Mayer-Rokitansky-Kuster-Hauser Syndrome: From Radiological Diagnosis to Further Challenges-Review and Update.

Mayer-Rokitansky-Küster-Hauser (MRKH) syndrome encompasses a range of Müllerian duct anomalies characterized by congenital absence of the uterus and the upper two-thirds of the vagina in young women who otherwise exhibit normal endocrine function and a 46,XX karyotype. MRKH syndrome can occur in an isolated form (type I) or in association with other congenital anomalies (type II or MURCS association), which may include renal, vertebral, auditory, and cardiac defects. It represents one of the most frequent causes of primary amenorrhea, affecting approximately 1 in every 4000-5000 women. MRKH sy

The genetic background of Mayer-Rokitansky-Küster-Hauser (MRKH) syndrome: A systematic review.

INTRODUCTION: Mayer-Rokitansky-Küster-Hauser (MRKH) syndrome is characterized by the congenital absence of the uterus and upper vagina, resulting from failed Müllerian duct development during embryogenesis. Given advancements in reproductive medicine enabling genetic motherhood, this study aims to systematically review published research on the genetic and epigenetic factors contributing to MRKH etiopathogenesis. METHODS: Original peer-reviewed studies were identified through a PubMed search and manual screening of references in included papers and relevant reviews. RESULTS: From 494 records

A Cognitive Behavioural Therapy for Mayer-Rokitansky-Küster-Hauser Syndrome Adapted for Other Disorders of Sex Development: A Qualitative Study.

BACKGROUND: Variations in Sex Characteristics (VSC) also called Differences in/Disorders of Sex Development (DSD) include a range of conditions of congenital abnormalities of the reproductive system. Either the internal and/or external genital tract are affected, presenting as androgenisation of the external genitalia, or incomplete formation of the vagina and uterus, and/or gonads. We previously developed a model of the core psychological impact of diagnosis/medical treatment of one of these conditions, namely utero-vaginal-agenesis or Mayer-Rokitansky-Küster-Hauser Syndrome (MRKH), and a Cog

Case Report: Persistent urogenital sinus (PUGS) associated with complete bicorporeal uterus, double cervix and partial longitudinal non-obstructing vaginal septum: a one-stop diagnostic and operative hysteroscopic approach.

INTRODUCTION: Persistent urogenital sinus (PUGS) is a rare congenital malformation characterized by the confluence of the urethra and vagina into a single distal channel. It may be associated with other congenital anomalies, including Müllerian malformations. Although typically diagnosed in childhood, delayed diagnosis may occur, particularly in patients with mild or nonspecific symptoms. CASE PRESENTATION: We report the case of a 39-year-old nulliparous woman referred for suspected complex genital malformation, presenting with chronic pelvic pain, dyspareunia, and recurrent urogenital infect

Diagnostic Accuracy of Imperforate Hymen at a Single Referral Center.

STUDY OBJECTIVE: Imperforate hymen (IH) is a rare congenital anomaly that results in vaginal outlet obstruction. IH can cause significant morbidity if not managed appropriately, which depends on accurate identification of the condition. However, data on the accuracy of IH diagnosis is limited. This study aimed to investigate the accuracy of IH diagnosis at a single referral center. METHODS: Institutional Review Board approval was obtained for this retrospective chart review. ICD-10 codes identified patients with "hymenal abnormalities" between 2018 and 2023 at a single children's hospital. Th

H-type Rectovestibular Fistula Presenting as a Left Labial Abscess: A Diagnostic Masquerade.

Anorectal malformations (ARMs) is a congenital abnormality that is commonly seen in newborns without a normal or ectopic anal opening. H-type fistula is a rare subtype and is commonly diagnosed in females after a history of passing stool from the vagina which may be the first and only sign of an underlying ARM which necessitates further diagnostic modalities, treatment, and follow-up. These children may have a normal anal opening. We report on a rare case of rectovaginal fistula presenting with a left labial abscess.

Prevalence of endometriosis in Mayer-Rokitansky-Küster-Hauser syndrome variants: a systematic review and meta-analysis.

