Torsion of a tumor within an accessory ovary.

BACKGROUND: An accessory ovary is a rare congenital anomaly of the female reproductive tract. This report illustrates a case of a cyst arising from an accessory ovary complicated by torsion. CASE: A 39-year-old nulliparous woman presented with lower abdominal pain and was found on physical examination to have an 8-cm mass anterior to the uterus. Ultrasonography of the pelvis showed a 9.3×6.7 cm cystic lesion with a solid component anterior to the uterus suggestive of a left ovarian cyst. Laparoscopy was performed for presumed ovarian torsion. Torsion of a tumor arising from an accessory ovary

Bilateral malrotation and lobulation of kidney with altered hilar anatomy: a rare congenital variation.

Variations of the structure and position of the kidney along with the variations of renal vessels are the most frequently reported. Rotational variations form a rare entity that is not cited in most of the embryology textbooks. During educational cadaveric dissection of a 57-year-old female, a complex picture of bilateral anatomical variants was encountered. Bilateral malrotation of kidney, bilateral lobulated kidneys along with open hilum of kidney was observed, so that the contents of the sinus were exposed. The renal pelvis was present anterior to the renal vessels instead of posterior posi

[Girls' abdominal pains and gynaecological emergencies].

Among many causes of abdominal pain in children, gynaecological ones have to be systematically considered in even no pubescent girls. Diagnosis leans on physical and US examinations and has to screen first urgent causes before frequent ones. Rare but urgent are ovarian torsions either on a normal ovary or an underlying ovarian tumor, an earlier surgery may avoid oophorectomy. Hematocolpos linked to imperforate hymen is the most common obstructive congenital abnormality of the female genital tract and the diagnosis is easy with inspection of external genitalia. Ovarian tumors are infrequent and

Surgical treatment of neonatal ovarian cysts.

Medical experts are still at issue over the most suitable management of simple neonatal ovarian cysts exceeding 40mm and complex cysts of any size. The authors present surgical treatment of these cysts by classical laparotomy and laparoscopy. The study included 13 newborn babies surgically treated for 6 simple and 7 complex ovarian cysts. The diameter of the cysts ranged from 29 to 102mm. The age of children was from 2 days to 10 months. The open classical laparotomic approach was performed in 8 babies. In the laparotomy group, cystectomy was done in 3 infants with simple cysts. The other 5, p

Clinical, biological and genetic analysis of anorchia in 26 boys.

BACKGROUND: Anorchia is defined as the absence of testes in a 46,XY individual with a male phenotype. The cause is unknown. METHODS: We evaluated the clinical and biological presentation, and family histories of 26 boys with anorchia, and sequenced their SRY, NR5A1, INSL3, MAMLD1 genes and the T222P variant for LGR8. RESULTS: No patient had any associated congenital anomaly. At birth, testes were palpable bilaterally or unilaterally in 13 cases and not in 7; one patient presented with bilateral testicular torsion immediately after birth. The basal plasma concentrations of anti-Müllerian horm

Ovarian cyst torsion in a patient with congenital lipoid adrenal hyperplasia.

Congenital lipoid adrenal hyperplasia (CLAH) is the most severe form of congenital adrenal hyperplasia, characterized by lack of synthesis of all gonadal and adrenal steroid hormones. Ovarian cysts can develop as complications of CLAH. However, the precise mechanism of development and natural history of such cysts in patients with CLAH have not yet been determined. We recently treated a 14-year-old female patient with CLAH and ovarian cyst torsion. As a neonate, she presented with vomiting, skin pigmentation, and electrolyte imbalance. At that time, her serum adrenocorticotropic hormone concen

Autoamputation of the fallopian tube after chronic adnexal torsion.

Autoamputation of the adnexa, although very rare, can either be congenital or acquired. For affected women it can have future fertility implications. Although in some women it is asymptomatic, in most cases a history of acute followed by chronic pelvic pain can be elicited. This is a case of autoamputation of a fallopian tube after chronic adnexal torsion. We suggest that all patients of reproductive age with acute lower abdominal pain should have a pelvic ultrasound scan and, if symptoms persist, early recourse to laparoscopy. Misdiagnosis can lead to ovarian damage, loss of tubal function, i

Unilateral ovarian and fallopian tube agenesis.

