Intravascular Large B-Cell Lymphoma Presenting with Rhabdomyolysis and Inflammatory Myositis: A Diagnostic Challenge.

INTRODUCTION: Intravascular large B-cell lymphoma (IVLBCL) is a rare and aggressive extranodal lymphoma characterised by the proliferation of lymphoma cells within small blood vessels. Distinctively, IVBCL typically lacks the lymphadenopathy, discrete tumour masses, or circulating lymphoma cells characteristic of other lymphomas. This results in substantial diagnostic delays and historically high rates of postmortem diagnosis. Common clinical features of IVLBCL include fever of unknown origin, cytopenias, elevated lactate dehydrogenase, and neurological dysfunction. This case highlights the di

Association between job insecurity and cardiovascular diseases in workers with type 2 diabetes mellitus.

In organs, the correct architecture of vascular and ductal structures is indispensable for proper physiological function, and the formation and maintenance of these structures is a highly regulated process. The analysis of these complex, 3-dimensional structures has greatly depended on either 2-dimensional examination in section or on dye injection studies. These techniques, however, are not able to provide a complete and quantifiable representation of the ductal or vascular structures they are intended to elucidate. Alternatively, the nature of 3-dimensional plastic resin casts generates a pe

High-sensitivity serum interferon-α and interferon-γ as immunometabolic mediators of metabolic syndrome in systemic lupus erythematosus.

OBJECTIVES: SLE is associated with a higher risk of metabolic syndrome compared with the general population. IFN-α and IFN-γ have been implicated in the pathogenesis of SLE. However, their serum quantification has been challenging until the development of ultrasensitive detection techniques. In this study, we aimed to examine the relationship between IFN-α and IFN-γ values and lipid profile abnormalities as well as pancreatic β-cell dysfunction, both of which are linked to the metabolic syndrome observed in patients with SLE. METHODS: : A total of 313 patients with SLE were recruited. A panel

Acute pancreatitis as a severe complication of scoliosis surgery in pediatric patient: a case report and literature review.

Acute pancreatitis (AP) in children is a rare but severe inflammatory disease of the pancreas. Unlike in adults, where alcohol consumption and gallstones are the most common causes, pediatric AP results from various factors such as abdominal trauma, infections, biliary tract disease, systemic disorders, genetic predisposition, and drug-induced injury, with many cases remaining idiopathic. Adolescent idiopathic scoliosis (AIS) is the most common form of pediatric scoliosis. Treatment options include physical therapy, bracing, and surgical correction in severe cases. A case of an adolescent pati

The natural killer-cellular humoral activation test as a functional biomarker of antibody-mediated rejection after kidney transplantation.

Antibody-mediated rejection (ABMR) is a major cause of graft failure after kidney transplantation, yet current diagnostics do not directly assess effector immune activity. Natural killer (NK) cells contribute to ABMR through antibody-dependent cell-mediated cytotoxicity, but clinically applicable functional assays are lacking. We evaluated the Natural Killer-Cellular Humoral Activation Test (NK-CHAT), a functional assay quantifying antibody-dependent NK cell activation, as a non-invasive biomarker of ABMR. Fourteen kidney transplant recipients within six months post-transplantation were studie

Extreme Hyperlipasemia in End-Stage Renal Disease Mimicking Acute Pancreatitis: A Diagnostic Pitfall.

Marked elevations of pancreatic enzymes are commonly interpreted as evidence of acute pancreatitis. However, in patients with end-stage renal disease (ESRD), serum amylase and lipase may be elevated without pancreatic inflammation, posing a significant diagnostic challenge. We present a case illustrating the diagnostic dilemma of extreme pancreatic enzyme elevation in an ESRD patient undergoing peritoneal dialysis without clinical or radiologic evidence of pancreatitis. A 50-year-old man with ESRD on peritoneal dialysis and known gastroparesis presented with intractable nausea and vomiting. In

The Prevalence of Pancreas Divisum and Its Association with Pancreatic Diseases: A Systematic Review and Meta-Analysis.

Background and Objectives: Pancreas divisum (PD) is the most common congenital anomaly of the pancreatic ductal system and has been suggested to contribute to pancreatic pathology. However, its true prevalence and relationship with pancreatic diseases remain debated. This systematic review and meta-analysis aimed to estimate the global prevalence of PD and evaluate its association with pancreatic disease. Materials and Methods: A comprehensive search of Google Scholar, Scopus, and PubMed was conducted to identify studies reporting the prevalence of PD across all populations and diagnostic moda

Cellular signaling crosstalk between osteoporosis and diabetes: Common mechanisms and therapeutic targets (Review).

