Progesterone resistance in atypical endometrial hyperplasia: Expression and mechanisms of hormone-responsive molecules.

This study aimed to delineate the molecular profile underlying progesterone resistance in atypical endometrial hyperplasia (AEH), a precancerous condition associated with a high risk of malignant transformation. Using a retrospective cohort of 20 AEH patients who completed a 6-month progestin therapy, we compared protein expression between 10 progesterone-resistant and 10 progesterone-sensitive tissues using immunohistochemistry and Western blot analysis. The results revealed a distinct molecular signature in resistant tissues, characterized by significant upregulation of estrogen signaling co

Single-cell transcriptomic analysis of endometrium pre-receptivity and receptivity stages in porcine.

Endometrial receptivity is a key factor influencing embryo implantation and development, and is therefore of significant research importance. In this study, we constructed single-cell transcriptome maps of the porcine endometrium during the pre-receptivity and receptivity phases, and identified the major cell types within the endometrial tissue. It is worth noting that there are significant differences in the composition of epithelial cell subpopulations during the pre-receptivity and receptivity phases. We examined the functional characteristics of endometrial epithelial cells and stromal cel

Understanding the impact of Mayer-Rokitansky-Küster-Hauser syndrome on sexual wellbeing-a qualitative study.

BACKGROUND: Mayer-Rokitansky-Küster-Hauser syndrome (MRKH) is characterized by the absence or underdevelopment of the uterus, cervix, and/or upper vagina. Individuals with MRKH often experience significant psychosexual impacts, including reduced self-esteem and sexual wellbeing. Yet, little is known about their lived experiences of sexual wellbeing. AIM: To explore sexual wellbeing among individuals with MRKH. METHODS: Participants (n = 12) were recruited through MRKH Canada and partook in semi-structured interviews centered on their experiences of navigating sexuality. Transcripts were subj

Cervicovaginal Virome Restructuring Associated with HPV Status, Bacterial Dysbiosis, and Cervical Cytological Abnormalities.

The cervicovaginal virome remains poorly characterized in relation to human papillomavirus (HPV) infection and bacterial community structure. We performed shotgun metagenomic sequencing of 311 cervicovaginal specimens from a Kazakhstani cohort spanning seven groups defined by HPV status and cervical cytology. Viral community composition differed according to HPV status, with increasing representation of the oncogenic Alphapapillomavirus 9 clade across more abnormal cytological categories. Predicted phage functional profiles also differed between NILM HPV-positive and NILM HPV-negative women. I

Biomarkers for Endometrial Receptivity: Implications for Infertility, Implantation Failure, and Advances in Diagnosis and Treatment.

Female infertility affects millions of women worldwide and is frequently caused by ovulatory disorders and uterine pathologies. Among these, endometrial abnormalities play a central role, especially in cases of embryo implantation failure during assisted reproductive techniques (ARTs). Endometrial receptivity, the period during which the endometrium prepares to receive and support embryo implantation, is a complex process regulated by essential morphological, molecular, and immunological changes necessary for a successful pregnancy. This review examines the mechanisms that govern endometrial r

YWHAZ loss is associated with endometrial dysfunction in proliferative-phase endometriosis.

Endometriosis is an estrogen-dependent inflammatory disorder frequently associated with infertility and characterized by progesterone resistance and impaired endometrial receptivity. While ectopic lesions define the disease, accumulating evidence indicates that molecular abnormalities within the eutopic endometrium substantially contribute to infertility. In this study, we performed transcriptomic analysis of proliferative-phase eutopic endometrium from women with and without endometriosis and identified 44 dysregulated genes, including 13 upregulated and 31 downregulated genes, in women with

Revealing an enhanced cytotoxic immune microenvironment at the human maternal-fetal interface in preeclampsia.

