Perioperative outcomes in primary neonatal pullthrough versus pullthrough in older children with Hirschsprung disease: a systematic review and meta-analysis.

Primary pullthrough for Hirschsprung disease is the current standard in most uncomplicated standard segment disease. In this review and meta-analysis, we aimed to compare perioperative outcomes and functional outcomes for pullthrough done in the neonatal period compared to that in older age. PubMed, Medline, Embase, CINAHL, SCOPUS, and Web of Science were searched for relevant publications in English from inception to June 29, 2024. Twenty studies were included in the qualitative analysis with a total 3197 patients matching the review criteria. All included studies were observational in nature

Outcomes of robotic-assisted soave pull-through procedure for Hirschsprung disease: a systematic review and meta-analysis.

Pull-through procedures remain the mainstay of treatment for Hirschsprung disease (HD). While laparoscopic-assisted procedures are widely performed, robotic-assisted Soave pull-through (RSPT) has emerged as a minimally invasive alternative with potential technical advantages. We systematically reviewed the available literature to evaluate the perioperative and functional outcomes of RSPT. PubMed, Embase, Scopus, Cochrane CENTRAL, and ClinicalTrials.gov were systematically searched through February 2025. Eligible studies included pediatric (< 18 years) patients undergoing RSPT. Data were pooled

Proximal ganglionic intestine in Hirschsprung Disease is fibrotic and stiff.

Hirschsprung disease (HSCR) is a congenital intestinal disorder characterized by the absence of ganglia in the distal intestine. Despite surgical resection of the aganglionic intestine and pull-through surgery, patients with HSCR still experience bowel dysfunction, indicating that latent abnormalities may also exist in the proximal ganglionic intestine. To elucidate possible causes of postoperative bowel dysfunction in HSCR, we investigated differences in the proximal ganglionic intestine using an animal model of HSCR (Ednrb-null mice) and validated our findings in tissue from patients with HS

Thrombospondin-1-mediated macrophage efferocytosis dysfunction exacerbates intestinal pathology in Hirschsprung's disease.

BACKGROUND: Hirschsprung's disease (HSCR) is a prevalent pediatric congenital enteropathy characterized by disrupted intestinal homeostasis, persistent inflammation, and progressive intestinal fibrosis. However, the cellular and molecular mechanisms underpinning these pathological hallmarks remain incompletely elucidated. Given the pivotal role of macrophages in inflammation resolution and tissue repair, and the established function of thrombospondin-1 (THBS1) in regulating macrophage polarization and efferocytosis, we focused on THBS1 to dissect the mechanisms of HSCR-associated pathology. Indirect Evidence for the Volume-Outcome Relationship in Corrective Surgery for Hirschsprung Disease: Insights from Adult Colorectal Surgery.

Hirschsprung disease (HD) is a rare malformation that requires complex corrective surgery. Low caseload, heterogeneous patient cohorts, and difficulties in linking long-term outcomes to the initial corrective surgery limit the available direct evidence for a volume-outcome relationship. Indirect evidence from adult colorectal cancer (CRC) surgery may be transferable, given the partial similarity of the procedures.Using the Institute for Quality and Efficiency in Health Care (IQWiG) V24-07 rapid report's methodology, this study evaluated the transferability of the volume-outcome relationship fr

Familial cartilage-hair hypoplasia: prenatal ultrasound features and clinical outcomes in three siblings with identical RMRP variants.

Cartilage-hair hypoplasia (CHH) is a rare autosomal recessive skeletal dysplasia. It is often associated with short stature, metaphyseal abnormalities, hair hypoplasia and immune dysfunction. This case describes a pregnant woman in her mid-30s with two previous children diagnosed with CHH with molecular confirmation of two pathogenic variants of the ribonuclease mitochondrial RNA processing gene. The couple is healthy and non-consanguineous. Routine ultrasound examination of the current pregnancy suggested this fetus was also likely to be affected. The couple opted not to perform invasive pren

Skip segment Hirschsprung Disease: Unicentric experience with four cases and systematic literature review.

