Serous cystadenoma of the pancreatic head associated with pancreatic duct atresia and acinar cell loss: a case report.

BACKGROUND: Serous cystadenoma (SCA) of the pancreas is typically a benign neoplasm, and variants are exceptionally rare. While tumors in the pancreatic head often cause upstream ductal dilatation and parenchymal atrophy due to obstruction, the concurrent presentation of SCA with congenital distal pancreatic duct atresia and complete acinar cell loss is unique. We report this case to highlight a potential congenital pathogenesis linking ductal developmental defects to the formation of these lesions. CASE PRESENTATION: A 56-year-old woman presented with persistent right upper abdominal distens

Cardiometabolic comorbidities in overt Cushing's syndrome.

Endogenous Cushing's syndrome (CS) is characterized by chronic glucocorticoid excess that drives a severe cardiometabolic phenotype responsible for most morbidity and mortality associated with the disorder. Cortisol excess induces a constellation of metabolic derangements-including visceral adiposity, insulin resistance, hypertension, dyslipidemia, and ectopic fat deposition-that closely mirror and amplify the components of metabolic syndrome. These abnormalities result from widespread glucocorticoid actions on adipose tissue, liver, skeletal muscle, pancreas, vasculature, and the heart, media

Prevalence and Clinical Significance of Abnormal Findings on Pre-Diagnostic Imaging in Pancreatic Ductal Adenocarcinoma.

OBJECTIVES: Pancreatic ductal adenocarcinoma (PDAC) is the third leading cause of cancer-related death. Although CT/MRI are used for initial evaluation, early imaging findings suggesting PDAC may be unappreciated, resulting in diagnostic delays. We aimed to determine the prevalence and types of pancreatic abnormalities on pre-diagnostic imaging in patients later diagnosed with PDAC. METHODS: We identified patients diagnosed with PDAC between 2022 and 2025 who had a pre-diagnostic abdominal CT/MRI performed 3-36 months before diagnostic imaging. These patients were compared with a non-PDAC coh

A case of small undifferentiated pancreatic carcinoma detected during follow-up for focal pancreatic parenchymal atrophy with fatty replacement.

Undifferentiated carcinoma (UC) is a rare and aggressive subtype of pancreatic cancer, typically diagnosed at an advanced stage. We report a case of UC detected as a small lesion (10 mm on imaging) during follow-up for focal pancreatic parenchymal atrophy (FPPA). A 75-year-old woman with a history of nontuberculous mycobacterial disease underwent routine computed tomography, revealing focal parenchymal atrophy in the pancreatic body without any abnormality in the main pancreatic duct (MPD). During follow-up, CA19-9 levels increased, and diffusion-weighted magnetic resonance imaging identified

Super-resolution deep learning-enhanced 2D MRCP improves main pancreatic duct visualization compared with 3D MRCP.

PURPOSE: The relative merits of 2D and 3D MRCP remain incompletely defined, and the effect of super-resolution deep learning reconstruction (SR-DLR) on main pancreatic duct (MPD) visualization remains unclear. We compared image quality and MPD visualization among 3D MRCP with SR-DLR (3D-SR), conventional 2D MRCP with compressed sensing (2D-CS), and 2D MRCP with SR-DLR (2D-SR), and exploratorily assessed diagnostic performance for MPD abnormalities. MATERIALS AND METHODS: This prospective study included 174 patients undergoing MRCP for pancreatic evaluation. Quantitative metrics included signa

Geometric simplification by ductal unification of dual pancreatic ducts during pancreaticojejunostomy.

BACKGROUND: Anatomical variations of the pancreatic duct are common; however, two closely approximated ductal lumens at the margin of pancreatic transection present a distinct reconstructive challenge during pancreaticoduodenectomy (PD). When the inter-ductal distance is minimal, conventional assumptions underlying pancreaticojejunostomy may not apply. CASE PRESENTATION: In this two-case report, two patients undergoing PD were preoperatively diagnosed with dual pancreatic ducts by contrast-enhanced computed tomography and magnetic resonance cholangiopancreatography. Histopathological assessme

Focal Pancreatic Parenchymal Atrophy and Delayed Enhancement in Early Pancreatic Ductal Adenocarcinoma: A Narrative Review of Pathologic Basis, Diagnostic Pitfalls, and Evidence Limitations.

