Machine learning-based prediction model for risk factors associated with conversion to open surgery in pediatric laparoscopic radical resection of choledochal cysts.

OBJECTIVE: To identify risk factors for conversion to open surgery during laparoscopic choledochal cyst excision in pediatric patients and develop a predictive model using machine learning. METHODS: We retrospectively analyzed 214 children who underwent laparoscopic excision between 2015 and 2023. Patients were classified into laparoscopic (n = 173) or conversion (n = 41) groups. Seven machine learning models were developed and evaluated using AUC and decision curve analysis (DCA). SHAP analysis identified top predictors, which were validated via multivariable logistic regression. A nomogram w

Preoperative nutritional risk and adverse perioperative outcomes in children with congenital choledochal cysts: a retrospective cohort study.

BACKGROUND: Congenital choledochal cysts (CCC) are rare biliary anomalies associated with significant morbidity. The impact of preoperative nutritional status on surgical outcomes in pediatric CCC patients remains unclear. This study aimed to investigate this relationship, using a validated nutritional risk screening tool to stratify patients. METHODS: We conducted a retrospective cohort study of pediatric patients who underwent CCC excision with Roux-en-Y hepaticojejunostomy at a single center between January 2011 and September 2025. Nutritional risk was assessed within 24 h of admission usi

Malignant transformation of choledochal cyst 38 years after cystoduodenostomy mimicking Lemmel's syndrome: a rare diagnostic pitfall.

Malignant transformation of choledochal cysts after internal drainage is a known risk, but presentation nearly four decades later is exceptional. We report a case of cholangiocarcinoma developing 38 years after cystoduodenostomy in a 39-year-old female. Diagnosis was confounded by a periampullary diverticulum, mimicking Lemmel's syndrome. Multimodal imaging suggested a hilar mass, but endoscopy revealed a large diverticulum. Exploratory laparotomy and frozen section analysis confirmed adenocarcinoma arising from the remnant cyst. The patient underwent complete cyst excision, lymphadenectomy, a

RNA-Binding Proteins Alter Redox Gene Splicing in Biliary Atresia: Insights from Expression Profiling.

BACKGROUND: Biliary atresia (BA), the most common cause of extrahepatic obstructive jaundice in infants, is a severe infant disease with a poor prognosis and unclear etiology. RNA-binding proteins (RBPs) are key regulators of alternative splicing and are implicated in various liver pathologies. However, whether RBP dysfunction and resultant aberrant splicing contribute to BA pathogenesis remains unknown. METHODS: We performed an integrated transcriptomic analysis using RNA-seq data from BA patients and normal donor livers (GEO: GSE159720). This encompassed systematic identification of differe

Feasibility and safety of transumbilical single-incision laparoendoscopic surgery for giant choledochal cysts in infants: a single-center retrospective study.

BACKGROUND: Radical resection of giant choledochal cysts (diameter ≥ 10 cm) in infants presents technical challenges, particularly when attempted with minimally invasive approaches. The primary difficulty lies in the cyst’s mass effect in the limited abdominal space. Although transumbilical laparoendoscopic single-site surgery (LESS) offers potential cosmetic and minimally invasive advantages, evidence regarding its feasibility and safety for managing these giant cysts remains limited. METHODS: This single-center, retrospective study with a historical control group included infants who underwe

Pediatric urethroscope versus flexible choledochoscope for removal of distal protein plugs and calculi in choledochal cyst: A prospective cohort study.

BACKGROUND: Choledochal cyst (CC) is a common congenital biliary malformation that occurs frequently in children. CC is often complicated by protein plugs, calculi, and other debris, predisposing patients to cholangitis, pancreatitis, and other adverse events. Thorough intraoperative removal of obstructive debris is critical for improving postoperative outcomes. At present, intraoperative endoscopy represents the main modality for removing distal obstructions; however, consensus regarding the comparative efficacy of pediatric urethroscope and flexible choledochoscope remains lacking. Development and validation of a preoperative nomogram for predicting postoperative high drainage output after robot-assisted Roux-en-Y hepaticojejunostomy for children with choledochal cysts: based on cyst diameter, age, and other indicators.

