Surgical repair of prolapsed stoma via the buttonpexy approach: a case series.

Stoma prolapse is a frequent complication following stoma formation in pediatric patients and often necessitates surgical intervention. The buttonpexy technique offers a minimally invasive, low-cost, and anesthesia-sparing alternative to formal stoma revision. This case series highlights its safety and effectiveness in managing pediatric stoma prolapse. Five pediatric patients with stoma prolapse secondary to various underlying conditions, including anorectal malformation, Hirschsprung's disease, and Currarino syndrome, were managed using the buttonpexy technique. In each case, the prolapsed b

Regulation of PHOX2B gene expression by the long non-coding natural antisense RNA PHOX2B-AS1.

Paired mesoderm homeobox protein 2B (PHOX2B) is a transcription factor essential for autonomic nervous system development. Heterozygous mutations in the PHOX2B gene are associated with neurodevelopmental disorders, including congenital central hypoventilation syndrome and Hirschsprung's disease. Additionally, PHOX2B plays a role in the genetic landscape of neuroblastoma, with mutations detected in both familial and sporadic forms of this rare cancer. Notably, PHOX2B is highly expressed in most neuroblastoma cells. Despite its significance, little is known about the regulation of PHOX2B gene ex

Post-Surgical Gut Microbiota Alterations in Pediatric Patients with Intestinal Disorders.

This detailed narrative review focuses on the current understanding of unique alterations in GM colonization and subsequent complications following surgery for significant childhood conditions, such as necrotizing enterocolitis (NEC), Hirschsprung's disease (HD), inflammatory bowel disease (IBD), and short bowel syndrome (SBS). Surgical interventions can alter the diversity and structure of the GM and potentially cause post-surgical complications. Although the data are well-established in adults, there is a lack of pediatric-specific data on post-surgical GM dysbiosis and its complications, in

ETB receptor deficiency amplifies allergic airway inflammation and hyperresponsiveness.

BACKGROUND: Endothelin-1 (ET-1) is a proinflammatory mediator that plays a crucial role in regulating airway tone by activating G protein-coupled endothelin receptors A (ETA) and B (ETB). The endothelin system has been linked to asthma, but the impact of ETB receptor deficiency on allergic airway inflammation remains uncharted. This study explores how the endothelin system influences allergic airway inflammation and hyperresponsiveness. METHODS: We used rescued ETB receptor-deficient (ETB -/-) mice to obviate lethal inherited Hirschsprung disease, prepro-ET-1 overexpressing (preETtg), and wil

Predictors of successful transitions in a bowel management program.

BACKGROUND: Bowel management programs (BMPs) have demonstrated effectiveness in treating severe constipation and encopresis. We aimed to evaluate the transition rate from more invasive to less invasive treatments (e.g. enema to laxative therapy) and identify clinical characteristics that increase likelihood of downgrading treatment over time. METHODS: Patients participating in our institution's BMP (2020-2024) were identified. Those without adequate follow-up after BMP induction were excluded. Follow-up was collected at 6-, 12-, 18-, and 24-month clinical visits. 'Downgrade' was defined as a

Sod1 trisomy causes ENS developmental defects and susceptibility to Hirschsprung disease via neuronal Ret suppression and glial remodeling.

Down syndrome (DS; trisomy 21) confers a ~100-fold increased risk of Hirschsprung disease (HSCR), yet the causal contributions of specific chromosome 21 genes remain unresolved. Here we show that increased dosage of SOD1 alone is sufficient to perturb enteric nervous system (ENS) development. We engineered a humanized SOD1 trisomic mouse line by inserting a 28 kb human SOD1 locus into ROSA26 and genetically profiled the distal colon at postnatal day 0 using single-cell RNA-seq and immunofluorescence. Sod1/SOD1 was elevated ~1.5X in the ENS but varied by cell type, with transcriptionally active

Anorectal changes and clinical outcomes after the Duhamel operation.

BACKGROUND: Surgery for Hirschsprung's disease (HD) generally results in a satisfactory outcome, but some patients continue to have bowel dysfunction. There are very few studies in the literature that compare the early and late clinical outcomes. AIM: To evaluate changes in anorectal function and clinical outcome with age in patients who underwent Duhamel operation (DO) for HD. METHODS: Twenty-eight patients who had previously undergone DO to treat HD were invited to an outpatient clinic upon reaching adulthood to participate in follow-ups. The patients' clinical data from the early and mid-

DNER as a novel protein contributes to HSCR pathogenesis: multi-omics combined Mendelian randomization analysis.

BACKGROUND: Hirschsprung disease (HSCR) is a congenital disorder characterized by the aganglionosis, which remains disputable and limited biomarkers about pathogenesis. METHODS: This study employed a multidimensional approach to explore the potential new targets and molecular mechanism for HSCR, including proteome-wide Mendelian randomization (MR) analysis, single-cell RNA sequencing (scRNA-seq), bioinformatics, and experimental validation through cellular and tissue studies. RESULTS: Our results confirmed RET and AGRN as key proteins implicated in HSCR genesis and development, further under

Surgical Complications in Hirschsprung Disease and the Impact of Botulinum Toxin Injection on Hirschsprung-Associated Enterocolitis.

Background: Hirschsprung disease (HD) is associated with substantial postoperative morbidity, particularly due to Hirschsprung-associated enterocolitis (HAEC), despite definitive surgical treatment. Postoperative outcomes may vary according to the length of the aganglionic segment and the surgical technique used, and optimal management of recurrent HAEC remains a significant clinical challenge. Methods: The medical records of patients who underwent corrective surgery for HD between 2011 and 2023 were retrospectively reviewed. Demographic characteristics, disease segment length, surgical techni

Neurotrophin-3 rs1805149A>G variant in Hirschsprung disease: An investigative study.

BACKGROUND: Hirschsprung disease (HSCR) is a congenital disorder of the enteric nervous system (ENS) caused by defective migration of neural crest cells. Genetic factors, including neurotrophic genes such as neurotrophin-3 (NTF3), may contribute to its pathogenesis. AIM: To investigate the association between the NTF3 rs1805149A>G variant and susceptibility to HSCR in a southern Chinese Han population. In addition, this study also aims to provide population-specific genetic data on HSCR and to explore whether this neurotrophin-related variant contributes to disease pathogenesis, potentially b

A Novel Rapid 3D Tissue-Clearing and Staining Approach for Enteric Neurovascular Imaging and Pathology Applications.

Background and Aims: Neurovascular abnormalities, such as aberrant nerve migration in Hirschsprung's disease and reduced vascular density in necrotizing enterocolitis, are frequently observed in intestinal diseases. Traditional 2-dimensional (2D) staining methods are complicated, time-consuming and fail to comprehensively visualize the intricate neurovascular structures and morphology of the intestine. This study focuses on evaluating a novel 3D staining technique that promises simpler, faster, and more effective visualization of intact neurovascular structures in the colon. Additionally, it a

Phenotypic Expansion and Molecular Implications in Recessive FUZ -Related Ciliopathy.

FUZ, a component of the CPLANE (ciliogenesis and planar polarity effector) complex, regulates primary ciliogenesis. Five patients of various types of skeletal dysplasia with biallelic FUZ variants have been reported to date, yet the gene-disease relationship has not been established. Here, we report a patient with ciliopathy with a novel homozygous missense variant in FUZ. This patient shares phenotypes with all the previously reported patients and presents with novel phenotypes: aorto-pulmonary window (AP window) and Hirschsprung disease. These phenotypes can be explained by the inhibition of
