Embryonic Gut Explant Model for Enteric Neural Crest Cells Migration.

Migration of enteric neural crest cells (ENCCs) within the developing embryonic gut is key to the formation of the enteric nervous system (ENS). Investigating factors that guide the migration of these cells along the gut tube could aid in understanding the pathogenesis and developing potential therapy for ENS developmental defects, such as those present in Hirschsprung disease. However, limitations exist on appropriate study models to allow the live tracing of these migrating cells within the developing intestine. This protocol describes a mouse embryonic gut explant model utilizing a Sox10-Ve

Emerging livestock science and technologies can drastically alter the projections of global food security models.

The incidence and prevalence of all types of diabetes mellitus is increasing at an alarming rate. Modern therapy involves greater and earlier use of intensive insulin regimens in order to achieve better control of blood glucose levels and reduce the long-term risks associated with the condition. Insulin therapy is associated with important cutaneous adverse effects, which can affect insulin absorption kinetics causing glycemic excursions above and below target levels for blood glucose. Common complications of subcutaneous insulin injection include lipoatrophy and lipohypertrophy. The developme

Visualization of a Y-configuration and peristalsis on prenatal sonography: The pathognomonic sign of an ileal duplication cyst.

OBJECTIVE: Enteric duplication cysts are a rare congenital malformation that consists of a cystic formation adjacent to the native gastrointestinal tract. A double wall appearance on fetal abdominal sonography is highly suggestive of its diagnosis. However, this sign also can be seen in an ovarian cyst, a mesenteric cyst, and a Meckel's diverticulum. To the best of our knowledge, this case is the first report of accurate prenatal diagnosis based upon the presence of a Y-configuration and peristalsis on prenatal sonography. We herein present prenatal and postnatal images including ultrasonogram

Long-Term Quality of Life and Functional Outcomes in Patients With Anorectal Malformations: A Retrospective Multicenter Study in Brussels, Belgium.

Introduction Anorectal malformations (ARMs) are congenital conditions requiring surgical correction. Despite surgical intervention, patients often experience persistent fecal and urinary dysfunction, significantly impacting their quality of life (QoL). This multicenter retrospective study aimed to assess QoL in ARM patients, identify factors influencing QoL deterioration, and propose improved management strategies. Methods We retrospectively analyzed data from 39 ARM patients who underwent surgery between 1999 and 2019 at two Belgian hospitals. Patients completed the Hirschsprung's Disease Ano

Neuroendocrine tumor arising in Meckel's diverticulum presenting with bowel obstruction.

Meckel's diverticulum (MD) is a common developmental abnormality of the gastrointestinal tract that, in adults, is usually asymptomatic but can present with complications including malignancy. Although malignancy in MD is rare overall, MD is recognized a high-risk site for tumorigenesis compared to the surrounding ileum. Neuroendocrine neoplasms are the most common malignancy arising in MD and are often associated with nodal metastases, even if the primary tumor is small. We report the case of a 66-year-old male who underwent exploratory laparotomy for small bowel obstruction and was intraoper

Study of the gut microbiome profile in full-term infants with necrotizing enterocolits.

BACKGROUND: Necrotizing enterocolitis (NEC) is a severe gastrointestinal disease that primarily affects preterm. Recently, cases with similar clinical manifestations have been observed in full-term infants without Hirschsprung's disease. Dysbiosis suggested to have role in development of NEC. AIM: To compare the gut microbiome composition between full term infants with NEC and healthy controls and to evaluate the impact of feeding sources on microbial diversity. METHODS: Ten full term infants with NEC and six matched healthy breastfed control infants were enrolled in the study. Triplicate st

Perforated Meckel's Diverticulum in a Neonate With Bucket-Handle Anorectal Malformation: A Rare Surgical Emergency.

BACKGROUND: Neonatal pneumoperitoneum is a life-threatening condition requiring urgent surgical intervention. Although Meckel's diverticulum (MD) and anorectal malformations (ARMs) are individually recognised congenital anomalies, their coexistence is uncommon, and perforated MD in a neonate with a bucket-handle ARM has been rarely described in the literature. CASE PRESENTATION: A term male neonate (birthweight 2600 g) presented on Day 4 of life with progressive abdominal distension and nonbilious vomiting. Examination revealed tachycardia, tachypnoea, delayed capillary refill and a bucket-ha

Long-term disease-free survival following preoperative diagnosis and laparoscopic radical resection for Meckel's diverticulum adenocarcinoma: A case report.

BACKGROUND: Meckel's diverticulum (MD) is most commonly encountered during emergency interventions for complications or as an incidental finding during an unrelated procedure. Definitive pre-operative diagnosis of MD-associated adenocarcinoma, permitting radical resection, is very rare in clinical practice. CASE SUMMARY: A 36-year-old male presented with recurrent dark-red hematochezia. Initial gastroscopy, colonoscopy, computed tomography, and tumor marker assessments were unremarkable. A technetium-99m pertechnetate Meckel scan (Meckel scan) revealed ectopic gastric mucosa, suggesting an MD

PRDM9 Deficiency Drives Mosaic Promoter Deletions in Sporadic Hirschsprung Disease and Supports Blood-Based Molecular Stratification.

