Management of incidentally diagnosed Meckel's diverticulum in pediatric patients: a questionnaire survey and a literature review.

INTRODUCTION: The presence of Meckel's diverticulum is an indication for its surgical removal. However, the procedure for treating incidentally found Meckel's diverticulum is controversial. Is its resection tangential to the intestinal wall using, for example, a surgical stapler sufficient? Should wedge/segmental intestinal resection with end-to-end anastomosis be performed? Is primary diverticulectomy always safe, or should the procedure be postponed and classified as an elective? AIM: The objective of the study was to assess the management of incidentally found Meckel's diverticulum based o

ACR Appropriateness Criteria® Gastrointestinal Bleeding-Child.

Gastrointestinal (GI) bleeding is a relatively common presenting complaint in pediatric practices and may arise from either the upper or lower GI tract. Many common causes of GI bleed are benign and self-limited, often requiring only supportive care with no imaging necessary. When indicated, the goal of imaging is to diagnose and localize the source of bleeding to help guide timely, targeted interventions. Abdominal radiography is often the initial imaging study obtained for patients presenting with GI bleeding. Although it is usually unable to precisely localize the site of bleeding, it may p

Rectointercostal Fascial Plane Block With or Without Modified Thoracoabdominal Plane Block for Postoperative Analgesia in Pediatric Abdominal Surgery: A Two-Case Report.

Postoperative pain after pediatric abdominal surgery includes both somatic and visceral components, making opioid-sparing strategies desirable. We report two pediatric cases in which ultrasound-guided rectointercostal fascial plane block was used alone or combined with modified thoracoabdominal nerves block through perichondrial approach (M-TAPA) as part of multimodal analgesia. In a 9-year-old undergoing laparoscopic Meckel diverticulum excision, a bilateral rectointercostal block provided sustained low FLACC (Face, Legs, Activity, Cry, Consolability) scores with no opioid requirement. In a 5

Clinical characteristics and predictors for the presence of pediatric intussusception secondary to pathological lead points.

PURPOSE: To analyze the clinical characteristics and identify independent predictors of intussusception secondary to pathological lead points (PLPs). METHODS: We conducted a retrospective study of children surgically treated for intussusception between 2010 and 2025. Patients were categorized into PLPs and non-PLPs groups. Demographic, clinical, laboratory, imaging, surgical, and pathological data were compared between the groups. RESULTS: Of 575 children included, 126 (21.9%) had PLPs and 449 (78.1%) were idiopathic. PLPs encompassed a range of etiologies, including Meckel's diverticulum, i

Necrotic Ileal Diverticulum With Features Mimicking Meckel's Diverticulum: A Diagnostic Challenge.

Small bowel diverticulosis is an uncommon condition that most frequently involves the duodenum and jejunum. In contrast, ileal diverticula are rarely encountered and may present a diagnostic challenge, particularly in the setting of an acute abdomen. We report the case of a 60-year-old male with no prior surgical history who presented with a 48-hour history of abdominal pain, nausea, vomiting, and obstipation. Contrast-enhanced CT revealed dilated small bowel loops with a transition point in the ileum, raising concern for small bowel obstruction. The patient underwent emergency exploratory lap

Management of a mixed Littre hernia.

A hernia containing a Meckel's diverticulum is termed a Littre hernia, with the mixed subtype describing the presence of additional abdominal viscera. Mixed Littre hernias are rare and often identified intra-operatively. We report the case of a man in his 60s undergoing a right inguinoscrotal hernia repair who was found to have a 10 cm Meckel's diverticulum with a loop of ileum within the hernia sac. The diverticulum appeared normal and was managed non-operatively with no complications at 6 months. Management of an incidental Meckel's diverticulum remains controversial, divided between non-ope

Failed ultrasound-guided hydrostatic reduction in pediatric intussusception: surgical-pathological characterization of failure phenotypes under a standardized protocol.

BACKGROUND: Ultrasound-guided hydrostatic reduction (UGHR) represents the first-line treatment for pediatric intussusception. However, treatment failure remains a persistent clinical challenge, driven largely by unrecognized pathological lead points (PLPs) before intervention. Large-cohort studies with complete surgical and pathological verification of all failed cases under a unified protocol remain scarce, leaving the true etiology and clinical phenotype of UGHR failure incompletely defined. PURPOSE: To define the distinct clinicopathological phenotype and surgical findings associated with

Infraumbilical Abscess Due to Meckel's Diverticulum Co-occurring With a Urachal Remnant: A Case Report.

Meckel's diverticulum is the most common congenital anomaly of the gastrointestinal tract, and urachal remnants, another congenital anomaly, arise from the incomplete involution of the fetal urachus. Although both conditions have been studied extensively, their co-occurrence is rare and may complicate the diagnosis, particularly if an infection is present. A 22-month-old male patient presenting with abdominal pain was found to have an infraumbilical abscess in the preperitoneal space. Initial imaging studies demonstrated a urachal abscess extending toward the bladder. Following antibiotic ther

An 18-Year-Old Man with Bleeding From a Meckel's Diverticulum of the Terminal Ileum Diagnosed by 99mTc-Pertechnetate Scintigraphy (Meckel's Scan).

