AIM: Biliary atresia (BA) has a known association with other anomalies and specifically the Biliary Atresia Splenic Malformation syndrome (BASM) in about 10-20% of infants in Western series. We aimed to analyse the effect of BASM and other anomalies on long-term outcome.
METHODS: Single-centre prospective database, using 2:1 isolated BA (IBA) as contemporaneous controls. The effect of various sub-groups (specifically cardiac) was also analysed. Clearance of jaundice (COJ) post Kasai-portoenterostomy (KPE) was defined as bilirubin <20 μmol/L. Overall survival (OS) and native liver survival (NL

BACKGROUND: Biliary atresia (BA) is the leading cause of cholestatic jaundice in the first months of life. Liver stiffness measurement by shear wave elastography (2D-SWE) could help discriminate BA from other causes of cholestasis.
OBJECTIVES: To assess the use of abdominal ultrasound with bidimensional shear wave elastography and liver histology to diagnose Biliary Atresia in cholestatic infants. To compare the use of elastography to estimate the stage of liver fibrosis with the histologic classification.
METHODS: Cholestatic infants younger than three months were divided into BA and non-BA

Pediatric populations differ from adults in drug elimination capacity. While current scaling methods account for enzyme and transporter maturation, they overlook comorbidities, such as biliary atresia (BA), a liver disease appearing within the first 2-8 weeks of life that can progress to cirrhosis. Such conditions may impair hepatic drug clearance, requiring dose adjustments. Physiologically based pharmacokinetic (PBPK) tools aim to address such cases and have been advocated to fill gaps in clinical data instead of less formalized and evidence-based guesswork. However, the paucity of systems d

OBJECTIVES: To establish a semi-quantitative hepatic subcapsular flow (HSF) score using color Doppler ultrasonography (CDUS) and evaluate the performance of both the HSF score and hepatic capsular retraction (HCR) sign for identifying biliary atresia (BA), while simultaneously correlating with liver fibrosis.
MATERIALS AND METHODS: This study prospectively recruited 170 infants (35 BA and 135 non-BA; 124 males and 46 females) with a median age of 50 days (interquartile range 35-71). Multimodal ultrasound (grayscale ultrasound, CDUS, elastography) was utilized to evaluate the HSF score, HCR si

BACKGROUND: Historical pediatric data recommend surgical resection for type I choledochal cysts because of the risk of malignant progression over time (up to 60%). With the widespread use of high-resolution cross-sectional imaging, isolated dilation of the extrahepatic biliary tree has become more frequently observed. The pediatric management strategy for choledochal cysts has been extrapolated to adults, without any specific long-term data. This study aimed to characterize the natural history of choledochal cysts in adults.
METHODS: We have updated our previously published institutional seri

PURPOSE OF REVIEW: Timely diagnosis of biliary atresia remains critical in infants with neonatal cholestasis, as delays in recognition are associated with reduced native liver survival and poorer outcomes following hepatoportoenterostomy. However, distinguishing biliary atresia from other causes of neonatal cholestasis remains challenging because of overlapping clinical, laboratory, and imaging features. This review highlights emerging diagnostic strategies aimed at improving earlier and more accurate identification of biliary atresia.
RECENT FINDINGS: Recent advances include development of s

BACKGROUND: Vascular anomalies are rare, heterogeneous disorders that threaten the health of children. Sirolimus is an important drug therapy, but its high incidence of adverse drug reactions (ADRs) may cause treatment discontinuation and therefore poor disease control. Although prior studies suggest a strong link between sirolimus exposure and ADRs, the quantitative relationship remains unclear. This study aimed to systematically quantify the relationships between steady-state trough sirolimus concentrations (Cmin) and three ADRs: myelosuppression, abnormal liver function, and dyslipidemia.

OBJECTIVES: Integration of the treatment for biliary atresia, including Kasai portoenterostomy and liver transplant, has had an important role in the marked improvement of the prognosis. We reviewed our experience with Kasai portoenterostomy and living donor liver transplant, both performed by the same pediatric surgical team and described the clinical features and outcomes of patients with biliary atresia.
MATERIALS AND METHODS: We reviewed 41 patients with biliary atresia who underwent Kasai portoenterostomy at Kumamoto University from August 2000 to March 2017.
RESULTS: The median age at

BACKGROUND AND AIMS: Hereditary Fructose Intolerance (HFI), a rare autosomal recessive metabolic disorder, has historically been considered benign when treated with a lifelong fructose-, sucrose and sorbitol-restricted diet. However, adverse metabolic manifestations have recently been reported in treated HFI patients. As serum metabolomics offers a valuable tool for assessing metabolic manifestations underlying inherited metabolic disorders, we aim to compare the serum lipidomic profile of HFI-treated patients with age-, gender-, and body mass index-matched healthy controls.
METHODS: Long-ter

An early adolescent male from a consanguineous family presented with progressive neurological deterioration, including developmental delay, seizures, left hemiparesis and cognitive decline from early childhood. He subsequently developed gastrointestinal bleeding, chronic liver disease with oesophageal varices, severe anaemia and thrombocytopenia. Initial investigations suggested TORCH (Toxoplasmosis, Other, Rubella, Cytomegalovirus, Herpes simplex) infection due to intracranial calcifications and multisystem involvement. However, detailed neuroimaging revealed extensive periventricular calcifi

