Modern hysteroscopic management of congenital uterine anomalies: current techniques, emerging technologies, and future directions.

BACKGROUND: Congenital uterine anomalies are clinically relevant in reproductive medicine, particularly among women with infertility, recurrent pregnancy loss, adverse obstetric outcomes, or obstructive symptoms. Hysteroscopy has transformed the management of correctable intracavitary anomalies by enabling conservative, image-guided reconstruction of the uterine cavity while avoiding the morbidity of historical abdominal metroplasty. SCOPE: This narrative review critically examines contemporary hysteroscopic management of septate uterus, dysmorphic uterus, bicorporeal septate uterus and rare

Unicornuate Uterus With a Rudimentary Horn Adenomyoma, Ipsilateral Renal Agenesis, and Deep Infiltrating Endometriosis: A Case Report.

Congenital uterine malformations are often difficult to classify, and an incorrect label can carry over for years across successive clinical records. We describe a 39-year-old woman, gravida 1 para 1, referred for chronic pelvic pain and intermenstrual bleeding, whose uterus had previously been recorded as didelphys. Pelvic magnetic resonance imaging, with T2-weighted sequences in three planes, T1-weighted and fat-saturated sequences, diffusion-weighted imaging, and post-gadolinium imaging, instead demonstrated a unicornuate uterus with a left, cavitated, non-communicating functional rudimenta

Missed Until Found: Diagnosis and Hysteroscopic Management of a Uterine Septum in a Woman With Primary Infertility.

Congenital uterine anomalies are an important yet frequently overlooked cause of female infertility, with a septate uterus representing the most common developmental anomaly of the uterine cavity. Although some affected women remain asymptomatic, others present with infertility, recurrent pregnancy loss, or adverse obstetric outcomes, making early recognition essential for appropriate management. We report the case of a 26-year-old nulligravid woman from Azerbaijan who presented with primary infertility of three years' duration despite regular menstrual cycles and an otherwise unremarkable inf

Obstructed Hemivagina and Ipsilateral Renal Anomaly (OHVIRA) Syndrome: A Case Report.

Obstructed hemivagina and ipsilateral renal anomaly (OHVIRA) syndrome is a rare congenital genitourinary tract anomaly characterized by the triad of uterine didelphys, unilateral obstructed hemivagina, and ipsilateral renal agenesis. Patients typically present with symptoms of cyclical pain after menarche. Cases are usually complex and may pose difficulties in diagnosis and management. Accurate diagnosis and prompt treatment are essential to relieve patient symptoms and prevent complications. We present a case of OHVIRA syndrome that was successfully managed surgically.

Surgical Correction of a Complete, Symmetric, Longitudinal Vaginal Septum Using LigaSure Technology: A Case Study.

A longitudinal vaginal septum is a congenital anomaly resulting from incomplete Müllerian duct development and is often associated with uterine didelphys, which can lead to reproductive symptoms and challenges. Surgical correction may be necessary for symptomatic individuals, those facing reproductive barriers, or women planning for vaginal birth. We present the case of an 18-year-old woman with uterine didelphys and a complete symmetric longitudinal vaginal septum who elected surgical resection of the septum. The procedure was performed using a LigaSure device and was uncomplicated. This case

Successful Treatment of Hematopyometra in One Horn of a Didelphys Uterus While Maintaining a Viable Pregnancy in the Other Horn.

Congenital uterine anomalies are rare, with didelphys uterus being one of the least common. Although generally asymptomatic, it can be associated with infertility, dysmenorrhea, and various pregnancy complications including spontaneous abortion and preterm delivery. We present the case of a 29-year-old gravida one para zero female at 6 weeks and 4 days gestational age who presented with pelvic pain during pregnancy. She was incidentally found to have a didelphys uterus during evaluation. She was diagnosed with a hematopyometra in the right horn, likely the cause of her pelvic pain, while carry

Pregnancy Outcomes in Three Distinct Uterine Anomalies: A Case Series.

