Discovering Hereditary Risk Through Surveillance: A Prospective Genetic Analysis of Individuals With Familial Pancreatic Cancer.

BACKGROUND: Little is known about the genetic background of individuals with familial pancreatic cancer (PC). Integrating germline testing into surveillance may uncover previously unrecognized hereditary susceptibility and expand prevention strategies beyond BRCA testing alone. This study evaluated the genetic landscape of high-risk individuals due to familiality (HRI-FHs) enrolled in a national surveillance program. METHODS: Five hundred HRI-FHs from seven centers underwent surveillance and germline testing with a 41-gene NGS panel. Pathogenic/likely pathogenic variants (PGVs) and variants o

Functional evaluation of pancreatic islets from patients with Beckwith-Wiedemann syndrome and congenital hyperinsulinism.

CONTEXT: Beckwith-Wiedemann Syndrome (BWS) is an overgrowth syndrome caused by various genetic or epigenetic abnormalities in a cluster of imprinted genes on chromosome 11p15. Congenital hyperinsulinism (HI) is one of the cardinal features of BWS, but the pathophysiology of HI in BWS has not been clearly defined. OBJECTIVE: We describe the islet phenotype of a series of infants with severe HI and comorbid BWS who required pancreatectomy for intractable hypoglycemia. METHODS: The cases are a subset of HI patients who required pancreatectomy and had Beckwith-Wiedemann Syndrome. Molecular testi

Unveiling the Invisible: A Case of Canine Diffuse Microscopic Pancreatic Adenocarcinoma With Normal Imaging and Gross Appearance of the Pancreas Complicated by Sterile Peritonitis and Disseminated Intravascular Coagulation.

Pancreatic adenocarcinoma is an uncommon neoplasm in dogs, typically presenting as a discrete mass with metastasis frequently identified at the time of diagnosis. Herein, we describe an unusual presentation of canine pancreatic adenocarcinoma, characterised by diffuse microscopic infiltration of the pancreas without gross abnormalities on advanced imaging or exploratory laparotomy. An 11-year-old male neutered Shetland Sheepdog presented with recurrent anorexia, lethargy, and haematuria. Diagnostic investigations revealed progressive anaemia, thrombocytopenia, pulmonary consolidations and ster

Treating Pancreatic Ductal Adenocarcinoma: The Targeted Revolution is Here.

Pancreatic ductal adenocarcinoma (PDAC) is one of the most lethal malignancies and is a rising cause of morbidity and mortality. An immunosuppressive, hostile tumor microenvironment, and KRAS-driven biology have contributed to poor outcomes in PDAC. Recent breakthroughs in targeting tumors with homologous repair deficiency, KRASG12C mutations, rare gene fusions, and other molecular abnormalities have improved outcomes in subsets of patients. KRAS inhibitors, both allele specific and pan(K)RAS, claudin-targeting biologics, and PRMT5 inhibitors have demonstrated single agent activity in pretreat

Establishing Normative Values and Clinician Assessment Accuracy for the Single Leg Bridge Endurance Test.

The Single Leg Bridge (SLB) is widely prescribed for core and posterior chain strengthening, but normative endurance values are lacking. Establishing benchmarks may enhance rehabilitation, performance screening, and injury risk assessment. # Hypothesis/Purpose The purpose of this study is two-fold: The first is to establish normative values for the SLB endurance test, which physical therapists can use as a reference for rehabilitation. The second is to evaluate whether visually assessing SLB form loss is as accurate as using an app with angle detection in measuring the total duration of SLB du

[Management of pain in chronic pancreatitis].

Pain in chronic pancreatitis remains difficult to treat, due to a lack of understanding of the underlying nociceptive process. Therefore, treatments are often based on anatomical changes, although morphological features are not correlated with pain intensity and severity. It is well established that the cause of pain in chronic pancreatitis involves multiple mechanisms. Traditional theories focused solely on the role of morphological abnormalities of the pancreas in pain are flawed; there is now clear evidence showing that the resulting pain resembles that observed in neuropathic pain. Therefo

Mitochondria-associated programmed cell death in pancreatic β cell of T2DM.

Mitochondria, as essential organelles that integrate energy metabolism and intracellular signaling, have recently gained increasing attention in the study of pancreatic β-cell dysfunction in type 2 diabetes mellitus (T2DM). Programmed death of β-cells is recognized as a mechanism contributing to both disease progression and remission, primarily by impairing insulin secretion and disrupting glucose homeostasis. Accumulating evidence indicates that mitochondria serve as central hubs coordinating multiple programmed cell death (PCD) pathways. Structural or functional abnormalities of mitochondria

Scytosiphon lomentaria Fucoidan Ameliorates Glucose and Lipid Abnormalities via Modulation of the Microbiota-Gut-Liver/Pancreas Axis: Insights from Multiomics Analysis.

