PHOX2B polyalanine repeat mutation alters the transcriptome of neuronal progenitor cells in congenital central hypoventilation syndrome.

Mutation in paired-like homeobox 2B (PHOX2B) is used as the diagnostic marker of Congenital Central Hypoventilation Syndrome (CCHS). The mutant gene/protein affects neural crest cells embryonic development which leads to congenital central hypoventilation syndrome (CCHS). When individuals also have Hirschsprung's disease (HSCR) with CCHS it is known as Haddad syndrome (HS). Previous studies on CCHS/HS have mainly focused on the conformational dynamics of the mutant protein and have remained controversial. Here we performed RNA-sequencing on the patient derived neuroepithelial stem cells (NESCs

Outcomes of Transition Zone Pull-Through in Hirschsprung Disease: A Systematic Review.

AIM: Transition zone pull-through (TZPT) can be associated with morbidity and the need for redo pull-through (RDPT), but not in all patients. We aimed to systematically review outcomes following TZPT in patients with Hirschsprung disease (HD). METHODS: A PICO-based systematic search of Embase and Medline was conducted in accordance with PRISMA guidelines. Search terms included variations of: Hirschsprung disease, pull-through (PT), transition zone, and complications. Eligible studies included cohorts of PT with TZ on final histology and reporting functional outcomes, Hirschsprung-associated e

Neonatal Intestinal Perforations: When Should We Perform a Rectal Biopsy to Rule Out Hirschsprung's Disease?

BACKGROUND: Intestinal perforation (IP) is one of the most critical surgical emergencies in neonates. It most often occurs in premature infants, with necrotizing enterocolitis (NEC) as the leading cause. Hirschsprung's disease (HD) is another important etiology. In this study, we aimed to investigate the frequency of HD among neonates with non-NEC IP and assessed the value of performing rectal biopsy in these patients. METHODS: Neonates who were treated for non-NEC IPs between 2005 and 2021 were evaluated retrospectively. Demographic data, clinical features, operative details, and rectal biop

Does surgical approach affect Hirschsprung-associated enterocolitis risk? A comparison between transanal Swenson-like and endorectal pull-throughs.

Hirschsprung-associated enterocolitis (HAEC) is a complication of Hirschsprung disease (HSCR) that may occur both before or after surgery. Transanal endorectal pull-through (TEPT) is one of the standard treatments for HSCR. In addition, transanal Swenson-like pull-through (TSLPT) is a recent technique combining the Swenson method with the posterior sagittal approach. We aimed to compare the incidence of HAEC following TSLPT versus TEPT in HSCR patients and examine their association with predictor factors. We retrospectively reviewed the medical records of HSCR patients who underwent either TSL

Retinol dehydrogenase 10-mediated retinoic acid signaling regulates the neural crest cell microenvironment during enteric nervous system formation.

The enteric nervous system (ENS) is formed from vagal neural crest cells (NCC), which generate most of the neurons and glia that regulate gastrointestinal function. Defects in the migration or differentiation of NCC in the gut can result in gastrointestinal disorders such as Hirschsprung disease (HSCR). Although mutations in many genes have been associated with the etiology of HSCR, a significant proportion of affected individuals have an undetermined genetic diagnosis. Therefore, it's important to identify new genes, modifiers, and environmental factors that regulate ENS development and disea

Xq28 Variants as Novel Male-Specific Susceptibility Factors for Hirschsprung Disease.

INTRODUCTION: Hirschsprung's disease (HSCR) is a complex congenital digestive tract disease affected by multiple genes. Despite the identification of >20 risk loci, no susceptibility variants map to sex chromosomes-a striking gap given the 4- to 6-fold male predominance in short-segment HSCR. This study investigates the contribution of X-chromosomal variation to HSCR pathogenesis. METHODS: We conducted a large case-control study involving a South Chinese population. Four single nucleotide polymorphisms (SNPs) in a disease hotspot region spanning on Xq28 were genotyped through MassARRAY genoty

Safety and feasibility of one-stage neonatal approach for short-segment Hirschsprung's disease.

BACKGROUND: Early definitive surgery for Hirschsprung disease (HD) in neonates is increasingly adopted to reduce preoperative morbidity and preserve long term bowel function. However, comparative data across minimally invasive approaches in neonates with short segment disease remain limited. This study compared outcomes of single incision laparoscopic assisted endorectal pull through (SILEP), conventional laparoscopic assisted endorectal pull through (CLEP), and complete transanal endorectal pull through (TERPT) for rectosigmoid HD. METHODS: We conducted a retrospective cohort study of 55 neo

Dyggve-Melchior-Clausen syndrome in three siblings: a unique case series with dual diagnosis of Down syndrome and Hirschsprung disease.

OBJECTIVES: Dyggve-Melchior-Clausen (DMC) syndrome is a rare autosomal recessive skeletal dysplasia caused by mutations in the DYM gene. It is characterized by progressive spondyloepimetaphyseal dysplasia, short stature, coarse facial features, microcephaly and intellectual disability. While it clinically resembles Morquio syndrome (mucopolysaccharidosis type IV, MPS IV), DMC is distinguished by cognitive impairment, absence of corneal clouding, normal urinary glycosaminoglycans and distinctive radiological features. CASE PRESENTATION: We reported three siblings with DMC syndrome. Two 4-year-

Perlecan, CollagenXVIII, and Agrin Expression in Normo-, Hypo-, and Aganglionic Segments in Hirschsprung's Disease.

