Pancreatic pathology in trisomy 13: specificity of the morphologic lesion.

Pancreases from 8 patients with trisomy 13 were studied. Ectopic splenic tissue was present in 6: 3 had accessory spleens and ectopic splenic tissue embedded in the pancreas, one had intrapancreatic splenic tissue without an accessory spleen, and one had an accessory spleen without intrapancreatic splenic tissue. The intrapancreatic splenic tissue was multiple, generally poorly or nonencapsulated, and contained pancreatic acini, islets of Langerhans, and ducts lined by tall columnar epithelium with goblet cells. The rest of the pancreas in these and other cases of trisomy 13 had microcyst form

Clinical significance of anomalous pancreaticobiliary union.

Anomalous pancreaticobiliary union was found in nine cases among 300 consecutive adult patients examined by endoscopic retrograde cholangiopancreatography. Three had congenital choledochal cysts: one had a cyst of the choledochus associated with a cyst of the intrahepatic bile duct, another had a fusiform dilation of the choledochus, and the third had a choledochal diverticulum. Five of the nine patients had biliary malignancies (55.6%): four carcinomas of the gallbladder and one carcinoma of the choledochus. On the other hand, 18 of 291 patients without anomalous pancreaticobiliary union had

Chronic pancreatitis and congenital strictures of the pancreatic duct.

Chronic pancreatitis may occasionally be due to chronic incomplete obstruction of the pancreatic duct. Pancreas divisum is associated with a high incidence of recurrent pancreatitis or pancreatic pain, which may be due to relative stenosis of the accessory papilla through which most of the pancreatic secretions drain. Stenosis of the pancreatic duct at the site of fusion of he ventral and dorsal ducts has been demonstrated in 3 percent of autopsy subjects and in patients studied with ERCP. Two patients with idiopathic chronic pancreatitis presented with dilatation of the pancreatic duct which

Surgical management of pancreas divisum.

Pancreas divisum is a congenital anatomic variant characterized by nonunion of dorsal and ventral pancreatic ducts in an otherwise fused pancreas. Of 21 patients with divisum documented by endoscopic retrograde cholangiopancreatography, 6 (28 percent) were found to have no reason other than divisum to account for multiple attacks of pancreatitis. Cholelithiasis was present in one patient, who remains free of recurrent pancreatitis after cholecystectomy only. The remaining five patients underwent surgical treatment directed at pancreas divisum in the belief that stenosis of the duct of Santorin

Endoscopic retrograde cholangiopancreatography in children.

Gastroduodenoscopy and retrograde cholangiopancreatography has been performed on 25 occasions in 20 children aged between 7 and 16. Radiographs of the clinically relevant duct or ducts were achieved in 96% of attempts, with no complications. The diagnostic information proved useful clinically; in particular it provided a precise map if biliary or pancreatic surgery was being contemplated. Several unexpected congenital duct anomalies were found. This and other recent reports, particularly from Germany, indicate that endoscopic retrograde cholangiopancreatography deserves greater application in

Surgical applications of sphincteroplasty and choledochoduodenostomy.

The papilla of Vater is subject to a variety of abnormalities that include congenital anomalies, inflammation, neoplasms, fibrosis, and motor dysfunction. Some of these lesions are clinically obvious (such as cancer or an impacted gallstone) and require aggressive therapy in order to provide relief from pain or biliary-pancreatic ductal obstruction. Other lesions, such as stenosing papillitis, are more subtle and provide few clues to their existence except recurrent episodes of abdominal pain. I have focused my discussion on the relative indications for sphincteroplasty and choledochoduodenost

"The choledocho-pancreatic long common channel disorders" in relation to the etiology of congenital biliary dilatation and other biliary tract disease.

The amylase level of bile and various cholangiograms was studied in 36 cases of congenital biliary dilatation. (C.B.D.) The amylase level above 10000 U/L was considered to suggest the existence of choledocho-pancreatic long common channel formation. Radiologically, the dilatation of intrahepatic bile duct was seen in 9 out of 19 (47%) Type I cases and in all (100%) Type II cases. The cystic dilatation of intrahepatic bile duct was seen in all Type I cases which have cystic choledochal dilatation. On the other hand, fusiform dilatation was seen in all Type II cases with fusiform choledochal dil

Gardner syndrome and periampullary malignancy.

In a family with colonic polyposis and the typical associated findings of Gardner syndrome (osteomas and soft-tissue tumors), two and possibly four of the affected members developed periampullary malignancy. A review indicates that individuals with Gardner syndrome may have a 100- to 200-fold increased risk of developing periampullary carcinoma when compared to the general population. While certain families and certain individuals (those with other duodenal involvement, males and those with all of the characteristics of Gardner syndrome expressed) may be most susceptible, all patients with Gar

[Ectopic openings of the common bile duct. A contribution to the physiological study of the sphincter of Oddi (author's transl)].

