Case Report: Nephrocalcinosis from pancreatic hypoplasia in HNF1B disease: a multigenerational expression with genetic confirmation in the youngest generation.

BACKGROUND: HNF1B-associated disease is a genetic disorder caused by heterozygous pathogenic variants in the HNF1B gene, leading to a variety of clinical phenotypes primarily affecting the kidneys, pancreas, liver, and genitourinary tract. First identified in 1997 as the cause of MODY5 (maturity-onset diabetes of the young, type 5), the disease spectrum has since expanded to include kidney disease, pancreatic hypoplasia, genital malformations, gout, hypomagnesemia, liver abnormalities, and primary hyperparathyroidism. Variable expressivity and limited penetrance complicate diagnosis, with some

Neonatal diabetes due to a truncating RFX6 mutation in an extremely low birth weight preterm infant with Mitchell-Riley syndrome.

OBJECTIVES: Mitchell-Riley syndrome (MRS) is a rare autosomal recessive disorder caused by pathogenic variants in the RFX6 gene and characterized by neonatal diabetes, pancreatic hypoplasia with exocrine insufficiency, intestinal atresia, and hepatobiliary abnormalities. Despite increasing recognition of the syndrome, the number of reported cases remains limited and clinical management remains challenging. We aimed to report a preterm infant with an extremely low birth weight and a truncating RFX6 mutation to highlight the severe metabolic instability that may occur in such patients. CASE PRE

A high-fat/high-sucrose/high-cholesterol diet leads to early indications of metabolic dysfunction-associated steatohepatitis-like lesions in obese diabetic mice.

Chronic hyperlipidemia and/ or hyperglycemia can impair various organs, such as the liver, kidney, and pancreas, through metabolic abnormalities. Metabolic dysfunction-associated steatohepatitis (MASH), related from nonalcoholic steatohepatitis (NASH), exhibits complex pathophysiological features, and animal models of MASH are essential for elucidating its underlying mechanisms. This study aimed to induce MASH-like lesions in obese type 2 diabetic mice by feeding them a high-fat/high-sucrose/high-cholesterol (HFSC) diet. C57BL/6J, db/db, and KK-Ay mice at 6 weeks of age were fed an HFSC diet f

Type 2 diabetes mellitus is not associated with a clinically relevant exocrine pancreatic phenotype: A prospective multimodal assessor-blinded case-control study.

AIMS: Exocrine pancreatic abnormalities have been reported in type 2 diabetes mellitus (T2DM), but their clinical relevance remains uncertain. We assessed whether T2DM is associated with a clinically meaningful exocrine pancreatic phenotype. METHODS: Adults undergoing endoscopic ultrasound (EUS) for non-pancreatic indications were prospectively enrolled. Patients with known or suspected pancreatic disease were excluded. Non-diabetic controls were frequency-matched to patients with T2DM by sex and age. EUS abnormalities, pancreatic body diameter, elastography, faecal elastase-1 (FE-1), symptom

Minor Papillotomy for Treatment of Idiopathic Acute Pancreatitis With Pancreas Divisum: A Randomized Clinical Trial.

IMPORTANCE: Pancreas divisum is implicated as an obstructive cause for acute pancreatitis. Observational data suggest endoscopic retrograde cholangiopancreatography (ERCP) with minor papillotomy reduces the risk of pancreatitis episodes. Even though this endoscopic procedure is widely used in practice, clinical trials are lacking. OBJECTIVE: To determine whether ERCP with minor papillotomy reduces the risk of acute pancreatitis among adults with unexplained acute recurrent pancreatitis and pancreas divisum. DESIGN, SETTING, AND PARTICIPANTS: This multicenter, sham-controlled, double-blind ra

The Role of Endoscopic Ultrasound in the Diagnosis and Management of Congenital Gastrointestinal and Pancreaticobiliary Conditions.

PURPOSE OF REVIEW: This review synthesizes current literature on the use of Endoscopic ultrasound (EUS) in congenital gastrointestinal (GI) conditions. Special emphasis is placed on pediatric applications, where EUS is increasingly recognized as both feasible and safe. RECENT FINDINGS: EUS has transformed the evaluation and management of congenital anomalies affecting the GI tract and pancreaticobiliary system, and has been shown to be safe and effective in the pediatric population. By combining high-resolution imaging with the ability to perform real-time interventions, EUS addresses diagnost

A rare variant of celiac trunk hexafurcation: a case report with a literature review.

BACKGROUND: Additional branches of the celiac trunk (CT) pose a serious risk during procedures in the upper abdomen, as the CT supplies organs essential for the body's functioning. Therefore, it is important to document every rare anatomical variation, as this contributes to expanding specialists' knowledge, particularly because there are only a few studies describing the variation presented below. CASE REPORT: During the dissection of an 82-year-old man, a rare hexafurcated CT was identified in the epigastric region. Measurements were taken using a digital caliper with an accuracy of 0.02 mm

A Rare Pediatric Intraductal Papillary Mucinous Neoplasm Presenting as a Pancreatic Pseudocyst With Splenic Vein Thrombosis and Gastric Variceal Hemorrhage.

