Collagen and microvascular alterations contribute to neuromuscular degeneration and disease progression in chronic intestinal pseudo-obstruction.

BACKGROUND: Chronic intestinal pseudo-obstruction (CIPO) is a severe gastrointestinal motility disorder that may be idiopathic or associated with systemic disease. In idiopathic cases, the pathophysiological mechanisms remain poorly defined. Although mutations in angiogenic factors have been reported in mitochondrial forms of CIPO, their role in non-mitochondrial cases is still unclear. OBJECTIVE: To investigate genetic and molecular contributors to CIPO, with a specific focus on intestinal microvasculature. METHODS: Jejunal samples from patients with CIPO were analysed by whole exome sequen

Liquid-liquid phase separation couples MKRN2-mediated ubiquitination of CSDE1 with neurodevelopmental disorders.

BACKGROUND: Makorin-2 (MKRN2) is an E3 ubiquitin ligase involved in multiple biological processes, yet its role in neurological disorders remains poorly understood. This study aims to elucidate how MKRN2 regulates the RNA-binding protein CSDE1-a molecule linked to autism-related genes-and to explore the functional implications of this interaction in neurodevelopment. METHODS: Using mass-spectrometry screening, we identified CSDE1 as a direct substrate of MKRN2. Ubiquitination sites were validated through mutagenesis of conserved lysine residues. Liquid-liquid phase separation (LLPS) assays we

Impact of Graft Size Matching on Graft Survival in Pediatric Whole Liver Transplantations in Recipients With Biliary Atresia Weighing Less Than 10 kg.

BACKGROUND: The size mismatch between a liver graft and the recipient can result in complications and poor survival after pediatric whole liver transplantation (WLT). METHODS: A retrospective study was designed, which included 114 recipients. Multiple variable predictors of graft loss suggested the graft-to-native-liver weight ratio (GNLWR), and then the GNLWR was calculated and categorized into 2 groups. The demographic, operation, complications, and survival analysis data were collected and compared. RESULTS: Group 1 with a GNLWR < 0.41 had a higher incidence of hepatic artery thrombosis (

Handing off hope: transition of care in pediatric surgery.

BACKGROUND: The importance of transitioning care from pediatric to adult practitioners is an often overlooked aspect of chronic disease. The benefits of a planned transition in health care are that patients will learn valuable life-long lessons on healthcare maintenance, when and how to seek medical attention for new issues that may arise, and improvements on their overall well-being. The purpose of this SAGES White Paper was to summarize the available knowledge for several pediatric surgical conditions to aid in transition of care for this patient population. METHODS: The members of the SAGE

Partial Duodenal Obstructions: A National Surgical Quality Improvement Program Pediatric Analysis of Clinical Factors.

INTRODUCTION: Duodenal atresia is one of the most common causes of intestinal obstruction, typically presenting in the first few days of life. Partial duodenal obstructions (PDO) can allow for the passage of liquids and may not be diagnosed in early infancy until feeding intolerance becomes more apparent. We aimed to identify characteristics of patients who may have PDO and describe their outcomes. METHODS: Utilizing the National Surgical Quality Improvement Program-Pediatric database, patient characteristics and surgical outcomes were compared between patients who underwent operative repair

Nivolumab-induced Limbic Encephalitis in Gastric Cancer: A Case Report and Literature Review.

BACKGROUND/AIM: Immune checkpoint inhibitors (ICIs), including nivolumab, have markedly improved gastric cancer (GC) outcomes. However, immune-related adverse events involving the central nervous system are rare and potentially life-threatening. Limbic encephalitis is an exceptionally uncommon manifestation, and its clinical course remains poorly understood. We conducted a literature review of ICI-induced encephalitis specifically in patients with gastrointestinal cancers to clarify its clinical characteristics and management. CASE REPORT: We describe the case of an 83-year-old man with unres

Dental pulp stem cells maintain epigenetic chromatin architecture remodeling primed by the etiological stimulus of biliary atresia.

BACKGROUND: Biliary atresia (BA) is a nongenetic cholangiopathy characterized by biliary obliteration. However, the underlying pathological mechanism remains unclear. We aimed to explore the epigenetic BA pathology by using BA-specific deciduous dental pulp stem cells (BA-SHED), which develop in parallel with cholangiocyte progenitor cells in human embryos. METHODS: BA-SHED were isolated from human exfoliated deciduous teeth of patients with BA using the colony-forming unit fibroblast method. After sequential stimulation with cytokines and chemicals in cultured BA-SHED, the in vitro bile duct

Progression of esophageal atresia associated Barrett's esophagus in adulthood - Is endoscopic surveillance worth it?

