Autosomal Dominant Polycystic Kidney Disease: Core Curriculum 2025.

Autosomal dominant polycystic kidney disease (ADPKD) is the most common genetic cause of kidney failure. Scientific advances have improved the approach to diagnosis, prognosis, and management of renal and extrarenal manifestations. The combination of total kidney volume, kidney function, and the genetic mutation (if known), predicts risk for progression to kidney failure, thereby identifying patients in whom disease modifying therapy is recommended. Currently there is one therapy approved by the US Food and Drug Administration (FDA) for slowing ADPKD progression, the V2 receptor antagonist, to

A novel frameshift variation of PKD1 in familial autosomal dominant polycystic kidney diseases: expanding the clinical phenotype and genetic spectrum of PKD1 disorders.

BACKGROUND: Autosomal dominant polycystic kidney disease is a common monogenic hereditary kidney disorder. Mutations in the PKD1 gene lead to the development of fluid-filled cysts in the kidney, liver, and other organs, which can eventually result in end-stage renal failure. Affected individuals have a 50% chance of passing the mutation to each of their offspring. Families with ADPKD are advised to undergo genetic testing to causative variants and would benefit from genetic counseling to reduce the inherited risk. METHOD: We performed genetic analysis on a family with ADPKD. DNA samples extrac

Accumulation of altered serum bile acids predicts liver injury after portoenterostomy in biliary atresia.

BACKGROUND & AIMS: Little is known about the mechanism of liver injury mediated by bile acids following Kasai portoenterostomy (KPE) for biliary atresia (BA). We sought to quantify individual serum bile acids, 7-alpha-hydroxy-4-cholesten-3-one (C4), and fibroblast growth factor 19 (FGF19) after KPE and to evaluate their prognostic utility and transcriptomic regulation. METHODS: Serum (n = 244) and liver specimens (n = 105) prospectively obtained from patients with BA (n = 54) after KPE were included. Bile acids were analyzed using mass spectrometry, gene expression with quantitative PCR, and

Expression Patterns of Escape Genes in Turner Syndrome Fibroblasts and Induced Pluripotent Stem Cells.

Turner syndrome (TS) is an X monosomy-related disorder caused by X chromosome nondisjunction during embryonic development. Patients with TS have only one intact X chromosome, with the other either completely or partially lost. TS affects various tissues, including the liver, kidneys, brain, cardiovascular system, and ovaries. These abnormalities are suggested to involve an altered dosage of escape genes that evade X chromosome inactivation. However, the mechanisms and roles of these escape genes in the TS phenotype remain unclear. We hypothesized that the expression levels of escape genes diff

Serum FGF19 combined with GGT and other biomarkers predicted native liver survival following Kasai portoenterostomy in early biliary atresia.

BACKGROUND: Accurately predicting the prognosis of biliary atresia (BA) prior to Kasai portoenterostomy (KPE) remains a challenge. The identification of the specific BA population that may benefit from primary liver transplantation (pLT) instead of KPE remains elusive. METHODS: A total of 196 BA patients and 31 age-matched non-BA cholestasis patients were recruited. BA patients were divided into training (February 2018-February 2019) and validation (March 2019-December 2021) cohorts. C-index was applied to evaluate the utility of indicators and models. RESULTS: Serum fibroblast growth factor

Impact of Hepatoblastoma on Infectious Complications Following Pediatric Liver Transplantation.

BACKGROUND: Liver transplantation is the standard therapy for end-stage liver disease in pediatric patients with biliary atresia (BA), congenital and metabolic conditions, and for an unresectable malignant tumor like hepatoblastoma (HB). BA is the leading indication for pediatric liver transplantation, while HB is the most common childhood liver cancer. Despite improved outcomes through advanced surgical techniques and novel immunosuppression, pediatric liver transplantation (pLT) is complicated by post-transplant infections. METHODS: A retrospective review was performed of pLT recipients at

Successful long-term survival of patients with trisomy 18 and hepatoblastoma after cardiac surgery.

