The authors describe a newborn, in which a diagnosis of ascites was made before birth by ultrasound. Ascites without other signs of hydrops may be caused by meconium-peritonitis, perforation of the bile ducts, disorders of the portal or systemic circulation, flow disturbances in the lymphatic vessels, urinary obstruction or a ruptured ovarian cyst. In this case there was a chylous ascites in a child with Down's syndrome. Diagnosis, pathogenesis and treatment are discussed. With conservative treatment the child was free of symptoms at the age of 6 months.

Anomalous pancreaticobiliary union was found in nine cases among 300 consecutive adult patients examined by endoscopic retrograde cholangiopancreatography. Three had congenital choledochal cysts: one had a cyst of the choledochus associated with a cyst of the intrahepatic bile duct, another had a fusiform dilation of the choledochus, and the third had a choledochal diverticulum. Five of the nine patients had biliary malignancies (55.6%): four carcinomas of the gallbladder and one carcinoma of the choledochus. On the other hand, 18 of 291 patients without anomalous pancreaticobiliary union had

Congenital cystic dilatation of the intrahepatic bile ducts--Caroli's syndrome--is a rare disease of which about one hundred cases have been reported in the literature. Based on two observations, the value of various radiological procedures is discussed. Compared with other noninvasive procedures, computer tomography has the advantage of being able to show not only the changes in the liver, but also the changes in the liver, but also the medullary sponge kidney which often accompanies this condition. The indications for ERC can therefore be more strictly defined.

Reduction of the number of interlobular bile ducts (BDP) has potentially significant physiologic consequences. Most of the disorders currently recognized as BDP involve an easily discernible reduction in duct numbers, but actual duct quantitation needs to be done routinely in the examination of liver biopsies in order to recognize and assay the importance of less marked reduction. Ducts are seldom if ever completely absent, and may not be uniformly reduced throughout the liver, so a wedge biopsy is the only completely satisfactory sample for the morphologic evaluation of duct numbers. In some

Arteriohepatic dysplasia (AHD) is a multisystem disorder which is characterized by liver, heart, eye, and bony abnormalities. A characteristic facies is also present. The etiology is not clear, and some authors have favored an in utero toxin or infection as the cause. Other reports have shown parent-to-child transmission suggesting an autosomal dominant form of inheritance. This study documents the presence of AHD characteristics in four successive generations of a single kindred. Overall, 15 of 24 members had at least some characteristics of AHD. In addition to the usual findings, renal disea

Fifteen patients, all of whom underwent surgery at this institution from 1955 to 1981, were included in this study. Bile duct cysts were classified into Types I to V to include cysts of the intra- and extra-hepatic bile ducts. The association of this entity with other congenital abnormalities is discussed. Carcinoma of the biliary tree occurred in two cases. The treatment of this condition is surgical. Excision is preferred whenever technically possible; otherwise a bypass procedure is performed. The technique of excision described by Lilly [32,34] is preferred because it is safer, decreasing

Initially a simple anatomical curiosity discovered at operation or autopsy, agenesis of the ball bladder has now, and despite its rarity, become of the greatest interest from a pathogenic, physiopathological and diagnostic standpoint. Embryogenesis explains that this a developmental abnormality affecting the hepatic diverticulum which is probably of genetic origin, hence its frequent association with other malformations and the hereditary nature of the condition. The usual finding of a syndrome of blockage of the sphincter of Oddi with dilatation of the common bile duct raises the problem of s

Three patients with extrahepatic biliary atresia associated with cyanotic congenital heart disease are presented and their clinical courses reviewed. In these patients, the coincidence of cyanosis and jaundice caused an atypical green-appearing cutaneous discoloration which masked the early clinical detection of biliary atresia in one infant and cyanotic heart disease in the other two. In all infants, either partial or complete correction of the congenital cardiac lesion was possible. Two infants died from hepatic disease despite attempted surgical revision and one has satisfactory bile draina

This is studies concerning the absorption and excretion of cefmetazole in neonates and the excretion into the bile of infants with congenital biliary atresia. 1. The concentration of cefmetazole was measured in the serum and urine of 2 neonates at the postoperative period. 2. The concentration measured in the serum showed a level similar to those of cases in pediatrics at other institutes regardless of the operation. 3. The recovery value of cefmetazole in the urine of the neonate is not different from those of other institutes even if it is a postoperative case, provided that the urine quanti

Thirteen infants and children from 42 days to 14 years old with hepatobiliary disease underwent Tc-99m-diethyl-IDA hepatobiliary imaging. Five patients had biliary atresia, four had biliary hypoplasia, two had neonatal hepatitis, and there were single examples of alpha-1-antitrypsin deficiency and congenital hepatic fibrosis. Tc-99m-diethyl-IDA imaging accurately differentiated biliary atresia (complete bile duct obstruction) from other forms of childhood hepatobiliary disease by demonstrating no intestinal radioactivity in the former and the presence of intestinal radioactivity in the latter.

