Mixed mesodermal tumors and carcinosarcomas of the uterus are classified as sarcomas. However, in other sites, malignant biphasic tumors may be classified as carcinomas, mesotheliomas, or sarcomas. Mullerian mixed tumors, as a group, are uncommon. Only 27 cases of this tumor of the fallopian tube have been reported to date. We report the 28th case of mixed mesodermal tumor, originating in the fallopian tube. A 79-year-old woman had mistaken a bloody vaginal discharge for hematuria. A patient with Stage III fallopian tube carcinosarcoma treated with cisplatin, doxorubicin, and cyclophosphamide, surviving 31 months disease-free after diagnosis, is presented. Review of the literature revealed this is the 28th case report of malignant mixed Mullerian tumor of the fallopian tube, and fifth instance in which chemotherapy was used as a primary therapy. Modern chemotherapy appears to have improved survival.

Four cases of carcinosarcoma and mixed müllerian tumors of the fallopian tube are presented. Each patient presented with abnormal bleeding and a pelvic mass. All underwent total abdominal hysterectomy, bilateral salpingo-oophorectomy, staging, and cytoreduction. The diagnosis and treatment of sterility due to abnormalities of the infundibulum of the Fallopian tube are reviewed. Knowledge on the physiology of the distal part of the tube explains the mechanisms of sterility when a lesion occurs, although diagnosis is based mainly on hysterosalpingography and laparoscopy. The requisite treatments are either percoelioscopic or surgical, and techniques and results with these treatments are reviewed. Special attention is given to tubal stenosis as a consequence of embryonic cysts of the tube.

We report a case of non-ruptured tubal pregnancy at 38 weeks of pregnancy with the survival of mother and baby. Following salpingotomy a 1860 gr and 43 cm normal healthy male infant was delivered. The fallopian tube with placental tissue was removed by salpingectomy. Fetal development post partum and recovery of the mother were normal.

A case of a patient with bilateral ovarian cancer and a uterine malignant mesodermal mixed tumor with ascites and metastatic disease is presented. Flow cytometry, DNA fingerprinting, and immunohistochemistry were performed to assess the origin of these malignancies. Urogenital cysts are retroperitoneal or mesenteric cysts that are derived from vestigial remnants of the embryonic urogenital apparatus. Although their precise embryonic origins are usually obscure, occasionally they display histologic features indicative of a mesonephric or metanephric origin. This report describes an unusual mesenteric müllerian cyst consisting of an endosalpingial lining and two-layered smooth muscle walls, probably representing a müllerian duct duplication cyst.

Several articles have been published recently that discuss the role of electron microscopy in the diagnosis and study of gynecologic neoplasms. Malignant mixed müllerian tumor of the fallopian tube is an extremely rare neoplasm, with less than 30 cases reported. Clinical signs and symptoms are usually nonspecific and include lower abdominal pain, fever, and vaginal bleeding. The age at diagnosis varies from 14 to 76 years, with a mean of 55 years. Prognosis is poor, with only one woman surviving more than five years. Surgery is the treatment of choice, although chemotherapy may be of some benefit.

Two recent pathologically oriented reports suggest that surgically significant parovarian cysts are most often of mesothelial origin rather than mesonephric or paramesonephric origin, as traditionally thought. In a series of eight large intraligamentous parovarian cysts, seven were considered to be of paramesonephric origin on the basis of both surgical and histologic observations. Determining the origin of these cysts has relevance from a surgical point of view in the younger patient.
