Single-cell transcriptomics reveals anti-inflammatory effects of prednisolone on the hepatic niche in biliary atresia.

Biliary atresia (BA) is a life-threatening paediatric liver disease, characterized by bile duct obstruction, progressive inflammation and fibrosis. Treatment options are limited, often necessitating surgery or liver transplantation to prevent irreversible liver damage. Prednisolone has shown promise in treating BA; however, there is currently no consensus on its effectiveness, and the mechanism of action remains unknown. We investigated the effects of prednisolone in a BA model induced by Rhesus Rotavirus (RRV). Liver samples were collected from day-21 and day-28 post-RRV infected mice injecte

Genetic diagnosis in fetuses with biliary tract system abnormalities: a meta-analysis and systematic review.

BACKGROUND: Biliary tract system (BTS) anomalies in fetuses are rare, but carry a wide range of clinical prognoses. The present meta-analysis aims to determine the prevalence and spectrum of genetic etiologies in fetuses with BTS anomalies and evaluate their clinical implications. METHODS: PubMed and Ovid Medline were searched up to June 2025. Studies reporting prenatal genetic results and outcomes of fetuses with BTS anomalies were included. The spectrum of genetic etiologies detected across included studies was systematically summarized. Pooled prevalence of genetic etiology was calculated

Prevalence and maternal-child clinical and socioeconomic factors associated with congenital anomalies in a Mexican hospital-based setting.

There is controversial evidence that some selected congenital anomalies (CA) are associated with sex, maternal age, urban-rural residence, or socioeconomic status among the Hispanic population. This study aimed to assess the prevalence and maternal-child clinical and socioeconomic factors across a wide range of CA in a hospital-based setting from northwest Mexico. From January to December 2023, a cross-sectional study for CA and live births at Durango General Hospital was performed. Hospital-based prevalence was calculated for all CA subtypes and grouped anatomical system defects. Associations

Feeding difficulties in children with esophageal atresia: A parent-reported multicenter study.

OBJECTIVE: Feeding difficulties (FDs) are common among children with esophageal atresia (EA) and tracheoesophageal fistula (TEF), but knowledge about their prevalence and risk factors is limited. This multicenter study aimed to assess the prevalence, subtypes, and associated factors of FD in children with EA/TEF. METHODS: Parents of children who underwent surgery for EA/TEF in four tertiary centers in Israel (2005-2022) completed a structured questionnaire. Pediatric feeding disorder (PFD) was diagnosed by means of the Montreal Children's Hospital Feeding Scale and classified by consensus cri

Preservation of the azygos vein versus ligation of the azygos vein during surgical repair of esophageal atresia-tracheoesophageal fistula - A systematic review and meta-analysis.

INTRODUCTION: Esophageal atresia-tracheoesophageal fistula (EA-TEF) is amongst the most common life-threatening developmental anomalies. For the surgical repair of esophageal atresia-tracheoesophageal fistula (EA-TEF), several interventional modifications have been suggested. One such modification is the preservation of the azygos vein, a key draining vein for the esophagus and surrounding structures. However, there is no clear consensus on the subject. OBJECTIVE: To evaluate whether preservation of the azygous vein offers clinical benefits compared with its ligation during primary repair of

Membranous esophageal atresia without tracheoesophageal fistula in a neonate: a rare case report and literature review.

BACKGROUND: Membranous esophageal atresia (MEA) without tracheoesophageal fistula (TEF) is a rare form of esophageal atresia (EA) that is often misdiagnosed as Gross type A esophageal atresia (isolated EA). However, the management strategies for these two conditions differ substantially. We present a case initially diagnosed as Gross type A EA, in which intraoperative recognition of key findings led to a revised diagnosis of MEA without TEF, enabling a single-stage repair and avoiding unnecessary staged reconstruction. Additionally, we have reviewed the literature to summarize the clinical cha

Early Living Donor Liver Transplant Is Associated With Improved Graft- and Patient-Survival Outcomes in Children With Biliary Atresia.

INTRODUCTION: Biliary atresia (BA) is the most common indication for pediatric liver transplantation. Pediatric end-stage liver disease (PELD) scores primarily determine deceased donor liver transplant (DDLT) waitlist priority. Due to persistent donor supply-demand mismatch, children typically have advanced disease and high PELD scores at the time of DDLT. Proactive living donor liver transplant (LDLT) may facilitate transplantation before critical illness. We hypothesized that early, low PELD, LDLT is associated with improved graft and patient survival in children with BA. METHODS: We retros

Revisiting Turkish Esophageal Atresia Regisrty data for quality indicators.

AIM: The Turkish Esophageal Atresia Registry (TEAR) data were revisited using quality indicators (QIs) to evaluate quality care provided to patients with esophageal atresia (EA). METHODS: Among 36 centers registering data in the TEAR database, only those treating more than four patients per year were included. Based on predefined QIs, each center was assessed for structural, procedural and outcome indicators. Mean percentages were calculated for each QI. Centers with a lower-than-mean percentage for adverse outcomes were determined as 'meeting' that QI, while those with higher percentage were

Liver mobilization during Kasai portoenterostomy: retrospective multicentre analysis.