This case series presents three patients with distinct Müllerian anomalies, highlighting variability in diagnosis, clinical management, and obstetric outcomes. Case 1 involved uterine didelphys with pregnancy in the right hemi-uterus, resulting in non-reassuring fetal heart rate tracings, prompting repeat cesarean delivery at 37 weeks' and six days' gestation. Case 2 involved a unicornuate uterus with a noncommunicating rudimentary horn and persistent breech presentation, requiring a scheduled cesarean delivery at 38 weeks' gestation. Case 3 involved a partial septate uterus associated with br

Concomitant Septate Hymen and Bicornuate Uterus: Emphasizing the Need for Thorough Internal Genital Tract Evaluation.

Congenital anomalies of the hymen are uncommon, and their co-occurrence with congenital uterine anomalies is considered even rarer. This case report describes a patient with a septate hymen combined with a bicornuate uterus. A teenage girl presented to the emergency department with a retained intravaginal tampon. Local examination revealed a septate hymen, and hymenectomy was performed. Over the following years, multiple transabdominal pelvic ultrasounds were performed because of persistent functional hypothalamic amenorrhea, revealing no underlying abnormalities of the internal genital tract.

Maternal and fetal outcomes in women with uterine congenital abnormalities: a systematic review and meta-analysis.

BACKGROUND: Congenital uterine anomalies (CUA) are structural abnormalities resulting from disrupted Müllerian duct development and have been associated with adverse reproductive and obstetric outcomes. However, the magnitude of these risks and variation across anomaly subtypes remain incompletely defined. OBJECTIVES: To evaluate the association between CUA and adverse maternal and fetal outcomes and to examine subtype-specific risk patterns using contemporary evidence. METHODS: We conducted a systematic review and meta-analysis of studies identified from MEDLINE, Web of Science, and Cochran

Intrauterine Pregnancy in a Uterus Didelphys Mimicking an Ectopic Pregnancy.

Uterus didelphys is a rare congenital uterine anomaly resulting from the complete failure of fusion of the Müllerian ducts. Early pregnancy in a duplicated uterus can present a significant diagnostic challenge, as the gravid lateralized hemi uterus may be mistaken for an adnexal mass. A 31-year-old woman presented at six weeks of gestation with mild abdominal cramping. A two-dimensional transvaginal ultrasound revealed an empty main uterine cavity and a gestational sac adjacent to the right ovary. Given a serum beta-hCG level of 4,200 mIU/mL and concern for an ectopic pregnancy, urgent diagnos

Unintentional Diagnosis of Uterus Didelphys in a Primigravida During Emergency Cesarean Section: A Case Report.

Uterus didelphys is a rare congenital Müllerian duct anomaly defined by the presence of two uteri, typically with two cervices and occasionally with a vaginal septum. Although in many cases it is asymptomatic, it is linked to several complications, including miscarriage, premature birth, malpresentation, and an increased rate of cesarean section. We report a 19-year-old primigravida incidentally diagnosed with uterus didelphys at emergency cesarean section for fetal distress at 38 weeks. Uterus didelphys is one of the rare Müllerian duct anomalies and is often missed on routine antenatal ultra

Symptomatic prepubertal obstructed hemivagina and ipsilateral renal anomaly (OHVIRA)-spectrum anomalies: two case reports with individualized management and no recurrence during follow-up.

BACKGROUND: Obstructed hemivagina and ipsilateral renal anomaly (OHVIRA) syndrome is a congenital anomaly characterized by uterus didelphys, obstructed hemivagina, and an ipsilateral renal anomaly, most commonly renal agenesis. Most patients become symptomatic after menarche and require surgical treatment. In prepubertal girls, asymptomatic fluid collections are often managed conservatively, whereas symptomatic cases have often prompted concern for ectopic ureter-related urinary accumulation requiring intervention. We report two early-childhood cases to show that integrated assessment of imagi

Tubo-ovarian abscess in an adolescent with uterine didelphys and prior cloacal repair: case report and literature review.

Tubo-ovarian abscess (TOA) is an uncommon complication of pelvic infection, particularly in patients with Müllerian anomalies. We report the case of a 16-year-old girl with uterine didelphys and a history of cloacal repair who presented with fever, abdominal pain, and a large multiloculated left adnexal mass. Laparoscopy revealed a TOA within a severely distorted pelvis, requiring extensive adhesiolysis, excision of the abscess cavity with adherent tubal remnants, and partial cystectomy with ovarian preservation. Histopathology confirmed a purely inflammatory process, excluding malignancy. Thi
