Prevalence and Predictors of Abnormal Cervical Cytology Among Women Living With HIV and Without HIV From the MACS/WIHS Combined Cohort Study.

BACKGROUND: Cervical cancer incidence has markedly declined among US women living with HIV (WLWH) and women without HIV (WWoH). We examined the effects of HIV and immunosuppression on cervical abnormalities in the current antiretroviral treatment era. METHODS: We conducted a cross-sectional analysis of WLWH and WWoH aged 29-74 years enrolled in the MACS/Women's Interagency Study Combined Cohort Study. For each participant, the first Papanicolaou smear (Pap) performed between October 2020 and March 2024 was included. Wilcoxon rank sum tests and Pearson χ 2 tests compared participant characteri

Immune remodeling in HPV-associated cervical disease.

INTRODUCTION: Persistent high-risk HPV infection is the principal cause of cervical dysplasia, but variation in disease phenotype may also be associated with differences in the local immune and microbial environment. METHODS: We characterized the cervicovaginal immune microenvironment in relation to HPV status, vaginal community state type (CST), and cytological grade. RESULTS: HPV-positive samples showed reduced IL-4 and IL-17E/IL-25, while IFN-α2 and IL-3 varied across community state types in a non-monotonic, CST-specific manner: IFN-α2 was relatively elevated in the Lactobacillus gasseri

Understanding the impact of Mayer-Rokitansky-Küster-Hauser syndrome on sexual wellbeing-a qualitative study.

BACKGROUND: Mayer-Rokitansky-Küster-Hauser syndrome (MRKH) is characterized by the absence or underdevelopment of the uterus, cervix, and/or upper vagina. Individuals with MRKH often experience significant psychosexual impacts, including reduced self-esteem and sexual wellbeing. Yet, little is known about their lived experiences of sexual wellbeing. AIM: To explore sexual wellbeing among individuals with MRKH. METHODS: Participants (n = 12) were recruited through MRKH Canada and partook in semi-structured interviews centered on their experiences of navigating sexuality. Transcripts were subj

Immunohistochemical expression of Ki-67, estrogen receptor, and human epidermal growth factor receptor 2 in p16-positive premalignant and malignant cervical squamous lesions: associations with clinicopathological parameters.

PURPOSE: Human papillomavirus is the dominant etiological factor underlying atypical cervical squamous epithelial cell abnormalities and cervical carcinoma. Currently, only a limited number of drugs targeting specific biomarkers in cervical cancer are available. This study aimed to assess the expression of estrogen receptor (ER), human epidermal growth factor receptor 2 (HER2), and the Ki-67 proliferative index (Ki-67) in p16-positive cervical squamous premalignant and malignant lesions, which may help clarify the potential role of targeted therapies in cervical cancer. METHODS: In p16-positi

Systemic Immune-Inflammation Markers Associate With Cervical Human Papillomavirus Infection in Asymptomatic Women.

BACKGROUND: Human papillomavirus (HPV), a major cause of cervical cancer, typically remains localized to the cervix. However, there is limited understanding of whether this localized infection is associated with systemic immune responses in asymptomatic women, beyond the immediate cervical microenvironment. This study aimed to assess whether systemic immune-inflammation markers are linked to HPV infection and HPV-related cytological abnormalities in asymptomatic women undergoing health screening. METHODS: This cross-sectional study included women from the Kangbuk Samsung Health Study who unde

Cervicovaginal Virome Restructuring Associated with HPV Status, Bacterial Dysbiosis, and Cervical Cytological Abnormalities.

The cervicovaginal virome remains poorly characterized in relation to human papillomavirus (HPV) infection and bacterial community structure. We performed shotgun metagenomic sequencing of 311 cervicovaginal specimens from a Kazakhstani cohort spanning seven groups defined by HPV status and cervical cytology. Viral community composition differed according to HPV status, with increasing representation of the oncogenic Alphapapillomavirus 9 clade across more abnormal cytological categories. Predicted phage functional profiles also differed between NILM HPV-positive and NILM HPV-negative women. I

AI-assisted point of care ultrasound and an innovative vaginal speculum: a proposed combined approach to reducing spontaneous preterm birth.

The Bouquet Speculum™ is an innovative vaginal speculum developed to enhance patient comfort, improve cervical visualization, and facilitate ease of use for healthcare providers during gynecologic examinations and procedures. Studies show AI-assisted ultrasound can identify women at risk for spontaneous preterm birth (s-PTB) more accurately than standard ultrasound. Cervical cerclage, often performed to delay delivery, is sometimes difficult with standard 2-bladed speculums. The Bouquet Speculum™ combined with AI-assisted ultrasound offers the potential for improved outcomes and reduction of m

Incidence of uterine cancer after supracervical hysterectomy.

