Sodium arsenite induces islets β-cells apoptosis and dysfunction via SET-Rac1-mediated cytoskeleton disturbance.

Sodium arsenite (NaAsO2), the most common form of inorganic arsenic prevalent in the environment, has been closely linked to islet β-cell dysfunction, a critical pathological hallmark of type 2 diabetes (T2D). Even though apoptosis plays a pivotal role in arsenic-induced islet β-cell dysfunction, the explicit underlying mechanisms remain elusive. Here, we have identified that the SET-Rac1 signaling pathway is instrumental in the apoptosis and dysfunction of islet β-cells induced by NaAsO2. During NaAsO2-induced islet β-cell apoptosis and dysfunction, our observations indicated downregulation o

Exocrine pancreatic insufficiency as an unusual extrarenal manifestation of proximal renal tubular acidosis associated with a novel SLC4A4 mutation.

Autosomal recessive proximal renal tubular acidosis (AR-pRTA) with ocular abnormalities is a rare syndrome caused by variants in the SLC4A4 gene, which encodes Na/HCO3 cotransporter (NBCe1). The syndrome primarily affects the kidneys, but also causes extra-renal manifestations. Pancreatic type NBCe1 is located at the basolateral membrane of the pancreatic ductal cells and together with CFTR chloride channel, it is involved in bicarbonate secretion. In vitro models have demonstrated that mutations in the pancreatic type NBCe1 lead to a reduction in pancreatic bicarbonate secretion. Although ele

Characteristic TARC/CCL17 expression in the salivary gland of IgG4-related disease: potential diagnostic utility and insights into pathogenesis.

Immunoglobulin G4-related disease (IgG4-RD) is a chronic inflammatory condition of unknown etiology characterized by lymphocytic infiltration, fibrosis, and infiltration of IgG4-positive plasma cells. It affects various organs, including the pancreas and salivary glands. Immunological abnormalities are suspected to play a role in its pathogenesis, and there is an epidemiological link to allergic conditions and type 2 inflammation. This study focused on the expression of thymus and activation-regulated chemokine (TARC)/CCL17, which is involved in the migration of T helper 2 and/or regulatory T

Cerium-based nanoplatform for severe acute pancreatitis: Achieving enhanced anti-inflammatory effects through calcium homeostasis restoration and oxidative stress mitigation.

Severe acute pancreatitis (SAP), a life-threatening inflammatory disease of the pancreas, has a high mortality rate (∼40 %). Current therapeutic approaches, including antibiotics, trypsin inhibitors, fasting, rehydration, and even continuous renal replacement therapy, yield limited clinical management efficacy. Abnormally elevated calcium levels and reactive oxygen species (ROS) overproduction by damaged mitochondria are key factors in the inflammatory cascade in SAP. The combination of calcium chelators and cerium-based nanozymes loaded with catalase (MOF808@BA@CAT) was developed to bind intr

Multiple low-dose radiation ameliorates type-2 diabetes mellitus via gut microbiota modulation to activate TLR4/MyD88/NF-κB pathway.

BACKGROUND: Type 2 diabetes mellitus (T2DM) is the fastest-growing metabolic disease in the world. The gut microbiota is linked to T2DM. Recent studies have showed that the metabolism of gut microbiota can trigger T2DM. Low dose radiation (LDR) has been proved to activate various protective bioeffects on diabetes. However, the underlying mechanisms remain unclear. METHODS: In this study, T2DM model was established using high fat diet combined with streptozocin (STZ) injection in C57BL/6 mice, and then exposed to multiple 75 mGy LDR every other day for one month. The changes of blood glucose l

[Joint chance discovery of a jejunal ectopic pancreas and a pancreas divisum of the native pancreas].

Ectopic pancreas (EP) and pancreas divisum are well-known anatomical abnormalities of the pancreas. Although pancreas divisum is the most common birth defect of the pancreas, EP is not such a rare anomaly. It is defined as the presence of pancreatic tissue outside the pancreas with no anatomical connection to the orthotopic pancreas. We present the case of a patient who combined these two anomalies that were discovered by chance with an abdominal CT scan.