Osteoporosis and diabetes typically occur together. The present review aimed to summarize the molecular mechanisms and intracellular signaling pathways that connect osteoporosis and diabetes. In diabetic conditions, persistent hyperglycemia triggers excessive oxidative stress, sustains low-grade inflammation and perturbs insulin signaling pathways. In turn, bone metabolic abnormalities affect glucose regulation through the bone-pancreas axis and osteoimmune signaling pathways, highlighting a bidirectional relationship between skeletal and metabolic health. Several antidiabetic agents (metformi

Variant mesenteric and pancreatic vasculature. A case report.

PURPOSE: The vasculature of the pancreas and colon is highly variable and clinically relevant in pancreatic and colon surgeries. Although variant arterial branching between colonic and pancreatic arteries has been described, the variant in this study has not previously been reported. This study highlights variant arterial anatomy between the colon and pancreas to enhance anatomic knowledge for surgical planning. METHODS: During a routine cadaveric dissection in our medical school anatomy laboratory, we identified an unusual branching pattern of the superior mesenteric artery (SMA) on a 72 y/o

Unravelling the developmental origins of cystic fibrosis-related diabetes.

Cystic fibrosis (CF)-related diabetes (CFRD) is a growing and evolving concern for people living with cystic fibrosis. As its own unique clinical entity, there is an urgent need to better understand this complex health challenge to improve the quality of life for people with CF. Unfortunately, many aspects of CFRD pathophysiology and pathogenesis remain unclear, resulting in a disparity in the standard of care provided. An emerging body of evidence supports that CFRD may have developmental origins, as people with CF present with pancreatic pathology and glucose abnormalities as early as in ute

Pancreatitis in Familial Dysautonomia Patients.

INTRODUCTION: Familial dysautonomia (FD) is characterized by abnormal function of autonomic and sensory nervous systems. Gastrointestinal (GI) symptoms are extremely prevalent in FD patients, but the impact of FD on the pancreas is less well described. We aimed to elucidate the incidence and characteristics of acute pancreatitis (AP) in a population of FD patients and to identify potential risk factors. METHODS: A retrospective review was performed on FD patients treated at a single tertiary center between 1981 and 2020. FD patients were included in the study, except those who died before 6 m

[Arterial variations and management strategies in pancreatic surgery].

The pancreas is local in the retroperitoneal space,in close proximity to critical blood vessels such as celiac trunk, superior mesenteric artery, hepatic artery, splenic artery, and gastroduodenal artery. The high incidence of anatomical variations in these arteries significantly increases the complexity of pancreatic surgery. Such variations-particularly aberrant origins of the right hepatic artery, dorsal pancreatic artery, and gastroduodenal artery, as well as an anomalous course of the splenic artery, are strongly associated with complications such as intraoperative hemorrhage, positive res

Fish oil and evening primrose oil have ameliorative effects on obesity-associated complications in olanzapine-administered rats.

Olanzapine is an atypical antipsychotic medication utilized for the management of schizophrenia and bipolar disorder. Obesity is a negative consequence of olanzapine. Fish Oil (FO) and Evening Primrose Oil (EPO), are proposed to possess anti-obesity properties. The present study sought to examine the impacts of these oils on olanzapine-induced obesity and its associated abnormalities in female albino rats. The animals were categorized into four groups designated Normal, Oanzapine, Olanzapine + FO (200 mg/kg, oral) and Olanzapine + EPO (200 mg/kg, oral) groups. All animals, except the normal co

Pharmacological and non-pharmacological therapies for chronic pancreatitis pain: a narrative review.

IMPORTANCE: Chronic pancreatitis (CP) pain is the major driver of morbidity, reduced quality of life, healthcare use, and opioid exposure in affected patients. Pain in CP is heterogeneous and often correlates poorly with structural pancreatic abnormalities alone. It may arise from overlapping obstructive, neuropathic, nociplastic, and psychosocial mechanisms, making simple stepwise analgesic escalation insufficient for many patients. OBSERVATIONS: The emerging evidence supports a multidimensional strategy for treating CP pain. In some patients, pain is driven mainly by ductal obstruction, sto

Polysplenia syndrome with semi-annular pancreas, midgut malrotation and interrupted inferior vena cava: A rare adult case report.

Polysplenia syndrome is a rare congenital anomaly within the heterotaxy spectrum. It is typically diagnosed in childhood due to associated cardiac malformations, whereas detection in adults without cardiovascular involvement is uncommon. We report a 50-year-old woman presenting with recurrent abdominal pain and vomiting. Contrast-enhanced CT demonstrated multiple splenic nodules consistent with polysplenia, a semi-annular pancreas partially encircling the duodenum, midgut malrotation, and absence of the intrahepatic segment of the inferior vena cava (IVC) with azygos/hemiazygos continuation.
