Aspects of digestive tract tumors in Down syndrome: a literature review.

The purpose of this study was to describe the digestive neoplasms found in persons with Down syndrome. Due to intellectual disability, persons with Down syndrome do not convey their symptoms and pain, leading to delayed diagnosis and potentially worse outcome. It is thus important to know which organs are at risk for tumors and possible tumor risk factors. In a review of the literature, we found 13 benign tumors and 127 cancers in 1 fetus, 8 children, and 131 adults with Down syndrome. The review suggests a decreased incidence of digestive cancer, however, with a possible increased incidence o

Laparoscopic cholecystectomy in a patient with situs inversus totalis.

Currently, laparoscopic cholecystectomy is an undoubtfully optimal treatment of cholelithiasis. What about performing this procedure on a patient with situs inversus totalis and what are the difficulties of this operation for a right-handed surgeon? We presented a 35-year-old man with unknown situs inversus totalis who was admitted with epigastric pain and digestive problems. Ultrasonography and computed tomography of the abdomen confirmed the diagnosis of a gallstone. Besides, the liver and gallbladder were on the left side and the spleen was on the right. All systems were left-right reversal

Agenesis of the gallbladder: lessons to learn.

BACKGROUND: Congenital absence of the gallbladder is a rare, usually asymptomatic, anatomical variation. Some affected individuals may present with a clinical picture suggestive of gallbladder disease. This presentation, coupled with the inability of standard abdominal ultrasonography to convincingly diagnose agenesis of the gallbladder, can put the surgeon in a diagnostic and intraoperative dilemma. CASE REPORT: A 30-year-old lady presenting with clinical features of cholecystitis and diagnosed with shrunken gallbladder on ultrasonography was scheduled for laparoscopic cholecystectomy. Intra

Relief of intractable pruritus in Alagille syndrome by partial external biliary diversion.

BACKGROUND: Patients with Alagille syndrome (AGS) may develop pruritus, skin hypertrophy, and xanthomas because of chronic cholestasis and hypercholesterolemia. Partial external biliary diversion (PEBD) has been used successfully to treat chronic cholestasis in patients with progressive familial intrahepatic cholestasis (PFIC) and is a potentially useful treatment for patients with severe and intractable pruritus because of AGS. METHODS: Four children with chronic cholestasis and intractable pruritus were treated with PEBD, 1 by surgical cholecystostomy and 3 by cholecystojejunostomy. RESULT

Agenesis of the gallbladder: difficulties in management.

Gallbladder agenesis is a rare congenital biliary anomaly that may be associated with other biliary and extrabiliary congenital anomalies. Awareness of this entity by clinicians and radiologists is essential because many of these patients present with biliary symptoms and have unnecessary operations. In the present article, the relative epidemiological, etiological (embryology and development), pathophysiological, diagnostic tools and pitfalls and management aspects of this rare anatomic anomaly are briefly discussed through review of the literature. Particular reference to the difficulty in p

Laparoscopic cholecystectomy in situs inversus totalis with "inferior" cystic artery: a case report.

A 76-year-old man with known situs inversus totalis presented with left-sided discomfort. Abdominal ultrasonography and CT scan confirmed the diagnosis of a gallstone, as well as, situs inversus; the liver and gallbladder on the left side and the spleen on the right. The biliary system was thought to be left-right reversal, mirror image in the view of drip infusion cholangiogram and MRI. Laparoscopic cholecystectomy was safely performed, despite of unexpected aberrant cystic artery running inferior to cystic duct of situs inversus. Laparoscopic surgeon should be careful for view of reversed re

Laparoscopic cholecystocholangiography as an effective alternative exploratory laparotomy for the differentiation of biliary atresia.

PURPOSE: Exploratory laparotomy with cholecystocholangiography is often performed for the definitive diagnosis of biliary atresia (BA) when radiological and biochemical studies are inconclusive. Laparoscopic cholecystocholangiography (LCC) has recently been introduced as an alternative procedure to avoid unnecessary laparotomy. We discuss the advantages and indications of LCC based on our experience of performing this diagnostic procedure in four infants with prolonged direct hyperbilirubinemia. METHODS: We performed LCC in four infants by direct percutaneous puncture of the gallbladder by in

Multiseptate gallbladder with anomalous pancreaticobiliary ductal union: a case report.

Multiseptate gallbladder, characterized by the presence of multiple septa dividing the gallbladder lumen, is a very extremely rare congenital anomaly of the gallbladder. On the other hand, anomalous pancreaticobiliary ductal union is also one of the congenital anomalous biliary diseases and thought to be related with choledochal cyst or biliary tract malignancies. In this paper, we describe a unique and first patient of multiseptate gallbladder with anomalous pancreaticobiliary ductal union and a review of the literature. To clarify more characters of the multiseptate gallbladder, examination

Klippel-Feil syndrome with associated agenesis of lung and gall bladder presenting with asthma and allergic rhinitis.