Two cases of ectopic openings of the common bile duct are reported. In one case there was a congenital short duct opening into DI, the presence of a spontaneous duodenobiliary reflux due to absence of the papilla being demonstrated by clinical and radiological examinations. The other patient had a long duct opening into D3 and no duodenobiliary reflux, but with an intact sphincter of Oddi explored in detail by means of combined choledochoduodenal manometry. These two cases demonstrate that the duodenal wall has only a moderate effect in preventing duodenobiliary reflux. The essential element i

Intraoperative gastrointestinal endoscopy.

A four year experience with the adaptation of the flexible fiberoptic endoscope to the intraoperative environment is presented in 30 patients. The technique of intraoperative endoscopy was utilized in a wide variety of difficult gastrointestinal surgical problems to include the location of the site and cause of bleeding of obscure etiology; resolution of intraoperative dilemmas without the necessity of opening abdominal viscera; resection of lesions during operations conducted for other pathological processes; and enhancement of diagnosis at laparotomy. There were no complications from the use

Congenital biliary tract dilatation and anomalous junction of the pancreatico-biliary ductal system.

Anomalous junction of the pancreaticobiliary ductal system (AJPBDS) was found not only in 6 of 28 children with congenital dilatation of the bile duct, but in 3 of 57 children with congenital biliary atresia, in 2 of 17 children with infantile hepatitis and in one child with chronic pancreatitis during the past 17 yr at the Kagoshima University Hospital. A new technique of selective choledochopancreatography has been developed in which the mid-choledochus is clamped and dye is selectively injected into the distal choledochus to demonstrate the pathology of the bile duct. This technique has bee

[Annular pancreas (author's transl].

Eight cases of annular pancreas treated at the University Hospital of Hamburg between 1964 to 1977 are presented. In adults, the disease follows the pattern of chronic duodenal obstruction being often asymptomatic unless complications such as peptic ulceration, acute pancreatitis or bile duct obstruction occur. Preoperative diagnosis is only possible by using endoscopic retrograde pancreatography. Attention is drawn to the frequent co-existence of other malformations of the gastro-intestinal tract. Treatment should consist of relief of any obstruction to the duodenum by a bypass gastrojejunost

Renal dysplasia and asplenia in two sibs.

A family is reported in which two sibs, one male and the other female, both died within 24 hours of birth with enlarged polycystic kidneys. Postmortem histology in the second child showed gross renal dysplasia. In both children the pancreas was enlarged, nodular and cystic but the liver appeared macroscopically normal. In the second child, histological examination confirmed pancreatic fibrosis with cystic dilation of ducts, but showed portal fibrosis with bile duct proliferation in the liver. This combination of findings is very reminiscent of those in a girl and her brother reported by Ivemar

Endoscopic retrograde cholangiopancreatography (ERCP) performed in children.

The value of ERCP in children is demonstrated on 3 cases. By using a usual duodenofiberscope (JF-B2), in 2 cases under general anesthesia and in one case after premedication with 1 ml Buscopan i.m., a similar technique is used as in adults. In congenital choledochal cyst only ERCP allowed the exact judgement of the distal part of the common bile duct and its relation to the pancreatic duct. Therefore, an exact preoperative diagnosis can be established. When duodenofiberscope will be more improved ERCP will also be possible in the newborn infant. Than the differential diagnosis of congenital bi

Trisomy C and cystic dysplasia of kidneys, liver and pancreas.

Trisomy C is documented in a 17-hour-old full-term male infant with bilateral diffuse renal dysplasia, Potter facies, pulmonary hypoplasia and other congenital anomalies. In addition to renal dysplasia, intrahepatic bile duct and pancreatic dysplasia are demonstrated microscopically. The extra C-group autosome is morphologically consistent with a No. 11 and this is regarded as the etiology of the multiple malformations of this infant. The phenotype and the renal malformation are essentially similar to those described by Juberg et al (3) in an infant with trisomy C. It is suggested that the sam

Duodenal anomalies and the ampulla of Vater.

The results of the surgical correction of congenital duodenal anomalies in infants free of severe prematurity, mongolism or other intestinal anomalies are most favorable. Several uncommon defects involving the second part of the duodenum, which required special attention to the anatomic proximity of the ampulla of Vater and the terminal portion of the common bile ducts have been encountered. The recognition of this intimate relationship may prevent inadvertent injuries to these structures, further improving the results of operative procedures for these difficult congenital malformations.

Duodenal involvement in Gardner's syndrome.

It is suggested that duodenal polyps are more frequent in Gardner's syndrome than previously recognized. The polyps have a particular tendency to undergo malignant change. Two patients with Gardner's syndrome are reported in whom duodenal polyps were associated in one patient with periampullary cancer and in the other with a possible adenocarcinoma of the duodenum.
