The majority (about 75%) of patients who suffer from acute pancreatitis do so as a consequence of gallstones or alcohol abuse. The other 25% of patients often present difficult diagnostic problems. Over several years the author has accumulated a series of patients with remedial causes of pancreatitis. They include a group of congenital conditions such as pancreas divisum, choledochal cysts and congenital abnormalities of the pancreatic ductal system. Patients who have had pancreatitis and who have an intact gallbladder often have stones that are difficult to identify. Repeated attacks of pancr

The infrequent case of gallbladder torsion and accompanying Meckel's diverticulum in a 3-year-old girl, is described. Detorquation, cholecystectomy and resection of the diverticulum were performed. Condition of the development of the disease as well as its clinical aspects and therapy are discussed. It is suggested to explore the abdominal cavity for other developmental anomalies in the course of the operation. The patient recovered without complications and left the hospital 14 days after the operation.

Two daughters of non-consanguineous normal parents had phocomelia of both lower extremities with 4-toed feet. The older sister also had phocomelia of the left upper extremity with 5 finger rays; she died immediately after birth. Autopsy disclosed a congenital diaphragmatic hernia, common mesentery and agenesis of the gallbladder, and normal female genitalia. In addition, the younger sister showed a bony skull defect, diastasis recti, agenesis of the uterus and agenesis or atresia of the vagina, hypoplasia of the sacrum and hypo/dysplasia of the pelvic bones. Her growth and mental development w

We performed serial ultrasonic examinations to differentiate biliary atresia from neonatal hepatitis. The subjects studied were 144 children (100 normal neonates and infants, 31 patients with neonatal hepatitis and 13 patients with biliary atresia). They were examined by ultrasound before, during and after feeding. In 97 out of 100 normal children and all patients with neonatal hepatitis, the gallbladder was identified, and the change in size following oral feeding was observed. In four children with severe neonatal hepatitis which could not be differentiated from biliary atresia by clinical a

One hundred thirty-one consecutive infants with biliary atresia were operated on during the 15-year period between 1973 and 1988. Six patients did not have biliary reconstruction because of advanced cirrhosis or transplant preference. The other 125 infants had excision of all nonpatent extrahepatic bile ducts; biliary drainage was provided by a gallbladder-common bile duct conduit in 14 patients and by a Roux-en-Y portoenterostomy in 111 infants (including the seven patients with correctable biliary atresia). The bilioenteric conduit was temporarily exteriorized and, for the past 2 years, a co

The authors describe a case of xiphoomphalpagus; one infant was dead, the other alive. They had a common umbilical cord and omphalocele, joined liver, only one gallbladder and common duodenum in the shape of a wide sac. From this originated two thin guts, one was wide and belonged to the dead foetus, the gut of the live foetus beneath the duodenum was atretic. Already six hours after delivery the authors separated the infants and in the live infant they not only resected the liver and reinserted the choledochus but also repaired the wide duodenum and atresia. The second infant died on the 4th

Diffuse esophageal leiomyomatosis is characterized by diffuse muscular hypertrophy chiefly marked in the lower end of the esophagus looks at a large tumor. It involves the entire length of the esophagus and down to the upper part of the stomach (esogastric leiomyomatosis). Diffuse leiomyomatosis is a extremely rare pathology encountered in children (13 previous report cases) and young adults. Il may be isolated or found in association with other intrathoracic and genital localisations or Alport syndrome (nephropathy, sensorineural deafness, ocular lesions). Nonvisualization of the gallbladder by ultrasound in a fasting patient usually indicates calculous gallbladder disease. However, a number of other less common conditions can prevent gallbladder visualization or recognition. The latter occurs when the location or the morphology of the gallbladder is so altered that the gallbladder is difficult to identify. These conditions can be classified as: (1) congenital anomalies, (2) gallbladder contraction not due to stones, (3) conditions that cause shadowing or reverberation from the near wall of the gallbladder, and (4) the solid gallbladder pattern.

