Alpha-Fetoprotein as a Biomarker in Pregnancy: From Genetic Disorders to Obstetric Complications.

Alpha-fetoprotein (AFP) is a glycoprotein primarily produced by the fetal liver and yolk sac during development. It is a multifaceted biomarker with significant applications in the prenatal screening of congenital abnormalities, cancer, and other disorders. The level of AFP in maternal blood may indicate several obstetric concerns and complications during pregnancy. Atypical AFP levels are commonly utilized as a biomarker for detecting fetal anomalies, placental complications, and other pregnancy-related issues. These findings raise concerns regarding the effectiveness of screening maternal se

Single-Centre Experience of Gallbladder Cancer in the United Kingdom: Do Patients With Incidentally Discovered Malignancy After Gallbladder Surgery Have Better Outcomes?

Background Biliary tract cancers refer to a spectrum of invasive tumours, usually adenocarcinomas, arising from the gallbladder or cystic duct (gallbladder carcinoma or GBC) or the biliary tree (cholangiocarcinoma or CCA) and represent a minority of all human cancers. Risk factors of GBC are old age, female gender, and gallstones, which represent the strongest risk factor, in addition to congenital biliary tree malformations and obesity. Methods This was a retrospective review using electronic records of all the patients admitted at a single centre in the UK, between January 2014 and July 2024

Preliminary Report of a Nationwide Prospective Cohort of Congenital Diaphragmatic Hernia in Korea.

BACKGROUND: Most congenital diaphragmatic hernia (CDH) cases are identified prenatally, enabling early multidisciplinary interventions, including maternal transfer and preparation for extracorporeal membrane oxygenation (ECMO). These strategies may improve the historically stagnant survival rates in Korea. However, multicenter studies on the epidemiology and treatment outcomes of CDH are lacking. This study introduces the Korean CDH cohort (K-CDH), a nationwide prospective registry, and presents interim findings. METHODS: The cohort comprises data from 6 tertiary centers (4 in Seoul and 2 in

Integrated genomic and biochemical diagnosis of a novel homozygous start-loss variant in AKR1D1 associated with neonatal cholestasis.

INTRODUCTION: Congenital bile acid synthesis defects are rare autosomal recessive disorders that typically present in early infancy with cholestasis, progressive liver dysfunction, and, in severe cases, acute liver failure. These conditions may mimic other metabolic diseases detected in newborn screening, complicating early diagnosis. The AKR1D1 gene encodes Δ4-3-oxosteroid 5β-reductase, a key enzyme in primary bile acid synthesis, and pathogenic variants cause bile acid synthesis defect type 2 (OMIM #235555). CASE DESCRIPTION: We report a 3-month-old male infant with severe neonatal cholesta

Conjoined Parasitic Twins (Cephalo-Thoracopagus Parasiticus) in a Free Ranging Northern Bat (Eptesicus nilssonii): A Micro-CT Anatomic and Genetic Survey.

The detailed anatomical situation of male stillborn cephalo-thoracopagus twins in a wild ranging northern bat (Eptesicus nilssonii) is described by means of full body micro-CT scans in high resolution with three-dimensional computational reconstruction. The external anatomy was characterised by supernumerary wings and legs and marked cranial disfiguration. The most striking features of the internal anatomy were the presence of two hearts with different characteristics, three lungs with two tracheas, three kidneys as well as a doubled liver. Vascular aberrations and other organ deformities are

Assessing liver disease in cyanotic CHD using multiparametric MRI: a call to prevent future burden.

BACKGROUND & AIMS: Patients with cyanotic CHD and those with metabolic dysfunction-associated steatotic liver disease are at risk of liver fibrosis. We compared hepatic extracellular volumes and native T1 values to better understand the burden of liver disease in these populations. METHODS: The sample comprised 136 patients in 5 groups: control (n = 23), metabolic dysfunction-associated steatotic liver disease [mild (F0-F1) and significant (F2-F4) fibrosis; n = 45], repaired tetralogy of Fallot (n = 30), and Fontan circulation (n = 38). Differences were assessed using linear regression models

Staged compressive dressing method for primary giant omphalocele repair: a low-cost and successful strategy.