BACKGROUND: Liver mobilization (LM) during Kasai portoenterostomy (KPE) is used by some surgeons to improve exposure but remains controversial due to anaesthetic challenges and concerns about increased adhesions complicating liver transplantation (LT). This study evaluated the impact of LM on intraoperative, postoperative, and long-term outcomes in biliary atresia. METHODS: A multicentre case-control study involving high-volume biliary atresia centres in Europe and North America was conducted. Preoperative, intraoperative, and postoperative clinical and laboratory data were retrospectively co

Timing of definitive surgery after external biliary drainage for completely perforated congenital choledochal cysts: a single-center retrospective cohort study.

BACKGROUND: The optimal timing of definitive surgery after external biliary drainage (EBD) for children with completely perforated congenital choledochal cysts (CCs) remains controversial. Prolonged EBD may increase the risk of drainage tube–related complications. This study compared the safety and efficacy of early (≤ 14 days) versus delayed (> 14 days) definitive surgery after EBD. METHODS: This study is a single-center retrospective cohort study that included all consecutive pediatric patients with complete perforation of choledochal cysts (CC) admitted between January 2018 and February 202

Complex mosaic form of trisomy chromosome 18 and monosomy chromosome X in a girl with esophageal atresia and mild developmental delay: Diagnostic and therapeutic challenge and a review of literature.

The coexistence of mosaic trisomy 18 and Turner syndrome represents an extremely rare genetic constellation, with only a few cases reported in the literature to date. We present the case of a 7-year-old girl with esophageal atresia, full trisomy 18 in lymphocytes, and mosaic copy number changes involving trisomy 18 with coexisting monosomy X in fibroblasts, suggesting the presence of additional cell populations lacking trisomy 18. In addition, we provide a review of the relevant literature. This case is of particular clinical significance, as it describes long-term survival in a child with Edw

PRDM9 Deficiency Drives Mosaic Promoter Deletions in Sporadic Hirschsprung Disease and Supports Blood-Based Molecular Stratification.

BACKGROUND & AIMS: Hirschsprung disease (HSCR) is a congenital enteric neuropathy with distal aganglionosis and dysmotility. Germline mutations explain many familial cases, but most sporadic cases lack a molecular explanation. We sought noncanonical mechanisms and blood-accessible biomarkers. METHODS: We performed RNA-sequencing (RNA-seq) on paired aganglionic and ganglionic colon biopsy specimens from 103 sporadic HSCR patients, with control colon samples (n = 22). Blood whole-genome sequencing (WGS) was available for 41 patients with RNA-seq profiles. We assessed PR/SET domain 9 (PRDM9) in

Intestinal obstruction by Meckel's diverticulum in stricturing Crohn's disease: coincidence?

We report the case of a 56-year-old male with a long-standing history of stricturing CD involving distal ileum. He had previously been treated with multiple biologics agents (infliximab, adalimumab, ustekinumab) due to persistent stenotic disease requiring repeated endoscopic ileal dilations. He presented to the emergency department with a two-week history of generalized abdominal pain, nausea and vomiting. Physical examination revealed diffuse abdominal tenderness and distension without signs of peritonism. Laboratory tests were unremarkable. Abdominal radiography demonstrated dilated small b

Enrolment criteria for surgical plication in pediatric diaphragmatic eventration: a systematic review of the literature.

PURPOSE: Diaphragmatic eventration (DE) is an uncommon but clinically significant condition in children. Although diaphragmatic plication is widely performed in symptomatic patients, standardized criteria guiding surgical indication remain poorly defined. This review aimed to identify the clinical-radiological parameters most consistently used to select pediatric surgical candidates. METHODS: We performed a literature search for English-language studies (2000-2025) reporting pediatric DE cases. Studies limited to radiology, genetics, or unrelated diaphragmatic disorders were excluded. Extract

Transgenic mouse model of congenital choledochal cyst.

PURPOSE: Choledochal cyst is a rare, congenital dilation of the hepatobiliary tree. Due to the associated malignancy risk, complete resection is recommended. There remains a risk of metachronous cholangiocarcinoma despite resection necessitating lifelong surveillance. Choledochal cysts are increasingly prevalent with an incompletely understood connection to carcinogenesis. We sought to develop a mouse model to reliably mimic human disease process of choledochal cyst. METHODS: Experimental transgenic mice were bred with a genotype of Pdx-Cre, TGFα, LSL-Kras G12D Mu/Wt. Control C57 mice were use

Age-Dependent Dynamics of the Biliary Microbiome in Children with Choledochal Cysts: Functional Remodeling Underlying Taxonomic Conservation.

Choledochal cyst (CC), a congenital biliary anomaly, is associated with recurrent infections, chronic inflammation, and an increased risk of malignancy. Although emerging evidence implicates the biliary microbiome in disease pathophysiology, its developmental dynamics in pediatric CC remain unclear. Using deep metagenomic sequencing and comprehensive functional annotation, this study characterized age-dependent changes in the biliary microbiome of 201 pediatric CC patients stratified into infancy (<1 year), early childhood (1-5 years), and later childhood (5-12 years). We found that while the
