Understanding and managing dextrocardia with situs inversus: A case report.

Dextrocardia with situs inversus is an uncommon congenital disorder in which the heart and other organs in the chest and abdomen are reversed. Although it rarely causes symptoms, it can be linked to heart or systemic disorders and is challenging to diagnose due to its unique architecture. The present report describes the case of a 27-year-old man who presented with sudden breathing problems. A physical examination revealed no heartbeat on the left side and odd cardiac noises. Further electrocardiogram tests revealed classic symptoms of dextrocardia, including right axis deviation and inverted

MRI features of fetal type I congenital choledochal cyst and parameter measurement of hepatobiliary development.

OBJECTIVES: To observe fetal MRI findings of type I congenital choledochal cyst (CCC) and the developmental differences in abdominal organs between fetuses with CCC and normal controls. METHODS: This retrospective study included 90 normal fetuses and 31 fetuses with surgically confirmed CCC. MRI was used to observe CCC morphology, orientation, connection with bile duct or gallbladder, and its relationship with the liver edge. Cyst volume was calculated. Measurements, including the lung-to-liver signal intensity ratio (LLIR), liver (transverse and craniocaudal diameters, maximum cross-sectiona

Extreme conformal prediction: Reliable intervals for high-impact events.

BACKGROUND: Prehospital (ambulance) care can reduce morbidity and mortality from trauma. Yet, there is a dearth of effective evidence-based interventions and implementation strategies. Emergency Medical Services Traumatic Shock Care (EMS-TruShoC) is a novel bundle of five core evidence-based trauma care interventions. High-Efficiency EMS Training (HEET) is an innovative training and sensitization program conducted during clinical shifts in ambulances. We assess the feasibility of implementing EMS-TruShoC using the HEET strategy, and feasibility of assessing implementation and clinical outcomes

Ambiguous Compliments and Encouragement: Young Fathers' Experiences of Care, Responsibility and Youth.

Caroli's disease is defined as a abnormal dilatation of the intra-hepatica bile ducts: Its incidence is extremely low (1 in 1,000,000 population) and in most of the cases the whole liver is interested and liver transplantation is the treatment of choice. In case of dilatation limited to the left or right lobe, liver resection can be performed. For many year the standard approach for liver resection has been a formal laparotomy by means of a large incision of abdomen that is characterized by significant post-operatie morbidity. More recently, minimally invasive, laparoscopic approach has been p

DLK1 is a GATA1s-driven dependency and therapeutic target in Down syndrome-associated myeloid leukemia.

Children with Down syndrome have a markedly increased risk of developing myeloid leukemia. Although having an excellent prognosis, 10% to 20% develop relapsed or refractory disease with poor survival, highlighting the need for new targeted approaches. The pathogenesis of myeloid leukemia of Down syndrome (ML-DS) is tightly linked to fetal hematopoiesis and mutations in GATA1, generating the truncated GATA1 short (GATA1s) isoform. We identified Delta-like noncanonical Notch ligand 1 (DLK1) as a direct GATA1s target. DLK1, a paternally imprinted transmembrane protein, is highly expressed in feta

Cystic biliary atresia mimicking choledochal cyst: A diagnostic challenge in an infant.

Cystic biliary atresia (CBA) is a rare subtype of biliary atresia (BA), characterised by cystic dilation of the extrahepatic biliary tract. It can radiologically mimic choledochal cysts, often delaying accurate diagnosis and timely management. We report the case of a 2-month-old male infant who presented with cholestatic jaundice and an extrahepatic cystic lesion suggestive of a choledochal cyst. Initial imaging, including magnetic resonance cholangiopancreatography, revealed a cystic structure in the common bile duct. Intraoperative cholangiography demonstrated a dilated extrahepatic biliary

Computed tomographic evaluation of canine hepatic lymph nodes: Establishment of aorta-normalized dimension ratio cutoff values for differentiating normal from abnormal conditions.

BACKGROUND AND AIM: Hepatic lymph nodes (HLNs) drain lymph from the liver, gallbladder, pancreas, stomach, and duodenum. Enlargement of these nodes may indicate pathology in those organs, but objective, breed-independent computed tomography (CT)-based size criteria have not been established. This study aimed to determine quantitative cutoff values for HLN dimensions normalized to aortic diameter (Ao) to differentiate normal from abnormal HLNs in dogs. MATERIALS AND METHODS: This retrospective study analyzed contrast-enhanced abdominal CT scans (slice thickness <1.5 mm) from 84 dogs older than

Characteristics and outcomes of dogs undergoing cholecystectomy for diseases other than a mature gallbladder mucocele: a retrospective cohort study.

