Evaluation of prenatal and postnatal ultrasonography for the diagnosis of fetal double bubble sign.

BACKGROUND: Congenital duodenal obstruction (CDO) is commonly detected antenatally through the presence of the "double bubble" sign on prenatal ultrasound, denoting dilatation of the stomach and duodenum. Subsequent postnatal ultrasonography plays a pivotal role in determining the causes of obstruction, thereby informing surgical strategies and neonatal management. The aim of this study was to investigate the diagnostic accuracy of postnatal ultrasonography in comparison to that of prenatal ultrasound and surgical findings in a cohort of 43 patients with fetal double bubble sign. METHODS: A t

Minimally Invasive Surgery for Duodenal Obstructions: 10 Years of Experience in a Single Center.

PURPOSE: Duodenal obstructions are one of the most common causes of upper intestinal obstruction during the neonatal period. Minimally invasive surgical treatment is considered highly complex. We report our experience with 43 patients treated using this method. METHODS: We conducted a retrospective study at our institution from January 2013 to May 2023, including patients classified as having upper intestinal obstructions. All patients received preoperative diagnoses based on clinical findings, radiography, and abdominal ultrasound. Laparoscopic surgery was performed on all patients. RESULTS

Antenatal Diagnosis of Complete Annular Pancreas With Postnatal Surgical Follow-Up: A Case Report.

Annular pancreas is a rare congenital disorder, which is characterized by partial or complete pancreatic tissue surrounding the second part of the duodenum. It can be diagnosed antenatally on ultrasound. We are reporting a case where a double bubble sign on an antenatal scan was seen, which was followed up after the delivery of the baby, and radiography also confirmed the findings. Both investigations suggested duodenal obstruction (duodenal atresia and annular pancreas were kept as possible causes). The child was taken for surgery and pancreatic tissue surrounding the second part of the duode

Adult presentation of symptomatic annular pancreas treated with gastrojejunostomy.

Annular pancreas is a rare congenital abnormality that is often considered a neonatal condition, though in few cases it can present in adulthood. Patients diagnosed with annular pancreas in adulthood are often asymptomatic and diagnosed incidentally from radiologic studies. While there is no existing treatment protocol for symptomatic annular pancreas, multiple surgical approaches have been documented in the literature. In this case study we present a 49-year-old female patient who presented with gastric outlet obstruction secondary to annular pancreas. The patient was treated with nasogastric

Primary division of annular pancreas: a surgical technique.

The authors presented a case of duodenal obstruction in a 61-year-old man, resulting from an annular pancreas diagnosed on imaging (computed tomography, magnetic resonance cholangiopancreatography, and endoscopic ultrasound). The patient underwent a diagnostic laparoscopy. Intraoperatively, given a straightforward appearance and anatomy of the annular pancreas overlying the second part of the duodenum, and due to extensive adhesions in the abdomen, a primary division of the annular pancreas was performed, instead of a bypass procedure such as gastrojejunostomy. He had some residual symptoms 1

Association between early enteral nutrition and length of stay in neonates with congenital bowel obstruction: A retrospective cohort study.

BACKGROUND: The optimal feeding strategy for postoperative neonatal patients with congenital bowel obstruction is widely debated. This study aims to evaluate perioperative characteristics and postoperative nutrition practices for patients with congenital bowel obstruction. We hypothesized that earlier introduction of enteral nutrition (EN) is associated with shorter hospital stays and increased weight gain velocities. METHODS: We performed a retrospective cohort study on neonatal patients (<30 days old) admitted to a pediatric referral hospital who underwent an operation for bowel obstruction

Mimics of pancreatic neoplasms at cross-sectional imaging: Pearls for characterization and diagnostic work-up.

Interpreting imaging examinations of the pancreas can be a challenge. Several different entities can mimic or mask pancreatic neoplasms, including normal anatomic variants, non-pancreatic lesions, and both acute and chronic pancreatitis. It is important to distinguish these entities from pancreatic neoplasms, as the management and prognosis of a pancreatic neoplasm, particularly adenocarcinoma, have considerable impact on patients. Normal pancreatic variants that mimic a focal lesion include focal fatty atrophy, annular pancreas, and ectopic pancreas. Extra-pancreatic lesions that can mimic a

Robotic pancreaticoduodenectomy for portal annular pancreas: how to do it.

Portal annular pancreas (PAP) is an uncommon anomaly in which the pancreatic parenchyma surrounds the portal or superior mesenteric vein. An adequate operative approach is necessary to prevent clinically relevant postoperative pancreatic fistula after pancreaticoduodenectomy for PAP. We herein report a case of robotic pancreaticoduodenectomy for PAP. In PAP, dissection of the retroportal parenchyma is the most important aspect of surgery. Adequate retraction of the portal system using vessel loops allows for a safe dissection and transection of the retroportal parenchyma. The robotic approach

Heterotaxy Syndrome Diagnosed in an Adult.

