CONTEXT: Pathogenic variants in GATA6, GATA4, and PDX1 cause pancreatic hypoplasia or agenesis and early onset diabetes mellitus. There is a lack of information about long-term outcomes and clinical management of these complicated patients, including how best to approach their exocrine pancreatic insufficiency (EPI), weight gain, and glycemic management.
PARTICIPANTS: We investigated clinical features and treatment of patients with variants in GATA6, GATA4, and PDX1 identified through the US Monogenic Diabetes Registry. Data were self-reported or extracted from medical records.
RESULTS: Elev

BACKGROUND AND AIMS: Chronic pancreatitis (CP) is associated with several systemic metabolic abnormalities including diabetes. While the colonic microbiota and its association with diabetes in CP have been reported, the specific composition of the small intestinal microbiota and its function in CP remains poorly understood. In this pilot study, we primarily aimed to characterize the jejunal microbiota in patients with CP and explore potential associations with diabetes.
METHODS: Jejunal aspirates were collected in a RNAlater-containing sterile container from 29 patients with CP and 10 control

BACKGROUND AND OBJECTIVE: Type 2 diabetes mellitus (T2DM) is a metabolic disorder characterised by insulin resistance and β-cell impairment. Dedifferentiation of pancreatic β-cells contributes to the progression of T2DM, influencing insulin secretion through mechanisms such as mitochondrial dysfunction, Ca2+ overload and impaired autophagy. This study investigated the effects of terpinen-4-ol (T4O) on β-cell dedifferentiation and mitochondrial autophagy, with an emphasis on the role of the Parkin/MCU signalling pathway.
METHODS: A T2DM mouse model was generated using a high-fat diet (HFD, 60%

Polysplenia syndrome is a rare congenital disorder characterized by multiple spleens and complex visceral and vascular anomalies. We report an adult case admitted for multiple intrahepatic bile duct stones. Imaging revealed multiple congenital abnormalities, including polysplenia, a truncated pancreas, and absence of the superior inferior vena cava segment. Intraoperative findings confirmed extensive intrahepatic bile duct stones and biliary malformations. Surgical treatment included partial hepatectomy, bile duct exploration, and T-tube drainage. Postoperatively, the patient developed bactere

Tau pathology plays a central role in a number of neurodegenerative diseases, but its presence and relevance in epilepsy remain incompletely understood. Emerging evidence suggests that epilepsy may promote tau accumulation, yet whether this occurs in vivo, independent of comorbid dementia or amyloid pathology, is unclear. In this study, we combined [18F]flortaucipir (FTP) PET imaging with high-throughput plasma proteomics to characterise regional tau deposition and its clinical and molecular correlates in non-demented epilepsy patients. We enrolled 75 epilepsy patients and 47 age- and sex-matc

Acute pancreatitis most commonly results from gallstone disease, alcohol use, metabolic abnormalities, or medication effects. Malignant infiltration of the pancreas is a rare cause of pancreatic inflammation and may present with symptoms indistinguishable from more typical forms of pancreatitis. Pancreatic involvement by lymphoma is particularly uncommon and may mimic pancreatic neoplasms or recurrent inflammatory pancreatic disease. We present the case of a 29-year-old woman with a recently diagnosed CD-20-positive B-cell lymphoma who developed recurrent severe abdominal pain and markedly ele

INTRODUCTION: Metabolic syndrome (MetS) comprises a cluster of interrelated metabolic abnormalities, including obesity, insulin resistance, dyslipidaemia and hypertension, mainly driven by chronic low-grade inflammation. Among innate immune pathways, the Nod-like receptor family pyrin domain-containing 3 (NLRP3) inflammasome has emerged as a critical molecular link between metabolic stress and inflammatory signalling, promoting caspase-1 activation, interleukin (IL)-1β/IL-18 maturation and pyroptosis across metabolically active organs such as adipose tissue, liver and pancreas. Persistent NLRP