BACKGROUND: Robot-assisted Roux-en-Y hepaticojejunostomy (RYHJ) is the standard treatment for choledochal cysts (CDC). Routine abdominal drainage is typically placed, but no reliable preoperative tool exists to identify patients at risk of high drainage output (≥ 2 mL·kg⁻1·d⁻1), limiting early drain removal or selective omission under close monitoring. This study aimed to develop and validate an intuitive preoperative nomogram using readily available clinical indicators (e.g., cyst diameter, age) to predict high-drainage risk after robot-assisted RYHJ and support individualized drainage manage

Navigating the Giant: A Case Series of Giant Choledochal Cysts in Infants Managed Laparoscopically.

The incidence of giant choledochal cysts (CDCs) in infants is the rarest of rare, and poses unique surgical challenges due to limited working space and close proximity to vital hepatobiliary structures. With advancements in minimally invasive surgery, laparoscopic excision has become the preferred approach. We present a case series of four infants diagnosed with giant CDCs who underwent laparoscopic cyst excision with biliary reconstruction at our institution. All four patients presented with progressively increasing abdominal distension and were diagnosed using ultrasonography followed by mag

Surgical outcomes in neonates and infants with congenital choledochal malformation: analysis of the Pediatric Health Information System (PHIS) database.

BACKGROUND: Optimal timing for surgical correction of congenital choledochal malformations (CCMs) remains controversial. This study aims to evaluate whether younger age at surgery is an independent risk factor for higher morbidity. METHODS: The Pediatric Health Information System (PHIS) database (October 2015-September 2024) was queried to identify children aged ≤ 1 year undergoing definitive surgery for CCMs. Patient characteristics and outcomes of children aged ≤ 3 months (Group A) were compared with children > 3 months old (Group B). RESULTS: 267 patients (Group A:71, Group B:196) were iden

Phrygian cap deformity in a pediatric patient: a case report.

The overall prevalence of congenital causes of gallstones is not well established. The Phrygian cap is a congenital anomaly of the gallbladder with an incidence of 4%. The literature provides a few case reports on Phrygian cap deformity with or without associated gallstones. This report presents a 15-year-old Hispanic, overweight female patient transferred to our service due to an aborted laparoscopic cholecystectomy. During the initial surgery, there was a suspicion of choledochal/gallbladder cyst. Due to intraoperative and technical difficulties, the patient was referred to our pediatric sur

Pathophysiology Underlying Manifestation of Pancreaticobiliary Maljunction Throughout Life.

Pancreaticobiliary maljunction (PBM) is an anomaly in which the bile and pancreatic ducts join outside the area of action of the sphincter. PBM is typically accompanied by choledochal cysts (dilated PBM); however, cases of PBM without biliary dilatation (nondilated PBM) have also been reported. PBM manifests throughout life. The presentation remains unclear due to age-dependent differences in its pathophysiology. PBM symptoms may be attributed to biliary obstruction caused by substances that differ with age: calcium bilirubinate debris in neonates, protein plugs in children, and gallstones in

Giant Todani Type IA Choledochal Cyst in an Elderly Patient Successfully Managed With Roux-en-Y Hepaticojejunostomy: A Case Report.

Choledochal cysts are uncommon congenital anomalies of the biliary tract typically diagnosed during childhood. Presentation in elderly patients is rare and may mimic malignant biliary pathology. We report the case of a 78-year-old female with progressive abdominal discomfort and marked intra- and extrahepatic biliary dilatation. Imaging studies demonstrated a common bile duct measuring 30.9 mm with distal biliary stenosis. Endoscopic retrograde cholangiopancreatography failed to achieve biliary cannulation or stent placement, prompting definitive surgical management. Exploratory laparotomy ide