OBJECTIVES: Barrett's esophagus (BE) is a late sequela after esophageal atresia (EA) repair. In order to assess the need for an endoscopic BE surveillance program, we studied evolution of BE in 71 adults with C-type EA in two successive endoscopies. METHODS: Endoscopic Prague Classification of BE was used. Endoscopic BE was graded as True BE (with goblet cell metaplasia) or BE without goblet cell metaplasia. RESULTS: The median patient age was 36 (IQR 28-43) years at E1 (index endoscopy) and 50 (41-58) years at E2 (follow-up endoscopy) with median interval of 16 (12-17) years. Prevalence of

Portable Neonatal Esophageal Atresia Simulator Improves MIS Acquisition Amongst Surgical Trainees: A Randomized Control Crossover Trial.

BACKGROUND: Minimally invasive surgical (MIS) skills are increasingly expected of surgical trainees; however, opportunities to practice these outside the operating theater are limited. Current pediatric surgery trainees have limited exposure to thoracoscopic repair of esophageal atresia (OA/TOF) during their training. We evaluated the utility of a neonatal OA/TOF simulator for the acquisition and retention of MIS skills amongst surgical trainees. METHODS: Randomized controlled cross-over trial of 20 surgical trainees in New Zealand. Participants were block-randomized into two groups: SIM vers

A genetic model of congenital intestinal atresia implicates Mypt1 in epithelial organisation.

Congenital intestinal atresia (IA) is a birth defect characterised by the absence or closure of part of the intestine. Although genetic factors are implicated, mechanistic understanding has been hindered by the lack of suitable animal models. Here, we describe a medaka (Oryzias latipes) mutant, generated by N-ethyl-N-nitrosourea (ENU) mutagenesis, that develops IA during embryogenesis. Positional cloning identified a nonsense mutation in mypt1, encoding myosin phosphatase target subunit 1. Mutant embryos exhibited ectopic accumulation of F-actin and phosphorylated myosin regulatory light chain

Chasing the ghost in the code: highlighting the mystery of a rare genetic condition.

Here, we recount the story of a remarkable young girl who demonstrated extraordinary courage and resilience while confronting significant health challenges over the span of 23 years. Her journey is a testament to her determination to survive and thrive despite overwhelming obstacles. We detail the investigative efforts aimed at associating her clinical symptoms with a specific membrane transporter: the Na-K-2Cl cotransporter-1 (NKCC1), which this laboratory has dedicated more than three decades to studying. The work summarized here covers experiments using patient-derived cells, cells engineer

Defining the Contribution of Genetic Variants in MRGPRX4 With Pruritus in Paediatric Cholestasis: Evidence From Case-Control Study.

BACKGROUND AND AIMS: Cholestatic pruritus is a debilitating symptom in paediatric liver disease with a complex and poorly understood pathogenesis. Recent evidence implicates the Mas-related G protein-coupled receptor X4 (MRGPRX4)-a bile acid receptor expressed in human dorsal root ganglia-as a potential mediator of itch signalling. Although MRGPRX4 harbours multiple single nucleotide variants (SNVs), their role in modulating cholestatic pruritus remains unexplored. METHODS: We conducted a case-control study at a single paediatric tertiary care center, enrolling patients with Alagille syndrome

A Case of Laparoscopic-Assisted Thoracoscopic Diaphragmatic Plication for Diaphragmatic Eventration.

A female in her 40s who presented with epigastric pain after meals and exertional dyspnea was diagnosed with a left diaphragmatic eventration. Because her symptoms did not improve despite conservative treatment, diaphragmatic plication was planned. To avoid injury to the intra-abdominal organs, three-port thoracoscopic diaphragmatic plication was initiated with laparoscopic observation through the umbilical port. Securing an adequate surgical field was difficult due to the relaxed diaphragm. To improve visualization, two grasping forceps were inserted through the umbilical port and used to ret

Evaluation of surgical outcomes of thoracoscopic esophageal atresia repair with and without preservation of azygos vein: a retrospective cohort study.

BACKGROUND: Esophageal atresia is an incomplete formation of esophagus, resulting in discontinuity between proximal and distal segments. During the surgical repair, whether the azygos vein should be ligated or preserved is still up for debate. The study aims to compare the postoperative outcomes of esophageal atresia repair with and without preservation of azygos vein. METHODOLOGY: A retrospective cohort study was conducted at the Institute of Mother and Child Centre, Nawabshah, Pakistan, from 2010 to 2023. The study included neonates undergoing repair for esophageal atresia. Demographic, intr