BACKGROUND: Secretory heparan sulphate proteoglycans (HSPGs) interact with various morphogens, growth factors, and signaling molecules contributing to the development of the enteric nervous system (ENS). Thus, HSPGs have come into focus as pathomechanistic players of enteric neuropathies, such as Hirschsprung's disease (HSCR) in animal models. However, a detailed description of HSPG expression in human HSCR patients is missing. METHODS: We characterized the expression pattern of the secretory HSPGs perlecan, COL18A1, and agrin in the human ENS and investigated differences in affected and heal

Endothelin-3 and T-type Ca2+ channels drive enteric neural crest cell calcium activity, contractility and migration.

Enteric neural crest cells (ENCCs) colonize the gut during embryogenesis and migration defects give rise to Hirschsprung disease (HD). Mutations in GDNF/RET and EDN3/EDNRB are known to be causal in HD. Here, we show that migrating ENCCs in mice exhibit endogenous EDN3/EDNRB-gated calcium activity, mediated by chloride channels, T-type Ca2+ channels and inositol trisphosphate-sensitive intracellular-store release. We find that inhibiting Ca2+ activity results in ENCC migration defects, while exciting it promotes migration by increasing ENCC contractility and traction force to the extracellular

Artificial intelligence in the diagnosis of Hirschsprung disease: A scoping review and rationale for a multicentric approach.

BACKGROUND: Hirschsprung disease (HD) is a congenital disorder characterized by absence of enteric ganglion cells, leading to functional bowel obstruction. Despite advances in diagnosis, global disparities persist, particularly in low- and middle-income countries (LMICs), where access to pathology remains limited. Artificial intelligence (AI) offers potential to improve diagnostic accessibility through histopathologic analysis. OBJECTIVE: To systematically synthesize evidence on AI-based diagnostic strategies for HD and identify current gaps to guide future multicenter research. METHODS: Fol

Transcription factor ZEB2 is essential for ureteral smooth muscle cell differentiation.

Mowat-Wilson Syndrome (MWS) is an autosomal dominant genetic disorder caused by heterozygous mutations or deletions in the Zinc finger E-box-binding homeobox 2 (ZEB2) gene. Congenital anomalies of the kidney and urinary tract (CAKUT), including hydroureter and hydronephrosis, have been reported in patients with MWS. However, the role of the ZEB2 gene in urinary tract development and the cellular and molecular mechanisms underlying the CAKUT phenotypes in MWS remain unknown. In this study, we examined ZEB2 expression in the developing mouse ureter and generated Zeb2 ureteral mesenchyme-specific

Early functional outcomes of the Swenson ileoanal pull-through for total colonic Hirschsprung disease at a high-volume referral center.

INTRODUCTION: There is no consensus for the optimal surgical approach for total colonic Hirschsprung disease (TCHD) and few studies report functional outcomes for the ileoanal Swenson pull-through (IAPT). METHODS: A single-center retrospective review identified patients with TCHD who underwent IAPT between 1/2020 and12/2023. Complications and functional outcomes were reviewed descriptively. RESULTS: 25 patients (64 % male) underwent IAPT at a median age of 1.6 years (range 0.6, - 15.6; IQR, 0.9-7.0) with a median follow-up of 16 months (range, 1-39; IQR, 3-26). A case of Hirschsprung disease of total intestinal aganglionosis with intestinal dilatation and peristalsis on prenatal ultrasound.

OBJECTIVE: To present a case of total intestinal aganglionosis (TIA), which is the rarest form of Hirschsprung disease (HD) and is difficult to diagnose prenatally owing to nonspecific prenatal ultrasound findings. CASE REPORT: In a 34-year-old primipara with a history of HD (total colonic aganglionosis), intestinal dilatation with intestinal movement like peristalsis was observed in the fetus starting at 36 weeks of gestation. By 38 weeks of gestation, the fetal abdominal circumference had increased, prompting cesarean section delivery. Prenatal ultrasound and postnatal barium enema findings

Diagnostic assessment of Hirschsprung disease using fluorescence confocal microscopy: A feasibility study.

INTRODUCTION: Hirschsprung disease (HD) is a congenital disorder marked by aganglionosis of intestinal nerve plexuses, leading to bowel obstruction. Determining the extent of the aganglionic segment is essential for successful surgical correction. Consequently, the extent of aganglionosis must be examined histologically. Fluorescence confocal microscopy (FCM) may serve as an alternative diagnostic modality. Using a laser to scan the unfixed specimen, it enables rapid intraoperative imaging without causing tissue alteration. This study assessed the feasibility and diagnostic accuracy of FCM in

Familial adult-onset chronic idiopathic megacolon: diagnostic and surgical challenges-a case report.

Chronic idiopathic megacolon is a rare disorder characterized by persistent colonic dilatation without obstruction or secondary causes. It is usually diagnosed in childhood, often linked to Hirschsprung's disease; adult-onset cases are exceedingly rare, especially with familial predisposition. We report a man in his 30s with progressive abdominal distension, prior adolescent colectomy, and family history suggesting genetic susceptibility. After excluding secondary causes, chronic idiopathic megacolon was diagnosed. Conservative management failed, necessitating subtotal colectomy with ileorecta
