The Christianson syndrome protein, sodium hydrogen exchanger isoform 6, is required for fat accumulation.

Mutations in NHE6 (human gene name SLC9A6) lead to the rare X-linked disorder Christianson syndrome (CS) characterized by intellectual disability, autism and ataxia. NHE6 is a Na+/H+ exchanger that regulates pH and ionic equilibria across endosomal membranes in a variety of tissues, although its non-neuronal role is understudied. A common co-morbidity in CS is low body weight, with body mass index in the 3-5th percentile range. In the present study, we use a Nhe6KO mouse model of CS to demonstrate a role for NHE6 in fat accumulation and glucose homeostasis. On both chow and a high-fat diet, Nh

Intestinal failure outcomes in children with small-intestinal Hirschsprung disease: A matched study.

INTRODUCTION: Small intestinal-Hirschsprung disease (SI-HD) causing intestinal failure (IF) leads to parenteral nutrition (PN) dependence, liver disease (IFALD), and infections (CLABSI). We aim to identify factors associated with enteral autonomy in patients with SI-HD. METHODS: Retrospective cohort study of 25 patients with IF from SI-HD matched on expected small bowel length and rehabilitation duration to patients with IF of other etiologies. RESULTS: SI-HD patients required 53.6 months (31.1, 97.7) of PN versus 22.7 months (9.4, 54.9) for non-SI-HD patients (p = 0.015). Patients with SI-H

Cholangiocarcinoma - Morphology, Immunohistochemistry, and Genetics.

Cholangiocellular carcinoma (cholangiocarcinoma, CCA) is a heterogeneous group of malignant epithelial tumors of the bile ducts, with varying classifications and molecular characteristics. CCA can arise in intrahepatic, perihilar, or extrahepatic bile ducts, with its incidence rising globally, particularly in Southeast Asia due to liver fluke infections. Other risk factors include primary sclerosing cholangitis, viral hepatitis, gallstones, congenital bile duct malformations, and cirrhosis. CCA is often diagnosed at advanced stages and has a poor prognosis. This article summarizes the complex

Untargeted Plasma Metabolomics Extends the Biomarker Profile of Mitochondrial Neurogastrointestinal Encephalomyopathy.

Mitochondrial neurogastrointestinal encephalomyopathy (MNGIE) is caused by pathogenic mutations in the nuclear TYMP gene, which encodes the cytosolic enzyme thymidine phosphorylase. In addition to the systemic accumulation of thymidine and deoxyuridine, several case studies have reported abnormalities in a range of other metabolites in patients with MNGIE. Since metabolites are intermediates or end-products of numerous biochemical reactions, they serve as highly informative indicators of an organism's metabolic activity. This study aimed to perform an untargeted metabolomic profiling to determ

Incidence, gross features, and histologic characterization of feline fibrocystic hepatobiliary lesions and extrahepatic biliary malformations.

Feline fibrocystic hepatobiliary lesions (FHLs) and extrahepatic biliary malformations (EBMs) are supposedly common, but prevalence studies and detailed gross and histologic descriptions within a large cohort are lacking. Our aims were to identify the prevalence of FHLs and EBMs within a large feline population, characterize gross and histologic features, and report comorbidities. Of 140 cats, 20 (14.3%) had either FHLs and/or EBMs. FHLs were most common (15/20, 75%) and included cystic biliary lesions (CBLs, 14/20, 70%) and congenital hepatic fibrosis (CHF, 1/20, 5%). CBLs had gross variabili

Causal links between congenital malformations, birth weight, and neuroblastoma: insights from Mendelian randomization.

BACKGROUND: While it is commonly observed that congenital malformations (CM), birth weight and neuroblastoma (NB) often co-occur clinically, there is a scarcity of studies investigating their relationship. AIM: To investigate the causal relationship between CM, birth weight and NB using two-sample mendelian randomization (MR) analysis. METHODS: This study utilized data from the Genome-Wide Association Studies (GWAS) database for CM, birth weight, and NB. We identified and analyzed single nucleotide polymorphisms (SNPs) related to CM from various organ systems, ensuring robust instrumental va

Ectopic Liver Mimicking Neoplastic Lesions: Report of Five Patients and Literature Review.

Ectopic liver is a rare congenital anomaly characterized by the presence of liver tissue outside the normal liver without any direct connection to it. These lesions are usually detected incidentally during clinical evaluation for other issues. However, they sometimes mimic neoplastic lesions both clinically and radiologically, which poses diagnostic challenges. Here, we present 5 patients with ectopic liver, 4 of which mimicked neoplastic lesions. The first was a 54-year-old woman diagnosed with multiple liver lesions and an enlarged gastro-hepatic lymph node, suspected to be a primary hepatic

Cholecystohepatic duct (type IIIA) - fifth report of aberrant biliary pathology in a paediatric case of oesophageal atresia and imperforate anus with review of literature.

It is known that the presence of associated anomalies has become one of the most critical prognostic factor for infants born with oesophageal atresia (OA). Associated VACTERAL group of abnormalities are common, but other associations are rare. Anomalies of the hepatobiliary system are even more rare among other groups. There are only a few such cases in the literature including adults and paediatric population. We describe the findings and management of one infant born with OA with tracheoesophageal fistula (TEF) and Ano rectal malformation (ARM), who was later shown to have features of obstru

Severe neonatal cholestasis in HNF1β deficiency: a case report and literature review.

