Integrative transcriptomic and single-cell analysis reveals a convergent cholangiocyte-associated host inflammatory program in biliary atresia and SARS-CoV-2-associated hepatobiliary injury.

SARS-CoV-2-associated hepatobiliary injury and biliary atresia (BA) differ markedly in etiology and biological context, but both involve injury to the biliary epithelial compartment. Here, we performed integrative transcriptomic and single-cell analyses to investigate whether these distinct hepatobiliary injury contexts converge on a shared cholangiocyte-associated inflammatory host-response program. Bulk transcriptomic datasets from SARS-CoV-2-infected human liver organoids and BA liver tissues were integrated to identify shared transcriptional alterations, followed by functional enrichment,

Ambiguous Compliments and Encouragement: Young Fathers' Experiences of Care, Responsibility and Youth.

Caroli's disease is defined as a abnormal dilatation of the intra-hepatica bile ducts: Its incidence is extremely low (1 in 1,000,000 population) and in most of the cases the whole liver is interested and liver transplantation is the treatment of choice. In case of dilatation limited to the left or right lobe, liver resection can be performed. For many year the standard approach for liver resection has been a formal laparotomy by means of a large incision of abdomen that is characterized by significant post-operatie morbidity. More recently, minimally invasive, laparoscopic approach has been p

Implementing Continuous Nursing Care After Kasai Procedure in Infants with Biliary Atresia.

Biliary atresia (BA) is a severe cholangiopathy of infancy characterized by progressive obstruction of the extrahepatic bile ducts. Kasai portoenterostomy remains the standard initial treatment, but postoperative complications such as cholangitis, malnutrition, and progressive jaundice remain common and can compromise long-term outcomes. Conventional discharge guidance often lacks continuity, leading to poor family adherence and delayed recognition of complications once the infant returns home. To address this gap, we developed and implemented a standardized protocol for continuity of nursing

Genetic Analysis of Isolated Hepatic Ductal Plate Malformation Reveals Novel Variants and Insights into Phenotypic Correlation.

BACKGROUND: Polycystic liver disease and other manifestations of ductal plate malformation (DPM) may be incidental or indeed develop significant complications. The genetic basis of 'isolated' DPM differs from that associated with polycystic kidney disease and remains incompletely understood. AIMS: We sought to genetically characterize a sizeable single-center cohort of patients with 'isolated' DPM, defined as absence of polycystic kidney disease. METHODS: 55 consecutive patients with isolated DPM were analyzed retrospectively with next-generation sequencing used to identify genetic variants.

Localized cystic dilatation of the intrahepatic bile ducts as a distinct subtype of Caroli disease: a case report.

BACKGROUND: Caroli disease encompasses multiple distinct entities. We report two cases of localized cystic dilatation of the intrahepatic bile ducts exhibiting a unique morphology. CASE PRESENTATIONS: Both patients were Japanese, diagnosed with a cyst in the liver prenatally (27 weeks' gestation) and at 3 months of age. The dilatation was limited to the anterior section branch and its first upstream branches, and the cystically dilated bile ducts abruptly connected to normal-sized ducts on both the upstream and downstream sides. One case transiently had debris in the dilated duct during the n

Plasma fractionation in Latin America: a path towards sovereignty and sustainability in the supply of plasma-derived medicines.

PURPOSE: Increased reactive oxygen species (ROS) released by NADPH oxidase and inflammation are associated with arterial hypertension and eye diseases associated with high blood pressure, including glaucoma, retinopathies (e.g., age-related macular degeneration), and choroidopathies affecting ocular function; however, the mechanisms underlying these adverse outcomes remain undefined. The present study was designed to highlight the importance of oxidative stress in severe hypertension-related eye damage. METHODS: Male Wistar rats (n = 7, unless otherwise specified for specific experiments) wer

Prenatal Diagnosis of Caroli's Disease by Ultrasound and MRI Imaging.

OBJECTIVE: To present the imaging features of Caroli's disease (CD) on prenatal ultrasound and magnetic resonance imaging (MRI). METHODS: This was a retrospective case series of prenatally diagnosed CD between 2017 and 2024. Clinical data from these cases were collected and reviewed. RESULTS: Five fetuses with CD were included, three of which had a definite combination of ARPKD and suspected in the other 2. Prenatal ultrasonography revealed multiple intrahepatic bile duct dilatations in four fetuses, each of which displayed the "horn comb" sign in a cross-section of the liver. All five fetus

Therapeutic opportunities in polycystic kidney and liver disease through extracellular matrix dynamics.

