Metformin Reverses Progesterone Resistance in Endometrial Cancer by Targeting the AMPK-FOXO1-CALB2 Pathway.

INTRODUCTION: Progesterone therapy is standard for fertility-preserving endometrial cancer (ECC) patients, but primary and secondary resistance limit its efficacy. Metformin (MET) combined with progesterone shows promise, yet the synergistic mechanism remains unclear. MATERIALS AND METHODS: In vitro cellular studies and nude mouse xenograft models were used. Cell proliferation, migration, and apoptosis were assessed by CCK8, flow cytometry, Western blot, and immunofluorescence. RNA sequencing and cell transfection verified that Forkhead box protein O1(FOXO1) inhibits ECC cell proliferation by

Chronic Mild Stress Protocol Applied to Male Rats, From Peripuberty to Adulthood, Negatively Affects Reproductive Outcomes and Offspring Development.

BACKGROUND: Depression is often observed in reproductive age. After the COVID-19 pandemic, it became more evident mainly in adolescents. The exposure to stressful situations is strongly related to mental diseases development. In 2009, the Food and Drug Administration (FDA) approved the escitalopram for treatment of depression in children and adolescents. AIMS: To evaluate the effects of paternal chronic mild stress (CMS), with or without escitalopram treatment, on sperm chromatin protamination, testicular SIRT1 expression, reproductive outcomes, and offspring development using a rat model. Staged Management of Recurrent Obstructed Hemivagina in Herlyn-Werner-Wunderlich Syndrome: A Case Report and Literature Review.

BACKGROUND: Herlyn-Werner-Wunderlich syndrome (HWWS) is a rare congenital Müllerian anomaly characterized by uterus didelphys, obstructed hemivagina, and ipsilateral renal anomaly. Recurrence after prior surgical treatment is particularly rare. We report a complex recurrent case of obstructed hemivagina in a patient with HWWS and duplicated vagina, successfully managed with a staged surgical approach. CASE PRESENTATION: A 19-year-old woman with a history of two prior vaginal septum resections and excision of a perineal mass (histopathology confirming ectopic colonic tissue) presented with rec

Detection of female genital tract abnormalities in antenatally detected fetal unilateral renal agenesis: Original retrospective case series with review of literature.

OBJECTIVE: In females, unilateral renal agenesis is associated with genital tract abnormalities, which can cause complications in childhood and beyond. However, its prevalence and characteristics are not consistently integrated into antenatal counselling. METHODS: An original retrospective case series study (2005-2024) at Guy's and St Thomas' Hospital identified fetuses with isolated antenatally diagnosed unilateral renal agenesis from an Astraia database, with standardized 6-week postnatal imaging. RESULTS: 110 cases of renal agenesis were identified. Of these, fifteen cases (n = 15) were e

Global and US standards for intrapartum surveillance of the pregnant and the fetal patient.

OBJECTIVE: Cardiotocography is a dual-signal physiologic modality monitoring the pregnant and fetal patient. The objective of the present study was to compare global guidelines with the recently updated US Clinical Practice Guideline. METHODS: A comparative analysis was conducted across global guidelines and the US guideline, evaluating three domains: uterine physiology, fetal heart rate interpretation, and management models. Definitions, interpretive structures, and triggers for intervention were compared without attempting to reconcile divergent frameworks. RESULTS: Global guidelines (card

Clinical Insights Into Linear Enamel Hypoplasia: A Case Report.

Linear enamel hypoplasia (LEH) is a failure or aberration of enamel to form correctly, resulting in bands of decreased enamel on the tooth surface. Many reasons, including genetics, malnutrition, and childhood diseases, can cause LEH, which solely affects the neonatal line. LEH is a kind of environmental hypoplasia with many symmetrical and ring-like abnormalities. Amelogenesis is a one-time procedure, and any abnormality might result in permanent enamel abnormalities. Enamel, unlike other human tissues, cannot be restored. The consequences of enamel hypoplasia vary based on illness severity,

Modern hysteroscopic management of congenital uterine anomalies: current techniques, emerging technologies, and future directions.

BACKGROUND: Congenital uterine anomalies are clinically relevant in reproductive medicine, particularly among women with infertility, recurrent pregnancy loss, adverse obstetric outcomes, or obstructive symptoms. Hysteroscopy has transformed the management of correctable intracavitary anomalies by enabling conservative, image-guided reconstruction of the uterine cavity while avoiding the morbidity of historical abdominal metroplasty. SCOPE: This narrative review critically examines contemporary hysteroscopic management of septate uterus, dysmorphic uterus, bicorporeal septate uterus and rare

Unicornuate Uterus With a Rudimentary Horn Adenomyoma, Ipsilateral Renal Agenesis, and Deep Infiltrating Endometriosis: A Case Report.