BACKGROUND: Most patients with trisomy 18 (18T) die of cardiac or other severe anomalies within the first few years. Although surgical intervention for cardiac lesions or other diseases has improved these patients' prognoses in recent years, some survivors face the risk of developing neoplasms, such as hepatoblastoma. However, the treatment strategy remains controversial because of the poor prognosis. We evaluated the merits of aggressive therapeutic interventions for malignant tumors in 18T patients. METHODS: We encountered five patients who underwent hepatoblastoma treatment within the last

Mutation of CHD7 impairs the output of neuroepithelium transition that is reversed by the inhibition of EZH2.

Haploinsufficiency of CHD7 (Chromo-Helicase-DNA binding protein 7) causes a severe congenital disease CHARGE syndrome. Brain anomaly such as microcephaly and olfactory bulb agenesis seen in CHARGE patients have not been mimicked in previous animal models. Here, we uncover an indispensable function of CHD7 in the neuroepithelium (NE) but not in the neural stem cells (NSCs) after NE transition. Loss of Chd7 in mouse NE resulted in CHARGE-like brain anomalies due to reduced proliferation and differentiation of neural stem and progenitor cells, which were recapitulated in CHD7 KO human forebrain o

Prevalence of portal vein thrombosis in pediatric patients with cirrhosis and intrahepatic non-cirrhotic portal hypertension.

INTRODUCTION: Portal vein thrombosis (PVT) has been increasingly diagnosed in pediatric patients owing to the widespread use of non-invasive radiological techniques. Although the prevalence of PVT in adults with cirrhosis and intrahepatic non-cirrhotic portal hypertension ranges from 0.6 to 26 % and 13 to 46 %, respectively, no available data exist in the pediatric population. The prevalence of PVT in children with cirrhotic and intrahepatic non-cirrhotic portal hypertension was evaluated in this study. METHODS: This retrospective study included children with cirrhosis and intrahepatic non-ci

Long-Term Reduction of Liver Fibrosis Surrogates in Syndromic Biliary Atresia.

AIM: Biliary atresia (BA) has a known association with other congenital anomalies and specifically the Biliary Atresia Splenic Malformation syndrome (BASM). Although initial response to the Kasai portoenterostomy (KPE) appear similar, it seems that they have less liver fibrosis in the longer term. We aimed to test this hypothesis. METHODS: Single-centre prospective database with a 2:1 ratio of contemporaneous controls. We used native liver survivors (at 3+ yrs) as the principal comparison. Three surrogate markers of liver fibrosis were used: AST-to-Platelet-index (APRi), Varices Prediction Ru

Situs Inversus Totalis in Conjunction with an Anomalous Artery Connecting the Common Hepatic Artery and a Jejunal Branch of the Superior Mesenteric Artery: A Cadaveric Case Report.

We encountered a case of situs inversus totalis in conjunction with an anomalous artery connecting the common hepatic artery and a jejunal branch of the superior mesenteric artery during anatomical dissection of a cadaver. The heart was located in the center of the thorax, with the apex located anterioinferiorly to the right of the midline and the base located posterosuperiorly to the left of the midline. The aortic arch coursed from right anterior to right posterior. The right lung was divided into two lobes, and the left lung was divided into three. The entire intraperitoneal gastrointestina

Congenital absence of the portal vein identified following the onset of esophageal-gastric variceal rupture.

A 36-year-old woman with an unremarkable medical history was admitted to our hospital owing to hematemesis. Upper gastrointestinal endoscopy revealed F3 esophageal varices and F3 gastric varices with erosions, which were treated with endoscopic injection sclerotherapy with histoacryl. Contrast-enhanced computed tomography showed that the superior mesenteric vein and splenic vein converged and drained into the inferior vena cava, with no identifiable intrahepatic portal vein branches. Additionally, collateral circulation and thrombosis in the superior mesenteric vein were identified. On the bas

Spontaneous Bowel Evisceration Through Umbilical Hernia Sites in Adult Patients: A Systematic Review of the Literature.