Neonatal cholestasis can be caused by several conditions, with biliary atresia being the major cause. Genetic and endocrinological etiologies represent other possibilities, with most of them requiring a rapid diagnosis and a specific treatment. We describe a neonatal case of severe cholestasis with low gamma glutamyl transferase in a child presenting with multiple abnormalities, including pituitary stalk interruption syndrome and consequent hypopituitarism. The cholestasis was rapidly resolved with hormone therapy. Genetic analysis showed a de novo 17q chromosome deletion, including the HNF1β

Impairment of Innate Immunity and Depletion of Vaccine-Induced Memory B and T Cells in the Absence of the Spleen.

Splenectomy or congenital asplenia is associated with severe reduction of memory B cells and increased risk of fulminant sepsis by encapsulated bacteria. Current guidelines recommend vaccinations against these pathogens before or after splenectomy, but the longevity of immunity acquired after splenectomy has not been determined. The impact of splenectomy on innate immune cells is unknown. We analyzed frequency, differentiation stage, and function of innate and adaptive immunity in the peripheral blood of adult (n = 41) and pediatric (n = 14) patients splenectomized or born asplenic and in sple

Bilateral Renal Fungal Bezoars and Perinephric Abscess in an Infant With Arthrogryposis-Renal Dysfunction-Cholestasis Syndrome: A Clinico-pathologic Case Report.

The patients with Arthrogryposis-Renal dysfunction-Cholestasis (ARC) syndrome have genetic susceptibility to the opportunistic infections due to the involvement of VPS33B (vacuolar protein sorting 33 homolog B) in phagolysosome fusion in macrophages. Detailed pathologic studies in ARC patients are missing in literature due to the lack of autopsy. We described the first autopsy case of ARC syndrome in a 2-month-old male infant. His death was due to recurrent sepsis and multiorgan failure despite the appropriate poly-antibiotic therapy and supportive care. The autopsy showed invasive renal candi

A Mysterious Case of Recurrent Confusion: An Unusual Presentation of Congenital Intrahepatic Portosystemic Shunt.

Congenital portosystemic shunts (CPSS) are rare vascular malformations that divert portal blood into the systemic circulation, bypassing hepatic metabolism. Although typically diagnosed in infancy due to associated congenital anomalies or complications such as hepatic encephalopathy, adult presentations are exceptionally uncommon and diagnostically challenging. We report a 71-year-old female patient with recurrent episodes of confusion and drowsiness who was found to have markedly elevated serum ammonia levels despite normal liver function. Extensive investigations excluded infective, metaboli

Hereditary haemorrhagic telangiectasia.

Hereditary haemorrhagic telangiectasia (HHT) is a vascular dysplasia inherited as an autosomal dominant trait and caused by loss-of-function pathogenic variants in genes encoding proteins of the BMP signalling pathway. Up to 90% of disease-causal variants are observed in ENG and ACVRL1, with SMAD4 and GDF2 less frequently responsible for HHT. In adults, the most frequent HHT manifestations relate to iron deficiency and anaemia owing to recurrent epistaxis (nosebleeds) or bleeding from gastrointestinal telangiectases. Arteriovenous malformations (AVMs) in the lungs, liver and the central nervou

Multi-Organ Transplantation in Adult Congenital Heart Disease: Navigating the Unique Challenges of a Distinct Patient Population.

The prevalence of adult congenital heart disease (ACHD) is increasing, with heart failure being the leading cause of death. For many ACHD patients, heart transplantation is the only treatment option for advanced heart failure, though significant extracardiac involvement may require multi-organ transplantation. Despite the rising number of ACHD transplants, multi-organ transplants in this population remain challenging, and a substantial gap remains between those in need and those who receive a transplant. While short-term outcomes may be worse for ACHD patients, long-term outcomes are comparabl

Pancreaticobiliary Maljunction With a Non-Dilated Extrahepatic Bile Duct Masked by Adenomyomatosis: A Potential Cause of Unresectable Gallbladder Carcinoma.

Adenomyomatosis of the gallbladder (ADM) is generally considered benign. The presented cases were initially diagnosed with ADM via ultrasound during an annual health check-up and monitored for several years without further investigations due to the absence of significant changes or symptoms. Subsequently, one case was diagnosed with pancreaticobiliary maljunction (PBM) with a non-dilated extrahepatic bile duct (EBD) after symptom onset, and underwent prophylactic laparoscopic cholecystectomy to avoid the risk of carcinogenesis. Although the other case remained asymptomatic, ultrasound later re

Characterizing the Healthcare Utilization and Costs of Hereditary Hemorrhagic Telangiectasia.

Hereditary hemorrhagic telangiectasia (HHT) is the second-most common inherited bleeding disorder worldwide, afflicting one in 4000-5000 people, and is the most morbid inherited bleeding disorder of women. HHT causes recurrent severe epistaxis, chronic gastrointestinal bleeding, heavy menstrual bleeding, and arteriovenous malformations in the lung, liver, and brain that cause serious bleeding and nonbleeding complications. There are no approved treatments worldwide, and the direct medical costs of HHT have not been well-characterized. We utilized the Komodo Healthcare Map claims database to cr

Belzutifan for patients with Von Hippel-Lindau (VHL) disease-associated heterogeneous tumors - a retrospective single center analysis.

BACKGROUND: Von Hippel-Lindau (VHL) disease is a hereditary tumor predisposition syndrome characterized by benign and malignant tumors affecting multiple organ systems. Standard treatment primarily involves surgical resection and tumor ablation, which can lead to progressive organ dysfunction. Belzutifan, an oral hypoxia-inducible factor-2 alpha (HIF-2α) inhibitor, has emerged as a novel therapeutic option for VHL-associated tumors. However, data on its efficacy and safety outside of clinical trials remain limited. This study evaluates the outcomes of off-label belzutifan treatment in patients
