Autoinflammation with infantile enterocolitis induced by a heterozygous variant (c.1357C > T) in the NLRC4 gene: a case report.

BACKGROUND: Necrotizing enterocolitis (NEC) is s a life-threatening inflammatory intestinal disorder primarily affecting preterm infants, though rare in full-term neonates. NLR-family CARD domain-containing protein 4 (NLRC4), a cytosolic inflammasome component driving IL-1β-mediated inflammation, is linked to autoinflammatory syndromes via germline gain-of-function mutations, though its role in term infant NEC remains underexplored. CASE PRESENTATION: A full-term male infant developed NEC shortly after birth, requiring emergency surgical interventions including ileostomy and intestinal resect

Mid-ureteric Ectasia Masquerading as Hirschsprung Disease: A Rare Cause of Infantile Large Bowel Obstruction.

Mid-ureteric dilatation with stenosis is extremely rare in children. We discuss a case of a three-month-old infant presenting with a large bowel obstruction and a left-sided abdominal lump. While the large bowel was grossly normal on exploration, a cystic dilatation in the middle of the ureter was causing colonic compression, with the proximal and distal ureteric segments being normal in diameter and exhibiting normal peristaltic activity. Thus, an unusual segmental cystic dilatation of the ureter with stenosis was noted. Excision of the dilated segment of the ureter and end-to-end ureteric an

Biofeedback Therapy for Incontinence in a Post-Duhamel Hirschsprung Patient: A Case Report.

Fecal incontinence is a recognized but underreported postoperative complication in patients with Hirschsprung disease (HD) Retentive fecal incontinence may result from stool retention in the Duhamel pouch, altered rectal sensation, or dyssynergic defecation. We report the use of biofeedback therapy in a 17-year-old male with a history of HD and total colonic aganglionosis (TCA), who underwent a Duhamel pull-through in infancy and later developed recurrent fecal soiling and constipation. Clinical examination demonstrated stool retention with preserved perianal sensation and sphincter tone, cons

Adult Hirschsprung disease: salvage to total colectomy - a rare case report.

A 17-year-old male with chronic constipation presented with acute obstipation and abdominal distension. Computed tomography showed a dilated colon with rectosigmoid transition. Initial laparotomy and segmental resection confirmed Hirschsprung disease (HD) on histopathology. One month later, he re-presented with abdominal distension. Urgent total colectomy and ileostomy were performed. He recovered after ICU care and was discharged with a functioning ileostomy. This case underscores complications of incomplete resection in adult HD and the need for vigilant follow-up.

Survival in congenital central hypoventilation syndrome (CCHS): data from the European CCHS Consortium.

INTRODUCTION: Congenital central hypoventilation syndrome (CCHS) is a rare disorder characterised by autonomic dysfunction and impaired respiratory control. While advances in ventilatory support have improved survival, risk factors for mortality remain incompletely understood. Our objective was to investigate the impact of genetic and clinical parameters on mortality in children and young adults with genetically confirmed CCHS. METHODS: Data were collected from the European CCHS Consortium registry, encompassing patient information collected from 2012 to 2021. This dataset included informatio

Aberrant Expressions of EDNRB and EDN3 in a Multifactorial Hirschsprung Disease.

BACKGROUND: Hirschsprung disease (HSCR) is a multifactorial disorder due to the enteric nervous system (ENS) development failure. At least 35 genes have been responsible for HSCR, including EDNRB and EDN3. Here, we aimed to determine the EDRNB and EDN3 expressions effects in HSCR subjects. METHODS: Our study analyzed EDNRB and EDN3 expressions in the colon of HSCR subjects and controls by a quantitative PCR. The EDNRB and EDN3 expressions were analyzed by the Livak method (2-ΔΔCT). RESULTS: Twenty-seven HSCR patients and 20 controls were ascertained. EDNRB and EDN3 expressions downregulated

International Congenital Central Hypoventilation Syndrome (CCHS) Registry: Analysis of Patient-Reported Symptoms by PHOX2B Variant.

INTRODUCTION: CCHS is a rare disorder caused by PHOX2B gene variants. While CCHS hallmarks are hypoventilation and respiratory control dysfunction necessitating lifelong artificial ventilatory support, affected individuals also experience widespread, but less studied, autonomic nervous system (ANS) dysregulation. PHOX2B variants may be divided into moderate and severe groups based upon molecular and in silico data. The International CCHS Registry is a secure repository for longitudinal data from individuals with a PHOX2B variant-confirmed diagnosis, including information on seven systems serve

FDA-approved RET protein-tyrosine kinase inhibitors in the management of RET-driven thyroid and lung cancer.

RET is a transmembrane receptor protein-tyrosine kinase that is required for the (i) survival and maturation of the enteric nervous system, the autonomic nervous system, and sensory neurons, (ii) renal development, and (iii) spermatogenesis. RET activation by its glial-cell derived neurotrophic factor (GDNF) ligands differs from that of all other receptor protein-tyrosine kinases because of the requirement for additional GDNF family receptor-α co-receptors (GFRα1/2/3/4, GFRAL). Activating RET-point mutations occur in multiple endocrine neoplasia syndromes (MEN2A, MEN2B) and in isolated medulla

Down-regulation of Laminin and its Correlated Significance to Interstitial Cells of Cajal in Hirschsprung's Disease.

INTRODUCTION: Hirschsprung's Disease (HSCR) is characterized by aganglionosis in the distal gut, but the role of Extracellular Matrix (ECM) components in its pathogenesis remains unclear. This study investigated the relationship between laminin, a key ECM protein, and Interstitial Cells of Cajal (ICC) in HSCR. METHODS: Immunofluorescence staining was used to analyze the expression and localization of laminin and ICC in paraffin-embedded colon sections from HSCR patients. Whole-mount preparations and confocal microscopy were employed to visualize the ICC network. Laminin and c-Kit expression l
