Distal cholangiocarcinoma in a patient with portal annular pancreas and aberrant common hepatic artery: a case report.

BACKGROUND: Portal annular pancreas (PAP) is a rare congenital anatomical variation characterized by pancreatic parenchyma encircling the portal vein (PV) or superior mesenteric vein (SMV). An aberrant common hepatic artery (CHA) originating from the superior mesenteric artery (SMA) represents another uncommon vascular anomaly. The coexistence of these two variations with a distal common bile duct (CBD) malignancy is extremely rare and poses significant technical challenges for pancreaticoduodenectomy (PD). CASE PRESENTATION: We report a case of a 55-year-old male with obstructive jaundice ca

Duplicated ventral pancreatic ducts arising separately from the duodenal wall: anatomical evidence suggestive of persistence of the left ventral pancreatic bud.

PURPOSE: Human embryological studies have documented the transient presence of paired ventral pancreatic buds during early development. The persistence of the left ventral bud into adulthood could theoretically result in duplicated ventral pancreatic ducts in adulthood. Here, we report a case of duplicated ventral ducts incidentally observed in a human pancreas retrieved from an organ donor. ANATOMIC VARIATION: The pancreas was obtained from a 46-year-old male donor after brain death for the purpose of islet transplantation. During the islet isolation procedure, we unexpectedly identified two

Portal Annular Pancreas: Prevalence and Surgical Importance.

PURPOSE: To determine the prevalence of portal annular pancreas (PAP) in a defined population using multidetector computed tomography (CT) and to assess its potential surgical implications, and to report our experience of three cases. METHODS: A retrospective analysis was conducted on 1000 consecutive triphasic contrast-enhanced abdominal CT scans performed between June 2024 and January 2025, evaluating the presence and subtype of PAP. RESULTS: PAP was identified in 27 patients (2.7%). The suprasplenic variant was the most common (n = 25), followed by the mixed variety (n = 2). Of the affect

Rare Association of Duodenal Obstruction and Situs Inversus Abdominis: A Two Case Report.

Duodenal obstruction in the setting of situs inversus abdominis is an uncommon clinical finding. We report 2 neonates who presented with bile-stained vomiting and abdominal distension. Diagnostic workup, including radiography and ultrasonography, confirmed the diagnosis of situs inversus abdominis with duodenal obstruction due to annular pancreas in the first case and due to intestinal malrotation in the second. A Ladd's procedure was successfully performed to relieve the obstruction. Both patients recovered well, initiating full enteral feeds and were subsequently discharged. These cases unde

Robot-Assisted Distal Pancreatectomy for Pancreatic Cancer with Type IIIa Portal Annular Pancreas Using a Novel Strategy of Ventral Pancreas Preservation: A Case Report.

INTRODUCTION: Portal annular pancreas (PAP) is a rare congenital anatomical variation, first reported in 1987, characterized by an uncinate process encircling the portal vein and fusing with the pancreatic body. While often asymptomatic, the presence of PAP poses a significant technical challenge during pancreatectomy because of its altered anatomy and the elevated risk of postoperative pancreatic fistula (POPF). Consensus is lacking on the optimal pancreatic transection method (1- or 2-cut margin approach) to minimize this complication. We report the first documented case of robot-assisted di

Outcomes of robotic lateral duodenoduodenostomy surgery for neonatal duodenal obstruction.

BACKGROUND: Robotic-assisted duodenoduodenostomy being used for congenital duodenal obstruction (CDO) is sparsely reported. Our study aims to compare outcomes of the robotic lateral duodenoduodenostomy (RLD) with laparoscopic lateral duodenoduodenostomy (LLD) in the treatment of CDO. METHODS: We retrospectively analyzed consecutive neonates with CDO who received RLD or LLD in our center between March 2015 and January 2025. Preoperative information, intraoperative and postoperative outcomes were compared. RESULTS: A total of 46 patients were analyzed in this study (RLD group = 26, LLD group =

Annular Pancreas Presenting with Intermittent Duodenal Obstruction in Early Childhood: A Diagnostic Masquerade.

Background and Clinical Significance: Annular pancreas is a rare congenital anomaly in which pancreatic tissue partially or completely encircles the duodenum, potentially causing duodenal obstruction. Clinical presentation varies from asymptomatic cases to persistent vomiting, feeding intolerance, and failure to thrive, often leading to delayed diagnosis. Case Presentation: We report a 2-year and 10-month-old girl with a long-standing history of intermittent, recurrent vomiting since the neonatal period, without growth impairment or other alarming symptoms. Initial imaging suggested proximal d

Polysplenia syndrome with semi-annular pancreas, midgut malrotation and interrupted inferior vena cava: A rare adult case report.

