Expanded Hepatic Progenitor Cells Featured with Aggregation of α-Synuclein Contribute to Pathologic Bile Duct Regeneration in Biliary Atresia.

Ductular reaction (DR) is a hallmark of biliary atresia (BA), but its underlying mechanisms remain unclear. In this study, we identified an expanded NCAM1+EpCAM+ hepatic progenitor cells (HPCs) as the predominant DR-related cell cluster using single-nucleus RNA sequencing, and NCAM1+EpCAM+ HPC-derived organoids exhibited impaired biliary differentiation in BA. α-synuclein was characterized as the signature gene of this cluster, exhibiting upregulated expression in liver tissues from both human BA and mouse BA models, as well as in the serum of patients with BA. Biliary organoids derived from m

Revisiting fundoplication in esophageal atresia: Implications for nutrition, stricture management, and wrap type.

BACKGROUND: Patients with esophageal atresia with or without tracheoesophageal fistula (EA-TEF) frequently require fundoplication to manage gastroesophageal reflux disease (GERD). Data is limited on whether fundoplication improves GERD-related outcomes and if outcomes vary by type of fundoplication. METHODS: This is a retrospective cohort study of patients <18 years-old who underwent EA-TEF repair and fundoplication at a single institution from January 2013 to September 2025. In the first aim, we compared GERD symptoms, medications, nutrition, and esophageal stricture dilation rate before and

Thymic Stromal Lymphopoietin-Driven B-Cell Hyperactivation Impairs Intestinal Epithelial Homeostasis Predisposing to Enterocolitis in Hirschsprung Disease.

BACKGROUND & AIMS: Hirschsprung-associated enterocolitis, a lethal complication of Hirschsprung's disease-a prevalent congenital disorder with high morbidity-has a poorly understood pathogenesis. We investigated how aberrant immune-stromal crosstalk drives mucosal injury in Hirschsprung's disease. METHODS: Integrated single-cell RNA sequencing, spatial transcriptomics, bulk RNA sequencing, and proteomics were performed using human Hirschsprung's disease tissues. Validation was conducted using murine Hirschsprung-associated enterocolitis (Ednrb-/-) models, flow cytometry, multiplex immunohisto

α7 nicotinic acetylcholine receptor activation promotes anxiety- and depressive-like behaviors associated with CREB/CRH signaling in the hypothalamic paraventricular nucleus.

Dysregulation of the hypothalamic-pituitary-adrenal (HPA) axis is a central feature of stress-related affective disorders, and corticotropin-releasing hormone (CRH) neurons in the hypothalamic paraventricular nucleus (PVN) initiate neuroendocrine stress responses. Although the α7 nicotinic acetylcholine receptor (α7 nAChR) has been implicated in emotional regulation, its role in PVN CREB/CRH signaling and anxiety- and depressive-like behaviors remains unclear. Here, using male C57BL/6 mice, we found that pharmacological activation of α7 nAChR with PNU282987 induced anxiety- and depressive-like

Gastrodin alleviates cardiomyocyte senescence by regulating autophagy through the AMPK/mTOR/4EBP1 pathway.

OBJECTIVE: To investigate whether Gastrodin (Gas) attenuates doxorubicin (Dox)-induced cardiomyocyte senescence and apoptosis by regulating the AMPK/mTOR/4EBP1 signaling pathway and autophagy-related processes. METHODS: A Dox-induced senescence model was established in H9c2 cardiomyocytes, and different concentrations of Gas (0.5 mM and 1 mM) were used for intervention. Senescence phenotypes were evaluated using SA-β-gal staining and senescence-associated proteins (P16, P21, and P53). Furthermore, the autophagy inhibitor 3-MA and the AMPK inhibitor Compound C (CC) were applied. Combined with

ARM in ARM? Investigating the co-occurrence of anorectal malformations and labioscrotal anomalies.