Early pancreatic ductal adenocarcinoma (PDAC), including carcinoma in situ (CIS)/high-grade pancreatic intraepithelial neoplasia (PanIN) and very small invasive tumors, is increasingly recognized, but the significance of pancreatic parenchymal atrophy and delayed enhancement remains unsettled. This narrative review summarizes their pathological basis, technical determinants, observer variability, differential diagnoses, and evidence limitations across computed tomography (CT), magnetic resonance imaging (MRI)/magnetic resonance cholangiopancreatography (MRCP), endoscopic ultrasonography (EUS),

Synchronous Multifocal Intraductal Tubulopapillary Neoplasm of the Pancreas Treated with Total Pancreatectomy: A Case Report.

INTRODUCTION: An intraductal tubulopapillary neoplasm (ITPN) is a rare pancreatic tumor characterized by scant mucin production. Even when associated with an invasive carcinoma component, its prognosis is generally considered more favorable than that of conventional pancreatic ductal adenocarcinoma. However, its pathogenesis and optimal treatment strategy remain unclear. Herein, we report a case of a synchronous multifocal ITPN treated with total pancreatectomy. CASE PRESENTATION: A man in his 50s was referred to Gunma University Hospital for evaluation of pancreatic duct abnormalities after

Unsupervised Machine Learning of Intrapancreatic Fat Deposition Reveals Distinct Clinical Phenotypes: A CT Study.

INTRODUCTION: Intrapancreatic fat deposition (IPFD) is an ectopic fat phenotype associated with metabolic abnormalities. However, the clinical heterogeneity of IPFD in relation to generalized adiposity remains insufficiently characterized. This study aimed to explore clinical profiles associated with IPFD using unsupervised clustering and characterize their metabolic features. METHODS: In this retrospective, single-center cross-sectional study, 558 adults undergoing non-contrast abdominal computed tomography were included. K-means clustering was performed using age, body mass index (BMI), pan

Retroperitoneal-First Laparoscopic Approach (Retlap) for Bile Duct Resection in Pancreaticobiliary Maljunction (PBM): A Novel and Reasonable Approach to Intrapancreatic Bile Duct.

BACKGROUND: Pancreaticobiliary maljunction (PBM) is a congenital anomaly in which the pancreatic and bile ducts join outside the duodenal wall.1,2 Reflux of pancreatic juice induces chronic inflammation and epithelial regeneration, promoting carcinogenesis. Biliary dilation increases the risk of cholangiocarcinoma, and prophylactic resection of the extrahepatic bile duct and gallbladder is recommended even for asymptomatic patients.3,4 To prevent postoperative carcinogenesis, the intrapancreatic bile duct should be transected just above the pancreatobiliary junction. The authors describe a nov

Clinical analysis of Meckel diverticulum with ectopic pancreas in children: 3 case reports.

RATIONALE: Lower gastrointestinal bleeding caused by Meckel diverticulum (MD) with ectopic pancreas is rare and diagnostically challenging in children, particularly due to the frequent absence of gastric mucosa leading to negative Technetium pertechnetate. This study aimed to summarize and analyze the clinical characteristics and management of this condition. PATIENT CONCERNS: A retrospective analysis included 3 pediatric patients (2 males, 1 female; aged 2-13 years) treated between July 2013 and March 2023. All presented with hematochezia as the primary symptom. One case was accompanied by a

An extrarenal renin-producing tumor associated with a 3 cm mature cystic teratoma.

