Ectopic Liver Mimicking Neoplastic Lesions: Report of Five Patients and Literature Review.

Ectopic liver is a rare congenital anomaly characterized by the presence of liver tissue outside the normal liver without any direct connection to it. These lesions are usually detected incidentally during clinical evaluation for other issues. However, they sometimes mimic neoplastic lesions both clinically and radiologically, which poses diagnostic challenges. Here, we present 5 patients with ectopic liver, 4 of which mimicked neoplastic lesions. The first was a 54-year-old woman diagnosed with multiple liver lesions and an enlarged gastro-hepatic lymph node, suspected to be a primary hepatic

Cholecystohepatic duct (type IIIA) - fifth report of aberrant biliary pathology in a paediatric case of oesophageal atresia and imperforate anus with review of literature.

It is known that the presence of associated anomalies has become one of the most critical prognostic factor for infants born with oesophageal atresia (OA). Associated VACTERAL group of abnormalities are common, but other associations are rare. Anomalies of the hepatobiliary system are even more rare among other groups. There are only a few such cases in the literature including adults and paediatric population. We describe the findings and management of one infant born with OA with tracheoesophageal fistula (TEF) and Ano rectal malformation (ARM), who was later shown to have features of obstru

Pancreaticobiliary Maljunction With a Non-Dilated Extrahepatic Bile Duct Masked by Adenomyomatosis: A Potential Cause of Unresectable Gallbladder Carcinoma.

Adenomyomatosis of the gallbladder (ADM) is generally considered benign. The presented cases were initially diagnosed with ADM via ultrasound during an annual health check-up and monitored for several years without further investigations due to the absence of significant changes or symptoms. Subsequently, one case was diagnosed with pancreaticobiliary maljunction (PBM) with a non-dilated extrahepatic bile duct (EBD) after symptom onset, and underwent prophylactic laparoscopic cholecystectomy to avoid the risk of carcinogenesis. Although the other case remained asymptomatic, ultrasound later re

Nationwide Expert Survey on Appropriate Bile Duct Transection Line in Adult and Pediatric Congenital Biliary Dilatation Surgery in Japan: AOB Consensus Meeting.

BACKGROUND: Resection of the extrahepatic bile duct including the gallbladder and the dilated segment, is a widely accepted surgical concept in congenital biliary dilatation (CBD) in both adult and pediatric surgeons. However, owing to the existence of various types of CBD and differences in the extent of dilatation, no consensus has been reached regarding the appropriate resection margins. METHODS: Consensus statements regarding the appropriate surgical margins for both the duodenal and hepatic hilum sides were developed as two clinical questions (CQs) during a multidisciplinary consensus me

Investigation of the Genetic Determinants of Telangiectasia and Solid Organ Arteriovenous Malformation Formation in Hereditary Hemorrhagic Telangiectasia (HHT).

Telangiectases and arteriovenous malformations (AVMs) are the characteristic lesions of Hereditary Hemorrhagic Telangiectasia (HHT). Somatic second-hit loss-of-function variations in the HHT causative genes, ENG and ACVRL1, have been described in dermal telangiectasias. It is unclear if somatic second-hit mutations also cause the formation of AVMs and nasal telangiectasias in HHT. To investigate the genetic mechanism of AVM formation in HHT, we evaluated multiple affected tissues from fourteen individuals. DNA was extracted from fresh/frozen tissue of 15 nasal telangiectasia, 4 dermal telangie

Congenital absence of the gallbladder in a child: a case report.

BACKGROUND: Congenital absence of the gallbladder (CAGB) is an exceedingly rare embryological anomaly of the biliary system, with a complex etiology involving the failure of gallbladder formation during embryogenesis. Clinical manifestations are diverse; most patients are asymptomatic, while some present with symptoms such as biliary colic. The complexity of its clinical presentation and radiological features renders diagnosis challenging. CASE PRESENTATION: Fetal ultrasound at 22 weeks of gestation revealed an absent gallbladder. At 9 years and 11 months of age, the child exhibited significa

Biliary Disorders, Anomalies, and Malignancies in Children.

Biliary abnormalities in children are uncommon, and the spectrum of biliary disorders is broader than in adult patients. Unlike in adults, biliary disorders in children are rarely neoplastic and are more commonly rhabdomyosarcoma rather than cholangiocarcinoma. Pediatric biliary disorders may be embryologic or congenital, such as anatomic gallbladder anomalies, anomalous pancreaticobiliary tracts, various cholestatic processes, congenital cystic lesions, or genetic conditions. They may also be benign, such as biliary filling anomalies, biliary motility disorders, and biliary inflammatory and i

Agenesis of the gallbladder: A multicenter case series and review of the bibliography.

Gallbladder agenesis is a rare condition in pediatrics that is usually asymptomatic and represents a diagnostic challenge for physicians seeing these cases for the first time. Some patients may, however, present with symptoms that mimic other diseases of the bile ducts, and many of them undergo surgery due to such suspicion. Still, a timely diagnosis of gallbladder agenesis allows for medical treatment that is often sufficient to resolve the patient's problem. Although it is a benign condition, patients often present with other associated, more serious malformations and should be actively stud

A Rare Case of Duplicated Gallbladder in the United Arab Emirates.

