Laparoscopic management of cystic duct cyst (Type VI): case report.

Cystic duct cyst (CDC) is a rare biliary anomaly that may occur in isolation or in association with other choledochal cysts. While individual variants have been described, reports focusing on dual biliary pathology managed by a minimally invasive approach are limited. Surgical resection is recommended to prevent complications, including malignant transformation. We report the case of a 68-year-old woman with a remote history of cholecystectomy who presented with acute pancreatitis. Magnetic resonance cholangiopancreatography identified a CDC associated with dilatation of the common bile duct.

Timing of definitive surgery after external biliary drainage for completely perforated congenital choledochal cysts: a single-center retrospective cohort study.

BACKGROUND: The optimal timing of definitive surgery after external biliary drainage (EBD) for children with completely perforated congenital choledochal cysts (CCs) remains controversial. Prolonged EBD may increase the risk of drainage tube–related complications. This study compared the safety and efficacy of early (≤ 14 days) versus delayed (> 14 days) definitive surgery after EBD. METHODS: This study is a single-center retrospective cohort study that included all consecutive pediatric patients with complete perforation of choledochal cysts (CC) admitted between January 2018 and February 202

Arteriogenic hemorrhage of the anastomotic loop after choledochojejunostomy: case series and literature review.

Arteriogenic hemorrhage of the anastomotic loop after choledochojejunostomy is a life-threatening, and complex postoperative complication, necessitating a comprehensive diagnostic approach integrating clinical manifestations, imaging findings, and results from endoscopy or angiography. We present 3 clinical cases: a 22-year-old male who developed hemorrhagic shock 28 days after laparoscopic choledochal cyst resection, due to an anomalous communication between the right hepatic artery stump and the choledochojejunostomy, and achieving hemostasis via transarterial embolization; a 71-year-old fem

Transgenic mouse model of congenital choledochal cyst.

PURPOSE: Choledochal cyst is a rare, congenital dilation of the hepatobiliary tree. Due to the associated malignancy risk, complete resection is recommended. There remains a risk of metachronous cholangiocarcinoma despite resection necessitating lifelong surveillance. Choledochal cysts are increasingly prevalent with an incompletely understood connection to carcinogenesis. We sought to develop a mouse model to reliably mimic human disease process of choledochal cyst. METHODS: Experimental transgenic mice were bred with a genotype of Pdx-Cre, TGFα, LSL-Kras G12D Mu/Wt. Control C57 mice were use

Age-Dependent Dynamics of the Biliary Microbiome in Children with Choledochal Cysts: Functional Remodeling Underlying Taxonomic Conservation.

Choledochal cyst (CC), a congenital biliary anomaly, is associated with recurrent infections, chronic inflammation, and an increased risk of malignancy. Although emerging evidence implicates the biliary microbiome in disease pathophysiology, its developmental dynamics in pediatric CC remain unclear. Using deep metagenomic sequencing and comprehensive functional annotation, this study characterized age-dependent changes in the biliary microbiome of 201 pediatric CC patients stratified into infancy (<1 year), early childhood (1-5 years), and later childhood (5-12 years). We found that while the

Predictive value of cyst size for clinical symptoms in patients with prenatally diagnosed choledochal cysts.

BACKGROUND: Choledochal cysts (CDCs) are common congenital biliary malformations in children. However, current research on whether cyst size links to postnatal clinical symptoms in prenatally diagnosed CDCs remains scarce and unclear. This study aimed to explore the predictive value of cyst size for the occurrence of clinical symptoms in patients with prenatally diagnosed CDCs. METHODS: A retrospective review of medical records was conducted for patients with prenatally diagnosed CDCs who were admitted to Fujian Children's Hospital between January 2018 and May 2025. The patients were divided

Robot-assisted choledochal cysts resection in children across all ages: a larger cohort single-center experience with 318 Cases.

Robotic-assisted surgery (RAS) is an emerging alternative to laparoscopic surgery (LAS) for the treatment of choledochal cysts. We aim to evaluate the safety and efficacy of robotic-assisted choledochal cyst resection (RACC) in children from neonates to 16-year-olds. In this retrospective study, 318 patients undergoing RAS between April 2020 and July 2025 were compared with 489 patients undergoing LAS between January 2015 and July 2025. The robotic cohort was stratified into six age-based subgroups: R1 (neonates), R2 (> 1 to ≤ 3 months), R3 (> 3 to ≤ 12 months), R4 (> 12 to ≤ 36 months), R5 (>

Robot-assisted Cyst Excision, Ductoplasty, and Embedding Portoenterostomy for Choledochal Cysts with a Small Caliber Hepatic Duct less than 6 mm.

Anastomotic stricture is a primary concern following the surgical treatment of choledochal cysts (CC), particularly in pediatric patients with narrow bile ducts (<6 mm), and remains the main indication for re-operation. Robotic-assisted surgery (RAS), with its superior 3D visualization and instrument dexterity, offers an ideal platform for the complex reconstruction required to prevent this complication. This study presents a robot-assisted modified technique designed to mitigate the risk of stricture, as demonstrated in a 34-month-old female with a Type Ia CC. The procedure involves complete

Technical feasibility should not outweigh long-term outcomes in biliary reconstruction for children.

