Stricture classification of pediatric esophageal strictures (SCOPES): A novel tool to predict response to endoscopic therapy.

OBJECTIVES: Esophageal anastomotic stricture is a common complication following esophageal atresia (EA) repair. While multiple endoscopic therapies exist, predicting individual stricture response remains challenging. Existing classification systems focus primarily on luminal diameter and do not account for complex physical characteristics of the stricture. We developed and evaluated a novel endoscopic scoring tool, stricture classification of pediatric esophageal strictures (SCOPES), to determine if physical features are predictive of treatment response. In addition to its predictive utility,

Development of a Bedside Scoring Tool to Screen for Biliary Atresia in Infants with Neonatal and Infantile Cholestasis in a Resource-limited Setting: an Observational Study.

OBJECTIVE: To develop an easy-to-use bedside scoring system to help physicians distinguish extrahepatic biliary atresia (EHBA) from other causes of neonatal and infantile (NIC) cholestasis. METHODS: Medical records of all infants between 0 and 12 months of age diagnosed as NIC between January 2016 and December 2024 were reviewed. A diagnosis of EHBA was made based on histopathology findings, per-operative cholangiogram, and/ or intraoperative findings. Using logistic regression analysis, candidate variables that predicted biliary atresia were identified, and a scoring tool was developed. RES

Focused Clinical Observation of Functional Outcomes in Children With OEIS Complex.

Omphalocele, Cloacal Exstrophy, Imperforate Anus, Spinal Defects (OEIS) complex is an extremely rare congenital disorder, and data on long-term functional and developmental outcomes remain scarce. We aimed to evaluate ambulatory, urological, and intellectual outcomes in a cohort of patients with OEIS complex. We retrospectively reviewed 10 patients with OEIS complex born at our institution between 1991 and 2021. Spinal defects were classified radiologically or intraoperatively. Ambulatory and urinary outcomes were assessed at the most recent follow-up, and intellectual status was evaluated in

Conserved Ductular Reaction Mechanisms in Biliary Atresia and Primary Sclerosing Cholangitis Derived From Single-Cell and Spatial Transcriptomics.

Primary sclerosing cholangitis (PSC) and biliary atresia (BA) both demonstrate the ductular reaction (DR), including biliary ductules, immune infiltration, and fibroblast activation. Advances in single-cell RNA sequencing and spatial transcriptomics have revolutionized our understanding of the DR fibro-inflammatory niche of these disorders. Recent studies using these techniques have also demonstrated that there are conserved mechanisms of fibro-inflammation across diseases and organ systems. Notably, epithelial, mesenchymal, and innate immune processes in the DR are shared between BA and PSC,

Early postoperative inflammatory markers associated with anastomotic leakage after repair of congenital esophageal atresia.

PURPOSE: Early clinical signs of anastomotic leakage (AL) after congenital esophageal atresia (EA) repair are often subtle; therefore, adjunctive early indicators are needed. METHODS: This was a single-center retrospective study of a cohort of infants undergoing radical EA repair between 2000–2024. White blood cell count (WBC), C-reactive protein (CRP), and the Noble and Underwood (NUn) score were assessed on postoperative day (POD) 1 and POD2. AL was diagnosed by routine contrast esophagography on POD7–9. Discrimination was evaluated using receiver operating characteristic analysis. RESULTS:

Toward the simultaneous detection of multiple diseases with a highly cost-effective cell-free DNA methylome test.

Plasma cell-free DNA (cfDNA), originating from multiple organs, holds significant potential for noninvasive diagnostics and prognostics. Current cfDNA methylation assays primarily focus on single clinical indications by targeting specific genomic loci. In contrast, comprehensive profiling of cfDNA methylome can enable simultaneous detection of multiple diseases by capturing organ-specific methylation signatures, thereby offering a holistic view of health, when disease etiology is unclear or when conventional biochemical diagnostics are unavailable. However, deep sequencing required for sensiti

Pediatric oral and dental manifestations of biliary atresia and Alagille syndrome: a comparative retrospective study.

BACKGROUND: Case reports, small case series, and studies not specifically focused on a single type of cholestatic disorder suggest the presence of oral and dental manifestations in Alagille syndrome (AGS) and Biliary Atresia (BA). While some may be directly due to cholestasis, others may be phenotypic of these conditions. OBJECTIVES: The aim of this study was to describe the oral characteristics of children with AGS and BA and to distinguish features specific to each condition from those attributable to cholestasis. METHODS AND SETTING: Oral characteristics where retrospectively compared bet

Anorectal malformation in adulthood: a systematic review of biological, psychological, and sociological outcomes and experiences.