Polysplenia syndrome is a rare congenital anomaly within the heterotaxy spectrum. It is typically diagnosed in childhood due to associated cardiac malformations, whereas detection in adults without cardiovascular involvement is uncommon. We report a 50-year-old woman presenting with recurrent abdominal pain and vomiting. Contrast-enhanced CT demonstrated multiple splenic nodules consistent with polysplenia, a semi-annular pancreas partially encircling the duodenum, midgut malrotation, and absence of the intrahepatic segment of the inferior vena cava (IVC) with azygos/hemiazygos continuation. Familial congenital duodenal obstruction.

Duodenal webs are an uncommon cause of duodenal stenosis arising from incomplete bowel recanalisation during embryological development. Despite troubling symptoms such as bilious vomiting in affected infants, prognosis is generally favourable when promptly recognised and treated surgically. Though the condition is recognised as embryological in origin, evidence for familial predisposition is lacking in the medical literature. The authors present a case of duodenal web and concurrent annular pancreas with a positive family history of congenital duodenal obstruction in the infant's father. We di

Adult Annular Pancreas Presenting With Gastric Outlet Obstruction Following Semaglutide Initiation: A Case Report.

Annular pancreas is a rare congenital anomaly that may remain clinically silent until adulthood. Its symptoms can be masked or exacerbated by glucagon-like peptide-1 receptor agonists (GLP-1 RAs), which delay gastric emptying. We report a 55-year-old male with type 2 diabetes who developed progressive postprandial vomiting and nine-kg weight loss following semaglutide initiation. Imaging revealed a partial annular pancreas causing second-part duodenal stenosis without malignancy. Symptoms persisted despite semaglutide discontinuation, supporting a fixed mechanical obstruction as the primary ae

Neonatal Lesser Omental Hernia Associated with Intestinal Malrotation and Congenital Duodenal Stenosis.

INTRODUCTION: Lesser omental hernia is a rare type of internal hernia. Almost all previously reported cases are adult patients and neonatal cases are extremely rare. CASE PRESENTATION: A 2-day old female baby was referred to our hospital due to vomiting with dark colored vomitus. The abdomen was slightly distended and enteral feeding failed to establish. A gastrointestinal series revealed intestinal malrotation, and the patient was taken to the operation room. Laparoscopic investigation revealed non-rotation type intestinal malrotation without volvulus. After extraction of the entire gastroin

Intraoperative Discovery of Type II Choledochal Cyst, Partial Annular Pancreas, and Duodenal Stenosis in a 45-Day-Old Term Born Female Infant.

Type II choledochal cysts (CCs), a rare form of congenital biliary dilation characterized as a true diverticulum along the extrahepatic duct, comprise less than 2% of all cases. The coexistence of CC, annular pancreas (AP), and duodenal stenosis (DS) has been scarcely reported. We present the case of a 45-day-old female with a rare combination of congenital upper gastrointestinal anomalies discovered intraoperatively after imaging identified malrotation with concern for possible volvulus. The patient underwent exploratory laparotomy revealing a Type II CC, partial AP, and DS. Management includ

Maternal metabolomic signatures for risk of preterm and early term birth in African American women.

BACKGROUND AND PURPOSE: Compared to healthy controls, adult patients with Sickle Cell Disease (SCD) are anemic, and therefore have higher cardiac output and Cerebral Blood Flow (CBF) to maintain brain oxygenation. They also demonstrate comparatively more cognitive deficits due to either overt strokes or silent cerebral ischemia. However, there are few correlative studies between CBF and cognitive deficits, specifically processing speed in SCD. Such studies are important to develop biomarkers of central brain processing and ischemia for diagnosis, prognosis, and evaluating the effectiveness of

Pancreatitis Risk Associated with GLP-1 Receptor Agonists, Considered as a Single Class, in a Comorbidity-Free Subgroup of Type 2 Diabetes Patients in the United States: A Propensity Score-Matched Analysis.

Introduction: Glucagon-like peptide-1 receptor agonists (GLP-1 RAs) are commonly prescribed for the management of type 2 diabetes mellitus (T2DM). However, the potential connection between GLP-1 RAs and the risk of pancreatitis presents a complex and nuanced issue. Although these drugs are effective in improving blood sugar control and cardiovascular health, their association with pancreatitis remains an area of concern. Our study aims to evaluate the association between the use of GLP-1 RAs, considered as a single class, and the risk of pancreatitis in a comorbidity-free subgroup of patients