Autosomal Dominant and Autosomal Recessive Polycystic Kidney Disease (ADPKD and ARPKD) are, respectively, common and rare forms of polycystic disorders, characterized by the formation and progressive growth of cysts from tubules in the kidneys and bile ducts in the liver. Alterations in the extracellular matrix (ECM) and in the activity of matrix metalloproteases (MMPs), both associated with fibrosis, have been shown to be important factors in cystic growth and progression of these diseases. We used tandem mass spectrometry (LC-MS/MS) to identify the most enriched proteins and pathways in an o

Identification of Biliary Atresia in Infantile Cholestasis: Integrating Radiomics With MRCP for Unobservable Extrahepatic Biliary Systems.

PURPOSE: Magnetic resonance cholangiopancreatography (MRCP) may assist in the workup of infantile cholestasis as nonvisualization of the biliary tree is seen with biliary atresia (BA). However, this finding can also be seen with other causes of infantile cholestasis. The purpose of this study is to differentiate BA from other causes of infantile cholestasis using a classification tool integrating MRCP-based radiomics and clinical signatures in patients with nonvisualization of the extrahepatic biliary tree on MRCP. METHODS: Data from infants with cholestasis due to BA, cytomegalovirus infecti

Adeno-associated Virus-mediated Silencing of Sox4 Leads to Long-Term Amelioration of Liver Phenotypes in Mouse Models of Alagille Syndrome.

BACKGROUND & AIMS: In patients with Alagille syndrome (ALGS), bile duct paucity often leads to severe cholestatic phenotypes for which liver transplantation remains the only definitive treatment. No Food and Drug Administration-approved mechanism-based strategies exist to enhance biliary development in ALGS or other diseases with bile duct paucity. We aimed to identify a therapeutic target to address this unmet need. METHODS: Preclinical ALGS mouse models lacking 1 copy of Jag1 with or without conditional deletion of 1 or both copies of Sox9 were used. Sox4 levels were reduced genetically or

Role of Mast Cells in Assessing the Degree of Fibrosis in Histopathological Scoring of Extrahepatic Biliary Atresia.

INTRODUCTION: Extrahepatic biliary atresia (EHBA) is a fibroinflammatory disorder characterised by the fibrosed development of extrahepatic biliary ducts, resulting in cholestatic jaundice. Inflammation and inflammatory cytokines have been implicated as causative factors in the development of fibrosis. The disease, in essence, is a rare entity with an incidence varying from 1 in 5000 to 1 in 18,000 live births in India. The role of inflammatory cells, such as mast cells, in causing fibrosis in liver biopsies is being researched in recent times to determine their impact on patient prognosis. A

Risk factors for anastomotic stenosis after congenital choledochal cyst surgery and efficacy analysis of laparoscopic reoperation.

BACKGROUND: Congenital choledochal cyst (CCC) is a common congenital biliary dysplasia in children, with higher incidence in Asia, treated by complete cyst excision plus Roux-en-Y hepaticojejunostomy (laparoscopy as preferred). Anastomotic stenosis, a common long-term postoperative complication, increases patient burden and treatment difficulty. Previous studies lacked systematic analysis of its influencing factors, and laparoscopic reoperation for it was once contraindicated, with only small-scale studies showing its efficacy comparable to open surgery. OBJECTIVE: To systematically analyze i

Diversity and heterogeneity in human pancreaticobiliary maljunction revealed by single-cell RNA sequencing.

PURPOSE: The etiology and pathogenesis of pancreaticobiliary maljunction (PBM) remain unclear, thus a comprehensive investigation of cellular diversity and microenvironmental differences is pivotal to elucidate the mechanisms driving PBM. METHODS: We performed single-cell RNA sequencing on bile duct tissues from six patients, including three with PBM and three without (non-PBM). Pathway enrichment, transcription factor analysis, and cell-cell communication were analyzed to explore cellular interactions and functional states. RESULTS: A total of 90,996 single cells and 11 distinct cell lineag

Cholangiocarcinoma - Morphology, Immunohistochemistry, and Genetics.

