The wandering mimic: Meckel's diverticulum with enterolith simulating an intraperitoneal hydatid cyst.

Meckel's diverticulum (MD) is the most common congenital anomaly of the gastrointestinal tract and is often asymptomatic. Unusual complications such as enterolith formation or foreign body impaction are rare, particularly in pediatric patients, and may produce atypical imaging findings that mimic other intra-abdominal pathologies. We report a case of a previously healthy 4-year-old boy who presented with intermittent colicky abdominal pain and non-bilious vomiting. Contrast-enhanced CT demonstrated a cystic lesion in the left paracolic gutter containing multiple internal rounded low-attenuatio

Complicated Meckel's diverticulum due to axial torsion and fibrous band formation: a rare cause of acute abdomen.

Acute abdomen is a potentially life-threatening condition requiring prompt evaluation and surgical intervention. Symptomatic Meckel's diverticulum in adults is uncommon and may mimic other intra-abdominal pathologies. We report a 25-year-old man presenting with diffuse abdominal pain, nausea, diarrhea, and signs of peritoneal irritation suggestive of complicated appendicitis. Exploratory laparotomy revealed axial torsion of a Meckel's diverticulum with irreversible vascular compromise caused by a fibrous band. Diverticulectomy with wedge resection was performed, resulting in complete recovery.

Jejunal diverticulitis with misleading biochemical improvement: a diagnostic challenge.

Jejunal diverticulitis is a rare and often under-recognized cause of acute abdominal pain, frequently presenting with non-specific clinical, biochemical, and radiological findings. A 52-year-old female presented with acute abdominal pain and elevated inflammatory markers, but non-diagnostic imaging. Despite biochemical improvement with conservative management, her symptoms persisted. Diagnostic laparoscopy revealed jejunal diverticulitis with an incidental Meckel's diverticulum requiring dual segmental small bowel resection. The postoperative course was uneventful. This case highlights the lim

Littré's hernia within a recurrent inguinal hernia sac: an unusual cause of acute abdomen in the elderly-a case report.

Littré's hernia is defined as the protrusion of a Meckel's diverticulum through a hernia defect; it represents fewer than 0.1% of complicated hernias, and its presentation as a recurrent hernia in the elderly is exceptional. We report an 81-year-old man with a previous open right inguinal herniorrhaphy who presented with abdominal pain, vomiting, and a non-reducible right inguinal mass consistent with intestinal obstruction. Exploratory laparotomy revealed a strangulated recurrent right inguinal hernia Nyhus IVB/European Hernia Society (EHS) L3R (lateral inguinal hernia, size 3 [>3 cm], recurr

Migrated biliary stent causing Meckel's diverticulitis and ileal perforation: a rare cause of acute abdomen.

Migration of plastic biliary stents is a recognized complication of endoscopic retrograde cholangiopancreatography (ERCP), although it is usually asymptomatic and self-limiting. Intestinal perforation due to stent migration is rare, and involvement of Meckel's diverticulum is exceedingly uncommon. We report a 70-year-old woman who presented with acute abdomen approximately one year after ERCP with placement of a 10F-10 cm plastic biliary stent for choledocholithiasis. Computed tomography revealed small bowel obstruction and a linear hyperdense intraluminal structure consistent with a foreign b

Etiology-based scoring for pediatric secondary intussusception: a retrospective analysis of clinical heterogeneity.

PURPOSE: To investigate clinical heterogeneity in pediatric secondary intussusception and to develop four simplified, etiology-specific scoring systems to facilitate preoperative etiologic prioritization after intussusception has been confirmed. METHODS: This retrospective study analyzed 92 pediatric patients diagnosed with secondary intussusception from 2018 to 2023. Disease patterns across PLP subtypes were analyzed using a four-dimensional framework including etiology, age, sex, and clinical features. Candidate variables were selected based on clinical plausibility and univariable screenin

Laparoscopic Management of Adult Ileocolic Intussusception Secondary to an Inverted Meckel's Diverticulum.

Intussusception is the telescoping of one part of the bowel into an adjacent part, and it is most common in the pediatric population. In the adult population, intussusception is rare; however, it is likely secondary to a pathological lead point, such as a neoplasm. Intussusception has been described secondary to Meckel's diverticulum, but ileocolic intussusceptions from an inverted Meckel's diverticulum are rare. We describe here a rare case of a 33-year-old male with no past medical or surgical history presenting with an acute abdomen and imaging findings of ileocolic intussusception with bow

Determinants of ultrasound-guided reduction failure and pathological lead points in pediatric intussusception.

PURPOSE: Intussusception is a leading cause of acute intestinal obstruction in children. Ultrasound-guided hydrostatic saline enema (USGSE) is widely accepted as first-line non-surgical management, but predictors of reduction failure and pathological lead points (PLPs) are not well defined. METHODS: We retrospectively reviewed pediatric patients (< 18 years) with ileocolic intussusception treated between 2012 and 2022. Clinical variables included symptom duration, vomiting, bloody stools, and age. Univariable and multivariable logistic regression analyses were used to identify predictors of f

Case Report: Complicated Meckel's diverticulum masquerading as gastroenteritis: a near-fatal presentation in a toddler.

