Predictive role of ultrasonography in the early diagnosis of Hirschsprung's disease in neonates and infants: an observational diagnostic study.

PURPOSE: We aimed to evaluate the diagnostic performance of conventional transabdominal ultrasound (US) in detecting Hirschsprung's disease (HD). MATERIALS AND METHODS: This cross-sectional observational study enrolled infants (≤ six months) with clinical suspicion of HD. They subsequently underwent a standardized transabdominal US, followed by a confirmatory rectal biopsy. US parameters included the presence and location of a transition zone (TZ), proximal colonic dilation, upstream and downstream bowel lumen diameter and wall thickness, and luminal diameter ratio (LDR). Diagnostic performan

Optimized chemogenetic ablation and regeneration of enteric nervous system neurons in zebrafish.

The enteric nervous system (ENS) is the intrinsic nervous system of the gut and regulates essential functions, including motility. ENS loss is associated with gastrointestinal disorders such as Hirschsprung disease. Restoring missing neurons by stimulating neuronal regeneration presents a promising therapeutic approach for ENS disorders. To study ENS regeneration, we developed a targeted chemogenetic neuron ablation model in zebrafish using the Gal4/UAS-NTR 2.0 system. Spatially and temporally controlled neuronal death was confirmed by morphological changes, neuron loss quantification, and TUN

Mapping and Characterization of Isolated Lymphoid Follicles in Patients with Hirschsprung-Associated Enterocolitis.

Hirschsprung-associated enterocolitis (HAEC) is a severe complication of Hirschsprung's disease (HSCR), but its pathogenesis remains unclear. Gut-associated lymphoid tissue (GALT), including isolated lymphoid follicles (ILFs), plays a key role in mucosal immunity. This study investigated the presence, distribution, and phenotype of ILFs in colonic tissue from 35 HSCR patients and 12 controls. Histological and immunohistochemical analyses revealed mature ILFs in both ganglionic and aganglionic bowel, with no significant differences in densities between HSCR patients with and without HAEC. Howev

Thymic Stromal Lymphopoietin-Driven B-Cell Hyperactivation Impairs Intestinal Epithelial Homeostasis Predisposing to Enterocolitis in Hirschsprung Disease.

BACKGROUND & AIMS: Hirschsprung-associated enterocolitis, a lethal complication of Hirschsprung's disease-a prevalent congenital disorder with high morbidity-has a poorly understood pathogenesis. We investigated how aberrant immune-stromal crosstalk drives mucosal injury in Hirschsprung's disease. METHODS: Integrated single-cell RNA sequencing, spatial transcriptomics, bulk RNA sequencing, and proteomics were performed using human Hirschsprung's disease tissues. Validation was conducted using murine Hirschsprung-associated enterocolitis (Ednrb-/-) models, flow cytometry, multiplex immunohisto

BxbI-mediated insertion of a 77kb human RET sensitive haplotype into the mouse genome to generate a humanized model of Hirschsprung disease.

Hirschsprung disease (HSCR) is a complex developmental disorder of the enteric nervous system, primarily driven by regulatory variants within enhancer elements of the RET gene. To investigate how these variants lead to aganglionosis, we developed a humanized mouse model by inserting an intact 77kb human RET genomic locus into the Rosa26 safe-harbor locus. Utilizing "big DNA" synthetic biology and Bxb1-mediated recombination, we integrated the complete human locus including all exons, introns, and upstream regulatory elements which we validated via nanopore and short-read sequencing. Functional

Extracellular vesicles in perinatal conditions: A minimally invasive approach to regenerative medicine.

Extracellular vesicles (EVs) are lipid bilayer-delimited nanoparticles released by cells to act as mediators of intercellular communication during organ development, injury, and repair. EVs carry cargo (bioactive proteins, lipids, and nucleic acids) that reflects the status of the parent cell and is transferred to recipient cells to regulate biological processes, such as inflammation, immune responses, and tissue regeneration. These properties have made EVs promising tools for investigating disease pathogenesis, improving diagnostic and prognostic accuracy, and developing cell-free regenerativ

An Innovative and Alternative Technique in Total Colonic Hirschsprung Disease (TCHD) Treatment, Two Case Reports.