Klippel-Feil syndrome KFS, a triad of short neck, limitation of neck movement and low posterior hairline, is characterized by presence of congenitally fused cervical vertebrae and is often associated with multiple congenital anomalies. A 35-year-old male was referred for evaluation of an opaque hemithorax. This led to a diagnosis of KFS, agenesis of left lung and gall bladder. The patient had history of wheezing dyspnea with nasal symptoms, which were diagnosed as asthma and allergic rhinitis. A high index of suspicion is required to recognize such a patient, and efforts should be made to seek

[The role of hepatobiliary scintigraphy in neonates with persistant jaundice].

Hepatobiliary scintigraphy of infants being examined for persistant neonatal jaundice is best carried out using mebrofenin, a 99m Tc-labeled IDA derivative. This article summarizes the most frequently occurring etiologies of persisting neonatal jaundice and the diagnostic work-up of this condition. Perinatal obliteration of extrahepatic bile ducts requires immediate surgery, whereas other causes of persisting jaundice may be treated medically and/or conservatively.

Left-sided gallbladder associated with congenital hypoplasia of the left lobe of the liver: a case report and review of literature.

Left-sided gallbladder is a rare anomaly that is often associated with other abnormal anatomy in the hepatobiliary system. We report our experience of a case of left-sided gallbladder associated with the congenital hypoplasia of the left lobe of the liver. A 71-year-old woman underwent cholecystectomy for acute cholecystitis. Intraoperative findings revealed the absence of the left lobe of the liver. The gallbladder was located in the left side of the round ligament, which was associated with abnormal intrahepatic portal branching. The incidence of left-sided gallbladder without situs inversus

Pancreaticobiliary maljunction in monozygotic twins--a case report.

We experienced a case of pancreaticobiliary maljunction in monozygotic twins. While one of the twins suffered from gallbladder cancer with poor prognosis, the other was not associated with biliary malignancy. Ultrasonography, endoscopic ultrasonography, and magnetic resonance cholangiopancreatography are effective in diagnosis of this disorder before occurrence of biliary cancers. This case is not only of academic interest for familial occurrence, but also of clinical interest in the early detection of pancreaticobiliary maljunction.

Initial experience with complex laparoscopic biliary surgery in children: biliary atresia and choledochal cyst.

BACKGROUND: Laparoscopic approaches are possible for many pediatric surgical diseases, including complex biliary disorders. The authors describe their early experience with laparoscopic procedures for biliary atresia and choledochal cysts. METHODS: A retrospective review was performed with Institutional Review Board (IRB) approval of children undergoing laparoscopic Kasai portoenterostomy (n = 2) or laparoscopic excision of a choledochal cyst with Roux-en-y biliary reconstruction (n = 3). The authors report the operative technique for these procedures and present their initial results. RESUL

Surgical treatment for anomalous arrangement of the pancreaticobiliary duct with nondilatation of the common bile duct.

BACKGROUND/PURPOSE: For anomalous arrangement of the pancreaticobiliary duct (AAPBD) with nondilatation of the common bile duct (CBD), the optimal surgical procedure remains controversial. The authors investigated which procedure would be most effective for AAPBD with nondilatation of the CBD. METHODS: The authors encountered 60 children with AAPBD in our institution between 1979 and 2002. Six of the 60 were classified as the nondilated type (CBD diameter; less than 8 mm), whereas the other 54 were classified as the dilated type (CBD diameter; more than 9 mm). Amylase levels in serum, CBD, an

Risk factor for extrahepatic bile duct cancer in patients with anomalous pancreaticobiliary ductal union.

BACKGROUND/AIMS: Anomalous connection between the choledochus and pancreatic duct is considered to be a factor in the development of biliary tract diseases such as choledochal cyst, pancreatitis, cholangitis, gallbladder cancer, and bile duct cancer. Our purpose was analysis of combined disease, especially biliary neoplasm and evaluated microscopic changes of extrahepatic bile ducts. METHODOLOGY: To study the clinical characteristics of anomalous pancreaticobiliary ductal union (APBDU), we reviewed 14 APBDU cases from June 1994 to June 1998. We studied the associated disease, surgical treatme

Laparoscopic cholecystectomy for triple gallbladder.

A very rare case involving true triplication of the gallbladder in a 38-year-old man with no other congenital abnormalities is reported. All three gallbladders had signs of chronic inflammatory disease and lithiasis. Acute cholecystitis and a double gallbladder were diagnosed preoperatively, but the diagnosis of a second accessory gallbladder was made only at the time of surgery. The patient underwent successful laparoscopic cholecystectomy. When a triplicate gallbladder is encountered, complete dissection of Calot's triangle and intraoperative cholangiographic evaluation should be performed t