INTRODUCTION: The unilateral agenesis of the ovary and fallopian tube is an extremely uncommon event with only few cases described in literature. CASE REPORT: Here, we report a case of a 30-year-old woman admitted to our Unit for increasing pelvic pain. In the suspect of a corpus luteum hemorrhage, we performed a laparoscopy identifying the rare anomaly. DISCUSSION: Together with the predisposing genetic and/or environmental factor not yet discovered, two are the hypotheses explaining the absence of one or both the adnexes, the mechanical hypothesis, i.e., the asymptomatic torsion of both Fa

Pregnancy outcome of patients with dermoid and other benign ovarian cysts.

OBJECTIVE: To investigate pregnancy outcome of patients with dermoid and other benign ovarian cysts. METHODS: A population-based study comparing all pregnancies of women with and without benign ovarian cysts was conducted. Deliveries occurred during the years 1988-2007 at Soroka University Medical Center. A multivariate logistic regression model, with backward elimination, was constructed to find independent risk factors associated with benign ovarian cysts. RESULTS: During the study period there were 212,114 deliveries, of which 93 occurred in patients with benign ovarian cysts. Most of the

Laparoscopic treatment of acute ovarian incarceration into the pelvic peritoneal sac.

Pelvic pain is a common symptom in women of reproductive age. Acute pelvic pain with rapid onset demands prompt diagnosis and treatment. We report the case of a patient with ovarian incarceration of acute onset. To our knowledge, this is the first report of ovarian incarceration into the pelvic peritoneal sac in a woman of reproductive age. In the present case, laparoscopy was useful in establishing the cause of pelvic pain. The patient reported severe lower right quadrant abdominal pain of sudden onset. At laparoscopic examination, the right fallopian tube was normal; however, the right ovary

Two-port laparoscopic management of an autoamputated ovarian cyst in a newborn.

A 4-week-old newborn underwent laparoscopic removal of a hemorrhagic cyst measuring 4 cm in diameter. Preoperative diagnostics suggested autoamputation, which is a rare complication of fetal ovarian torsion. The laparoscopic procedure, lasting 26 minutes, was performed, without any major intraoperative complications--bleeding, rupture, or leakage. The modified 2-port technique was used. The identification of all structures was exact. The hemorrhagic cyst was freed of the cyst bed, suctioned to fit the size of the umbilical port, and removed. The presentation of a blind adnexal stump proved aut

Acute abdomen due to twisted ovarian immature teratoma in a 7-year-old girl: magnetic resonance findings with histopathologic correlation.

Immature teratomas represent only the 1% of ovarian teratomas, and they are the third most common primitive germ cell tumor; generally, they are congenital lesions and affect mainly the first 2 decades. We present the case of a 7-year-old child who came at our attention because of an acute low abdominal pain due to the torsion of an ovarian immature grade 1 teratoma on its pedicle. After ultrasound in the emergency department, preoperative evaluation was done by performing a magnetic resonance study, whose findings helped in suggesting the curative approach. The magnetic resonance imaging char

Ovarian histological findings in an adult patient with the steroidogenic acute regulatory protein (StAR) deficiency reveal the impairment of steroidogenesis by lipoid deposition.

CONTEXT: The steroidogenic acute regulatory protein (StAR) is essential for the production of steroid hormones. The mutations in the StAR gene typically cause congenital lipoid adrenal hyperplasia (lipoid CAH), characterized by severe adrenal insufficiency in both sexes and complete female external genitalia in genetic males. Affected 46, XX females feminize at puberty and menstruate but have progressive hypergonadotropic hypogonadism. It has been hypothesized that the cholesterol accumulation in the steroidogenic cells destroys the residual steroidogenic capacity and progressive ovarian failu

[Fetal juvenile granulosa cell tumor with hermaphroditism verus - prenatal diagnosis, management and outcome].

Fetal ovarian cysts are common during pregnancy and after delivery. Most of these cysts are simple cysts that involute during pregnancy or in the first months of life. However, complicated cyst with a heterogeneous structure and also possible and can result in various complications: rupture, hemorrhage, ascites, edema of the labia, compression of other viscera, and ovarian torsion. In this case report we describe rare diagnosis of a complicated fetal ovarian cyst with edema of the labia and moderate ascites. The neonate had ambiguous genitalia with clitoromegaly. The newborn underwent surgery