Congenital uterine malformations are often difficult to classify, and an incorrect label can carry over for years across successive clinical records. We describe a 39-year-old woman, gravida 1 para 1, referred for chronic pelvic pain and intermenstrual bleeding, whose uterus had previously been recorded as didelphys. Pelvic magnetic resonance imaging, with T2-weighted sequences in three planes, T1-weighted and fat-saturated sequences, diffusion-weighted imaging, and post-gadolinium imaging, instead demonstrated a unicornuate uterus with a left, cavitated, non-communicating functional rudimenta

Missed Until Found: Diagnosis and Hysteroscopic Management of a Uterine Septum in a Woman With Primary Infertility.

Congenital uterine anomalies are an important yet frequently overlooked cause of female infertility, with a septate uterus representing the most common developmental anomaly of the uterine cavity. Although some affected women remain asymptomatic, others present with infertility, recurrent pregnancy loss, or adverse obstetric outcomes, making early recognition essential for appropriate management. We report the case of a 26-year-old nulligravid woman from Azerbaijan who presented with primary infertility of three years' duration despite regular menstrual cycles and an otherwise unremarkable inf

Immune remodeling in HPV-associated cervical disease.

INTRODUCTION: Persistent high-risk HPV infection is the principal cause of cervical dysplasia, but variation in disease phenotype may also be associated with differences in the local immune and microbial environment. METHODS: We characterized the cervicovaginal immune microenvironment in relation to HPV status, vaginal community state type (CST), and cytological grade. RESULTS: HPV-positive samples showed reduced IL-4 and IL-17E/IL-25, while IFN-α2 and IL-3 varied across community state types in a non-monotonic, CST-specific manner: IFN-α2 was relatively elevated in the Lactobacillus gasseri

Endometrial receptivity failure in assisted reproduction: immune-cellular niches, mechanistic evidence, and translational boundaries.

Endometrial receptivity failure and recurrent implantation failure (RIF) are clinically heterogeneous outcomes shaped by embryo competence, uterine anatomy, endocrine support, immune adaptation, and the timing of embryo-endometrial synchrony. This Review focuses on how immune-cellular niches, cell-specific and spatiotemporal mechanisms, disease-modified receptivity states, and evidence-to-translation boundaries should be interpreted in assisted reproduction. Single-cell, spatial, and multi-omics studies increasingly resolve abnormalities in stromal, sepithelial, vascular, perivascular, and imm

Primary Amenorrhea due to Mayer-Rokitansky-Küster-Hauser Syndrome Managed with Laparoscopic Davydov Procedure: A Case Report.

Mayer-Rokitansky-Küster-Hauser (MRKH) syndrome is a congenital Müllerian anomaly characterized by uterovag-inal agenesis in phenotypic females with normal second-ary sexual characteristics and preserved ovarian function. Vaginal reconstruction may be required to allow satisfac-tory sexual function, particularly when nonsurgical dilation is declined, not tolerated, or not preferred. Laparoscopic Davydov vaginoplasty uses autologous peritoneum to line the neovaginal canal and may provide favorable anatomical outcomes when postoperative dilation is performed consis-tently. We report the case of a

Inhibition of SLC38A2 and loss of its N-glycosylation impair decidualization by promoting autophagy and apoptosis.

Decidualization plays a crucial role in embryo implantation and successful pregnancy. Dysregulation and dysfunction of decidualization caused by any factor may lead to a variety of pregnancy related disorders. However, many underlying molecular pathways regulating decidualization remain unknown, especially the impact of glycosylation. Protein glycosylation is vital for maternal fetal crosstalk, and its aberrance may result in adverse pregnancy outcomes. This study focused on SLC38A2, a member of the solute carrier family (neutral amino acid transporter), to investigate its function in endometr

Clinical, Biochemical and Pathological Findings in Symptomatic and Asymptomatic Bitches With Intrauterine Content.

Intrauterine contents are a common ultrasound finding in bitches, with variable significance. Clinical, analytical and histopathological findings were compared between symptomatic and asymptomatic bitches with intrauterine contents that underwent ovariohysterectomy. In this cross-sectional study, the majority were symptomatic (87.9%, 123/140). Cystic endometrial hyperplasia was the most frequent lesion, with greater severity in symptomatic bitches. Uterine disease in bitches showed a heterogeneous presentation. Haematological and biochemical abnormalities were common in both groups and showed