Intraductal papillary mucinous neoplasm (IPMN) is a rare pancreatic cystic neoplasm in children. Pediatric pancreatic cystic lesions are more often inflammatory, traumatic, or congenital; therefore, IPMN may be mistaken for pseudocysts or pancreatitis-related changes. We report the case of a 15-year-old girl who initially presented with intermittent abdominal pain, mild lipase elevation, and a pancreatic tail cyst that was managed as a presumed pseudocyst after interval regression on surveillance imaging. Despite initial improvement, she later developed upper gastrointestinal bleeding. Subsequ

Short Pancreas in Children With Fanconi Anemia: A Report of Three Cases.

A short pancreas (SP), characterized by the absence of the pancreatic tail, is a rare anomaly resulting from abnormal development of the dorsal pancreas and has been documented to cause endocrine pancreatic insufficiency. Fanconi anemia (FA) is associated with multiple congenital malformations and abnormalities in glucose-insulin metabolism; however, dorsal pancreatic anomalies have not been recognized as part of its phenotype. We report three children with FA in whom computed tomography (CT) demonstrated an SP anomaly. SP may be an underrecognized feature of FA, with potential implications fo

Uromodulin: A Novel Regulator of the Kidney-Adipose Axis in Diabetic Kidney Disease.

The rising burden of diabetic kidney disease (DKD) and its associated lipid abnormalities underscores the need for new mechanistic insights. Uromodulin, a kidney-enriched protein, has been associated with metabolic disorders in human studies, yet its functional role in systemic lipid metabolism remains elusive. In this study, transcriptomic datasets were analyzed to investigate uromodulin expression and biological function in DKD. Subsequently, a diabetic model was induced in UMOD+/+ and UMOD-/- rats using a combination of a high-fat diet, unilateral nephrectomy, and streptozotocin to assess r

The Fatty Pancreas: Current Evidence, Treatment Implications, and Future Directions.

Here, we use the term fatty pancreas (FP), but more recent terminology includes pancreatic steatosis, nonalcoholic FP disease, pancreatic lipomatosis, and fatty atrophy. The absence of a universally accepted definition and standardized diagnostic criteria continues to hinder progress in both clinical practice and research. This condition is marked by abnormal fat content in pancreatic tissue and is becoming more recognized as clinically significant. The diagnosis has implications for metabolic dysfunction and other pancreatic diseases, including pancreatic ductal adenocarcinoma. This review hi

A case of isolated pancreatic sarcoidosis presenting with acute pancreatitis necessitating a differential diagnosis from pancreatic cancer.

Sarcoidosis is histopathologically characterized by non-caseating granulomas and can affect any organ; however, pancreatic involvement is rare. We report a case of isolated pancreatic sarcoidosis presenting with acute pancreatitis diagnosed using endoscopic ultrasonographic elastography and endoscopic ultrasonography-guided tissue acquisition. The patient presented abdominal pain and was diagnosed with acute pancreatitis. Computed tomography and magnetic resonance imaging revealed dilation of the main pancreatic duct and a mass in the pancreatic body, raising the suspicion of obstructive pancr

Retroperitoneal paraganglioma mimicking a solid pseudopapillary neoplasm of the pancreatic tail and confirmed by percutaneous biopsy: a case report.

BACKGROUND: Retroperitoneal paraganglioma (RPGL) is an uncommon extra-adrenal neuroendocrine tumor. When located adjacent to the pancreas, RPGL may mimic solid pseudopapillary neoplasm of the pancreas (SPN/SPTP) because both lesions may present as large cystic-solid masses with nonspecific symptoms and non-diagnostic conventional tumor markers. Percutaneous biopsy may establish the diagnosis but can be hazardous when RPGL has not been excluded. CASE PRESENTATION: A 44-year-old man presented with abdominal distension. He had no history of hypertension, headache, palpitations, or profuse sweati

CT and MR evaluation of main pancreatic duct dilation: A radiologist's guide to differentiating benign from malignant causes.

Main pancreatic duct (MPD) dilation is an imaging finding that encompasses a broad spectrum of benign and malignant etiologies. Differentiating between these conditions is critical for appropriate patient management, and radiologists play a central role in this diagnostic process. CT and MR, particularly with MR cholangiopancreatography (MRCP), are crucial for assessing ductal morphology, parenchymal changes, and associated lesions. Thin-slice dual-phase CT provides excellent spatial resolution and is particularly effective for evaluating pancreatic ductal adenocarcinoma (PDAC), vascular invas
