Outcomes in laparoscopic versus robotic-assisted surgery for median arcuate ligament syndrome: a systematic review and meta-analysis.

Median Arcuate Ligament Syndrome (MALS) is a rare condition caused by external compression of the coeliac artery by the median arcuate ligament (MAL), leading to postprandial abdominal pain, nausea, vomiting, and weight loss. This study aims to compare outcomes of robotic (RMALR) and laparoscopic (LMALR) MAL release. A systematic search of PubMed/MEDLINE, Scopus, and Embase databases was performed in accordance with the Preferred Reporting Items for Systematic Reviews and Meta-Analyses guidelines to compare perioperative and postoperative outcomes of LMALR versus RMALR. Comparative studies inc

Comparative analysis of three fecal continence scoring systems in anorectal malformations: inter-score agreement and prognostic factors.

PURPOSE: To evaluate inter-score correlation and categorical agreement among the Kelly, Holschneider, and Templeton fecal continence scores in patients with anorectal malformations (ARM), and to identify predictors of poor continence. METHODS: In this retrospective single-center cohort, 110 surgically treated ARM patients (2007-2017) were assessed at latest follow-up using all three scores. Correlation was tested with Spearman analysis, categorical agreement with weighted kappa, and independent risk factors with multivariable logistic regression for each scoring system. RESULTS: Correlations

Robot-assisted choledochal cysts resection in children across all ages: a larger cohort single-center experience with 318 Cases.

Robotic-assisted surgery (RAS) is an emerging alternative to laparoscopic surgery (LAS) for the treatment of choledochal cysts. We aim to evaluate the safety and efficacy of robotic-assisted choledochal cyst resection (RACC) in children from neonates to 16-year-olds. In this retrospective study, 318 patients undergoing RAS between April 2020 and July 2025 were compared with 489 patients undergoing LAS between January 2015 and July 2025. The robotic cohort was stratified into six age-based subgroups: R1 (neonates), R2 (> 1 to ≤ 3 months), R3 (> 3 to ≤ 12 months), R4 (> 12 to ≤ 36 months), R5 (>

Parents' experiences of condition management in children born with esophageal atresia-tracheoesophageal fistula during their early childhood.

BACKGROUND: Esophageal atresia-tracheoesophageal fistula (EA-TEF) is a rare congenital malformation of the esophagus and airways. Following surgical repair, children commonly face ongoing aerodigestive morbidity. To address the limited research in this area, this study explored parents’ experiences managing EA-TEF during early childhood, a time marked by high morbidity, and evolving parenting roles. METHOD: As part of a larger project, five standardized focus groups were conducted with 22 parents of children aged 6 months to 7 years treated at a quaternary pediatric surgical center in U.S. Dis

Risk factors for post-ERCP pancreatitis and assessment of stent exchange intervals in children with chronic pancreatitis: a single-center retrospective study.

BACKGROUND: ERPD serves as the first-line endoscopic interventional therapy for pediatric chronic pancreatitis (CP). However, pediatric-specific risk factors for post-endoscopic retrograde cholangiopancreatography pancreatitis (PEP) and the optimal stent exchange intervals remain poorly defined. METHODS: In this single-center retrospective study, pediatric CP patients (≤ 14 years) undergoing ERPD procedures (2015-2024) were enrolled. PEP was diagnosed per ESGE 2020 criteria (new/worsened abdominal pain + amylase/lipase > 3×ULN). Univariate and multivariate logistic regression identified PEP r

Development of a lifelong core outcome set for oesophageal atresia ± tracheoesophageal fistula: the OCELOT study.

BACKGROUND: Despite anatomical correction, people born with oesophageal atresia±tracheoesophageal fistula (OA-TOF) experience lifelong morbidity. Core outcome sets (COSs) are recognised as a means of improving research quality and, as a consequence, improving patient outcomes; one was not available for this population. OBJECTIVE: The scope of the study was to develop a COS for people born with OA-TOF that would be applicable regardless of age or geographic location. STUDY DESIGN: Patient input was paramount to this study. For long-list generation, in addition to the systematic review (SR), p

Prenatal biliary imaging for the diagnosis of biliary atresia: a protocol for a multicentre prospective diagnostic accuracy study.