With reported occurrences ranging from 1% to 20%, total colonic Hirschsprung disease (TCHD) accounts for about 8% of Hirschsprung disease cases. The selection and sequencing of the surgical approach, access to parenteral nutrition, and collaborative care between surgeons, gastroenterologists, and pathologists are all critical factors in achieving optimal long-term outcomes. Currently, there is no consensus on the best way to treat TCHD patients. Despite the accompanying morbidity and mortality, standardization of care is required.

TET Enzymes as Epigenetic Integrators in Intestinal Immunity, Inflammation, and Disease.

DNA methylation plays a fundamental role in maintaining intestinal homeostasis, immune tolerance, and inflammatory balance. Active DNA demethylation, mediated by the ten-eleven translocation family of dioxygenases (TET1, TET2, and TET3), has emerged as an important epigenetic mechanism linking environmental and metabolic cues to gene regulatory programs in the gut. In the intestinal epithelium, TET-dependent DNA hydroxymethylation contributes to intestinal stem cell maintenance, epithelial differentiation, regeneration, and barrier integrity. Perturbations in TET activity are associated with e

Objective: Minimally invasive surgery in Hirschsprung disease (HSCR) management was introduced in the mid-1990s. Despite decades of clinical application of various laparoscopic approaches, there remains a paucity of high-powered prospective studies and comprehensive systematic reviews in the literature. This study aimed to systematically review and summarize published techniques and outcomes of laparoscopic- and robotic-assisted surgery in HSCR. Methods: A systematic literature review was conducted using PubMed and the Cochrane Library. Studies reporting technical and outcome data of laparosco

Case Report: Identification of a de novo missense variant in the N-terminal zinc-finger domain of ZEB2 in a patient presenting with neurodevelopmental delay and recurrent pulmonary infections.

BACKGROUND: Heterozygous variants in the ZEB2 gene are known to cause Mowat-Wilson syndrome (MWS). The classical clinical spectrum of MWS includes characteristic facial features, intellectual disability, epilepsy, Hirschsprung disease (HSCR), and various congenital malformations. Reported pathogenic variants have predominantly been truncating variants or missense variants involving the C-terminal zinc-finger domain. To date, no disease-causing missense variant affecting the N-terminal zinc-finger domain has been documented. CASE PRESENTATION: We report a 3-year-old boy presenting with charact

Precision minimally invasive strategy for Hirschsprung's disease: from anatomical resection to whole-cycle functional management-a review.

BACKGROUND: Minimally invasive radical surgery for Hirschsprung's disease (HD) has achieved substantial advances in anatomical accuracy and perioperative safety, yet postoperative defecation dysfunction remains a key factor limiting long-term quality of life in pediatric patients. Existing reviews seldom elaborate the integrated whole-cycle functional management paradigm. OBJECTIVE: To summarize available study on precision minimally invasive strategies for HD and propose a whole-cycle functional management framework. METHODS: This review summarizes information from major databases, includin

The Impact of Delayed Diagnosis on Outcomes in Hirschsprung Disease: A Comparative Analysis.

INTRODUCTION: Hirschsprung disease (HD) is a congenital disease characterized by a lack of neural innervation of the distal bowel. This causes fecal stasis, which results in a wide variety of non-specific fecal obstructive symptoms. Identification of patients with HD can, therefore, be challenging, causing a delayed diagnosis. This may lead to complications such as Hirschsprung-associated enterocolitis (HAEC) and bowel perforation. Current evidence regarding the short postoperative and long-term outcomes in patients with delayed diagnosis is inconclusive. Therefore, the aims of this study were

Indirect evidence for the volume-outcome relationship for corrective surgery for anorectal malformations using the IQWIG V24-07 rapid report methodology.

PURPOSE: Anorectal malformations (ARM) are rare congenital anomalies requiring complex surgical correction. Due to low caseload and heterogeneous cohorts, prior studies failed to establish whether higher hospital caseloads improve outcomes. This study applies the IQWiG V24-07 framework to evaluate whether indirect evidence from other surgical procedures can inform a potential volume-outcome relationship in ARM surgery. METHODS: An interdisciplinary expert panel conducted a formal consensus using three steps: (1) identifying suitable source populations; (2) systematically comparing these with
