Uncovering Hidden Genetic Contributors to 46,XY Disorders of Sex Development Through Phenotype-Driven Rare Variant Assessment: A Pilot Study.

BACKGROUND: Despite advances in genetic testing, many 46,XY Disorders of sex development (DSD) cases remain unsolved after whole-exome sequencing (WES). This study intended to explore rare variants in patients with micropenis, cryptorchidism, or hypospadias using bioinformatics analysis to identify potential pathogenic contributors and pathways underlying 46,XY DSD. METHODS: A total of 35 patients with specific phenotypes (micropenis/cryptorchidism/hypospadias) and negative whole-exome sequencing results were enrolled. Bioinformatics analysis methods (SKAT-O test and GO enrichment) were appli

Comprehensive diagnosis and management of Musladin-Lueke Syndrome in a Beagle in Japan.

Musladin-Lueke syndrome (MLS) is an inherited connective tissue disorder in Beagle caused by a homozygous missense mutation in the ADAMTSL2 gene. It is rare, but recognized globally. This report presents the first case in Japan definitively diagnosed via genetic testing. The 6-month-old male Beagle exhibited characteristic "tiptoe" gait, joint stiffness, lateral deviation of the eye socket, and undescended testes. At 10 months of age, bilateral Cryptorchidectomy was performed, and echocardiography revealed no abnormalities. At 12 months of age, the patient demonstrated progressive forelimb sti

Identification of gene variants in 30 patients from southeastern China with severe hypospadias by whole-exome sequencing.

Hypospadias is a common congenital malformation of the male external genitalia, with severe cases presenting considerable surgical and long-term challenges. Despite the clinical importance of severe hypospadias demonstrated by prolonged hospital stays, repeated surgeries, and substantial costs, the genetic etiology of severe hypospadias remains incompletely understood, particularly in diverse populations. To determine the molecular basis, we performed whole-exome sequencing (WES) on 30 Chinese patients from southeastern China with confirmed 46,XY karyotypes. Our analysis identified clinically

OTUD5-related rare X-linked multiple congenital anomalies and neurodevelopmental syndrome: clinical findings and review of the literature.

The OTUD5 gene (OMIM #300713), located on Xp11.23, encodes the ovarian tumor deubiquitinase 5, a deubiquitinase enzyme with critical functions in chromatin remodeling, transcriptional regulation, neuroectodermal differentiation, and innate immune signaling. Hemizygous pathogenic variants in OTUD5 are associated with X-linked multiple congenital anomalies–neurodevelopmental syndrome (MCAND, OMIM #301056), a rare multisystem disorder characterized by developmental delay, intellectual disability, craniofacial dysmorphism, and variable involvement of the cardiac, skeletal, and genitourinary system

Use of apparent diffusion coefficient values to differentiate lymph nodes from testicular tissue in patients with undescended testes.

BACKGROUND: This study investigated the anatomical location of non-palpable undescended testes (UTs), which account for approximately 20% of all cases of UTs. Radiological imaging is essential to determine the location of such testes in the abdomen or groin. Other studies have examined the diagnostic performance of diffusion-weighted MRI (DWI) in detecting and localizing non-palpable UTs. However, no study has used apparent diffusion coefficient (ADC) values to differentiate UT tissue. METHODS: This study included consecutive children referred to a pediatric surgery clinic with a prediagnosis

Expanding the mutational spectrum of RAD50: a case report of Nijmegen breakage syndrome-like disorder in a Chinese child.

BACKGROUND: Nijmegen breakage syndrome-like disorder (NBSLD) is a rare chromosomal instability syndrome caused by biallelic pathogenic variants in RAD50, which is a key component of the MRE11-RAD50-NBS1 (MRN) complex involved in DNA double-strand break repair. Merely few cases have been reported worldwide, and its phenotypic spectrum remains incompletely defined. CASE PRESENTATION: We report a 6-year-old Chinese boy, who presented with bilateral cryptorchidism, severe microcephaly, growth retardation, multiple café-au-lait macules, brachydactyly, and distinctive craniofacial features, includi

Testicular regression syndrome revisited: Insights from a 15-year retrospective study and evolving perspectives on management.

INTRODUCTION: Testicular Regression Syndrome (TRS) represents a form of cryptorchidism characterised by the absence of testicular tissue with persistence of cord structures. While excision of remnants is routinely advocated due to potential malignant risk from seminiferous tubules (SNTs) or germ cells (GCs), the actual risk remains debated, and no universal consensus exists. MATERIALS AND METHODS: We conducted a retrospective review of 262 children with clinically impalpable testes who underwent diagnostic laparoscopy and/or inguinal exploration at a single tertiary centre between 2008 and 20

Comparison of Scrotal Orchiopexy Versus Traditional Inguinal Orchiopexy for Palpable Undescended and Retractile Testis in Children: Insights from a Greek Surgical Center.

Introduction/Purpose: Congenital cryptorchidism and retractile testis represent the most commonly presented abnormalities of the male genitourinary system. Orchiopexy is the surgical treatment for both conditions and can be performed either via the conventional two-incision surgical approach or via a singular scrotal incision. The present study firstly investigated the complications associated with each orchiopexy approach in a single-center pediatric surgical department and secondly compared the surgical outcomes in children with congenital cryptorchidism or retractile testes. Methodology: A

Polyglutamine-binding protein 1 protects mouse testes from heat stress.