Extrarenal renin-producing tumors are a cause of secondary hypertension and are most commonly reported to arise in the adrenal gland, lung, and pancreas; however, they rarely occur in the ovary, and renin-producing ovarian tumors are mainly larger than 5 cm or malignant. We report a case of a 3 cm renin-producing ovarian tumor. A 27-year-old woman presented with newly diagnosed hypertension. Laboratory evaluation revealed hyperreninemic hyperaldosteronism. Renal artery ultrasonography and abdominal magnetic resonance angiography showed no renal artery stenosis or adrenal abnormalities, making

Case Report: Recurrent pulmonary infections with neck masses: a diagnostic challenge and literature review.

This article presents a case characterized by recurrent pulmonary infections and a poor therapeutic response. Initially, cystic fibrosis (CF) or Primary ciliary dyskinesia was suspected; however, the patient subsequently sought medical attention for a neck mass. The mass was initially misdiagnosed as lymphoma, but a comprehensive examination and assessment ultimately confirmed the diagnosis of activated phosphoinositide 3-kinase delta syndrome type I(APDS1). Through the patient's tortuous diagnostic journey, this report aims to enhance clinicians' understanding of this rare primary immunodefic

Case Report of Taeniasis with Gastric Manifestations on CT and Worm Expulsion Post CT Examination.

INTRODUCTION/BACKGROUND: In March 2025, a 31-year-old female patient was admitted to Mangkang County People's Hospital in Tibet, complaining of upper abdominal pain for two days, which worsened after meals. She reported a habit of consuming raw beef. This case is unique as it highlights the diagnostic value of CT imaging in identifying taeniasis, particularly when worms are expelled post-CT examination. Previous imaging studies often lacked specific signs, with many cases only confirmed post-surgery. This case provides a valuable reference for the radiological characteristics of taeniasis. CA

A prognostic model based on seven cell adhesion molecule-related genes to predict the prognosis of colorectal cancer.

BACKGROUND: Magnetic resonance cholangiopancreatography (MRCP) is useful to assess exocrine pancreatic function by combining rapid imaging acquisition with the administration of secretin, a gastrointestinal hormone that stimulates the secretion of bile and pancreatic juice. However, extensive data on this method are lacking. This study aimed to determine whether MRCP with secretin administration is able to simultaneously detect alterations of both the pancreatic ducts and exocrine pancreatic function. METHODS: All subjects older than 18 years who underwent magnetic resonance imaging (MRI) and

Maternal metabolomic signatures for risk of preterm and early term birth in African American women.

BACKGROUND AND PURPOSE: Compared to healthy controls, adult patients with Sickle Cell Disease (SCD) are anemic, and therefore have higher cardiac output and Cerebral Blood Flow (CBF) to maintain brain oxygenation. They also demonstrate comparatively more cognitive deficits due to either overt strokes or silent cerebral ischemia. However, there are few correlative studies between CBF and cognitive deficits, specifically processing speed in SCD. Such studies are important to develop biomarkers of central brain processing and ischemia for diagnosis, prognosis, and evaluating the effectiveness of

Effect of Microcurrent Stimulation on Cartilage Regeneration in Engineered Chondrocyte Spheroids.

Microcurrent stimulation has emerged as a promising biophysical cue for cartilage regeneration, but its dose-dependent effects in 3-dimensional engineered cartilage constructs remain unclear. Here, we investigated the effects of continuous direct microcurrent stimulation on auricular chondrocytes and engineered cartilage spheroids using 0-, 10-, 25-, 50-, 100-, and 200-μA stimulation for 30 min daily. In 2-dimensional culture, microcurrent stimulation did not markedly increase the DNA content or Cell Counting Kit-8 value, indicating that it did not markedly promote cell expansion or overall me

Ultrasound Assessment of the Fetal Pancreas in Normal and Abnormal Pregnancies: A Narrative Review.

Fetal pancreatic development is central to intrauterine glucose-insulin homeostasis and may influence lifelong metabolic risk. Advances in obstetric ultrasonography now allow in-utero assessment of pancreatic size, morphology, and echotexture, offering a non-invasive window into fetal metabolic adaptation across normal and high-risk pregnancies. A narrative review was conducted using electronic databases to identify human studies reporting sonographic evaluation of the fetal pancreas, including measurements of circumference, diameter, area, segmental lengths, derived ratios, and echogenicity g