Knowledge of congenital anomalies in abdominal viscera is important for surgeons, if they are to treat such patients and solve their problems adequately. This article presents an unusual case of reversed rotation of the intestine associated with anomalies in the biliary tract and gallbladder. This patient underwent surgery because of a bleeding duodenal ulcer. During surgery the following viscera positions were detected: the gallbladder was located to the left of the round ligament, the choledochus was in front of the first portion of the duodenum, the small intestine was in the left portion o

Nine cases of specific type (subtype d) biliary atresia were experienced at the Tohoku University Hospital and were classified into three groups on the basis of their clinical and pathological findings. Group 1 cases were defined as cases showing obstruction on the side of the hepatic duct and cystic dilatation, communicating with the gallbladder, in the common bile duct. Group 2 cases were those showing dilatation similar to Group 1, but measuring less than 20 mm, the dilated part being covered with thick connective tissue. Group 3 cases were those showing isolated dilatation in the common he

Agenesis of the gallbladder without extrahepatic biliary atresia is a rare disorder. At the UCLA-affiliated hospitals, 12 patients were classified in the following groups: (1) multiple fetal anomaly, (2) asymptomatic, and (3) symptomatic. All four patients in the multiple fetal anomaly group died of their other congenital defects. In the three patients in the asymptomatic group, the absent gallbladder was an incidental finding at autopsy. The five patients in the symptomatic group underwent operations for symptoms suggestive of biliary tract disease, with no gallbladder found; all were symptom

While cystic malformations of the main biliary tract are wellknown and classified, it is exceptional to find cystic malformations of the accessory biliary tract (cystic duct). An adolescent aged 15 years and an adult of 49 years were operated for cysts of the cystic duct revealed by associated lesions, a cyst of the common bile-duct and gallbladder stones respectively. Ultrasonography showed cystic lesions which opacification during intravenous cholangiography and endoscopic retrograde cholangiography (EPRC) proved to be related to the cystic duct. The radiography outline of the biliary pathwa

Congenital cystic segmentary dilatation of the intrahepatic biliary ducts is a rare disease. Although several cases has been described in medical literature, its occurrence such as it was originally referred is exceptional. Some cases coexist with congenital hepatic fibrosis and portal hypertension. A characteristic phenomenon is the formation of stones and canalicular infection. In this article, three cases of Caroli disease (females, 57, 53 and 58 years old) are described. Case n 1 was a complex malformation including: a) cystic and segmentary dilatations of the intrahepatic biliar ducts; b)

Familial adenomatous polyposis coli is now known to encompass a more complicated spectrum of anomalies than was initially suspected. The term Gardner's syndrome, broadened from its original definition, currently includes all cases of familial polyposis coli with extracolonic neoplasms. Small-bowel adenomata occur in approximately 12% of these patients, and sporadic instances of biliary neoplasia have also been reported. We describe multifocal adenomatous change with severe dysplasia of the gallbladder in a young woman known to have familial adenomatous polyposis coli. Review of this and other

The choledochus and pancreatic duct ordinarily enter the duodenum either separately or via a common channel located in the duodenal wall. The usual maximal length of the common channel is approximately 0.5 cm. An anomalous junction of the pancreaticobiliary tract is defined by the presence of an unusually long common channel whose length is 1 cm or longer. During the last 8 years, we studied 9 cases of congenital dilatation of the bile duct. Besides the choledochal cyst, all patients had a common channel which measured 1.5 cm or longer. The mean age of patients (6 females, 3 males) was 36.5 ye

Aminoguanidine (AG) sulfate was injected into the thin albumen of fertile eggs on the different days of incubation. The lethality of AG sulfate on the developing chick embryos became lower when the treatment was done at later developmental stage. When AG was injected on the 5th day of incubation, the frequency of retardation of development of the body and liver, aplasia of gallbladder and enlarged spleen was highest without high mortality compared with the other experiments. Embryos with liver damage could not hatch. 14C-AG hydrochloride injected on the 5th day of incubation easily distributed

Pancreas divisum is a congenital variant of pancreatic duct drainage in which the dorsal duct dominates, most likely due to the failure of the two independent pancreatic ductal systems to fuse embryologically. Although 5 to 10 percent of the population have this congenital variation, few demonstrate symptoms related to their pancreaticobiliary ductal systems. However, patients may present with symptoms referable to this system, and the diagnosis is difficult. In this series, endoscopic retrograde cholangiopancreatography with cannulation and radiographic injection of the dorsal duct demonstrat