Heat exposure of the testes, a condition seen in fevers or cryptorchidism, is a well-established cause of compromised spermatogenesis and male infertility. Cells typically respond to such stresses by forming protective RNA-protein assemblies known as stress granules. While the RNA-binding factor Polyglutamine binding protein 1 (PQBP1) is known to be involved in stress responses in neurons, its specific role in the thermally sensitive testis remains unclear. Here we identify PQBP1 as a critical factor that safeguards germ cell function by orchestrating the stress granule response during testicu

Successful conservative management of isolated hemiscrotal agenesis: case report.

Hemiscrotal agenesis is an extremely rare congenital anomaly characterized by unilateral absence of scrotal skin with preserved midline raphe. Optimal management, particularly conservative approaches in isolated cases without cryptorchidism, remains poorly defined. We report a 5-year-old boy with normal 46,XY karyotype presenting for circumcision revision, who had isolated right hemiscrotal agenesis, penile torsion, and excess preputial skin. The right testis was palpable subcutaneously with adequate perineal space. After penile detorsion and circumcision revision, sufficient skin elasticity a

Receptivity of the human male and female fetal caudal genital ligament to gonadal hormones.

In briefCurrent understanding of testicular descent mechanisms, mainly based on rodent models, attributes a central role to testicular hormones. This study examines the hormonal receptivity and potential sex differences of human caudal genital ligaments (also named gubernaculum in males) during organogenesis. Abstract In both sexes, fetal gonads are connected to the abdominal wall by caudal genital ligaments (CGLs). The male CGL (gubernaculum testis) drives testis descent under the influence of testicular hormones, whereas the fate of the female CGL is thought to result from the absence of the

Laparoscopic Orchiopexy for Pediatric Ectopic Testis.

BACKGROUND: Ectopic testis (ET), testicular descent outside the normal pathway, is a rare condition with variants such as perineal ET (PET), femoral ET (FET), and transverse testicular ectopia (TTE). Most studies on ET have reported open surgical techniques, whereas laparoscopic evidence remains limited. This retrospective single-center study evaluated the feasibility and safety of laparoscopic orchiopexy for pediatric ET. METHODS: We retrospectively reviewed 2306 pediatric patients aged <10 years who underwent laparoscopic orchiopexy for abnormal testicular descent between January 2016 and D

Testicular atrophy rate in single-stage versus two-stage fowler-stephens orchidopexy: a systematic review and meta-analysis.

BACKGROUND: Managing intra-abdominal testes is particularly challenging, especially when short testicular vessels require division. Fowler–Stephens orchiopexy remains the standard approach in such cases, but whether it is more effective when performed in a single or two stages remains uncertain. This review aims to provide a clear, updated comparison of testicular atrophy rates following single-stage (SS) versus two-stage (TS) FSO. METHODS: A systematic search of PubMed/MEDLINE and Google Scholar was conducted until May 2025 in accordance with PRISMA 2020 guidelines. Studies comparing SS-FSO a

Molecular mechanism of testicular oxidative stress-induced ferroptosis in Leydig cells caused by cryptorchidism in yak.

Cryptorchidism is a prevalent reproductive disorder triggering severe testicular damage and male infertility. However, the underlying molecular mechanism in plateau yaks remains largely unclear. This study explored the pathogenesis of androgen deficiency and Leydig cell ferroptosis induced by yak cryptorchidism, and illuminated the relevant regulatory pathways. Testicular specimens were collected from healthy and cryptorchid yaks, the Leydig cells were isolated, purified and cultured in vitro combined with the histopathological staining, immunofluorescence, Western blotting and ELISA were cond

Cryptorchidism in Sarda Sheep: Incidence, Morphology, Ultrasonography and Behavioral Insights.

Cryptorchidism is the most common non-lethal congenital defect of the male reproductive system in sheep, with potential economic consequences for flock management. This study investigated the incidence, testicular morphology, ultrasonographic characteristics, semen quality, and sexual behavior of bilateral cryptorchid Sarda rams. Slaughterhouse inspections of 2360 lambs showed an incidence of 0.87% cryptorchidism. Cryptorchid testes were significantly rounder and lighter than intact testes, indicating impaired development in affected animals. Ultrasonography of 15 adult bilateral cryptorchid r

Discontinuous splenogonadal fusion mimicking a testicular tumor in a 3-year-old boy: Successful testis-sparing resection - A case report.

Splenogonadal fusion (SGF) is a rare congenital anomaly that can mimic testicular malignancy, potentially leading to unnecessary orchiectomy. We report a 3-year-old boy with bilateral cryptorchidism who underwent bilateral orchiopexy via inguinoscrotal approach. Intraoperatively, a reddish, firm extra-testicular mass was identified at the upper pole of the left testis. Due to its uncertain nature, the mass was not manipulated to avoid orchiectomy, and the testis was descended uneventfully. Postoperative Doppler ultrasound showed a hypervascular, hypoechoic heterogeneous mass suggestive of SGF.

A Novel Prediction Model for Monorchidism in Boys With Unilateral Nonpalpable Testes: Diagnostic Utility of Preoperative Ultrasonography.

INTRODUCTION: In boys with non-palpable testis (NPT), the affected side may present as a viable testis (VT) or a nonviable testis (NVT), the latter representing true monorchidism. This study evaluated the utility of ultrasonography (US) for predicting monorchidism and testicular location in unilateral NPT. METHODS: We retrospectively reviewed boys with unilateral NPT who underwent preoperative US between 2009 and 2025. Testicular volumes were converted to age-adjusted standard deviation (SD) scores. A rule-based model classified NVT when the affected testis was visualized with an SD score ≤ c