PURPOSE: In giant omphalocele cases, small abdominal cavity leads to high intraabdominal pressure during closure, making repair challenging. This study aimed to evaluate our staged compressive dressing technique and its outcomes. METHODS: Eighteen giant omphalocele cases treated with staged compressive dressing/primary closure between 2014 and 2024 were retrospectively reviewed. All cases received compressive dressing every other day with wet sterile gauze and transparent adhesive surgical drapes from the first postnatal day until surgery to reduce intra-sac organs. Primary closure was perfor

Understanding and managing dextrocardia with situs inversus: A case report.

Dextrocardia with situs inversus is an uncommon congenital disorder in which the heart and other organs in the chest and abdomen are reversed. Although it rarely causes symptoms, it can be linked to heart or systemic disorders and is challenging to diagnose due to its unique architecture. The present report describes the case of a 27-year-old man who presented with sudden breathing problems. A physical examination revealed no heartbeat on the left side and odd cardiac noises. Further electrocardiogram tests revealed classic symptoms of dextrocardia, including right axis deviation and inverted

Ambiguous Compliments and Encouragement: Young Fathers' Experiences of Care, Responsibility and Youth.

Caroli's disease is defined as a abnormal dilatation of the intra-hepatica bile ducts: Its incidence is extremely low (1 in 1,000,000 population) and in most of the cases the whole liver is interested and liver transplantation is the treatment of choice. In case of dilatation limited to the left or right lobe, liver resection can be performed. For many year the standard approach for liver resection has been a formal laparotomy by means of a large incision of abdomen that is characterized by significant post-operatie morbidity. More recently, minimally invasive, laparoscopic approach has been p

DLK1 is a GATA1s-driven dependency and therapeutic target in Down syndrome-associated myeloid leukemia.

Children with Down syndrome have a markedly increased risk of developing myeloid leukemia. Although having an excellent prognosis, 10% to 20% develop relapsed or refractory disease with poor survival, highlighting the need for new targeted approaches. The pathogenesis of myeloid leukemia of Down syndrome (ML-DS) is tightly linked to fetal hematopoiesis and mutations in GATA1, generating the truncated GATA1 short (GATA1s) isoform. We identified Delta-like noncanonical Notch ligand 1 (DLK1) as a direct GATA1s target. DLK1, a paternally imprinted transmembrane protein, is highly expressed in feta

Prenatal ultrasound diagnosis of a rare morgagni hernia: a case report.

BACKGROUND: Morgagni hernia is a rare type of congenital diaphragmatic hernia, and its prenatal diagnosis by ultrasound is extremely challenging, especially when combined with massive pericardial effusion, as it can be easily confused with other diaphragmatic hernias. This article reports a case of fetal Morgagni hernia definitively diagnosed by serial prenatal ultrasound and magnetic resonance imaging, followed by successful intrauterine pericardial drainage and postnatal surgical correction. It aims to emphasize the importance of standardized imaging follow-up for prenatal identification and

Genomic and transcriptomic profiling of hepatocellular carcinoma in patients with Fontan-associated liver disease.

BACKGROUND AND AIMS: Fontan-associated liver disease (FALD) could lead to liver cirrhosis and hepatocellular carcinoma (HCC). The prognosis of FALD patients is worse when HCC develops. Therefore, a better understanding of the mechanisms of FALD hepatocarcinogenesis is critical. We investigated the genomic and transcriptomic signatures of FALD HCC. APPROACH AND RESULTS: Ten patients with FALD HCCs, 2 patients with non-Fontan congenital heart surgery (CHS) HCC, and 1 patient with FALD focal nodular hyperplasia (FNH) were enrolled. Whole-genome sequencing and transcriptome sequencing were perfor