INTRODUCTION: Skip lesion aganglionosis is characterized by intestinal aganglionosis interrupted by portions of normally ganglionated bowel that is located in between segments of aganglionosis. Even though this is a well-known entity, only scattered cases have been reported so far. The aim of this manuscript is to present our series of patients and perform a systematic literature review, to offer a critical overview of possible management. PATIENTS AND METHODS: We hereby present a case series of 4 patients diagnosed during the last five years in our Centre in accordance to SCARE guidelines. A

Mobile health applications for parents of children with Hirschsprung disease: a requirement analysis.

UNLABELLED: Hirschsprung disease (HD) is a congenital gastrointestinal disorder that occurs due to the absence of nerve cells in segments of the large intestine, resulting in impaired propulsive bowel movements. Postoperative complications and the need for continuous care place parents of affected children under significant educational challenges. Mobile health (mHealth) applications can serve as effective tools to enhance parental education and self-management. The aim of this study was to identify the requirements and functional needs of a mHealth application specifically designed for parent

The PODCAST-question: postoperative dilation versus calibration after surgical treatment of Hirschsprung's disease-what is necessary?

PURPOSE: Postoperative anal dilation after surgery for Hirschsprung disease (HD) still remains common practice. This study aimed to evaluate the benefit of routine dilations compared with a single postoperative calibration. METHODS: Patients who underwent HD surgery within the first year of life at the University Children’s Hospital Zurich or the University Hospital Frankfurt were retrospectively reviewed. Two groups were compared: those with serial postoperative dilations (PD) and those with a single calibration (PC). Primary outcomes included anastomotic stricture, Hirschsprung-associated en

Definitive surgery for Hirschsprung Disease between 3 and 12 months achieves best outcomes: A systematic review with meta-analysis.

INTRODUCTION: Optimal age for definitive pull-through in Hirschsprung disease (HD) is debated. Both early and delayed interventions may influence short- and long-term outcomes, but current guidelines do not provide a clear recommendation. METHODS: A systematic review was conducted following Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) guidelines. Nineteen studies published between 1998 and 2025, including 3980 pediatric patients with HD undergoing definitive pull-through surgery, were included. Age at surgery was stratified using predefined thresholds (neonatal,

Dual roles of GDNF in enteric glial cell plasticity: direct transdifferentiation via the CaMKII/NeuroD1 pathway and cooperative regulation in a neural stem cell-inducing medium.

BACKGROUND: Hirschsprung disease (HSCR) is a congenital condition featuring aganglionosis in the distal colon, causing functional obstruction. While EGF and bFGF are well-characterized neurogenic factors, the precise mechanistic role of GDNF in modulating enteric glial cell plasticity remains incompletely understood. METHODS: EGCs were identified via proteomic profiling and immunofluorescence in Ednrb⁻/⁻ mice modeling HSCR. EGC/PK060399egfr and primary EGCs were induced with neural stem cell-inducing medium (NSC-Med). Morphological changes, EdU assay, immunofluorescence, RT‒qPCR, and Western

Systematic structure-based analysis of RET variants in MEN2A and Hirschsprung's disease, and the paradoxical co-occurrence of both conditions.

Variants in the human receptor tyrosine kinase RET can cause RET loss-of-function and Hirschsprung's disease (HSCR), while activating RET variants drive cancers including multiple endocrine neoplasia type 2 (MEN2). Paradoxically, some variants cause both HSCR and MEN2A. We curated 77 RET extracellular positions associated with HSCR, MEN2A or both and used a structure-based approach to predict the effects of variants at these positions on RET structure. Approximately 90% of HSCR-associated positions can, upon mutation, disrupt intramolecular interactions stabilizing RET tertiary structure via d

Management of pregnancy and outcomes in a woman with mannose-6-phosphate isomerase deficiency (MPI-CDG).

Congenital Disorder of Glycosylation type Ib (MPI-CDG) is a rare autosomal recessive metabolic disorder caused by deficiency of mannose-6-phosphate isomerase, resulting in impaired N-glycosylation. Unlike other CDG subtypes, MPI-CDG is not associated with neurodevelopmental impairment and is effectively treated with oral D-mannose supplementation. However, evidence guiding management during pregnancy is scarce, and D-mannose is generally avoided due to limited safety data. We describe the long-term course and pregnancy outcomes of a woman diagnosed with MPI-CDG in infancy. She was treated with