BACKGROUND & AIMS: Hirschsprung disease (HSCR) is a congenital enteric neuropathy with distal aganglionosis and dysmotility. Germline mutations explain many familial cases, but most sporadic cases lack a molecular explanation. We sought noncanonical mechanisms and blood-accessible biomarkers. METHODS: We performed RNA-sequencing (RNA-seq) on paired aganglionic and ganglionic colon biopsy specimens from 103 sporadic HSCR patients, with control colon samples (n = 22). Blood whole-genome sequencing (WGS) was available for 41 patients with RNA-seq profiles. We assessed PR/SET domain 9 (PRDM9) in

Intestinal obstruction by Meckel's diverticulum in stricturing Crohn's disease: coincidence?

We report the case of a 56-year-old male with a long-standing history of stricturing CD involving distal ileum. He had previously been treated with multiple biologics agents (infliximab, adalimumab, ustekinumab) due to persistent stenotic disease requiring repeated endoscopic ileal dilations. He presented to the emergency department with a two-week history of generalized abdominal pain, nausea and vomiting. Physical examination revealed diffuse abdominal tenderness and distension without signs of peritonism. Laboratory tests were unremarkable. Abdominal radiography demonstrated dilated small b

Redefining PHOX2B-Related Neurodevelopmental Disorders as Circuitopathies: A New Paradigm in Autonomic Network Medicine.

The paired-like homeobox transcription factor 2B (PHOX2B) is a master regulator whose mutations cause a spectrum of severe neurological and neurodevelopmental disorders, including Congenital central hypoventilation syndrome (CCHS), Hirschsprung disease, and neuroblastoma. For decades, a central paradox has challenged the field: how can PHOX2B's seemingly restricted expression in the brainstem account for the broad, multi-system clinical phenotypes observed in patients? Here, we elucidate how this paradox has been resolved by recent methodological advances. We synthesize evidence from multiple

Atypical presentation of Meckel's diverticulum in late childhood with dual ectopic gastric and pancreatic mucosa: a case report.

Meckel's diverticulum (MD) is a common congenital anomaly, usually symptomatic in early childhood. The presence of multiple heterotopic tissues within a single diverticulum is rare and poses significant diagnostic challenges. In this report, a 12-year-old male presented with a month-long history of painless lower gastrointestinal (GI) bleeding (melena and hematochezia). While endoscopy was normal, 99mTc scintigraphy revealed MD in the right pelvic region. Laparoscopic diverticulectomy was performed, and histopathology confirmed MD containing dual ectopic gastric and pancreatic tissues. Symptom

Enteric neuroimmune interactions in health and disease.

The intestine contains a dense intrinsic nervous system that is closely integrated with local immune cells to coordinate motility, barrier defense, and tissue repair. Recent studies, primarily in mice and supported by emerging human evidence, have identified diverse enteric neuroimmune pathways through which enteric neurons shape immune cell responses, while immune cells, in turn, influence neuronal survival and function across distinct intestinal compartments. These bidirectional interactions are increasingly implicated in intestinal disorders, including Hirschsprung's disease, irritable bowe

Urinary prostaglandin E major urinary metabolite in neonates: Gestational age at birth-dependent dynamics and clinical implications.

BACKGROUND: Prostaglandin E-major urinary metabolite (PGE-MUM) is a stable urinary marker of prostaglandin E2 metabolism and it is used for monitoring inflammation. However, its physiological dynamics in neonates across gestational ages remain unclear. Here, we aimed to investigate the longitudinal changes in PGE-MUM levels during the first 14 postnatal days (PND) in neonates with varying gestational age at birth (GA) and to examine its association with GA and neonatal inflammatory conditions. METHODS: This prospective observational study enrolled 40 neonates admitted to the neonatal intensiv

Meckel's diverticulum presenting as an umbilical granuloma in a 4-month-old Somali infant: a rare case report.

INTRODUCTION AND IMPORTANCE: Meckel's diverticulum (MD) is the most common congenital anomaly of the gastrointestinal tract, resulting from incomplete obliteration of the vitelline duct. Although usually asymptomatic, it may rarely present as an umbilical lesion resembling an umbilical granuloma, creating diagnostic challenges in low-resource settings. PRESENTATION OF CASE: A 4-month-old Somali male infant presented with a persistent umbilical mass and mucous discharge. The lesion had previously been diagnosed as an umbilical granuloma and treated with silver nitrate without improvement. Exam

Ultrasonographic diagnosis of pure inversion of Meckel's diverticulum without obstruction in a child: a case report and literature review.

INTRODUCTION: Meckel's diverticulum (MD) is the most common congenital malformation of the gastrointestinal tract. Most cases are asymptomatic, while a minority may develop complications such as hematochezia, diverticulitis, and intestinal obstruction. When a Meckel's diverticulum inverts into the lumen of the ileum rather than protruding outward from the intestinal wall, it is referred to as an inverted Meckel's diverticulum, which is a very rare condition. Although inverted MD frequently presents with intestinal obstruction, cases of pure inversion without obstruction are exceptionally rare.

Maternal nonylphenol exposure induces offspring intestinal injury and enteric neuronal defects involving proinflammatory macrophage polarization.

Nonylphenol (NP), an environmental endocrine disruptor with estrogenic activity, has been reported to exert marked neurotoxic effects. Hirschsprung's disease (HSCR) is a congenital intestinal disease characterized by the absence of enteric neurons in the gut. However, it remains unclear whether maternal exposure to NP would induce HSCR-related intestinal lesions in the offspring. Sprague Dawley rat model of maternal NP exposure during pregnancy was established to explore the effects of prenatal NP exposure on the development of enteric nervous system in offspring. In vivo and in vitro experime