Is It Anorexia? Congenital Duodenal Web.

Congenital duodenal web (CDW) is a rare cause of intestinal obstruction in adults, making diagnosis challenging. We present the case of a 26-year-old woman with chronic epigastric symptoms who recently developed vomiting and significant weight loss. Prolonged compensatory duodenal dilation obscured typical radiological signs and delayed preoperative identification. The diagnosis was confirmed intraoperatively and managed surgically. This case highlights the need to consider CDW in adults with unexplained proximal gastrointestinal obstruction.

Usefulness of endoscopic ultrasonography for differentiating between non-functional pancreatic neuroendocrine neoplasm and intrapancreatic accessory spleen.

In pancreatic hypervascular masses, it is often difficult to differentiate pancreatic neuroendocrine neoplasm (PanNEN) and intrapancreatic accessory spleen (IPAS) before surgery because of their similar features, such as a round shape and early enhancement. This study aimed to examine the efficacy of endoscopic ultrasonography (EUS) for differentiating between them. This retrospective pilot study enrolled 136 patients with pathologically confirmed non-functional PanNEN or IPAS who underwent EUS at our institution. The clinical features, conventional EUS findings, EUS elastography (EUS-EG) find

Undernutrition-Associated Diabetes Mellitus: Pathophysiology of a Global Problem.

Early-life undernutrition is known to be a potentially important risk factor for the development of diabetes mellitus in adult life. Additionally, some literature suggests that undernutrition during later life or adulthood may lead to metabolic consequences, including diabetes, despite an individual's normal nutritional status earlier in life. Notably, individuals with diabetes and undernutrition show some unique features that do not fit the usual phenotype of type 2 diabetes mellitus, such as a low body mass index of <18.5 kg/m2, resistance to ketosis, and low serum C-peptide levels. Many glo

The Role of Markedly Elevated Cyst Carcinoembryonic Antigen in Risk-Stratifying Pancreatic Cysts.

INTRODUCTION: Carcinoembryonic antigen (CEA) level is currently being used in algorithms of commercially available pancreatic cyst fluid analysis platforms. However, the evidence to date on its role in risk stratification for mucinous lesions is heterogenous. We therefore sought to examine the relationship between the magnitude of CEA elevation and the structural and molecular properties of pancreatic cysts. METHODS: All patients who underwent pancreatic cyst fluid analysis at a single institution from 2012 to 2019 were retrospectively examined. Structural features on endoscopic ultrasound we

Autoimmune Pancreatitis Presenting as Multifocal Masses: A Rare Case Report.

Autoimmune pancreatitis (AIP) is an uncommon inflammatory condition that can mimic pancreatic malignancy both clinically and radiologically, leading to diagnostic uncertainty. Two subtypes exist: Type 1 is linked to systemic IgG4-related disease, and Type 2 is confined to the pancreas. A 64-year-old woman was found to have submandibular and cervical lymphadenopathy during a routine physical examination. Further imaging revealed FDG-avid lymph nodes and pancreatic abnormalities, raising concern for malignancy. However, biopsy results confirmed Type 1 autoimmune pancreatitis (AIP) and IgG4-relat

HFE-Related Hemochromatosis May Be a Primary Kupffer Cell Disease.

Iron overload can lead to increased deposition of iron and cause organ damage in the liver, the pancreas, the heart and the synovium. Iron overload disorders are due to either genetic or acquired abnormalities such as excess transfusions or chronic liver diseases. The most common genetic disease of iron deposition is classic hemochromatosis (HH) type 1, which is caused by mutations of HFE. Other rare forms of HH include type 2A with mutations at the gene hemojuvelin or type 2B with mutations in HAMP that encodes hepcidin. HH type 3, is caused by mutations of the gene that encodes transferrin r
