Laparoscopic Assisted Neo-Cervico-Vaginal Anastomosis by Rail-Road Procedure In Cervico-Vaginal Atresia.

Cervico-vaginal atresia is a rare entity, difficult to diagnose and treat, with a higher rate of complications. It is classified as type 1b mullerian anomaly per ASRM and subclass C4 per ESHRE classification. The increased rate of re-stenosis and sepsis makes the surgery challenging. Also, the young adolescent age group makes compliance difficult, necessitating an empathetic yet efficient approach. We aim to compare and observe the two repair techniques: laparoscopic v/s open. A stepwise narrative protocol describing this modified technique, which uses a key-hole fundal incision to reach the e

The effects of endometrial polyps, leiomyomas, adenomyosis, and endometriosis on endometrial receptivity: phenotype, molecular pathways, and clinical implications.

PURPOSE OF REVIEW: This review synthesizes current evidence on how endometrial polyps, leiomyomas, adenomyosis, and endometriosis influence endometrial receptivity, emphasizing phenotype-specific effects, molecular mechanisms, and implications for embryo transfer outcomes. RECENT FINDINGS: Impaired endometrial receptivity, when present, is mediated by a limited set of convergent pathways, including Homeobox gene ( HOX ) A10/HOXA11 suppression, inflammatory cytokine dysregulation, and progesterone resistance. Leiomyomas and adenomyosis alter decidualization through paracrine signaling, includi

Ultrasound Sliding Sign Assessed by Transvaginal or Transrectal Route for Predicting Complete Pouch of Douglas Obliteration in Patients with Ovarian Endometriomas: An Exploratory Retrospective Study.

Background and Objective: The ultrasound sliding sign dynamically assesses pouch of Douglas (POD) obliteration in suspected endometriosis. Transvaginal (TVS) and transrectal (TRS) ultrasound are used in patients with and without a self-reported history of vaginal intercourse, respectively, and thus in clinically distinct populations. We evaluated sliding sign performance for predicting surgically confirmed complete POD obliteration in patients with ovarian endometriomas and described findings in route-defined TVS and TRS groups. Materials and Methods: This retrospective cohort study enrolled 3

Double Diagnostic Dilemma: Coexistence of Turner Syndrome and Mayer-Rokitansky-Küster-Hauser Syndrome Presenting as Primary Amenorrhea-A Case Report.

INTRODUCTION: Primary amenorrhea may result from chromosomal, endocrine, hypothalamic, developmental, or structural abnormalities. Turner syndrome is associated with gonadal dysgenesis and preserved Müllerian structures, whereas Mayer-Rokitansky-Küster-Hauser (MRKH) syndrome involves uterovaginal agenesis with normal ovarian function. Their coexistence is extremely rare and has significant reproductive and long-term health implications. CASE: An 18-year-old female presented with primary amenorrhea, short stature, and absent secondary sexual characteristics. Imaging showed an absent uterus and

Ruptured rudimentary horn pregnancy in a unicornuate uterus.

A ruptured rudimentary horn pregnancy is an obstetric emergency. It presents clinically similar to a ruptured tubal ectopic pregnancy. In patients with a known uterine anomaly, it is a risk that can be anticipated; however, in patients with a previous caesarean, who are unaware of having a uterine anomaly, it becomes a diagnostic challenge. We present such a case of a multigravida with a previous caesarean section who presented at 15 weeks gestation with severe abdominal pain and hypotension. On ultrasonography, a single extrauterine fetal demise with features of uterine rupture and massive ha

Irreducible inguinal hernia containing a noncommunicating rudimentary uterine horn: A case report.

Müllerian duct anomalies presenting within an inguinal hernia are exceptionally rare in adult women. We report a case of an irreducible inguinal hernia containing a noncommunicating (type III) rudimentary uterine horn with the ipsilateral fallopian tube and discuss its diagnosis and management. A woman in her 40s (gravida2 para1) presented with a 20-year history of a painless, irreducible mass in her right inguinal region. Physical examination revealed a firm, fixed, nontender solid mass measuring approximately 3 × 4 cm, while pelvic examination was unremarkable. Ultrasonography indicated a un

Mayer-Rokitansky-Küster-Hauser syndrome type II complicated by strangulated left inguinal ovarian-fallopian tube hernia in a pediatric patient: a case report.

BACKGROUND: Mayer-Rokitansky-Küster-Hauser (MRKH) syndrome is a congenital disorder characterized by Müllerian duct anomalies. Patients have a significantly higher incidence of inguinal hernia (6.4%-15.6%) compared to the general female population. These hernias frequently contain ovarian or tubal tissue and are associated with a high risk of strangulation, posing a threat to future fertility. This case report details a rare pediatric presentation of MRKH syndrome type II with a strangulated inguinal ovarian-tubal hernia. CASE PRESENTATION: A 7-year-5-month-old girl presented with a 28-hour h

Emergency Cesarean Delivery Complicated by Massive Postpartum Hemorrhage Revealing an Undiagnosed Unicornuate Uterus: A Case Report.

