In a prospective study during 1981-1983 42 boys aged 3-12 (mean 6.5) years were operated on for a palpable undescended testis. Twelve boys 3-8 (mean 4) years old were operated on for an unpalpable testis and 2 boys for an ectopic testis. The day before operation an ultrasound examination was performed. Reoperations were made on 11 patients and a bilateral operation on 2. In total, 69 ultrasound examinations were made. Three further cases of perineal ectopic testis are reported and the published literature reviewed. Etiopathogenicity is discussed and emphasis placed on the need for perineal exploration in all cases of absence of testes from the scrotum. The most effective route of approach for repair is an inguinal one allowing replacement of the testis into the corresponding hemiscrotum without difficulty. Because of the histopathologic features involved, prognosis is better than that of cryptorchidism.

We report a 5 1/2-year-old boy with preperitoneal ectopic testes. This condition has been reported only once before. Both testes were attached to the umbilicus and at operation were successfully placed in the scrotum.

A 70-year-old man with the complaint of dysuria and painless swelling of the right scrotal sac and inguinal region was operated on for suspected right inguinal hernia. The hernia sac contained two testis and immature uterine tissue, which were pathognomonic of left crossed ectopic testis complicated by hernia uteri inguinalis. The chromosomes were normal. Statistics on 57 similar cases indicated that this was the eldest of all such patients reported in Japan; since he had two children, he seems to have been fertile.

Crossed ectopic testis is an uncommon anomaly. A 10-month-old boy with a crossed ectopic testis and a common vas deferens is presented. At operation 2 testes were in the left inguinal canal. The seminal vesiculogram showed that both vasa deferentia fused to form a common lumen, and then formed a common stem on the left side and opened into the posterior urethra. The spermatic arteriogram revealed that each testis was supplied from the right and left sides, respectively. No testis or vas deferens was located in the right retroperitoneal space. Perineal testis is a very rare congenital anomaly. Since the first case of the disease reported by John Hunter in 1786, more than 113 cases have been reported in the English literature, and 13 cases have been reported in the Japanese literature. The 14th and 15th cases are presented herein. Case 1 was a 38-year-old man who came to our clinic with the chief complaint of discomfort of the perineum. The etiology of certain disorders of sexual differentiation is unclear. The authors have examined the hypothesis that hypospadias and other disorders compatible with a defect in androgen action, such as cryptorchidism, micropenis, chordee/penile torsion, and ectopic testis, might be explained by androgen receptor abnormalities. Abdominal testes represent 15 to 25% of all cases of cryptorchidism. In 5% the testis is so high in the abdomen that it is impossible to perform a conventional orchidopexy. The authors report an experience of 5 cases of testicular autotransplantation achieved by temporarily severing the testicular vessels and then reanastomosing them to the inferior epigastric vessels using a microsurgical technique with an operating microscope at 25 times magnification. A case of crossed ectopic testis is reported. On reviewing the literature according to the definition of the condition only 30 cases can be accepted as authentic cases. Analysing the clinical and other investigations, it is suggested that this is a local phenomenon without any endocrinal derangement, due to a developmental insult resulting in failure of gubernacular mechanism and consequently failure to open the inguinal rings and the descent to the opposite side is due to adhesions to the testis with normal gubernacular mechanism and inguinal rings.

Two infants aged four and eight weeks required an operation for an interstitial inguinal hernia. The clinical picture of this rare form of hernia is characteristic. There is a swelling in the abdominal wall between the internal inguinal ring and the anterior superior iliac spine. In addition there is almost always an ectopic testis on the affected side. Incidence, clinical picture and anatomy of the interstitial hernia are discussed.

During 28 orchiopexies in 22 boys, an analysis was made of the increased length of cord that was obtained at the end of 3 aspects of the operation--after division of the cremaster, after excision of the sac and after retroperitoneal dissection. The mean gain in length of the cord was analysed in relation to the original position of the testis and was greatest in the ectopic testis and least in the testis in the inguinal canal. Each part of the operation produced worthwhile increases in length, but bore no relationship to the age of the child at operation.

Tumors developing on ectopic testes, usually seminomas, are a rare occurrence, but when they do arise they are frequently associated with renal agenesis. Clinical manifestations vary, the presenting sign being generally an abdominal mass. Among the numerous exploratory techniques available the current examination of choice is computed tomography. The precise site of the lesion can be determined employing lower irradiation doses than those delivered during invasive methods.

The authors present three cases of perineal, testicle ectopia, two of which were diagnosed and treated in the Urology Department of the "La Fe" Hospital. The other observation was diagnosed and treated by one of us privately. The treatment was surgical in all the cases and good results were obtained in them all. They discuss the etiopathogeny, clinical characteristics and the uselessness of hormone treatment.

It has now been clearly demonstrated that cryptorchidism is accompanied by a very marked increase in malignancy of the ectopic testis as well as in that in place. Amongst 80 patients undergoing surgery for carcinoma of the testis 14 (17.5%) had a past history of unilateral undescended. A young man with a palpable ectopic testis underwent right testis biopsy and orchiopexy. Since histological findings of the biopsy specimen revealed an unsuspected intratubular carcinoma in situ and evidence of diminished spermatogenic potential, an inguinal orchiectomy was done. This appears to be the first reported case of a tumor of this type developing in an undescended testis.

A case of pubopenile testis and a case of perineal ectopic testis are presented. The mechanism of descensus is largely positive, possibly facilitated by raised intra-abdominal pressure. The testis is usually guided by the gubernaculum and ectopia results from gubernacular failure. The ectopic testis is relatively rare but is easily recognized and treated by orchiopexy.
