biopsies are essential for the diagnosis and treatment of Hirschsprung's disease and visceral myopathies. However, intraoperative sampling may be inconclusive, leading to diagnostic errors and unnecessary extensive resections. Current laparoscopic approaches remain technically demanding. The aim of this study is to present an unpublished, safe, and effective technique, Sequential Track and Intestinal Gathering (STRING), to perform multiple laparoscopic intestinal biopsies and ensure diagnostic accuracy in pediatric patients.
METHODS: Retrospective observational descript

INTRODUCTION: Meckel's diverticulum is a common congenital abnormality of gastrointestinal tract in children. Planar scintigraphy using Technetium-99m pertechnetate is widely used in the diagnosis of Meckel's diverticulum. Single photon emission computed tomography/computed tomography (SPECT/CT) fusion imaging may help to locate the Meckel's diverticulum lesion. We now present a Meckel's diverticulum case which tends to be missed.
CASE DESCRIPTION: The patient was diagnosed with Mecke's diverticulum by planar scintigraphy in 2007. After seven years, a recurrence of hematochezia made the patie

BACKGROUND: Robotic-assisted surgery has transformed the surgical management of complex oncologic diseases by enhancing precision, visualization, and surgeon ergonomics. However, access to expert robotic oncologic care remains geographically restricted. Telesurgery offers a potential solution to overcome this disparity, though concerns regarding latency, reliability, and oncologic safety have limited its widespread adoption.
OBJECTIVE: To evaluate the technical feasibility, safety, and perioperative outcomes of the first North-to-South India tele-robotic cancer surgeries performed using the S

Multiple endocrine neoplasia type 2 (MEN2) is caused by germline RET mutations and is characterized by the presence of medullary thyroid cancer (MTC) associated or not with other endocrine neoplasias. MEN2 is classified as MEN2A, including different variants, and MEN2B. The aim of the present study was to evaluate the impact of the RET genetic screening in redefining the prevalence of the different MEN2 variants inside the MEN2A group. This study included 223 MEN2 families: 202 MEN2A (55 MEN2A classical, 3 MEN2A + lichen cutaneous amyloidosis (CLA), 5 MEN2A + Hirschsprung disease (HD) and 139

INTRODUCTION: Hirschsprung's (HIRSH-sproongz) disease is a multifactorial disorder characterized by the failure of enteric nervous system (ENS) development and is associated with loss-of-function variants, primarily in the RET gene. Rearranged during transfection (RET) expression is tightly regulated within a complex gene regulatory network (GRN) involving many transcription factors, such as GATA2. Aberrant DNA methylation in GATA2 may indicate a reduced enhancer activity, resulting in the silencing of RET and contributing to the failure of ENS development in HIRSH. However, the epigenetic mec

Neonates undergoing major gastrointestinal surgery for congenital or acquired disorders require complex nutritional management to support their growth and recovery. Prolonged fasting can be detrimental, necessitating timely and appropriate nutritional support. This joint position paper by the Italian Society of Neonatology, the Italian Society of Pediatric Surgery, and the Italian Society of Pediatric Nutrition aims to provide evidence-based suggestions for the nutritional care of these vulnerable infants, addressing the lack of robust randomized controlled trials in this field through expert

Stoma prolapse is a frequent complication following stoma formation in pediatric patients and often necessitates surgical intervention. The buttonpexy technique offers a minimally invasive, low-cost, and anesthesia-sparing alternative to formal stoma revision. This case series highlights its safety and effectiveness in managing pediatric stoma prolapse. Five pediatric patients with stoma prolapse secondary to various underlying conditions, including anorectal malformation, Hirschsprung's disease, and Currarino syndrome, were managed using the buttonpexy technique. In each case, the prolapsed b

Paired mesoderm homeobox protein 2B (PHOX2B) is a transcription factor essential for autonomic nervous system development. Heterozygous mutations in the PHOX2B gene are associated with neurodevelopmental disorders, including congenital central hypoventilation syndrome and Hirschsprung's disease. Additionally, PHOX2B plays a role in the genetic landscape of neuroblastoma, with mutations detected in both familial and sporadic forms of this rare cancer. Notably, PHOX2B is highly expressed in most neuroblastoma cells. Despite its significance, little is known about the regulation of PHOX2B gene ex

This detailed narrative review focuses on the current understanding of unique alterations in GM colonization and subsequent complications following surgery for significant childhood conditions, such as necrotizing enterocolitis (NEC), Hirschsprung's disease (HD), inflammatory bowel disease (IBD), and short bowel syndrome (SBS). Surgical interventions can alter the diversity and structure of the GM and potentially cause post-surgical complications. Although the data are well-established in adults, there is a lack of pediatric-specific data on post-surgical GM dysbiosis and its complications, in

BACKGROUND: Endothelin-1 (ET-1) is a proinflammatory mediator that plays a crucial role in regulating airway tone by activating G protein-coupled endothelin receptors A (ETA) and B (ETB). The endothelin system has been linked to asthma, but the impact of ETB receptor deficiency on allergic airway inflammation remains uncharted. This study explores how the endothelin system influences allergic airway inflammation and hyperresponsiveness.
METHODS: We used rescued ETB receptor-deficient (ETB -/-) mice to obviate lethal inherited Hirschsprung disease, prepro-ET-1 overexpressing (preETtg), and wil

BACKGROUND: Bowel management programs (BMPs) have demonstrated effectiveness in treating severe constipation and encopresis. We aimed to evaluate the transition rate from more invasive to less invasive treatments (e.g. enema to laxative therapy) and identify clinical characteristics that increase likelihood of downgrading treatment over time.
METHODS: Patients participating in our institution's BMP (2020-2024) were identified. Those without adequate follow-up after BMP induction were excluded. Follow-up was collected at 6-, 12-, 18-, and 24-month clinical visits. 'Downgrade' was defined as a

Down syndrome (DS; trisomy 21) confers a ~100-fold increased risk of Hirschsprung disease (HSCR), yet the causal contributions of specific chromosome 21 genes remain unresolved. Here we show that increased dosage of SOD1 alone is sufficient to perturb enteric nervous system (ENS) development. We engineered a humanized SOD1 trisomic mouse line by inserting a 28 kb human SOD1 locus into ROSA26 and genetically profiled the distal colon at postnatal day 0 using single-cell RNA-seq and immunofluorescence. Sod1/SOD1 was elevated ~1.5X in the ENS but varied by cell type, with transcriptionally active

BACKGROUND: Surgery for Hirschsprung's disease (HD) generally results in a satisfactory outcome, but some patients continue to have bowel dysfunction. There are very few studies in the literature that compare the early and late clinical outcomes.
AIM: To evaluate changes in anorectal function and clinical outcome with age in patients who underwent Duhamel operation (DO) for HD.
METHODS: Twenty-eight patients who had previously undergone DO to treat HD were invited to an outpatient clinic upon reaching adulthood to participate in follow-ups. The patients' clinical data from the early and mid-