Anorectal malformations (ARMs) are known to be associated with other defects, however there is no evidence addressing the co-occurrence of ARMs with anomalies of structures derived from the labioscrotal folds. We performed a systematic review (Prospero: CRD420251013945) aiming at investigating correlations and patterns between ARMs and labioscrotal anomalies (LSAs), when presenting simultaneously. Our search identified 1642 articles. After abstract screening, 344 full texts were assessed and 144 met the inclusion criteria. In total, these reported on 319 individual cases with both ARM and LSA.

Balancing two defects: surgical decision-making in low-birth-weight neonates with esophageal atresia and tetralogy of Fallot-a case series.

BACKGROUND: Esophageal atresia with tracheoesophageal fistula (EA/TEF) and tetralogy of Fallot (TOF) are both life-threatening congenital malformations in the neonatal period. Their coexistence poses substantial challenges for treatment planning, particularly in low-birth-weight infants, for whom no consensus management strategy has yet been established. CASE PRESENTATION: We report the diagnostic and therapeutic courses of two preterm, low-birth-weight infants with concomitant EA/TEF and TOF. Case 1 was a male neonate with a birth weight of 1.7 kg. Intraoperative exploration revealed an appro

Feasibility and safety of transumbilical single-incision laparoendoscopic surgery for giant choledochal cysts in infants: a single-center retrospective study.

BACKGROUND: Radical resection of giant choledochal cysts (diameter ≥ 10 cm) in infants presents technical challenges, particularly when attempted with minimally invasive approaches. The primary difficulty lies in the cyst’s mass effect in the limited abdominal space. Although transumbilical laparoendoscopic single-site surgery (LESS) offers potential cosmetic and minimally invasive advantages, evidence regarding its feasibility and safety for managing these giant cysts remains limited. METHODS: This single-center, retrospective study with a historical control group included infants who underwe

Pediatric urethroscope versus flexible choledochoscope for removal of distal protein plugs and calculi in choledochal cyst: A prospective cohort study.

BACKGROUND: Choledochal cyst (CC) is a common congenital biliary malformation that occurs frequently in children. CC is often complicated by protein plugs, calculi, and other debris, predisposing patients to cholangitis, pancreatitis, and other adverse events. Thorough intraoperative removal of obstructive debris is critical for improving postoperative outcomes. At present, intraoperative endoscopy represents the main modality for removing distal obstructions; however, consensus regarding the comparative efficacy of pediatric urethroscope and flexible choledochoscope remains lacking. Endoplasmic reticulum autophagy in inflammatory diseases.

The endoplasmic reticulum (ER) is responsible for the synthesis, modification, and folding of various intracellular proteins. Under strong external stimuli, the ER often undergoes significant structural disorganization and functional abnormalities, a process that generally accelerates the onset and progression of inflammatory responses and related diseases, such as infections and sepsis, digestive system diseases, cancer, neurological disorders, circulatory diseases, and musculoskeletal diseases. Therefore, maintaining ER homeostasis is crucial for delaying the inflammatory process. As an impo

Robotic median arcuate ligament release in a pediatric surgery center.

PURPOSE: Median arcuate ligament syndrome is characterized by chronic abdominal pain due to compression and neurofibrosis of the celiac trunk. While it frequently presents in adulthood, adolescents can be affected. The syndrome is often treated with an open median arcuate ligament release. Robotic surgery has grown in popularity for complex cases to improve visualization and post-operative recovery, however there is limited data on the feasibility and benefits of robotic median arcuate ligament release compared to an open procedure, particularly in a pediatric cohort. METHODS: A retrospective

Clinical significance and natural history of esophageal histologic inflammation without macroscopic findings in children.

Routine esophageal biopsies during pediatric esophagogastroduodenoscopy (EGD) are recommended by current guidelines, even in the absence of macroscopic abnormalities; however, their clinical value remains uncertain. We conducted a retrospective review of all diagnostic EGDs performed at our institute between January 2019 and December 2021, a period during which esophageal biopsies were routinely obtained. Endoscopic findings and histologic results were analyzed to determine the diagnostic yield and clinical impact of biopsies in patients with a normal-appearing esophagus. A total of 1400 proce