Background: Heterotaxy syndrome, a condition in which the internal organs are abnormally arranged in the thorax or abdomen, is generally diagnosed early in life, often during the neonatal period. Case Report: We present the case of a 42-year-old male who was incidentally diagnosed with polysplenia syndrome and subsequently diagnosed with heterotaxy syndrome. Upon further investigation, he was determined to have a sinus venosus type atrial septal defect. Furthermore, the patient's inferior vena cava was interrupted in the infrarenal region and continued as the azygos vein with a coincidental re

[A Case of Invasive Intraductal Papillary Mucinous Carcinoma of the Pancreas with Portal Annular Pancreas].

A 70-year-old male patient presented with epigastric pain. Subsequent evaluation revealed a cystic lesion in the head of the pancreas. After 1 year, the cyst had enlarged and a nodular shadow appeared within it, prompting a referral to our hospital. CT imaging revealed a 65 mm cystic lesion extending from the head to the uncinate process of the pancreas, located posterior to the portal vein. Additionally, an indistinct nodule was apparent inside the cyst. Based on a diagnosis of invasive intraductal papillary mucinous carcinoma, pancreaticoduodenectomy was performed. Intraoperative findings re

Annular pancreas in adulthood: Two compelling cases, including one with pyloric stenosis and chronic pancreatitis.

We report the infrequent occurrence of the annular pancreas (AP) in adults by presenting two remarkable cases. The first case involved an incidental discovery of an AP in a patient diagnosed with chronic pancreatitis. Surgical intervention was warranted due to gastric outlet obstruction resulting from pyloric stenosis. The patient underwent loop gastrojejunostomy (GJ) and duodenojejunostomy (DJ). The second case was evaluated for dyspeptic symptoms, and the AP was diagnosed on the contrast-enhanced abdomen CT. A bypass procedure in the form of loop DJ was performed to address the obstruction.

Sonographic findings of transient marked proximal bowel dilatation in a growth-restricted fetus at 35 weeks' gestation.

Etiologies underlying the relatively infrequent third-trimester sonographic depiction of dilated fetal bowel include (functional or mechanical) bowel obstruction, intestinal atresia, volvulus, annular pancreas, intestinal malrotation, intussusception, gastrointestinal duplications, cystic fibrosis-associated meconium ileus, congenital chloride diarrhea, microvillus inclusion disease, intestinal neuronal dysplasia, and meconium plug syndrome. Fetal bowel obstruction may be associated with aneuploidy (mostly Trisomy 21 in association with esophageal or duodenal atresia), and rarely select microd

Adult-Onset Annular Pancreas: When to Intervene?

Annular pancreas in adults is a rare congenital abnormality, often detected after the onset of complications or incidentally during autopsy. Diagnosis in adults is challenging due to the similarity of symptoms with other conditions. We report a case of a 55-year-old female who presented with a six-month history of intermittent colicky pain in the epigastric region, radiating to the back, accompanied by periodic nonbilious vomiting. Abdominal examination revealed mild tympany and succussion splash in the epigastric region, with no organomegaly, lumps, or visible peristalsis. The patient was tre

[Surgical management of duodenal atresia due to annular pancreas and intestinal atresia IIIb].

BACKGROUND: The presence of duodenal atresia related to type IIIb intestinal atresia is a rare association, with few cases reported in the literature, representing a surgical challenge considering that even isolated cases of type IIIb intestinal atresia are a challenge. The objective was to report the successful surgical management of a case of a complex intestinal malformation, characterized by duodenal occlusion secondary to annular pancreas and type IIIb intestinal atresia, with intestinal malrotation by definition and the presence of Meckel's diverticulum. Recurrent non-bilious vomiting in a child: A case report of an uncommon diagnosis.

Annular pancreas (AP) is a congenital anomaly and a recognised cause of duodenal obstruction which can affect all age groups. It may manifest early in the neonatal period, but it may also have varied and often delayed symptoms. We report the case of AP in an 18-month-old girl with a long history of recurrent post-prandial non-bilious vomiting treated for a long time with the clinical diagnosis of gastroesophageal reflux. Upper gastrointestinal study was suggestive of partial duodenal obstruction and computed tomography revealed a complete ring of pancreatic parenchyma surrounding the second pa

Annular pancreas: Beneath the intestinal obstruction-A case report.

Annular pancreas is an uncommon congenital anomaly which is a rare cause of congenital duodenal obstruction. It is normally identified during the neonatal period, but may also be identified in adolescence or adulthood. This diagnosis is often overlooked in adult patients who present with symptoms suggestive of duodenal obstruction. We present a case of AP detected in a 23-year-old man, with complaints of continuous vomiting and abdominal discomfort over the last 6 months. An upper gastrointestinal study revealed a constricted second part of the duodenum. A computed tomography scan revealed a c

Can the transection plane be optimized in pancreatic resections?

PURPOSE: According to current protocol, the separation of pancreatic head and body is performed at the level of superior mesenteric vein (SMV). Previous data indicate that the resection plane should be modified in portal annular pancreas. We presumed that the optimal line of pancreatic resections could also be different in other cases. Our aim is to simulate pancreatic resections in different planes and find the optimal resection line with the minimum number of cut vessels. MAIN METHODS: 25 abdominal vascular corrosion casts were prepared, the aorta and the portal vein were cannulated. CT sca