STUDY QUESTION: In patients with Mayer-Rokitansky-Küster-Hauser syndrome (MRKHS; population), is the presence of functional endometrium (FE) in uterine remnants (URs) (exposure), compared with the absence of FE in UR/absence of UR (comparison), associated with a higher prevalence of endometriosis (outcome)? SUMMARY ANSWER: The aggregate prevalence of endometriosis was considerably higher in MRKHS patients with FE (MRKHSFE+) than in those without FE (MRKHSFE-). WHAT IS KNOWN ALREADY: The pathogenesis of endometriosis is not fully understood. The finding of pelvic endometriosis in patients wit

The incomplete cloaca and Bardet-Biedl syndrome: A remarkable association with broader implications on patient's care.

BACKGROUND: Incomplete cloaca is a rare cloacal variant that refers to a persistent urogenital sinus in girls with the bowel opening anteriorly in the perineum or the vestibule (summary figure). Most of these girls present primarily to pediatric surgeons/urologists as neonatal hydrocolpos. This study outlines the diagnosis and management of this condition while highlighting its association with Bardet-Biedl syndrome (BBS). The latter is a genetically heterogenous group of autosomal recessive disorders characterised by multisystem affection: pigmentary retinopathy, obesity, mental defect, polyd

Molecular pathogenesis, diagnosis, and management challenges in complete androgen insensitivity syndrome.

Complete androgen insensitivity syndrome (CAIS) is a rare X-linked recessive disorder of sex development (DSD) caused by androgen receptor (AR) gene mutation and present with female phenotypes with male chromosomal karyotype. Primitive bipotent gonads in CAIS differentiate into testes producing androgens and antimüllerian hormone (AMH). However, androgens cannot stimulate embryonic wolffian ducts into male internal reproductive organs owing to AR defect and hormone resistance, while AMH induces the regression of müllerian ducts with the absence of uterus, fallopian tubes, and upper third of th

Genetic Predisposition in Müllerian Malformations: A Case Report.

Müllerian malformations (MM) are congenital anomalies of the female reproductive tract that may have a hereditary component. We report the case of a 20-year-old woman with a notable family history of MM (mother and grandmother with a longitudinal vaginal septum and aunt with a bicornuate uterus) who presented with difficulty inserting tampons and menstrual cups. Physical examination revealed a 3 cm longitudinal vaginal septum, which was confirmed by magnetic resonance imaging (MRI) as a 4 cm septum in the lower two-thirds of the vagina, with no abnormalities in the uterus or cervix. Pregnancy in Women With Klippel-Trenaunay Syndrome Complicated by Uterine Vascular Malformation: A Case Report and Literature Review.

Klippel-Trenaunay syndrome (KTS) is a rare congenital vascular disorder characterized by the classical triad of cutaneous port-wine stain (capillary hemangioma), varicose veins, and hypertrophy of soft tissues and bones. KTS with uterine involvement is extremely uncommon. We present a 27-year-old pregnant woman with KTS complicated by uterine venous malformation. Comprehensive management was provided by a multidisciplinary team after 12 weeks gestation diagnosed with KTS. The patient wore full-length compression stockings and received low-molecular-weight heparin (LMWH) for thromboprophylaxis.

Uterus didelphys bicollis bicolpos-a case report.

Uterus didelphys refers to a congenital uterine anomaly that occurs due to failed fusion of the paired Müllerian ducts. It is characterized by two separate non-communicating uterine horns each with its cervix and fallopian tube. Duplication may be limited to the uterus, cervix or also involve the vagina. Here, we present a case of a 27-year-old primigravida incidentally diagnosed during pregnancy, followed up with cervical cerclage at 13 weeks due to a short cervix and had preterm delivery via cesarean section at 35 weeks. She had a successful pregnancy outcome and an uneventful postnatal cour

Familial occurrence of imperforate hymen in premature monozygotic twins and their mother: a case report and literature review.

BACKGROUND: Imperforate hymen is an uncommon obstructive anomaly of the developing female reproductive tract. There are occasional case reports of imperforate hymen occurring in family clusters, suggesting a plausible familial mode of inheritance. We describe a set of monozygotic premature twins with imperforate hymen noted at birth, whose mother was diagnosed with the same condition as a teenager. We also elucidate the likely underlying mode of inheritance of imperforate hymen. METHOD: We utilized the CARE (Case Report) guideline in reporting the cases. CASE PRESENTATION: These are monozygo