BACKGROUND: Cholecystectomy is well described for gallbladder mucocele management, but its role in other gallbladder diseases is less well understood. HYPOTHESIS/OBJECTIVES: Characterize and compare dogs undergoing cholecystectomy for mature gallbladder mucocele (M-GBM) versus "other gallbladder disease" (non-M-GBM). A secondary exploratory aim was to compare surgical complication and 10 to 14-day mortality rates between cohorts. ANIMALS: A total of 256 dogs undergoing cholecystectomy. METHODS: Retrospective cohort study (2018-2025). Dogs were assigned to M-GBM or non-M-GBM groups based on

Prenatal ultrasound diagnosis of a rare morgagni hernia: a case report.

BACKGROUND: Morgagni hernia is a rare type of congenital diaphragmatic hernia, and its prenatal diagnosis by ultrasound is extremely challenging, especially when combined with massive pericardial effusion, as it can be easily confused with other diaphragmatic hernias. This article reports a case of fetal Morgagni hernia definitively diagnosed by serial prenatal ultrasound and magnetic resonance imaging, followed by successful intrauterine pericardial drainage and postnatal surgical correction. It aims to emphasize the importance of standardized imaging follow-up for prenatal identification and

Genomic and transcriptomic profiling of hepatocellular carcinoma in patients with Fontan-associated liver disease.

BACKGROUND AND AIMS: Fontan-associated liver disease (FALD) could lead to liver cirrhosis and hepatocellular carcinoma (HCC). The prognosis of FALD patients is worse when HCC develops. Therefore, a better understanding of the mechanisms of FALD hepatocarcinogenesis is critical. We investigated the genomic and transcriptomic signatures of FALD HCC. APPROACH AND RESULTS: Ten patients with FALD HCCs, 2 patients with non-Fontan congenital heart surgery (CHS) HCC, and 1 patient with FALD focal nodular hyperplasia (FNH) were enrolled. Whole-genome sequencing and transcriptome sequencing were perfor

Segmental Cholangiectasia of the Intrahepatic Bile Duct: A Case Report.

INTRODUCTION: Segmental cholangiectasia is a rare cause of focal intrahepatic bile duct dilatation that can mimic congenital, inflammatory, or neoplastic conditions, posing diagnostic challenges. CASE PRESENTATION: We describe a 58-year-old man with a longstanding cystic lesion in hepatic segment 8 who developed multiple cholangitic episodes. Despite the exclusion of infectious, autoimmune, and overt neoplastic disease, an elevated carbohydrate antigen 19-9 (CA 19-9) raised concern for intraductal papillary neoplasm of the bile duct and prompted anterior hepatic sectorectomy. Histopathology c

Ventriculo gallbladder shunt as an alternative cerebrospinal fluid diversionary procedure.

BACKGROUND: In the complex management of hydrocephalus, ventriculoperitoneal (VP) shunt insertion is the mainstay for cerebrospinal fluid (CSF) diversion. However, when traditional routes fail or are contraindicated - due to congenital abnormalities, infections, extensive adhesions, or previous abdominal surgeries - surgeons must consider less conventional alternatives. The second line is the ventriculo-atrial shunt then the ventriculo-pleural. The ventriculo-gallbladder (VG) shunt, although rarely used, presents a fascinating option with unique physiological advantages. Here, we present an in

ULTRASONOGRAPHY, SHEAR WAVE ELASTOGRAPHY AND LIVER HISTOLOGY FOR THE DIFFERENTIAL DIAGNOSIS BETWEEN BILIARY ATRESIA AND OTHER CAUSES OF CHOLESTASIS.

BACKGROUND: Biliary atresia (BA) is the leading cause of cholestatic jaundice in the first months of life. Liver stiffness measurement by shear wave elastography (2D-SWE) could help discriminate BA from other causes of cholestasis. OBJECTIVES: To assess the use of abdominal ultrasound with bidimensional shear wave elastography and liver histology to diagnose Biliary Atresia in cholestatic infants. To compare the use of elastography to estimate the stage of liver fibrosis with the histologic classification. METHODS: Cholestatic infants younger than three months were divided into BA and non-BA

Unveiling Hepatic Protein Alterations in Neonatal and Infant Biliary Atresia.

Pediatric populations differ from adults in drug elimination capacity. While current scaling methods account for enzyme and transporter maturation, they overlook comorbidities, such as biliary atresia (BA), a liver disease appearing within the first 2-8 weeks of life that can progress to cirrhosis. Such conditions may impair hepatic drug clearance, requiring dose adjustments. Physiologically based pharmacokinetic (PBPK) tools aim to address such cases and have been advocated to fill gaps in clinical data instead of less formalized and evidence-based guesswork. However, the paucity of systems d