Fucoidan (FUC2), purified from Scytosiphon lomentaria, improved glucose and lipid metabolism in mice by modulating the gut-liver/pancreas axis through structural remodeling of the gut microbiota. Structural characterization revealed that FUC2 is a homogeneous acidic heteropolysaccharide mainly composed of fucose-, mannose-, and galactose-rich regions with possible glycosidic linkages. Mice experiments demonstrated that FUC2 attenuated insulin resistance and oxidative stress by activating the AMPK and Nrf2 signaling pathways. FUC2 administration significantly increased the abundance of benefici

Surgical Management of Pancreatic Head Arteriovenous Malformation: A Case Report.

Pancreatic arteriovenous malformations (PAVMs) are uncommon abnormalities of pancreatic blood vessels that result in direct, high-flow connections between arteries and veins. There are currently fewer than 200 reported cases in the literature of this particular anomaly. They are often clinically asymptomatic but may present with abdominal pain, gastrointestinal bleeding, or features of portal hypertension, and can resemble cystic or neoplastic lesions on imaging. We report a case of a 48-year-old female with a pancreatic head mass presenting with non-specific abdominal pain and jaundice. Owing

Sirolimus Versus Mycophenolate Mofetil in Simultaneous Pancreas-Kidney Transplantation: Impact on Urinary Tract Infection Rates.

BACKGROUND: Urinary tract infections (UTIs) are common complications following simultaneous pancreas and kidney transplantation (SPK). The role of specific immunosuppressive agents in modulating UTI incidence and recurrence remains poorly understood. METHODS: In this retrospective, single-center study, we analyzed 164 SPK recipients randomized to receive either sirolimus or mycophenolate mofetil (MMF) in combination with tacrolimus. The incidence of UTIs, relapses, recurrences, and UTI-related hospitalizations was assessed over a 10-year follow-up. Univariable and multivariable negative binom

Zebrafish gon4la mutants recapitulate human GON4L-related growth disorders and reveal novel metabolic organs abnormalities.

Biallelic mutations in human GON4L have been linked to short stature, craniofacial abnormalities, and microcephaly, yet the underlying mechanisms remain unclear. GON4L is a nuclear protein composed of a YY1AP1-like region, two PAH domains, and one SANT domain, all implicated in transcriptional or chromatin regulation. Here, we define the function of Gon4la (a zebrafish ortholog of human GON4L) in post-embryonic growth and tissue homeostasis. We generated three gon4la mutant lines using CRISPR-Cas9, each predicted to express truncated proteins lacking the C-terminal PAH/SANT domains and differi

Pancreatic cancer in situs inversus totalis: case presentation and literature review.

INTRODUCTION AND IMPORTANCE: Situs inversus totalis (SIT) is a rare anatomical variation of the internal organs in which the thoracic and abdominal viscera organs are mirrored from their normal anatomical position. Surgical treatment of cancer in this anatomical variation represents a great challenge due to the altered surgical anatomic landmark. SIT has been related to a higher risk of cancer without evidences behind this hypothesis. CASE PRESENTATION: We report a 61-year-old male with SIT who presented with synchronous primitive pancreatic-head and lung cancers. After multidisciplinary disc

The Jejunal Microbiota in Patients With Chronic Pancreatitis: Results From a Pilot Study.

BACKGROUND AND AIMS: Chronic pancreatitis (CP) is associated with several systemic metabolic abnormalities including diabetes. While the colonic microbiota and its association with diabetes in CP have been reported, the specific composition of the small intestinal microbiota and its function in CP remains poorly understood. In this pilot study, we primarily aimed to characterize the jejunal microbiota in patients with CP and explore potential associations with diabetes. METHODS: Jejunal aspirates were collected in a RNAlater-containing sterile container from 29 patients with CP and 10 control

Mechanism of Terpinen-4-ol in Suppressing β-Cell Dedifferentiation: Restoring Mitophagy and Alleviating Mitochondrial Calcium Overload via Parkin-Mediated MCU Degradation.

BACKGROUND AND OBJECTIVE: Type 2 diabetes mellitus (T2DM) is a metabolic disorder characterised by insulin resistance and β-cell impairment. Dedifferentiation of pancreatic β-cells contributes to the progression of T2DM, influencing insulin secretion through mechanisms such as mitochondrial dysfunction, Ca2+ overload and impaired autophagy. This study investigated the effects of terpinen-4-ol (T4O) on β-cell dedifferentiation and mitochondrial autophagy, with an emphasis on the role of the Parkin/MCU signalling pathway. METHODS: A T2DM mouse model was generated using a high-fat diet (HFD, 60%

In vivo tau in epilepsy reflects clinical severity and immune- and ageing-related proteomic changes.

Tau pathology plays a central role in a number of neurodegenerative diseases, but its presence and relevance in epilepsy remain incompletely understood. Emerging evidence suggests that epilepsy may promote tau accumulation, yet whether this occurs in vivo, independent of comorbid dementia or amyloid pathology, is unclear. In this study, we combined [18F]flortaucipir (FTP) PET imaging with high-throughput plasma proteomics to characterise regional tau deposition and its clinical and molecular correlates in non-demented epilepsy patients. We enrolled 75 epilepsy patients and 47 age- and sex-matc