INTRODUCTION: Biliary atresia (BA) is a rare but serious infantile cholangiopathy requiring timely diagnosis and surgical intervention. However, current screening methods are limited by their postnatal initiation. By contrast, antenatal imaging is increasingly considered promising due to growing evidence of a prenatal onset of BA, although high-quality diagnostic evidence remains scarce. This study aims to evaluate the diagnostic performance of antenatal biliary imaging features for the detection of BA. METHODS AND ANALYSIS: This is a multicentre, prospective cohort diagnostic accuracy study

Robot-assisted Cyst Excision, Ductoplasty, and Embedding Portoenterostomy for Choledochal Cysts with a Small Caliber Hepatic Duct less than 6 mm.

Anastomotic stricture is a primary concern following the surgical treatment of choledochal cysts (CC), particularly in pediatric patients with narrow bile ducts (<6 mm), and remains the main indication for re-operation. Robotic-assisted surgery (RAS), with its superior 3D visualization and instrument dexterity, offers an ideal platform for the complex reconstruction required to prevent this complication. This study presents a robot-assisted modified technique designed to mitigate the risk of stricture, as demonstrated in a 34-month-old female with a Type Ia CC. The procedure involves complete

Research progress on the etiology and pathogenesis of pancreatic cancer: a narrative review.

Pancreatic cancer (PC) is a highly malignant tumor of the digestive system, characterized by the lack of effective methods for early screening, diagnosis and treatment. Despite its low incidence rate, it has an extremely high mortality rate. In recent years, the number of confirmed cases worldwide has been on a continuous rise, making PC the sixth leading cause of cancer-related deaths across the globe. At present, clinical treatment strategies including surgical resection, chemotherapy, radiotherapy, molecular targeted therapy and biological immunotherapy have not significantly improved the s

Management of postoperative leaks and infections in patients with esophageal atresia.

Esophageal atresia (EA) with or without tracheoesophageal fistula (TEF) remains a common, complex congenital anomaly with high survival but significant postoperative morbidity. Despite advances in surgical technique and neonatal care, one of the main contributors to postoperative morbidity is infection with anastomotic leakage occurring in up to 25 % of cases. Leakage of the esophageal anastomosis is associated with mediastinal and thoracic infection, prolonged hospitalization, and long-term sequelae. Prompt recognition and management can minimize short- and long-term complications for these p

Mechanism of tcirg1b deficiency in disrupting central nervous system homeostasis of zebrafish.

Microglial phagocytosis is crucial for maintaining central nervous system (CNS) homeostasis, a process that depends on normal lysosomal acidification and is precisely regulated by vacuolar-type ATPase (V-ATPase). While mutations in the V-ATPase a3 subunit (encoded by the tcirg1b gene in zebrafish) are a major cause of human malignant osteopetrosis, the subunit's function in the CNS remains unknown. To investigate the role of the V-ATPase a3 subunit in the zebrafish CNS, we generated a tcirg1b knockout model. Although mutant zebrafish displayed no early neuronal defects, adult brains exhibited

Technical feasibility should not outweigh long-term outcomes in biliary reconstruction for children.

The choice of biliary reconstruction after excision of choledochal malformations in children should prioritize long-term physiological outcomes over operative convenience. In response to the article by Kanojia et al. on the management of aberrant bile ducts at the porta hepatis during minimally invasive surgery, we wish to highlight that while hepaticoduodenostomy is technically attractive (particularly with laparoscopic and robotic platforms) evidence suggests it is associated with increased biliary reflux and reflux-related morbidity, especially in cases with complex hilar anatomy requiring