BACKGROUND: Meckel's diverticulum poses diagnostic difficulties due to its nonspecific symptoms, which often resemble benign conditions like gastroenteritis. CASE PRESENTATION: A 2-year-11-month-old girl presented with abdominal pain and vomiting, initially diagnosed as acute gastroenteritis. Within 12 h, her condition deteriorated rapidly with fever, lethargy, and circulatory shock. Laboratory findings included marked leukocytosis (36.05 × 10⁹/L), severe metabolic acidosis (pH 7.32, HCO₃⁻ 5.2 mmol/L), and elevated lactate (3.3 mmol/L). Emergency abdominal CT showed small bowel obstruction wi

Enterolith-containing Meckel's diverticulum causing strangulated small bowel obstruction in an older adult: diagnostic implications of transition-point calcification on computed tomography.

INTRODUCTION: Preoperative diagnosis of Meckel's diverticulum in adults remains challenging because clinical presentation and computed tomography findings are often nonspecific. Enterolith-containing Meckel's diverticulum may present as strangulated small bowel obstruction with calcified lesions near the transition point. CASE PRESENTATION: An 83-year-old woman without prior abdominal surgery presented with abdominal pain, vomiting, and progressive malaise. Non-contrast computed tomography showed closed-loop small bowel obstruction with two adjacent high-density lesions near the pelvic transi

Neonatal and early infant intussusception: a 24-year retrospective review from a tertiary referral center.

BACKGROUND: Intussusception in neonates and young infants is rare, with limited data to guide management. OBJECTIVE: This study aims to characterize the presentation, management, and outcomes of intussusception in infants ≤100 days of age. MATERIALS AND METHODS: Radiology reports were searched for the keyword "intussusception" in infants ≤100 days over a 24-year period (January 1, 2000-July 31, 2024). Patient charts were reviewed, and cases with negative imaging studies were excluded. Data were analyzed for patient demographics, presenting symptoms, imaging, and management approach, includin

Failed air enema reduction leading to the diagnosis of small-bowel intussusception secondary to Meckel's diverticulum with ectopic gastric mucosa in a 5-month-old infant: A case report.

A 5-month-old infant presented with paroxysmal crying and vomiting for more than 4 h. Ultrasonography confirmed intussusception in the right abdomen, but intraluminal gas limited precise classification. On the assumption of ileocolic intussusception, air enema reduction was attempted but was unsuccessful. Due to the persistence of symptoms, low-dose computed tomography was performed to further characterize the intussusception subtype. The scan demonstrated concentric ring and pseudokidney-like signs within a segment of the ileum, strongly suggesting small-bowel intussusception. Exploratory lap

Small Bowel Obstruction Caused by Meckel's Diverticulum Entrapped behind Transperitoneal Sigmoid Colostomy: A Case Report.

INTRODUCTION: Meckel's diverticulum is the most common congenital anomaly of the gastrointestinal tract, often asymptomatic but occasionally causing inflammation, bleeding, or intestinal obstruction. Permanent colostomy after abdominoperineal resection (APR) is typically created via either a transperitoneal or extraperitoneal route. Although small bowel obstruction (SBO) is a known complication of transperitoneal colostomy, SBO caused by Meckel's diverticulum entrapped behind the stoma has not previously been reported. We describe an extremely rare case of SBO caused by adhesion between Meckel

Mesenteric Meckel's Diverticulum as a Missed Diagnosis in a Child With Recurrent Rectal Bleeding: A Report of a Challenging Case.

Meckel's diverticulum is a common congenital anomaly of the gastrointestinal tract and is a remnant of the omphalomesenteric duct. One of the classical identification and diagnostic criteria of Meckel's diverticulum is its location along the anti-mesenteric border of the ileum. Mesenteric Meckel's diverticulum (MMD) is an unexpected occurrence, rarely reported, with confounding diagnostic tests due to the atypical location, often forming a gray zone in diagnosis due to its similarity to an enteric duplication cyst. A 1.5-year-old male child presented to the pediatric surgery outpatient departm

Meckel's Diverticulum as a Rare Etiology of Small-Bowel Obstruction in an Otherwise Healthy Adult: A Case Report.

Meckel's diverticulum is the most common congenital anomaly of the gastrointestinal tract and is often asymptomatic in adults. When symptomatic, adult presentations frequently involve obstruction or inflammation and can be difficult to diagnose preoperatively due to nonspecific imaging findings. We report a case of a healthy 31-year-old male with progressive abdominal pain, distention, and emesis, ultimately found to have a high-grade small-bowel obstruction refractory to conservative management. Diagnostic laparoscopy identified a Meckel's diverticulum tethered to the retroperitoneum at the t

Imaging of ectopic gastric mucosa in Meckel's diverticulum: A case report and literature review.

Meckel's diverticulum (MD) is usually asymptomatic, with the most common presentation being painless gastrointestinal bleeding in children under the age of 2 years. We report a case of a 10-year-old male who presented with painless hematochezia and melena, accompanied by several months of intermittent abdominal pain, nausea, vomiting, and constipation. Initial evaluation and imaging were nondiagnostic for MD. However, technetium-99m pertechnetate scintigraphy established the diagnosis of MD, which was confirmed surgically.