Cholangiocellular carcinoma (cholangiocarcinoma, CCA) is a heterogeneous group of malignant epithelial tumors of the bile ducts, with varying classifications and molecular characteristics. CCA can arise in intrahepatic, perihilar, or extrahepatic bile ducts, with its incidence rising globally, particularly in Southeast Asia due to liver fluke infections. Other risk factors include primary sclerosing cholangitis, viral hepatitis, gallstones, congenital bile duct malformations, and cirrhosis. CCA is often diagnosed at advanced stages and has a poor prognosis. This article summarizes the complex

Incidence, gross features, and histologic characterization of feline fibrocystic hepatobiliary lesions and extrahepatic biliary malformations.

Feline fibrocystic hepatobiliary lesions (FHLs) and extrahepatic biliary malformations (EBMs) are supposedly common, but prevalence studies and detailed gross and histologic descriptions within a large cohort are lacking. Our aims were to identify the prevalence of FHLs and EBMs within a large feline population, characterize gross and histologic features, and report comorbidities. Of 140 cats, 20 (14.3%) had either FHLs and/or EBMs. FHLs were most common (15/20, 75%) and included cystic biliary lesions (CBLs, 14/20, 70%) and congenital hepatic fibrosis (CHF, 1/20, 5%). CBLs had gross variabili

Causal links between congenital malformations, birth weight, and neuroblastoma: insights from Mendelian randomization.

BACKGROUND: While it is commonly observed that congenital malformations (CM), birth weight and neuroblastoma (NB) often co-occur clinically, there is a scarcity of studies investigating their relationship. AIM: To investigate the causal relationship between CM, birth weight and NB using two-sample mendelian randomization (MR) analysis. METHODS: This study utilized data from the Genome-Wide Association Studies (GWAS) database for CM, birth weight, and NB. We identified and analyzed single nucleotide polymorphisms (SNPs) related to CM from various organ systems, ensuring robust instrumental va

Bilateral Renal Fungal Bezoars and Perinephric Abscess in an Infant With Arthrogryposis-Renal Dysfunction-Cholestasis Syndrome: A Clinico-pathologic Case Report.

The patients with Arthrogryposis-Renal dysfunction-Cholestasis (ARC) syndrome have genetic susceptibility to the opportunistic infections due to the involvement of VPS33B (vacuolar protein sorting 33 homolog B) in phagolysosome fusion in macrophages. Detailed pathologic studies in ARC patients are missing in literature due to the lack of autopsy. We described the first autopsy case of ARC syndrome in a 2-month-old male infant. His death was due to recurrent sepsis and multiorgan failure despite the appropriate poly-antibiotic therapy and supportive care. The autopsy showed invasive renal candi

Double Common Bile Duct Associated With Choledochal Cyst: A Rare Developmental Anomaly.

Duplication of the extrahepatic bile duct is a highly uncommon congenital variation of biliary anatomy and may occur in conjunction with other biliary or pancreatic abnormalities. We describe the case of a 3-year-old girl who presented with abdominal discomfort and vomiting. Magnetic resonance cholangiopancreatography (MRCP) revealed a cystic lesion consistent with a choledochal cyst (CC). Intraoperatively, a double common bile duct (DCBD), an atypical anomaly characterized by duplication of the extrahepatic bile duct, was identified. Treatment involved unifying the duplicated ducts into a sin

Pancreaticobiliary Maljunction With a Non-Dilated Extrahepatic Bile Duct Masked by Adenomyomatosis: A Potential Cause of Unresectable Gallbladder Carcinoma.

Adenomyomatosis of the gallbladder (ADM) is generally considered benign. The presented cases were initially diagnosed with ADM via ultrasound during an annual health check-up and monitored for several years without further investigations due to the absence of significant changes or symptoms. Subsequently, one case was diagnosed with pancreaticobiliary maljunction (PBM) with a non-dilated extrahepatic bile duct (EBD) after symptom onset, and underwent prophylactic laparoscopic cholecystectomy to avoid the risk of carcinogenesis. Although the other case remained asymptomatic, ultrasound later re

Nationwide Expert Survey on Appropriate Bile Duct Transection Line in Adult and Pediatric Congenital Biliary Dilatation Surgery in Japan: AOB Consensus Meeting.

BACKGROUND: Resection of the extrahepatic bile duct including the gallbladder and the dilated segment, is a widely accepted surgical concept in congenital biliary dilatation (CBD) in both adult and pediatric surgeons. However, owing to the existence of various types of CBD and differences in the extent of dilatation, no consensus has been reached regarding the appropriate resection margins. METHODS: Consensus statements regarding the appropriate surgical margins for both the duodenal and hepatic hilum sides were developed as two clinical questions (CQs) during a multidisciplinary consensus me