Gallbladder duplication is a rare congenital aberration that requires special attention due to its clinical, diagnostic, and surgical complexity. Its symptoms are usually consistent with cholecystitis and other gallbladder etiologies. This is a case report of a 39-year-old male patient with a known case of chronic cholecystitis and cholelithiasis. He presented with mild epigastric abdominal pain over two months; as a result, he opted for elective cholecystectomy. Subsequently, a type I septate duplicated gallbladder was incidentally diagnosed following a histopathology report. This literature

An animal model recapitulates human hepatic diseases associated with GATA6 mutations.

Heterozygotic GATA6 mutations are responsible for various congenital diseases in the heart, pancreas, liver, and other organs in humans. However, there is lack of an animal that can comprehensively model these diseases since GATA6 is essential for early embryogenesis. Here, we report the establishment of a gata6 knockout zebrafish which recapitulates most of the symptoms in patients with GATA6 mutations, including cardiac outflow tract defects, pancreatic hypoplasia/agenesis, gallbladder agenesis, and various liver diseases. Particularly in the liver, the zebrafish gata6 model exhibits the pau

The Identification of a Novel Pathogenic Variant in the GATA6 Gene in a Child with Neonatal Diabetes.

GATA6 syndrome is a rare monogenic disorder caused by heterozygous variants in the gene GATA6, which controls the early embryonic differentiation of germ layers and the development of different organs. We present the results of the 7-year follow-up of a child with this syndrome as well as the following conditions: diabetes mellitus, exocrine pancreatic insufficiency, gallbladder atresia, and congenital heart disease (CHD). At birth, the patient was diagnosed with neonatal diabetes mellitus (NDM) associated with heart (mitral valve prolapse) and gastrointestinal abnormalities (gallbladder atres

Intraoperative hepatic subcapsular spider-like telangiectasia sign for the definitive diagnosis of biliary atresia.

OBJECTIVE: To evaluate the accuracy of intraoperative hepatic subcapsular spider-like telangiectasia (HSST) sign for differentiating biliary atresia (BA) from other causes of hepatic cholestasis. METHODS: The data of 69 patients with jaundice treated from January 2019 to December 2021 were retrospectively analyzed. Based on intraoperative cholangiography (IOC), the patients were divided into two groups: the BA group (n = 49) and the non-BA group (n = 20). The biochemistry tests, liver ultrasound, liver stiffness value and HSST sign of the two groups were compared. RESULTS: The incidence of a

3D reconstruction of a gallbladder duplication to guide LC: A case report and literature review.

RATIONALE: Gallbladder duplication is a congenital aberration of the biliary tree, which is rarely encountered in the abdomen. It is a challenge that can be encountered by surgeons and is associated with an increased risk of complications after cholecystectomy. More than 50% of gallbladder duplication cases were undetected on preoperative traditional imaging. In this study, a case of gallbladder duplication in a patient with mild abdominal pain detected using preoperative 3-dimensional (3D) reconstruction of the gallbladder was described for the first time. PATIENT CONCERNS AND DIAGNOSIS: We

Role of autopsy in diagnosing asplenia and right bilobed lung in a fetus with dextrocardia.

We present a unique case of a fetus with dextrocardia, asplenia and a right bilobed lung in a primigravida woman in her 20s at 21 weeks' gestation. Prenatal ultrasound examination revealed dextrocardia and other anomalies such as atrioventricular septal defect and situs ambiguous with the gallbladder on the left, leading to termination of the pregnancy. Fetal autopsy confirmed the diagnosis, detected additional findings such as asplenia and right bilobed lung missed on ultrasound and highlighted the importance of autopsy in prenatal diagnosis. This rare case emphasises the value of a comprehen

Development and validation of a model for early diagnosis of biliary atresia.

BACKGROUND AND AIMS: Early diagnosis of biliary atresia (BA), particularly distinguishing it from other causes of neonatal cholestasis (NC), is challenging. This study aimed to design and validate a predictive model for BA by using the data available at the initial presentation. METHODS: Infants presenting with NC were retrospectively identified from tertiary referral hospitals and constituted the model design cohort (n = 148); others were enrolled in a prospective observational study and constituted the validation cohort (n = 21). Clinical, laboratory, and abdominal ultrasonographic features

Cardiac complications caused by biliary diseases: A review of clinical manifestations, pathogenesis and treatment strategies of cholecardia syndrome.

Gallbladder and biliary diseases (GBDs) are one of the most common digestive diseases. The connections between GBDs and several organs other than the liver have gradually surfaced accompanied by the changes in people's diet structure and the continuous improvement of medical diagnosis technology. Among them, cholecardia syndrome that takes the heart as the important target of GBDs complications has been paid close attention. However, there are still no systematic report about its corresponding clinical manifestations and pathogenesis. This review summarized recent reported types of cholecardia
