The findings associated with diethytlstilbestrol (DES) exposure in utero are expanding rapidly. Structural abnormalities in both the cervix and uterus are well documented; in addition the authors have noted indications of an abnormal healing response in these women. Locally destructive methods such as cryosurgery, cauterization, and excision have resulted in permanent and significant physical damage. Caution is advised in attempting even minor gynecologic procedures on DES-exposed offspring.

Seventy-four postmenopausal women presenting with vaginal atrophy were treated with either Ovestin vaginal cream (Group A, 23 women: 1 mg/day E3; Group B, 30 women: 0.5 mg/day E3) or vaginal suppositories (Group C, 21 women: 0.5 mg/day E3), applied daily for 3 wk (A and B) or 2 wk (C) before retiring. Out of a series of 219 women who had had a child with anencephaly or spina bifida (ASB) in the Manchester area, there were 69 where this ASB pregnancy had been both preceded and succeeded by at least one other pregnancy. There was a significant excess of spontaneous abortions when the preceding pregnancy was compared with the succeeding pregnancy. Half of these abortions were followed by curettage of the uterus, a fact which is a little against the trophoblastic rest hypothesis.

Diethylstilbestrol (DES) was widely prescribed to pregnant women from 1940 to 1960. This resulted in multiple benign abnormalities of the genital tract and occasional malignant transformation in women who had been exposed to DES in utero. Over 300 cases of clear cell adenocarcinoma of the vagina have been described to date. In this study, 18 DES-exposed women and 20 normal age-matched controls were studied with ultrasound. A total of 116 consecutive obstetric ultrasound examinations were reviewed. An anechoic area was observed at the placental-endometrial junctionin all 54 fundal placentas, 51 of the 52 posterior placentas, and 9 of the 36 anterior placentas. Detail was enhanced by use of a focused transducer with digital gray-scale equipment, and visualization of anterior placentas was improved by placing a water bath between the transducer and the skin surface. The anechoic area was seen beginning in the twelfth week of gestation and corresponds best to the dilated vessels of the decidua basalis.

Morphological data from 3,614 well-preserved human embryos derived from artificial termination of pregnancy were used to determine whether ectopic implantation or enlarged myomas could enhance the prevalence of localized malformations of the embryo. Twelve patients with a double uterus, unilateral vaginal obstruction, and ipsilateral renal agenesis are described. The clinical presentation varies, depending on whether the obstruction was partial or complete. In rare instances a communication existed between the obstructed vaginal pouch and the opposite patent vagina through a defect in the septum of the double uterus. Early accurate diagnosis followed by the excision of the obstructing vaginal septum offers complete relief of symptoms while preserving reproductive capacity.

To identify risk factors for various cervical abnormalities, 237 women with abnoromal cervical smears and 422 control women were interviewed. Cervical biopsy specimens taken from the patients with abnormal smears were reviewed according to standard criteria by one pathologist and classified as follows: 65 carcinoma in situ, 81 severe dysplasia, 44 mild dysplasia and 47 normal histology. A 16 1/2-year-old phenotypic female had primary amenorrhea, mild mental retardation, radioulnar synostosis, and other minor anomalies. Chromosome constitution of leukocytes and skin fibroblasts was 48,XXXX. Plasma levels of gonadotropins were increased, and those of estrogens, decreased consistent with ovarian failure. Laparoscopy showed a small midline uterus, and 2 fallopian tubes, and fimbriae. Neither ovaries nor gonadal streaks were seen on either side. This patient appears to represent the first instance of 48,XXXX aneuploidy with documented absence of ovaries.

We have conducted a phase II trial of cisplatinumdiamminodichloride (CPDD) which not only demonstrated its remarkable activity in embryonic carcinoma of the testes, but also in ovarian carcinoma, in melanoma, and in epidermoid carcinoma, especially of the head and of the uterus cervix. Its toxicity, manifested mainly in the digestive and renal tracts, confines its administration to hospitalized patients only. This compound is now indicated in combination therapy for the above-mentioned tumors.

In this survey of 210 women with a history and/or gross changes of the cervix and vagina characteristic of DES exposure, changes related to vaginal adenosis were correlated with age. The mean age of women with columnar epithelium alone was 18.5 +/- 3.2 years, columnar epithelium with squamous metaplasia was 20.2 +/- 3.4 years, and squamous metaplasia alone was 23.0 +/- 2.7 years.