Hereditary sensory neuropathy type IA (HSN1A) is a rare neurodegenerative condition caused by dominant mutations in the Serine Palmitoyl Transferase Long Chain base subunit 1 (SPTLC1) gene. There is no treatment available. Allele-specific silencing by antisense oligonucleotides (ASOs) to preferentially silence the mutant transcripts has shown therapeutic promise for dominant gain-of-function genetic disorders. In this study, we validated an allele-specific ASO therapy to selectively silence mutant SPTLC1 (p.S331F) in a disease mouse model carrying a heterozygous p.S331F mutation (S331F mice).

BACKGROUND: Extrahepatic portal vein obstruction (EHPVO) can be idiopathic or secondary to congenital abnormalities and other conditions in the pediatric population. The severity of the disease can be accurately determined by the precise degree of hepatic fibrosis. Most cases result from portal vein thrombosis (PVT). This study aimed to evaluate the accuracy of liver stiffness measurement (LSM) compared with liver biopsy for assessing liver fibrosis in patients with PVT.
METHODS: This retrospective cross-sectional study included children aged 3-18 years with EHPVO/PVT who were referred to the

BACKGROUND: Polycystic liver disease and other manifestations of ductal plate malformation (DPM) may be incidental or indeed develop significant complications. The genetic basis of 'isolated' DPM differs from that associated with polycystic kidney disease and remains incompletely understood.
AIMS: We sought to genetically characterize a sizeable single-center cohort of patients with 'isolated' DPM, defined as absence of polycystic kidney disease.
METHODS: 55 consecutive patients with isolated DPM were analyzed retrospectively with next-generation sequencing used to identify genetic variants.

BACKGROUND: Caroli disease encompasses multiple distinct entities. We report two cases of localized cystic dilatation of the intrahepatic bile ducts exhibiting a unique morphology.
CASE PRESENTATIONS: Both patients were Japanese, diagnosed with a cyst in the liver prenatally (27 weeks' gestation) and at 3 months of age. The dilatation was limited to the anterior section branch and its first upstream branches, and the cystically dilated bile ducts abruptly connected to normal-sized ducts on both the upstream and downstream sides. One case transiently had debris in the dilated duct during the n

Caroli syndrome (CS) is a rare congenital fibropolycystic liver disorder characterized by non-obstructive intrahepatic bile duct dilatation and congenital hepatic fibrosis, often masquerading as fever of unknown origin. A 38-year-old Hispanic male presented with four weeks of intermittent high-grade fevers and right upper quadrant pain following recent travel to Mexico. An extensive infectious workup was unrevealing, while inflammatory markers were markedly elevated despite normal liver enzyme levels. Cross-sectional imaging revealed multiple intrahepatic cystic dilatations with biliary ectasi

The increasing availability and decreasing costs of DNA sequencing have resulted in the re-grouping of rare, severe paediatric cases of progressive familial intrahepatic cholestasis (PFIC) with more frequent, later-onset cases of cholestasis (eg, intrahepatic cholestasis of pregnancy, benign recurrent intrahepatic cholestasis, low phospholipid-associated cholelithiasis) under the umbrella of genetic cholestasis. The common denominator is the presence of functional variants in the PFIC-associated genes, predominantly in ABCB4, ABCB11 and ATP8B1, which cause PFIC types 1-3. Several other congeni

Paediatric short bowel syndrome (SBS) with chronic intestinal failure (IF) is a rare, complex and potentially life-threatening condition. The main causes in children are necrotizing enterocolitis, volvulus, congenital malformations, and other enteropathies. The resulting reduced intestinal absorption capacity often requires parenteral nutrition (PN), which is associated with complications such as catheter infections, liver disease, and thrombosis, as well as significant limitations in quality of life. The goal of multidisciplinary treatment is to achieve enteral autonomy, prevent complications

Hereditary haemorrhagic telangiectasia (HHT) is an autosomal dominant disorder with an estimated prevalence of one in 5000 to one in 7000. Pathogenic variants in three genes, endoglin (ENG), activin receptor-like kinase 1 (ACVRL1) and mothers against decapentaplegic homolog 4 (SMAD4), all part of the transforming growth factor-β signalling pathway, account for over 90% of HHT cases. Clinically, HHT is characterised by recurrent epistaxis, gastrointestinal bleeding from mucocutaneous telangiectasias, and visceral arteriovenous malformations (AVMs), most commonly affecting the lungs, liver and b

BACKGROUND: Early diagnosis is the most critical determinant of native liver survival in infants with biliary atresia (BA). Accurate differentiation of BA from other neonatal cholestatic disorders remains particularly challenging within the first 28 days of life, a period during which clinical and biochemical features are often nonspecific. This study aimed to develop and validate a minimalist, highly interpretable diagnostic model that integrates serum matrix metalloproteinase-7 (MMP-7) with routinely available clinical parameters to facilitate early BA diagnosis in neonates.