Macroautophagy is a highly conserved system that degrades various materials inside the cell ranging from proteins to organelles. Atg5 is a protein essential for the formation of autophagosomes, which sequester materials by double membrane and degrade them after fusion with lysosomes. MIN6 cells derived from mouse pancreatic β cells retain the ability to secrete insulin in response to a change in glucose concentration from low to high. We knocked out the Atg5 gene in MIN6 cells and identified an abnormality in the endoplasmic reticulum (ER) under normal culture conditions using electron microsc

Coronavirus disease 2019 (COVID-19) is increasingly recognized as a multisystem disorder associated with persistent metabolic complications extending beyond the acute phase of infection. Accumulating evidence suggests that severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) may disrupt glucose homeostasis through mechanisms involving pancreatic β-cell dysfunction, insulin resistance, chronic inflammation, oxidative stress, mitochondrial dysfunction, and hypoxia-related signalling. This review summarizes current evidence regarding the molecular and cellular mechanisms linking SARS-CoV-

Pancreaticobiliary maljunction (PBM) is an anomaly in which the bile and pancreatic ducts join outside the area of action of the sphincter. PBM is typically accompanied by choledochal cysts (dilated PBM); however, cases of PBM without biliary dilatation (nondilated PBM) have also been reported. PBM manifests throughout life. The presentation remains unclear due to age-dependent differences in its pathophysiology. PBM symptoms may be attributed to biliary obstruction caused by substances that differ with age: calcium bilirubinate debris in neonates, protein plugs in children, and gallstones in

BACKGROUND: Cystic fibrosis-related diabetes (CFRD) is the most common extra-pulmonary complication in adults with cystic fibrosis (CF) and is primarily driven by progressive beta-cell dysfunction. The impact of CFTR modulators, particularly elexacaftor/tezacaftor/ivacaftor (ETI), on glucose metabolism remains unclear. This study aimed to assess the effect of ETI on beta-cell function and insulin sensitivity in people with CF (pwCF) without a history of CFRD.
METHODS: We conducted a prospective study involving pwCF who underwent oral glucose tolerance tests (OGTT) at baseline (prior to ETI in

The portal vein is crucial for supplying the liver, and its embryonic development is complex. Variations in portal vein formation can lead to significant surgical challenges, particularly in liver transplantation. Here, we report a unique case of a persistent left portal vein in a 2-year-old patient with Alagille syndrome. During the recipient hepatectomy, the portal vein was found to have an unusual course, traversing behind the pancreas with a more caudal confluence than typical. Although no adverse outcomes occurred during transplant, this variation required a longer and more angular anasto

A Japanese boy carried a paternally inherited single-nucleotide deletion in PTF1A exon 1 (c.775delC) and a maternally inherited nucleotide substitution in the distal enhancer region (g.23508356T>G). Both variants were hitherto unreported. The patient exhibited transient anemia in addition to typical clinical features of pancreatic hypoplasia, but no neurodevelopmental abnormalities. These results highlight the clinical importance and broad mutation spectrum of PTF1A variants as a cause of pancreatic hypoplasia. In addition, our data imply that phenotypes of PTF1A abnormalities are variable and

BACKGROUND: Pathogenic variants in the HNF1B gene cause a multi system disorder encompassing organ abnormalities-primarily affecting the kidneys and pancreas-as well as metabolic disturbances, collectively referred to as HNF1B-related disease. While maturity-onset diabetes of the young type 5 is a well-recognized manifestation, neonatal diabetes mellitus (NDM) associated with HNF1B is exceedingly rare, and has only been reported in patients harboring single nucleotide variants.
CASE PRESENTATION: We describe two unrelated female children presenting with transient NDM caused by a complete HNF1

INTRODUCTION: Immune checkpoint inhibitors (ICIs) are an important component of triple-negative breast cancer (TNBC) treatment, and the KEYNOTE-522 regimen has established pembrolizumab combined with chemotherapy, as the standard of care. While ICIs improve outcomes, they can also induce immune-related adverse events (irAEs), affecting diverse organs. However, renal, pancreatic, and ocular toxicities are rare, and there have been few reports of sequential multi-organ irAEs in breast cancer.
CASE PRESENTATION: A 63-year-old woman with stage IIA TNBC was treated with the KEYNOTE-522 regimen. Du