BACKGROUND Meckel's diverticulum represents the most common congenital anomaly of the gastrointestinal tract in the pediatric population. Although it is rarely symptomatic, painless rectal bleeding is the most common manifestation among pediatric patients. In contrast, Meckel's diverticulum is considerably rare in adults, in whom intestinal obstruction and diverticulitis are more frequent complications than bleeding. When it is suspected, a Meckel's scan is the most sensitive and specific diagnostic tool. Surgical excision remains the treatment of choice for Meckel's diverticulum. This report

Two Contrasting Scintigraphy-Negative Pediatric Meckel's Diverticula Identified and Localized by Intestinal Ultrasound and Double-Balloon Enteroscopy: A Report of Two Cases.

Scintigraphy-negative Meckel's diverticulum (MD) is diagnostically challenging in children, particularly when the underlying pathology differs between cases. Technetium-99m pertechnetate scintigraphy is widely used for diagnosis; however, false-negative results may occur when ectopic gastric mucosa is absent or when the diverticulum is inverted. We report two pediatric cases with contrasting scintigraphy-negative MD in which intestinal ultrasound (IUS) suggested target lesions, and double balloon enteroscopy (DBE) enabled direct diagnosis and preoperative localization. Case 1 was a 13-year-old

Perforated Meckel's Diverticulum in a 22-Year-Old Male: A Case Report.

Meckel's diverticulum (MD), the most common congenital anomaly of the gastrointestinal tract, results from incomplete regression of the vitelline duct. Although often asymptomatic, complications occur in 4%-7% of cases, with perforation being a rare but life-threatening presentation that may mimic other acute abdominal emergencies. A 22-year-old Pakistani male was discovered with progressive abdominal pain, high-grade fever, vomiting, and diarrhea. Clinical findings and radiographic evidence of pneumoperitoneum suggested enteric perforation. An exploratory laparotomy incidentally revealed a pe

Stepwise diagnosis and minimally invasive management of obscure gastrointestinal bleeding secondary to a Meckel's diverticulum: a case report.

BACKGROUND: Obscure gastrointestinal bleeding (OGIB) presents a significant diagnostic challenge for clinicians due to the anatomical characteristics and inaccessibility of the small intestine. Preoperative diagnosis is particularly challenging, and optimal management relies heavily on precise lesion localization. We report the successful diagnosis and minimally invasive resection of a bleeding Meckel's diverticulum using a multidisciplinary stepwise approach. CASE PRESENTATION: A 25-year-old Chinese man presented to the emergency department with melena that rapidly progressed to recurrent he

Clinicopathologic spectrum and outcomes analysis of Meckel's diverticulum in children.

Meckel's diverticulum is the most common congenital anomaly of the gastrointestinal system. Although often asymptomatic, it can present with a wide range of complications in children. Early diagnosis remains challenging due to nonspecific symptoms and the limited sensitivity of imaging modalities.

Omphalocele with Patent Omphalomesenteric Duct and Intestinal Prolapse in a Preterm Neonate: Histopathological Evidence of Incomplete Vitelline Duct Involution.

Patent omphalomesenteric duct (OMD) with intestinal prolapse coexisting with omphalocele is an exceptional ventral folding anomaly that may be misdiagnosed as gastroschisis. The vitellointestinal duct normally involutes between the seventh and ninth gestational weeks; failure of obliteration produces a spectrum of anomalies including Meckel diverticulum, fibrous bands, umbilico-ileal fistulae, or, rarely, a fully patent duct with bowel prolapse. Fewer than 15 cases of the combined anomaly have been reported since 1967. We describe a preterm neonate (32 + 4 weeks, 2160 g) with an intact omphalo

Pathological lead points in pediatric secondary intussusception: A single-center retrospective observational study of five common etiologies.

While most pediatric intussusceptions are idiopathic, secondary cases differ significantly in clinical presentation, treatment strategies, and prognosis due to the presence of pathological lead points (PLPs). Accurate PLP identification is crucial for optimizing diagnosis and treatment. This study aims to compare clinical and imaging features across PLP types to improve recognition and intervention efficiency for secondary intussusception. This study investigates common PLPs in pediatric secondary intussusception and compare their epidemiological characteristics, clinical manifestations, imagi

A fish out of the Meckel diverticulum, foreign body bowel perforation: a case report.

Meckel's diverticulum is the most common congenital abnormality of the gastrointestinal tract, caused by incomplete obliteration of the omphalomesenteric duct during the seventh week of gestation, and occurs in up to 4% of the general population. Most patients are asymptomatic; however, in a few cases, patients present with gastrointestinal bleeding and intestinal obstruction. Perforation of the diverticulum due to a foreign body is a very rare complication since most ingested foreign bodies may pass through the gastrointestinal tract without consequences. We present the unusual case of a 73-y