The choice of biliary reconstruction after excision of choledochal malformations in children should prioritize long-term physiological outcomes over operative convenience. In response to the article by Kanojia et al. on the management of aberrant bile ducts at the porta hepatis during minimally invasive surgery, we wish to highlight that while hepaticoduodenostomy is technically attractive (particularly with laparoscopic and robotic platforms) evidence suggests it is associated with increased biliary reflux and reflux-related morbidity, especially in cases with complex hilar anatomy requiring

Machine learning-based prediction model for risk factors associated with conversion to open surgery in pediatric laparoscopic radical resection of choledochal cysts.

OBJECTIVE: To identify risk factors for conversion to open surgery during laparoscopic choledochal cyst excision in pediatric patients and develop a predictive model using machine learning. METHODS: We retrospectively analyzed 214 children who underwent laparoscopic excision between 2015 and 2023. Patients were classified into laparoscopic (n = 173) or conversion (n = 41) groups. Seven machine learning models were developed and evaluated using AUC and decision curve analysis (DCA). SHAP analysis identified top predictors, which were validated via multivariable logistic regression. A nomogram w

Preoperative nutritional risk and adverse perioperative outcomes in children with congenital choledochal cysts: a retrospective cohort study.

BACKGROUND: Congenital choledochal cysts (CCC) are rare biliary anomalies associated with significant morbidity. The impact of preoperative nutritional status on surgical outcomes in pediatric CCC patients remains unclear. This study aimed to investigate this relationship, using a validated nutritional risk screening tool to stratify patients. METHODS: We conducted a retrospective cohort study of pediatric patients who underwent CCC excision with Roux-en-Y hepaticojejunostomy at a single center between January 2011 and September 2025. Nutritional risk was assessed within 24 h of admission usi

Malignant transformation of choledochal cyst 38 years after cystoduodenostomy mimicking Lemmel's syndrome: a rare diagnostic pitfall.

Malignant transformation of choledochal cysts after internal drainage is a known risk, but presentation nearly four decades later is exceptional. We report a case of cholangiocarcinoma developing 38 years after cystoduodenostomy in a 39-year-old female. Diagnosis was confounded by a periampullary diverticulum, mimicking Lemmel's syndrome. Multimodal imaging suggested a hilar mass, but endoscopy revealed a large diverticulum. Exploratory laparotomy and frozen section analysis confirmed adenocarcinoma arising from the remnant cyst. The patient underwent complete cyst excision, lymphadenectomy, a

RNA-Binding Proteins Alter Redox Gene Splicing in Biliary Atresia: Insights from Expression Profiling.

BACKGROUND: Biliary atresia (BA), the most common cause of extrahepatic obstructive jaundice in infants, is a severe infant disease with a poor prognosis and unclear etiology. RNA-binding proteins (RBPs) are key regulators of alternative splicing and are implicated in various liver pathologies. However, whether RBP dysfunction and resultant aberrant splicing contribute to BA pathogenesis remains unknown. METHODS: We performed an integrated transcriptomic analysis using RNA-seq data from BA patients and normal donor livers (GEO: GSE159720). This encompassed systematic identification of differe

Feasibility and safety of transumbilical single-incision laparoendoscopic surgery for giant choledochal cysts in infants: a single-center retrospective study.

BACKGROUND: Radical resection of giant choledochal cysts (diameter ≥ 10 cm) in infants presents technical challenges, particularly when attempted with minimally invasive approaches. The primary difficulty lies in the cyst’s mass effect in the limited abdominal space. Although transumbilical laparoendoscopic single-site surgery (LESS) offers potential cosmetic and minimally invasive advantages, evidence regarding its feasibility and safety for managing these giant cysts remains limited. METHODS: This single-center, retrospective study with a historical control group included infants who underwe

Children With Pancreatic Hypoplasia Experience Poor Weight Gain and Labile Diabetes but Low Incidence of DKA.

CONTEXT: Pathogenic variants in GATA6, GATA4, and PDX1 cause pancreatic hypoplasia or agenesis and early onset diabetes mellitus. There is a lack of information about long-term outcomes and clinical management of these complicated patients, including how best to approach their exocrine pancreatic insufficiency (EPI), weight gain, and glycemic management. PARTICIPANTS: We investigated clinical features and treatment of patients with variants in GATA6, GATA4, and PDX1 identified through the US Monogenic Diabetes Registry. Data were self-reported or extracted from medical records. RESULTS: Elev

Pediatric urethroscope versus flexible choledochoscope for removal of distal protein plugs and calculi in choledochal cyst: A prospective cohort study.

BACKGROUND: Choledochal cyst (CC) is a common congenital biliary malformation that occurs frequently in children. CC is often complicated by protein plugs, calculi, and other debris, predisposing patients to cholangitis, pancreatitis, and other adverse events. Thorough intraoperative removal of obstructive debris is critical for improving postoperative outcomes. At present, intraoperative endoscopy represents the main modality for removing distal obstructions; however, consensus regarding the comparative efficacy of pediatric urethroscope and flexible choledochoscope remains lacking. Altered hepatic metabolism in Down syndrome.

Trisomy 21 (T21) gives rise to Down syndrome (DS), the most commonly occurring chromosomal abnormality in humans. T21 affects nearly every organ and tissue system in the body, predisposing individuals with DS to congenital heart defects, autoimmunity, and Alzheimer's disease, among other co-occurring conditions. Here, using multi-omic analysis of plasma from more than 400 people, we report broad metabolic changes in the population with DS typified by increased bile acid levels and protein signatures of liver dysfunction. In a mouse model of DS, we demonstrate conservation of perturbed bile aci