The ocular findings in a father and two offspring with arteriohepatic dysplasia are reported in this paper. All three people had bilateral posterior embryotoxon, Axenfeld's anomaly and a pigmentary retinopathy. Other ocular findings were exotropia, an ectopic pupil, band keratopathy, choroidal folds, anomalous optic discs and infantile myopia. Persons with arteriohepatic dysplasia typically present with prolonged neonatal jaundice. Cognizance of the associated eye findings helps distinguish this syndrome from other types of familial intrahepatic cholestasis.

The papilla of Vater is subject to a variety of abnormalities that include congenital anomalies, inflammation, neoplasms, fibrosis, and motor dysfunction. Some of these lesions are clinically obvious (such as cancer or an impacted gallstone) and require aggressive therapy in order to provide relief from pain or biliary-pancreatic ductal obstruction. Other lesions, such as stenosing papillitis, are more subtle and provide few clues to their existence except recurrent episodes of abdominal pain. I have focused my discussion on the relative indications for sphincteroplasty and choledochoduodenost

The amylase level of bile and various cholangiograms was studied in 36 cases of congenital biliary dilatation. (C.B.D.) The amylase level above 10000 U/L was considered to suggest the existence of choledocho-pancreatic long common channel formation. Radiologically, the dilatation of intrahepatic bile duct was seen in 9 out of 19 (47%) Type I cases and in all (100%) Type II cases. The cystic dilatation of intrahepatic bile duct was seen in all Type I cases which have cystic choledochal dilatation. On the other hand, fusiform dilatation was seen in all Type II cases with fusiform choledochal dil

Clinical differential diagnosis of neonatal icteric diseases not infrequently appears to be difficult. Diagnostic means are limited. The needle biopsy of the liver has become an important aid in the diagnosis of neonatal liver diseases. Authors have analysed 21 biopsies obtained from children with congenital bile duct atresia and 8 biopsies from children with cholestatic hepatosis. Findings are compared with those revealed in neonatal hepatitis and other processes. The early changes (intracanalicular and intraductal cholestasis) are similar both in congenital bile duct atresia and in cholestat

Arteriohepatic dysplasia (AHD, Alagille's syndrome) is presumed to be one of the six known familial intrahepatic cholestatic syndromes, all of which present with neonatal jaundice or failure to thrive, or both. Accurate early diagnosis of the proper syndrome is important, as arteriohepatic dysplasia has a good prognosis, whereas the other syndromes usually lead to death in infancy or early adulthood. Posterior embryotoxon was found in all five of our patients and may be one of the hallmarks of this syndrome. Axenfeld's anomaly was present in three of five patients, and retinal pigmentary abnor

In 63 patients with malabsorption syndromes, 16 with congenital biliary atresia (BA) and 47 with cystic fibrosis (CF), axonal dystrophy in the gracile nucleus (ADG) was studied. Of the 16 patients with BA, ADG of considerable severity was observed in all 10 over one year of age. Of the 47 patients with CF, it was observed in 32, 61 and 80% of the cases in the first, second, and third decades, respectively. Evidence is presented that there has been a substantial decrease in the incidence of ADG in CF patients in recent years and that the decreased incidence is attributable to vitamin E (Aquasol

In a family with colonic polyposis and the typical associated findings of Gardner syndrome (osteomas and soft-tissue tumors), two and possibly four of the affected members developed periampullary malignancy. A review indicates that individuals with Gardner syndrome may have a 100- to 200-fold increased risk of developing periampullary carcinoma when compared to the general population. While certain families and certain individuals (those with other duodenal involvement, males and those with all of the characteristics of Gardner syndrome expressed) may be most susceptible, all patients with Gar

Two cases of ectopic openings of the common bile duct are reported. In one case there was a congenital short duct opening into DI, the presence of a spontaneous duodenobiliary reflux due to absence of the papilla being demonstrated by clinical and radiological examinations. The other patient had a long duct opening into D3 and no duodenobiliary reflux, but with an intact sphincter of Oddi explored in detail by means of combined choledochoduodenal manometry. These two cases demonstrate that the duodenal wall has only a moderate effect in preventing duodenobiliary reflux. The essential element i

A four year experience with the adaptation of the flexible fiberoptic endoscope to the intraoperative environment is presented in 30 patients. The technique of intraoperative endoscopy was utilized in a wide variety of difficult gastrointestinal surgical problems to include the location of the site and cause of bleeding of obscure etiology; resolution of intraoperative dilemmas without the necessity of opening abdominal viscera; resection of lesions during operations conducted for other pathological processes; and enhancement of diagnosis at laparotomy. There were no complications from the use
