Integrated genomic and biochemical diagnosis of a novel homozygous start-loss variant in AKR1D1 associated with neonatal cholestasis.

INTRODUCTION: Congenital bile acid synthesis defects are rare autosomal recessive disorders that typically present in early infancy with cholestasis, progressive liver dysfunction, and, in severe cases, acute liver failure. These conditions may mimic other metabolic diseases detected in newborn screening, complicating early diagnosis. The AKR1D1 gene encodes Δ4-3-oxosteroid 5β-reductase, a key enzyme in primary bile acid synthesis, and pathogenic variants cause bile acid synthesis defect type 2 (OMIM #235555). CASE DESCRIPTION: We report a 3-month-old male infant with severe neonatal cholesta

Conjoined Parasitic Twins (Cephalo-Thoracopagus Parasiticus) in a Free Ranging Northern Bat (Eptesicus nilssonii): A Micro-CT Anatomic and Genetic Survey.

The detailed anatomical situation of male stillborn cephalo-thoracopagus twins in a wild ranging northern bat (Eptesicus nilssonii) is described by means of full body micro-CT scans in high resolution with three-dimensional computational reconstruction. The external anatomy was characterised by supernumerary wings and legs and marked cranial disfiguration. The most striking features of the internal anatomy were the presence of two hearts with different characteristics, three lungs with two tracheas, three kidneys as well as a doubled liver. Vascular aberrations and other organ deformities are

Assessing liver disease in cyanotic CHD using multiparametric MRI: a call to prevent future burden.

BACKGROUND & AIMS: Patients with cyanotic CHD and those with metabolic dysfunction-associated steatotic liver disease are at risk of liver fibrosis. We compared hepatic extracellular volumes and native T1 values to better understand the burden of liver disease in these populations. METHODS: The sample comprised 136 patients in 5 groups: control (n = 23), metabolic dysfunction-associated steatotic liver disease [mild (F0-F1) and significant (F2-F4) fibrosis; n = 45], repaired tetralogy of Fallot (n = 30), and Fontan circulation (n = 38). Differences were assessed using linear regression models

Long-term follow-up after choledochal cyst excision in children.

INTRODUCTION: Choledochal cyst (CC) excision is indicated to avoid complications. There is no consensus on postoperative follow-up investigations, duration, or timing. At our institution, patients are followed for 5 years after CC excision with ultrasound and liver laboratory tests. We aimed to evaluate both the post-operative outcomes after pediatric CC excision, focusing on long-term complications, and the adherence to our postoperative follow-up protocol. METHODS: A retrospective chart review of children who underwent a CC excision at our institution between January 1986 and March 2025 was

Surgical management for aberrant bile ducts at porta in children undergoing laparoscopic and robotic excision of choledochal malformations.

INTRODUCTION: Choledochal malformations are complex biliary anomalies in children, and their definitive treatment involves cyst excision with bilioenteric reconstruction. The presence of aberrant bile ducts (ABD) at the porta hepatis adds significant technical difficulty and increases the risk of postoperative complications such as bile leak, cholangitis, and segmental stasis if not managed appropriately. The aim of this study was to describe the incidence, classification, and surgical management of ABD encountered during minimally invasive excision of choledochal cysts in children, and to eva

Staged compressive dressing method for primary giant omphalocele repair: a low-cost and successful strategy.

PURPOSE: In giant omphalocele cases, small abdominal cavity leads to high intraabdominal pressure during closure, making repair challenging. This study aimed to evaluate our staged compressive dressing technique and its outcomes. METHODS: Eighteen giant omphalocele cases treated with staged compressive dressing/primary closure between 2014 and 2024 were retrospectively reviewed. All cases received compressive dressing every other day with wet sterile gauze and transparent adhesive surgical drapes from the first postnatal day until surgery to reduce intra-sac organs. Primary closure was perfor

Biallelic Frameshift Variant in LAMB1 Associated with Occipital Meningoencephalocele and Pansutural Craniosynostosis: A Case Report and Review of the Literature.

The LAMB1 gene encodes the laminin β1 chain, an essential component of the pial basement membrane required for neural morphogenesis. While biallelic LAMB1 mutations typically result in cobblestone lissencephaly and brainstem abnormalities, we report an extreme, lethal syndromic phenotype that significantly expands the gene's known clinical spectrum. Through genomic sequencing of a 36-week stillborn male fetus from consanguineous parents, we identified a novel homozygous frameshift variant in LAMB1 (NM_002291.3:c.3330dup, p.Gly1111Argfs*23). This fetus exhibited severe growth restriction and a