BACKGROUND: The literature reports few instances of spontaneous bowel eviscerations through umbilical hernia sites. Spontaneous rupture of the hernia sac is a less common complication, primarily associated with persistent ascites or congenital wall defects. MATERIALS AND METHODS: A systematic review was conducted using the PubMed database-the United States National Library of Medicine, with the search terms "spontaneous bowel evisceration" and "umbilical hernia evisceration". However, several results were deemed unsuitable for this manuscript. From a total of 185 cases, this review was narrow

Multi-ancestry genome-wide association meta-analysis identifies novel associations and informs genetic risk prediction for Hirschsprung disease.

BACKGROUND: Hirschsprung disease (HSCR) is a rare, congenital disease characterized by the absence of enteric ganglia in the hindgut. Common genetic variation contributes substantially to the heritability of the disease yet only three HSCR-associated loci were identified from genome-wide association studies (GWAS) thus far. METHODS: We performed the largest multi-ancestry meta-analysis of GWAS to date, totalling 1250 HSCR cases and 7140 controls. Prioritized candidate genes were further characterized using single-cell transcriptomic data of developing human and mouse gut for their roles in de

Subcutaneous administration of adipose-tropic gene therapy for congenital leptin deficiency.

AAV-based gene therapy targeting adipose tissue has been underdeveloped due to lack of adipose-tropic AAV vectors with sufficient transduction efficiency. We previously demonstrated that an engineered capsid variant of Rec2 capsid with F503Y, Y708D and K709I substitution (named V7 capsid) exhibited highly selective adipo-tropism while ablating liver transduction upon intraperitoneal injection or intravenous injection. In this study, we investigated the feasibility of subcutaneous administration of V7 vector harboring human leptin (V7-LEP) in a congenital leptin deficiency model ob/ob mice. Sub

Ocular Involvement in Post-Transplantation Lymphoproliferative Disorder: A Rare Case with Rituximab Hypersensitivity.

BACKGROUND Post-transplantation lymphoproliferative disorder (PTLD) is a heterogeneous group of lymphoid proliferative conditions that occur in transplant recipients under sustained immunosuppression. Rituximab is widely used as first-line therapy for PTLD. However, hypersensitivity reactions to rituximab are rarely reported in pediatric patients. CASE REPORT A 1-year-old boy underwent living-donor liver transplantation for biliary atresia using his mother's graft. Six years after transplantation, he presented with mild graft rejection and abnormal liver function tests. Cervical lymphadenopath

Chronic kidney disease- mineral and bone disorder in autosomal dominant policystic kidney disease.

Autosomal dominant polycystic kidney disease (ADPKD) is the most common genetic disorder characterized by bilateral renal cysts that detach from parental tubules, progressively leading to renal function loss. Patients with ADPKD represent a unique subset of chronic kidney disease (CKD) individuals, sharing common CKD features while also exhibiting distinct traits specific to ADPKD. This review described the unique patterns of mineral and bone disorders present in ADPKD in the early CKD stages. The principal points are: the extraosseous FGF23 production by cystic tissues in the kidneys and live

Novel Reassortants of Oropouche Virus (OROV) Are Causing Maternal-Fetal Infection During Pregnancy, Stillbirth, Congenital Microcephaly and Malformation Syndromes.

Oropouche virus (OROV) is an orthobunyavirus endemic in the Brazilian Amazon that has caused numerous outbreaks of febrile disease since its discovery in 1955. During 2024, Oropouche fever spread from the endemic regions of Brazil into non-endemic areas and other Latin American and Caribbean countries, resulting in 13,014 confirmed infections. Similarly to other orthobunyaviruses, OROV can undergo genetic reassortment events with itself as well as other viruses. This occurred during this current outbreak, resulting in novel strains with increased pathogenicity and levels of transmission. For t