Preeclampsia is a multifactorial pregnancy complication characterized by hypertension and immune dysregulation, which can lead to adverse maternal and fetal outcomes. Decidual immune cells are critical for pregnancy homeostasis, yet their functional abnormalities and mechanisms in preeclampsia remain poorly understood. Herein, we sorted decidua immune cells from preeclampsia patients for single-cell RNA sequencing to characterize cellular composition and functional heterogeneity. We delineated the immune cell landscape of the decidua in preeclampsia. A hallmark of decidual immune microenvironm

Multi-Omics Analysis Reveals Inflammatory Activation and Maternal-Fetal Interface Remodeling in Spontaneous Abortion.

BACKGROUND: Spontaneous abortion (SA) is a common adverse outcome of early pregnancy, yet its underlying pathophysiological mechanisms remain incompletely understood. Accumulating evidence suggests that dysregulated inflammatory responses at the maternal-fetal interface play a critical role in pregnancy loss. However, the potential associations between alterations in gut microbiota, metabolic disturbances, and localized decidual inflammation in patients with SA have not been systematically characterized. METHODS: Women with SA (n = 30) and those with normal early pregnancy (NP, n = 28) were e

Multi-omics insights into immune tolerance at the maternal-fetal interface in recurrent pregnancy loss: mechanisms, integration challenges, and translational perspectives.

Recurrent pregnancy loss (RPL) is a heterogeneous reproductive disorder in which dysregulation of maternal-fetal immune tolerance, aberrant decidual immune remodeling, and altered inflammasome signaling have been implicated within a complex multi-omics landscape. Multi-omics profiling (genomics, epigenomics, single-cell/spatial transcriptomics, proteomics, metabolomics, microbiome analyses, and immunomics) is increasingly being used to characterize mechanistic heterogeneity in RPL and to support biomarker discovery and immune-informed stratification. Genomic studies have associated chromosomal

Recognition of intrapartum uterine rupture, anesthetic management, and maternal-fetal outcomes: strategies for rapid response.

PURPOSE OF THE REVIEW: Uterine rupture is a rare but life-threatening obstetric condition that can lead to catastrophic outcomes for pregnant patients and their offspring. As the trial of labor after cesarean births rises and other uterine procedures become more common, the observed rates of this complication have increased. We aim to provide guidance on risk stratification, timely recognition, anesthetic considerations, hemorrhage management, and associated outcomes. RECENT FINDINGS: The risk of uterine rupture varies by scar type, with the highest rates after classical cesarean incisions. T

Maternal death from ruptured rudimentary horn pregnancy: a rare case report.

Pregnancy in a rudimentary horn is rare but can lead to severe complications, including life-threatening complication like rupture of the rudimentary horn. Herein, we report a case of maternal death due to massive hemorrhage from a ruptured non-communicating rudimentary horn pregnancy. A 26-year-old female died at home after experiencing severe abdominal cramps three hours prior to her death. She contemplated seeking medical attention at the beginning of her cramps but decided to wait as her pain initially subsided. Tragically, her condition worsened suddenly, and she collapsed before accessin

Neonatal hyperoxia exposure causes cerebellar lesions and behavioral abnormalities in rats.

In humans, fetal cerebellar development peaks during the final stages of pregnancy. Preterm infants experience hyperoxia (excessive oxygen) outside the uterus. Hyperoxia exposure causes neurological deficits in preterm infants. However, detailed mechanisms underlying hyperoxia-induced neurological deficits remain unclear. Previous studies on neurological deficits have focused on cerebral lesions. However, recently, cerebellar lesions have been observed on brain magnetic resonance imaging in preterm infants. We herein aimed to investigate behavioral and cerebellar tissue–level changes in Spragu

Unraveling the Genetic Mysteries of Müllerian Anomalies: Research Approaches and Clinical Significance.

Müllerian anomalies are a collection of heterogeneous anatomical disorders of the female genital tract that present with complex clinical features of which severe subtypes like congenital aplasia of the vagina and uterus, may present with primary amenorrhea and dyspareunia, while mild cases like septate uterus, are often asymptomatic. Regardless of the types, the Müllerian anomalies impose both psychological and physical burdens on patients. Currently, the etiology of Müllerian anomalies remains largely unclear, which hinders early diagnosis and intervention. Although the advent of next-genera