PURPOSE: Anorectal malformations are congenital anomalies involving the rectum, anus, genital, and urinary tracts, and occur in approximately one in 5000 live births. A lack of transition from pediatric to adult health services means little is known about how this condition is experienced in adulthood. The purpose of this review was to discover biopsychosocial challenges and supports experienced by adults living with anorectal malformation. MATERIALS AND METHODS: A systematic quantitative literature review was the preferred method to collect and analyze data. Articles included in the review w

Seasonal and temporal trends in leading causes of hospitalisation among older adults in Hong Kong: a retrospective study across community and institutional care settings.

OBJECTIVES: With an ageing population, understanding leading causes of hospitalisation in older adults is critical for care strategies. These leading causes may vary across residential settings and by seasonal patterns. This study examines the temporal trends of leading causes of hospitalisation among older adults in community-dwelling and nursing home settings, specifically comparing patterns during winter and summer seasons. DESIGN, SETTING, PARTICIPANTS AND MAIN OUTCOME MEASURES: A retrospective analysis of electronic medical records from Hong Kong public hospitals (2012-2018) was conducte

[Thoracoscopic treatment for congenital diaphragmatic hernia and oesophageal atresia].

Thoracoscopic surgery is an alternative to open repair of congenital diaphragmatic hernia and oesophageal atresia. Studies show faster recovery compared with open approaches. Neonatal thoracoscopy was initially associated with longer operative times and higher complication rates. However, risks have declined with greater expertise, improved techniques, and careful patient selection. Neonatal anaesthetic management remains a demanding task and requires specialised skills. Thoracoscopy is now a safe option, though open surgery remains necessary in selected cases.

Molecular Probes for Pancreatic Cancer Diagnosis: Recent Advances and Further Perspectives.

Pancreatic cancer (PC) is a highly aggressive malignancy of the digestive system, characterized by an annual mortality rate nearly equivalent to its incidence. Early surgical resection remains the most promising curative approach; however, approximately 90% of patients are diagnosed at an advanced stage due to its insidious onset and rapid metastasis. Currently, anatomical imaging techniques such as endoscopic ultrasound, computed tomography, and magnetic resonance imaging (MRI) have limited sensitivity and specificity for detecting small and early-stage lesions of PC. Molecular probes, which

Analysis of terminal rectal fibrosis and expression of type I and type III collagen proteins in patients with mid-high anorectal malformations.

BACKGROUND: Patients with intermediate and high-type anorectal malformations (ARMs) demonstrate moderate to severe fibrosis in the distal rectal wall, which may underlie postoperative defecation dysfunction. This study aims to systematically evaluate and quantitatively analyze collagen deposition patterns in the distal rectal wall, providing new insights into the mechanism of defecation disorders following anorectal reconstruction. METHODS: We conducted an integrated analysis of 30 intermediate/high-type ARMs patients utilizing histochemical staining, immunohistochemistry, and Western blot to

International disparities in biliary atresia outcomes.

The Japanese Biliary Atresia Registry, reported by Okubo et al., provides a comprehensive long-term perspective on biliary atresia (BA) across 3951 patients (1989–2023). With jaundice clearance rates exceeding 60% and 10-year native liver survival of 50.5%, outcomes in Japan remain consistently superior to those reported in Western cohorts. These disparities are likely multifactorial. BA represents a heterogeneous phenotype shaped by genetic, immunological, environmental, and developmental factors that may differ across populations. The lower prevalence of biliary atresia splenic malformation

Intraluminal bougienage-based elongation for native-preserving salvage in long-gap esophageal atresia.

BACKGROUND: Preservation of the native esophagus is the preferred goal in long-gap esophageal atresia (LGEA); however, some infants remain unsuitable for anastomosis after spontaneous growth or failure of established lengthening strategies. METHODS: We retrospectively reviewed infants with LGEA who underwent intraluminal bougienage-based elongation followed by delayed anastomosis between July 2020 and July 2025. Patients were stratified by age at initiation of elongation (<3 months vs. ≥3 months) for descriptive comparison. Outcomes were compared, and multivariable linear regression analyses

Outcomes of pediatric median arcuate ligament release: A single-center experience.

INTRODUCTION: Median arcuate ligament syndrome (MALS) is a rare condition caused by compression of the celiac artery by the median arcuate ligament. The surgical treatment for MALS is a median arcuate ligament release (MALR). While outcomes of MALR are well described in adults, pediatric data remain limited. Leveraging our extensive institutional experience, we sought to address this gap by reporting the largest single-institution cohort of pediatric patients undergoing MALR at a standalone children's hospital. METHODS: We performed a single-institution retrospective review of all patients wh