BACKGROUND: Supracervical hysterectomy (SCH), commonly performed during sacrocolpopexy, involves removal of the uterine corpus, leaving the cervix in-situ. Significant anatomic variation exists in remnant tissue, which may develop into a uterine malignancy. No prior studies have sought to describe the future risk of developing de novo uterine cancer after SCH with benign pathology. This study aims to describe the incidence rate of uterine cancer following SCH performed for benign indication. METHODS: Retrospective cohort study of patients who underwent SCH between June 2000 and December 2022

Registry Study of the Working Group on Cervical Pathology and Colposcopy (AGCPC) on the Diagnostic Algorithm for the New Cervical Cancer Screening - Persistent HPV Infections and Squamous Cell Lesions: Evaluation of over 10000 Patients.

INTRODUCTION: Cervical cancer screening in Germany was newly regulated as of January 1, 2020. Since then, an organized cancer screening guideline (oKFE-RL) for cervical cancer screening has been effective. From the age of 20, a cytological smear is taken once a year and from the age of 35 a co-test is carried out every three years. In the event of abnormalities, an algorithm regulates the procedure. This algorithm states that even so-called low-risk groups should have a diagnostic colposcopy at an early stage. This approach has been widely discussed and continues to serve as the basis of this

Genetic and embryonic transcriptome analyses reveal the molecular and developmental basis of Mayer-Rokitansky-Küster-Hauser syndrome.

BACKGROUND: Mayer-Rokitansky-Küster-Hauser syndrome (MRKHS) is characterised by aplasia of the uterus, cervix and upper part of the vagina. The genetic aetiology remains incompletely understood. METHODS: We performed gene-level and gene set-level burden analyses based on exome sequencing/genome sequencing data from 727 probands with MRKHS and 2504 female control individuals. Single-cell RNA sequencing (scRNA-seq) was performed on human and mouse embryonic metanephros at different developmental stages. Genetic and transcriptomic data were integrated to prioritise suboptimal genetic signals, id

Maternal death from ruptured rudimentary horn pregnancy: a rare case report.

Pregnancy in a rudimentary horn is rare but can lead to severe complications, including life-threatening complication like rupture of the rudimentary horn. Herein, we report a case of maternal death due to massive hemorrhage from a ruptured non-communicating rudimentary horn pregnancy. A 26-year-old female died at home after experiencing severe abdominal cramps three hours prior to her death. She contemplated seeking medical attention at the beginning of her cramps but decided to wait as her pain initially subsided. Tragically, her condition worsened suddenly, and she collapsed before accessin

The Impact of Congenital Anomalies of the Male and Female Reproductive Organs on Infertility and Recurrent Pregnancy Loss: A Review.

Congenital anomalies of the reproductive system represent a heterogeneous group of structural and functional abnormalities affecting both male and female genital organs. These anomalies typically arise during embryogenesis and may remain asymptomatic until they are incidentally identified during evaluation for infertility, recurrent pregnancy loss, or disorders of sexual development. In females, abnormalities include Müllerian duct anomalies and congenital malformations of the uterus, cervix, vagina, and ovaries, such as Mayer-Rokitansky-Küster-Hauser syndrome, septate, unicornuate, bicornuate

Stepwise Colpotomy With Selective Vaginal Septum Trimming During Total Laparoscopic Hysterectomy for High-Reaching Longitudinal Vaginal Septum.

A high-reaching longitudinal vaginal septum presents specific technical challenges during total laparoscopic hysterectomy, particularly in achieving controlled colpotomy and tension-free vaginal cuff closure. In asymptomatic patients, the optimal timing and extent of septal resection remain undefined, and routine complete excision may represent overtreatment. We report a case of total laparoscopic hysterectomy performed using a stepwise colpotomy guided by independent vaginal landmarks in a patient with a complete uterine septum and complete longitudinal vaginal septum extending to the cervix.

Successful pregnancy after reconstructive surgery for a woman with complete cervical and vaginal agenesis - a case report and literature review.

Congenital cervical agenesis is a rare but important obstructive cause of primary amenorrhoea. Initial diagnosis is often delayed. With advances in reconstructive surgery, fertility restoration is now a realistic option for some women. We present a case of complete cervical and vaginal agenesis diagnosed in a 16-year-old girl who presented with cyclical pelvic pain and primary amenorrhoea. Following successful vaginoplasty and direct uterovaginal anastomosis, regular, pain-free menses, and later, uncomplicated sexual intercourse was achieved. Following a 10-year period of primary infertility a

Infertility in an American Quarter Horse mare with cervical duplication.

Cervical duplication is a disorder of sexual development (DSD) that may be present in the absence of chromosomal aberrations. It is a rare cause of infertility in mares and is attributed to the failure of the Müllerian ducts to fuse properly, leading to the persistence of the median walls along the whole length of the cervix. This case report involves a 13-year-old American Quarter Horse mare with cervical duplication. Prior to this diagnostic, the mare was bred multiple times, but pregnancy was not achieved. The mare presented for a breeding soundness evaluation. During the examination an abe