Increased first-trimester nuchal translucency (NT) may be associated with chromosomal, genetic, and structural abnormalities, although some pregnancies progress with normal invasive testing and reassuring interval imaging. This case involved a 32-year-old primigravida referred at 12+5 weeks' gestation following the identification of markedly increased NT measuring 5.6 mm, with associated body wall oedema. Chorionic villus sampling demonstrated normal fluorescence in situ hybridisation (FISH) and chromosomal microarray results, while subsequent antenatal imaging showed resolution of the body wa

Acquired Uterine Arteriovenous Fistulas After First-Trimester Pregnancy Loss: A Narrative Review with Case-Based Insights into Minimally Invasive Management.

Background and Objectives: Uterine arteriovenous fistulas (AVFs) and arteriovenous malformations (AVMs) are rare but potentially life-threatening vascular anomalies that most commonly develop after pregnancy-related uterine trauma, such as curettage or surgical evacuation. The widespread use of color Doppler ultrasonography has led to increased recognition of these lesions and a shift from hysterectomy toward fertility-preserving, minimally invasive management. This narrative review summarizes current evidence on acquired uterine AVF/AVM after early pregnancy loss, with particular emphasis on

Endometrial imaging.

AIM: Imaging remains fundamental to the diagnosis and management of endometrial disorders. Recognition of the normal endometrium in premenarchal, menstruating, post-partum and post-menopausal patients, as well as patients taking medications such as hormone replacement therapy and tamoxifen, is essential to exclude endometrial pathology. In addition, endometrial cancer is now the most common gynaecological malignancy in the United Kingdom, and imaging remains central to the diagnosis and management of these patients. MATERIALS AND METHODS: This paper explores multimodality imaging appearances

Gubernacular discontinuity and abnormal distal fixation in cryptorchidism: Challenging the classical concept.

BACKGROUND: The gubernaculum is essential for testicular descent, but its detailed surgical anatomy remains poorly understood. We have previously identified an unrecognized anatomy of the round ligament in female patients with sliding inguinal hernias. OBJECTIVE: This study investigated whether comparable anatomical features exist in the gubernaculum of male cryptorchidism patients, as compared to those identified in female sliding hernias. MATERIALS AND METHODS: We retrospectively analyzed undescended testes located in the inguinal canal that underwent open inguinal orchidopexy between 2016

Retained products of conception with uterine arteriovenous malformation: A case report.

RATIONALE: Uterine arteriovenous malformation (UAVM) is a rare but potentially life-threatening cause of abnormal uterine bleeding. Diagnosis is particularly challenging when UAVM coexists with retained products of conception (rPOC), as both entities can present with overlapping clinical and imaging features. Accurate differentiation is essential for appropriate management, especially in women desiring preservation of fertility. PATIENT CONCERNS: A woman of reproductive age presented with recurrent episodes of profuse vaginal bleeding, accompanied by progressive anemia. The severity, persiste

Single-Cell Transcriptomic Profiling and Machine Learning Integration Unveil Stromal Cell Heterogeneity in Endometriosis.

BACKGROUND: Endometriosis (EMs) affects approximately 10% of reproductive-age women worldwide, yet its pathogenesis remains incompletely understood. Abnormal cell differentiation and somatic mutations in the ectopic endometrial microenvironment play critical roles in disease progression and treatment response heterogeneity. This study is aimed at elucidating the molecular mechanisms underlying ectopic endometrial cell differentiation using machine learning (ML) approaches and single-cell RNA sequencing (scRNA-seq), and identifying novel prognostic biomarkers and therapeutic targets, with parti

Platelet-rich plasma as an adjuvant therapy for intrauterine adhesions: a narrative review of mechanisms, clinical efficacy and combination strategies.

BACKGROUND: Intrauterine adhesions (IUA), particularly moderate to severe cases, is a leading cause of decreased menstrual flow and secondary infertility in women, affecting an estimated 2.8%∼46% of infertile women. Hysteroscopy remains the gold standard for diagnosis, with high sensitivity and specificity (both exceeding 90%), while three-dimensional ultrasonography offers a non-invasive alternative with a reported sensitivity of 86% and specificity of 90% for detecting intrauterine abnormalities. The success of surgical management via transcervical resection of adhesions (TCRA) is primarily

Rudimentary horn pregnancy mimicking tubal ectopic pregnancy: a diagnostic dilemma.

Rudimentary horn pregnancy is a rare form of ectopic pregnancy associated with Müllerian duct anomalies and carries a high risk of uterine rupture and life-threatening haemorrhage. We report the case of a woman in her 20s, gravida 2, abortion 1, who presented with 6 weeks of amenorrhoea and a positive urine pregnancy test. Transvaginal ultrasonography demonstrated a 6-week 4-day gestational sac in the right adnexa with an empty uterine cavity, suggestive of an ectopic pregnancy. Diagnostic laparoscopy revealed a right rudimentary horn pregnancy, which was excised along with the ipsilateral fal
